Abstract

Purpose: This study aimed to evaluate the correlation between MCH, MCHC, and PHR with the severity of DVI in pediatric patients. Methodology: A cross-sectional observational study was conducted on 47 pediatric DVI patients at PKU Muhammadiyah Sampangan Hospital, Surakarta. Medical records from January to May 2024 were analyzed. Data were statistically evaluated using Spearman’s test to determine the correlation between variables. Results: Among 47 patients, 57.45% had dengue without warning signs, while 42.55% had dengue with warning signs, with no cases of severe dengue. There was no significant correlation between MCH (p=0.383, r=0.130), MCHC (p=0.457, r=-0.111), or PHR (p=0.064, r=-0.273) and the severity of DVI. Applications/Originality/Value: This study examines the roles of MCH, MCHC, and PHR in pediatric dengue virus infection, finding no significant correlation with disease severity. It provides a basis for future research on alternative hematological markers to enhance clinical assessment. This research introduces PHR as a novel parameter in dengue studies, providing a unique perspective on its potential relevance to disease pathophysiology.

Abstract

Purpose: To analyze the relationship between fever duration before admission, hematocrit, platelet count, and leukocyte count with length of hospital stay in pediatric patients with dengue infection. Methodology: This observational study with cross-sectional design was conducted at PKU Muhammadiyah Sampangan Hospital, Surakarta, using secondary data from medical records. Forty pediatric patients were selected through consecutive non-probability sampling technique. Statistical analysis was performed with Spearman correlation test using statistical applications. Results: There was a significant association between fever duration before admission and length of hospitalization (r = -0.407, p = 0.009). However, there was no significant association between hematocrit (r = 0.136, p = 0.403), platelet count (r = 0.101, p = 0.536), or leukocyte count (r = 0.111, p = 0.496) with length of hospitalization. Applications/Originality/Value: This study highlights the importance of early detection and intervention in managing pediatric dengue cases. The significant correlation found between fever duration prior to admission and length of stay underscores the need for prompt treatment to minimize hospital stay due to associated inflammation in the body. In contrast, the lack of significant associations between hematocrit, platelet count, leukocyte level and duration of hospitalization highlights the complexity of factors influencing outcomes and the need to explore other prognostic markers. This study conducted in an endemic region of Indonesia contributes to understanding the management of dengue virus infection and underscores the importance of a comprehensive multifactorial approach to improve patient care strategies.

Abstract

This research aims to evaluate the correlation between erythrocyte level, hematocrit, hemoglobin, and Hb/RDW ratio with the length of hospitalization in pediatric patients with DVI. This observational study used a cross-sectional design and analyzed secondary data from 41 pediatric DVI patients at PKU Muhammadiyah Sampangan Surakarta Hospital. The correlation between hematological parameters and length of hospitalization was analyzed using the Spearman test. Laboratory results showed a mean erythrocyte count of 5.05 million cells/nL (SD 0.61), a median hematocrit of 39.5%, hemoglobin of 13.6 g/dL, and Hb/RDW ratio of 1.03. Bivariate correlation test showed erythrocyte level (r = 0.027, p = 0.867), hematocrit (r = -0.004, p = 0.983), hemoglobin (r = 0.079, p = 0.625), and Hb/RDW ratio (r = 0.116, p = 0.472) showed no significant association with length of hospitalization. This study provides insights into the relationship between hematological parameters and the length of hospital stay in pediatric dengue patients. While no significant correlation was found, it highlights the complexity of factors influencing outcomes and the need to explore other prognostic markers. Conducted in an Indonesian endemic region, the research contributes to understanding DVI management and underscores the importance of a comprehensive, multifactorial approach to improving patient care strategies.

Abstract

Surakarta has a population of 148,992 people living in five sub-districts, namely Laweyan, Serengan, Pasar Kliwon, Banjasari, and Jebres. Jebres District has four Community Health Centers that routinely report on diabetes mellitus (DM) health services. DM cases in the Surakarta area were 12,105 patients. The highest DM cases were in the Jebres District area, with 3,392 (27.89%) residents suffering from DM. DM cases have increased from year to year. The cause of the increase in DM cases is lifestyle changes and a lack of DM screening. The hope is that this activity can reduce the number of DM sufferers in the Jebres District area. Activities have been carried out by checking blood sugar levels at any time. The results obtained were that the blood sugar levels of study participants on Sunday morning at the Jagalan Mosque were 124.5 (47.75) mg/dL. These results indicate that blood sugar levels are still within normal limits, so in the future education and regular screening are needed to prevent cases of DM.

Abstract

Diabetes mellitus involves many mechanisms, the collagen metabolism pathway with very complex interactions and its regulation can contribute to the development of diabetes complications. The collagen 1 alpha 1  (COL1A1) gene plays an important role in the diabetes melitus pathway in adipose tissue by regulating adipocyte function, insulin resistance, and glucose metabolism. COL1A1 expression  is positively correlated with insulin resistance and negatively correlated with insulin sensitivity and is identified as a potential therapeutic target for type 2 diabetes. The cat's whisker plant (Ortosiphon aristatus) pharmacologically has hypoglycemic activity and increases insulin expression. This study aims to observe the potential of cat whisker leaf extract (Ortosiphon aristatus) in increasing the expression of the COL1A1 gene and is protective against increased blood sugar levels in  hyperglycemia-induced Rattus norvegicus. The experimental design with pretest and posttest controlled group design was carried out by dividing 18 test animals into six groups of non-diabetic negative control, diabetic negative control, metformin as positive control, doses of 100, 200, and 400 mg/kgBW. Induction of hyperglycemia with streptozotocin 65 mg/kgBW and nicotinamide 230 mg/kgBW peritoneal injection. The extract treatment is given for 14 days. Blood sugar level measurements were carried out at the treatment’s beginning and end. Adipose tissue was taken to examine COL1A1 gene expression by real time PCR technique by calculating the ratio of the cycle of tresh-hold COL1A1 to the internal ?-actin gene. The results of Kruskall Wallis test showed no significant difference in COL1A1 gene expression between groups and the Spearmen correlation test showed no relationship between blood sugar levels and COL1A1 gene expression (p>0.05). There was a decrease in blood sugar levels before and after treatment for all treatment groups.

Abstract

Metabolic syndrome is a collection of metabolic disorders that are closely related to an increased risk of developing diabetes mellitus and cardiovascular diseases in obese adolescents. Metabolic syndrome in adolescents is still reversible with varying timeframes. In adolescents with metabolic syndrome, the transition from homeostasis with normal blood sugar levels to type 2 diabetes occurs more rapidly compared to adults. Early diagnosis of metabolic syndrome is essential for prevention through lifestyle changes. Living in urban areas has a higher risk of obesity compared to living in rural areas after controlling for other variables. This activity aims to initially screen metabolic syndrome in middle school adolescents in urban and rural areas. The measurement of metabolic syndrome parameters involved 107 students, consisting of 47 middle school students in urban areas and 60 in rural areas. The measurement and data analysis results showed significant differences in body mass index (p<0,001), waist size (p=0.03), waist circumference (p=0.007), and upper arm circumference (p=0.002) between students in urban and rural areas, while blood sugar levels, blood pressure, and neck circumference did not show significant differences.
