Abstract

Backgrounds and purpose: The road map of the National Health Insurance Scheme (JKN) aims to ensure that 75% of patients are satisfied with the quality of health facilities. This study was designed to determine differences in patients’ satisfaction in general, as well as at each individual first-level health facility (FKTP) and based on dimensions of quality. Methods: Study was a cross-sectional sample survey with the respondents of 148 JKN patients selected consecutively from 4 types of FKTP. A total of 4 health centers, 3 government clinics, 4 private clinics and 4 general practitioners randomly selected from 11 health centers, 7 government clinics, 13 private clinics and 70 general practitioners in the city of Denpasar. Data were collected through interviews over March-April 2015 in each health facility using questionnaires by trained interviewers. Data were analyzed by univariate, bivariate and multivariate. Results: A total of 69,59% of JKN patients said that they were satisfied with the quality of FKTP services. This figure was lower than the target of JKN i.e. 75%. The highest satisfaction (90,00%) was in the general practitioners while lowest was in the private clinics (47,46%). Based on the dimensions of quality, JKN patients’ satisfaction in the four FKTPs was significantly different (p=0,008) in physical and empathic dimensions (p=0,013). In the multivariate analysis, variables found related to the satisfaction of the respondents were the FKTP of general practitioners (adjusted PR=1,91; 95%CI: 1,05-3,47) and health centers (adjusted PR=1,76; 95%CI: 1,04-2,97). Conclusion: In general, the patients’ satisfaction rate was still lower than the target of JKN and when viewed per care facilities, the highest satisfaction was found in FKTP of general practitioners and health centers.

Abstract

Background and purpose: Screening for pre-cancerous cervical lesions is already being implemented in Bali. This study aims to determine hormonal contraceptives and STI history as risk factors of pre-cancerous cervical lesions. Methods: A case-control study was conducted involving 76 women of childbearing age diagnosed with pre-cancerous cervical lesions who screened at two health centers in Denpasar Bali from January to March 2015. Control was 76 women of childbearing age with no pre-cancerous cervical lesions who screened at the same period. Consecutive sampling was used to select cases and controls. Data were collected through interview using questionnaires and analyzed using logistic regression to calculate adjusted odd ratio of each risk factor. Results: Multivariate analysis indicated that the use of hormonal contraceptives over ≥5 or <5 years increase risk of pre-cancerous cervical lesions with OR=10,7 (95%CI: 1,04-108,17) and OR=3,0 (95%CI: 1,16-7,84) compared to those who were not using hormonal contraception. History of sexually transmitted infections also increases risk of pre-cancerous cervical lesions with OR=9,7 (95%CI: 3,83 24,18). Conclusion: The use of hormonal contraceptives and history of sexual transmitted infections increased risks of pre-cancerous cervical lesions.

Abstract

Background and purpose: The national family planning program currently promotes the role of husbands as acceptors of vasectomy as contraception. The Wanasaba Sub-district of the East Lombok Regency is one of the areas with a high number of acceptors. This phenomenon needs to be understood more deeply in order to increase vasectomy contraception uptake in other areas. This study aims to understand in depth the factors that contribute to the acceptance of vasectomy as contraception in the Sub-District of Wanasaba of East Lombok. Methods: The study used a qualitative design with phenomenology approach. The data were collected through in-depth interviews. The study was conducted among 20 participants consisting of five couples of vasectomy acceptors and five couples of non-vasectomy acceptors. Data were analyzed using thematic analysis. Results: Research showed that the decision to undergo vasectomy was voluntarily and swift without any pressure or coercion from providers or others. The main factor contributing to the acceptance of vasectomy was the availability of free, direct and fast services facilitated efficiently by providers and cadres. Another factor was economical owing to the fact that hormonal contraceptive was considered costly. The driving factor for the acceptance of vasectomy was active support from the spouse along with social acceptability of the process from the community. Conclusion: Service availability, in addition to the involvement from health workers and cadres were crucial in providing information on the procedure, and in promoting and facilitating vasectomy uptake. It is necessary to improve the capacity of cadres, family planning field workers and family planning motivators in order to increase the number of vasectomy acceptors.

Abstract

Background and purpose: The incidence of recurrence in patients with pulmonary tuberculosis (TB) in Denpasar in 2014 was relatively high, around 3.5% of 1082 patients. This study aims to determine the risk factors for recurrence of pulmonary TB patients in Denpasar. Methods: The study design was a case control with 46 patients with pulmonary TB recurrence aged over 15 years as cases, and 92 patients who had recovered as control. Respondents were selected using systematic random sampling from the TB register from 2013 to 2015 in public health centers in Denpasar. Data were collected by a search of documents, observations, measurements and interviews conducted using questionnaires. Bivariate and multivariate analysis were conducted using chi square test and logistic regression respectively. Results: Risk factors that associated with recurrence of pulmonary TB were co-morbidity of diabetes mellitus (AOR=9.6; 95%CI: 2.17-43.08), adherence (AOR=7.6; 95%CI: 2.85-20.17), exposure to cigarette smoke during treatment (AOR=3.6; 95%CI: 1.41-9.16), home ventilation <10% (AOR=3.4; 95%CI: 1.27-9.47), house contact with pulmonary TB patients (AOR=3.1; 95%CI: 1.31-7.46) and malnutrition (AOR=2.8; 95%CI: 1.02-7.72). Conclusion: Risk factors for recurrence of pulmonary TB among patients were co-morbidity of diabetes mellitus adherence, cigarette smoke exposure during the treatment period, home ventilation <10%, house contact with pulmonary TB patients and malnutrition.

Abstract

Background and purpose: The Indonesian Demographic Health Survey of 2012 indicated that the prevalence of contraceptive use in Bali Province has decreased compared to 2007. In addition, the proportion of the use an intrauterine device (IUD) also continues to decline. To increase number of IUD acceptors is to promote postplacental intrauterine device (PPIUCD), however the acceptance remains low. This study aims to determine factors associated with acceptance of post-placental intrauterine device in Denpasar. Methods: Study was cross sectional with a total of 100 respondents selected by consecutive sampling. Respondents were mothers who had gave birth in January-February 2016 at Wangaya General Hospital and three health centers (Dauh Puri Sub Health Center, East Denpasar I and South Denpasar IV). Data were collected by interview in hospital and health center. Chi square test was conducted for bivariate analysis and multivariate using logistic regression. Results: Proportion of PPIUCD acceptance was 35%. Multivariate analysis indicated that PPIUCD acceptance was associated with perception of benefits (AOR=10.39; 95% CI: 2.792-38.56), perception of low side effects (AOR=5.288; 95%CI: 1.085-25.761), role of health workers (AOR=7.1; 95%CI: 1.781-28.60) and support of the husband (AOR=12.020; 95% CI=2.888-50.01). Abstract

Tidur merupakan kebutuhan fisiologi tubuh untuk dapat berfungsi secara optimal. Apabila kualitas tidur mengalami penurunan maka salah satu risiko yang dapat muncul adalah depresi. Salah satu kelompok yang memiliki risiko mengalami penurunan kualitas tidur adalah mahasiswa kedokteran, dimana hal ini dapat memicu kenaikan tingkat depresi pada kelompok mahasiswa kedokteran. Oleh karena itu penulis ingin mengetahui apakah ada hubungan antara kualitas tidur dengan kenaikan tingkat depresi pada mahasiswa Program Studi Sarjana Kedokteran dan Profesi Dokter angkatan 2018 Fakultas Kedokteran Universitas Udayana. Desain penelitian ini adalah observasional analitik dengan pendekatan cross sectional. Setelah persetujuan komite etik institusional, mahasiswa kedokteran tahun terakhir yang bersedia menjadi subjek untuk studi dan telah diberikan persetujuan tertulis (n=80) diberikan kuesioner Indeks Kualitas Tidur (PSQI) untuk mengamati kualitas tidur dan Kuesioner Kesehatan Pasien-9 (PHQ-9) kuesioner untuk melihat tingkat depresi responden melalui google form. Skor PSQI dan PHQ-9 dihitung dan keterkaitannya diukur secara statistik. Analisis data dilakukan dengan uji chi square didapatkan nilai p=0,006 (p<0,05) yang berarti ada hubungan antara kualitas tidur dengan tingkat depresi pada mahasiswa kedokteran tahun terakhir. Diperlukan penelitian lebih lanjut terkait dengan hubungan antar komponen komponen dari kuesioner PSQI dengan tingkat depresi serta penelitian dengan jumlah sampel yang lebih besar agar penelitian ini dapat digeneralisasi lebih luas. Abstract

Semakin berkembangnya olahraga bola basket di Indonesia ditandai dengan partisipasi anak-anak dimulai dari usia dini. Pada atlet usia muda sangat rentan terjadi cedera yang disebabkan oleh berbagai faktor salah satunya flat foot yang banyak dialami oleh usia muda. Pada atlet basket sering mengalami cedera pada sendi engkel dan jika cedera tersebut berulang dapat menyebabkan terjadinya chronic ankle instability (CAI). Penelitian ini merupakan penelitian observasional analitik dengan pendekatan cross sectional. Pengambilan data bentuk kaki dilakukan dengan menggunakan wet footprint test lalu dilanjutkan dengan pengukuran clarke’s angle. Pengambilan data CAI mengunakan kuesioner Identification of Functional Ankle Instability (IdFAI). Teknik pengambilan sampel menggunakan sistem total sampling dan didapatkan 35 responden dari klub basket Elite Bali dan Merpati Bali. Analisis data dilakukan dengan uji chi square didapatkan nilai p<0.05 yang berarti adanya hubungan antara flat footdengan CAI. Tetapi pada sampel hanya ditemukan 1 orang dengan flat foot sekaligus menderita CAI pada kaki kiri maupun kaki kanan. Dapat disimpulkan bahwa flat foot dapat disingkirkan menjadi faktor risiko dari terjadinya CAI. Diperlukan penelitian lebih lanjut dengan jumlah sampel yang lebih besar untuk dapat mengeneralisasi hasil penelitian ini.

Abstract

Nowadays in this pandemic era, most of the living sectors are obligated to adapt and conduct their duty from home. However, home environment isn’t always designed as a place to study, unlike campus, which is risky to be unergonomic. Moreover, this condition can result in various musculoskeletal disorders, such as low back pain. The aim of this study is to find out the prevalence of low back pain in Undergraduate Study Program in Medicine and Medical Profession Students of Udayana University during online classes. The design of this research is description with a cross-sectional approach. The research subjects were 468 Undergraduate Study Programs in Medicine and Medical Profession Students of Udayana University. Research data are primary data obtained from the standardized Nordic questionnaire. Students with low back pain is found in a higher proportion, which is 59% compared to not having low back pain which is 41%. Factors that are related towards this condition are sex with the p value = 0.000, batch with the p value = 0.016, and sitting duration with the p value = 0.002.

Abstract

Dyspepsia is one of the digestive tract disorders that is often complained of and suffered by the community. According to WHO, the prevalence of dyspepsia in the world is 13-40% every year, while in Indonesia it reaches 40-50%. Students tend to have difficulty in adapting to a very dense routine that results in stress and a tendency not to maintain a diet, which then triggers the emergence of symptoms of dyspepsia. While not fatal, dyspepsia in students can have a negative impact on study habits, ability to concentrate, and performance on exams. This study was conducted to determine the correlation between dietary habits and stress levels on the incidence of dyspepsia. This study is a descriptive study with a cross-sectional analytics approach with a total sample of 268. The analysis techniques used in this research were univariate, bivariate, and multivariate analysis. The results of this study showed that 75.4% of respondents experienced symptoms of dyspepsia. It was also found from the bivariate analysis that there was a significant correlation between dietary habit and stress levels on the incidence of dyspepsia (p<0.005) and from multivariate analysis there was a significant partial effect between stress levels and dyspepsia (p<0.005). It can be concluded that diet and stress levels are associated with the appearance of dyspepsia symptoms and can be considered risk factors for dyspepsia. However, only stress level had an independent effect on the incidence of dyspepsia. It is hoped that the results of this study can be a reference for further research on risk factors for dyspepsia..

Abstract

Nyeri pergelangan tangan memiliki prevalensi yang cukup tinggi baik di Indonesia maupun luar negeri. Nyeri pergelangan tangan dapat disebabkan oleh penyebab traumatis dan non traumatis. Nyeri pergelangan tangan yang disebabkan oleh trauma fisik salah satunya adalah akibat aktivitas mengendarai sepeda motor yang turut dialami pula pada siswa sekolah menengah atas. Penelitian ini dilakukan untuk mengetahui prevalensi nyeri pergelangan tangan pada siswa pengendara sepeda motor di SMA Negeri 11 Denpasar tahun 2022. Penelitian ini menggunakan desain potong lintang deskriptif. Metode pengambilan sampling adalah purposive sampling. Subyek penelitian adalah 315 siswa SMA Negeri 11 Denpasar yang memenuhi kriteria inklusi yaitu siswa yang mengendarai sepeda motor dan bersedia menjadi subyek penelitian serta kriteria eksklusi berupa siswa yang mengalami infeksi atau trauma serta pernah dilakukan operasi pada pergelangan tangannya. Data primer diambil dari kuesioner dan pengukuran langsung oleh peneliti. Berdasarkan hasil penelitian, prevalensi nyeri pergelangan tangan pada siswa pengendara sepeda motor di SMA Negeri 11 Denpasar pada tahun 2022 adalah 33,0%. Berdasarkan faktor risiko yaitu lama pengalaman berkendara, durasi berkendara per hari, jarak berkendara per hari, indeks massa tubuh, dan jenis kelamin, proporsi nyeri pergelangan tangan lebih banyak terjadi pada siswa yang mengendarai sepeda motor lebih dari 3 tahun (34,1%), siswa yang berkendara lebih dari 4 jam per hari (50,0%), siswa yang berkendara lebih dari 5 kilometer per hari (40,5%), siswa yang mengalami kekurangan berat badan tingkat berat (46,3%), dan siswa perempuan (49,0%). Diharapkan penelitian ini dapat digunakan lebih lanjut untuk meneliti hubungan antar faktor risiko. Kata kunci : prevalensi, nyeri pergelangan tangan, pengendara sepeda motor, siswa SMA
