Abstract

Heat shock proteins-90 (HSP-90) is a protein that plays an important role in the life cycle of normal and cancer cells for their self protection from thermal stress, oxidative damage, and cell hypoxia. Inhibition of HSP90 is one way to suppress the growth of cancer cells. In this study, pharmacophore modeling and molecular docking were conducted to identify hit compounds as inhibitors of HSP-90. The pharmacophore feature consists of three hydrogen bond acceptors, one hydrogen bond donor and one hydrophobic feature with Area Under Curve of Receiver Operating Characteristics (AUCROC) is 0.5 and the Goodness of Hit (GH) value is 0.752. Screening in the ZINC database generated 1,500 hit compounds, were subjected to molecular docking to determine their binding energy and interactions with HSP-90. The range of binding energy (E) of all hit compounds is -5.68 to -12.24 kcal/mol and there are four best hit compounds namely lig_543, lig_527, lig_1337 and lig_337, when compared to native ligands (PU2, E=-8.25 kkal/mol) based on the binding energy and orientation, which indicate their potential as new HSP-90 inhibitors.

Abstract

Sacha inchi (Plukenetia volubilis L.) of the family Euphorbiaceae is native to Peruvian in the Amazon forest. SaIn seeds are known to contain a significant fatty acid such as linolenic acid (Omega-3), linoleic acid (Omega-6), and oleic acid (Omega-9). Several studies have shown the benefits of Sacha inchi (SaIn) for various pharmacological activities based on its compound content. The aim of this review article is to provide information about pharmacological activities and describe the physicochemical characteristics of SaIn seed oil. The method used to make this article review is a literature study gathered from national and international journals published in the last ten years through Google Scholar, NCBI-Pubmed, and Crossref with search keys in the form "Plukenetia volubilis L.", "Sacha inchi", "pharmacological activity of Sacha inchi", "physicochemical characteristics of Sacha inchi". Based on the literature review, it was found that the physicochemical characteristics of SaIn seed oil with the observed parameters of peroxide value, iodine number, saponification number and acid number showed different values depending on the extraction method used. SaIn has pharmacological activities as antioxidant, anticancer, antidiabetic, antiobesity, anticonvulsant, anti-aging, antihyperlipidemia, antibacterial and potential for alzheimer therapy.

Abstract

Pneumonia is one of the leading causes of morbidity and mortality worldwide. The irrational use of antibiotics in pneumonia management can reduce therapeutic effectiveness and increase the risk of antimicrobial resistance. This study aimed to evaluate the rationality of antibiotic use in pneumonia patients at Hospital X using the Gyssens method. This research employed a descriptive observational design with a retrospective approach, utilizing medical record data of adult inpatients diagnosed with pneumonia during the period of January–December 2024. A total of 86 patient records meeting the inclusion criteria were analyzed. Evaluation was conducted using the Gyssens method by comparing the appropriateness of therapy with the Guidelines for the Management of Adult Pneumonia issued by the Indonesian Ministry of Health (Regulation No. 28 of 2021) and the Infectious Diseases Society of America/American Thoracic Society (IDSA/ATS, 2023) guidelines. The results showed that the most frequently used antibiotics were Ceftriaxone (34.88%), Azithromycin (32.56%), and Levofloxacin (32.56%). The most common duration of therapy was five days (53.9%). Based on the Gyssens categories, rational antibiotic use (category 0) was found in 57 cases (66.28%), while the remaining cases were categorized as I (6.98%), IIa (11.63%), IIIa (9.30%), and IIIb (5.81%). These findings indicate that most antibiotic use at Hospital X was rational; however, continuous monitoring and periodic evaluation through the Antimicrobial Resistance Control Program (PPRA) are still necessary to optimize therapeutic effectiveness and prevent antibiotic resistance.
