Abstract

Every year number incident wound increasingly increased to become wrong one problem biggest if knowledge about care wound limited along with it care about wound . Introduction characteristics wound comprising of 13 BJWAT items and maintenance that is type dressinf and cleansing could help nurse for increase his knowledge related handling wound .Purpose: To knowing picture characteristics wound and maintenance in Room Dr. Wound Hospital Polyclinic Wahidin Sudirohusodo Makassar.Method: design research quantitative with design Cohort studies retrospect tif. The sampling technique used namely consecutive sampling with a total sample as many as 43 respondents . Data collection is done based on secondary data that is note record medical patient on visit first and last in Room Dr. Wound Hospital Polyclinic Wahidin Sudirohusodo Makassar.Result: From results research found that mostly patients who visit to clinic care wound is man as much as 72.09% (31 people), with type wound mostly is wound chronic . Results research on visit early with visit end no too significant changes that occur . Type dressings used mostly is Anti microbial as Primary dressing 37.2% (16 people ) , Natural fiber dressing as dressing secondary 81.4% (35 people ), and hypafix as dressing tert ier with amount 62.8 % ( 27 people ).Conclusions and Suggestions: characteristics wound very important for in value to get is known condition wound increasingly getting better or even increasingly worsening . By because that important for a nurse understand way rate characteristics wound and care wound .

Abstract

Pendahuluan: Penggunaan bahan alam sebagai penyembuhan luka merupakan alternatif yang banyak digunakan. Ekstrak Buah Naga Merah (EBNM) memiliki kandungan antioksidan dan anti-inflamasi yang dibutuhkan untuk mempercepat penyembuhan luka. Namun penelitian yang berhubungan dengan efek buah naga merah terhadap penyembuhan luka masih sangat terbatas. Tujuan penelitian ini adalah untuk mengetahui efek Ekstrak Buah Naga Merah terhadap penyembuhan luka DM dan luka Non DM pada wistar. Metode: Jenis penelitian ini adalah penelitian eksperimen dengan desain post test with control group design. Wistar jantan albino (n=30) berat badan (250-350 gr) dibagi menjadi 2 kelompok (kelompok A wistar DM + Eksisi dan kelompok B wistar Non-DM + Eksisi). Selanjutnya Wistar dilukai pada punggung kiri dan kanan dengan menggunakan punch biopsy 8 mm. Topikal krim EBNM 3.5%, 5% dan 7.5% dioleskan pada permukaan luka dengan menggunakan cotton bud. Jaringan diwarnai dengan Hematoksilin dan Eosin (HE) untuk mengetahui ketebalan granulasi, dan epitelisasi jaringan. Data dianalisis dengan menggunakan uji independent T test dan paired T test (SPSS 21, Chicago Inc.). Abstract

Background: Neuropathy and peripheral vascular disease are two major factors that lead to diabetic wounds. Additionally, diabetic wound healing process takes a long time. Collagen is one indicator to assess wound healing. Aim: This study aimed to compare the wound healing process DM and non-DM In Wistar with acute wounds modeling that uses the density collagen as indicators. Methods: This study used a randomized design Post Test Control Group who uses Wistar as research objects. Wistar rats were divided into 2 groups: Wistar DM and Non-DM groups. The total sample were 30 individuals that consisted of 15 rats each group. Wistar DM was induced by injection of Streptozotocin (STZ) intraperitoneal single dose of 40-50 mg / kg. The wound were created by using punch biopsy of each group. The wound were followed up for 14 days. Data analysis was done using the Kolmogorov - Smirnov test with a confidence level of 95%. All data were analyzed by SPSS version 21.0 software (SPSS, Inc. Chicago, IL). Results: The density of collagen in DM group (20%) was lower than in non DM group (40%). However there were no significant difference in collagen density in both groups (p=0,375). Nevertheless, the percentage of collagen density is higher in non DM group compared with DM group (20.0%). This shows the tendency of the wound healing process in the group of Non - DM is better than DM group. This shows the tendency of the wound healing process in the group of Non - DM is better than DM group. Conclusion : There is a tendency that the wound healing in non ?DM group is better than in DM groups. More studies are needed in the future to confirm this findings. Abstract

Latar belakang : Luka kaki diabetik merupakan salah satu komplikasi yang ditimbulkan oleh penyakit Diabetes Melitus. Proses penyembuhannya akan menjadi rumit jika tidak dilakukan manajemen perawatan dengan baik. Salah satunya adalah dengan melakukan penilaian atau pengkajian terstandar dalam pengelolaan luka kaki diabetik. Beberapa penilaian luka kaki diabetik dilakukan dengan menggunakan sistem klasifikasi dan penggunaan instrumen pengkajian luka. Tujuan dari studi literatur ini adalah untuk mengidentifikasi pengkajian yang dapat digunakan dalam menilai luka kaki diabetik. Metode : Data base yang digunakan dalam pembuatan literatur review ini adalah Pubmed, Science Direct, Google Scholar dan Cochrane. Hasil : Terdapat 47 artikel yang diidentifikasi dan dipublikasikan dari tahun 2010-2018. Dari 47 artikel 7 artikel yang memenuhi kriteria inklusi. Beberapa hasil penelitian menunjukkan penilaian luka kaki diabetik baik dengan menggunakan sistem klasifikasi maupun menggunakan instrumen pengkajian luka kaki diabetik. Diskusi : Pengkajian merupakan salah satu komponen penting dalam proses keperawatan. Selain itu pengkajian merupakan salah satu komponen manajemen luka kaki diabetik. Dengan adanya penilaian atau pengkajian dapat membantu praktisi dan peneliti dalam mengkaji dan mengelola kesehatan pasien serta melakukan pengembangan dan penilaian terapi yang baru. Kesimpulan : Pengkajian merupakan kegiatan pertama dan fase penting dalam proses keperawatan yang akan mempengaruhi proses keperawatan yang lain yang dapat digunakan sebagai dasar dalam membuat pertimbangan klinis dan rencana tindakan selanjutnya.

Abstract

ABSTRAK Latar belakang : Komplikasi penyakit Diabetes mellitus (DM) yang kronis dan serius adalah luka kaki diabetes (LKD) serta merupakan penyebab utama terjadinya amputasi tungkai bawah. Pada dasarnya hal tersebut dapat dicegah dengan manajemen luka yang optimal. Salah satu komponen dasar dari manajemen luka adalah pencucian luka. terdapat banyak solusi yang tersedia untuk penccucian luka salah satunya adalah sabun antiseptik. Tujuan dari studi literatur ini adalah untuk mengetahui efek sabun antiseptik terhadap penurunan kolonisasi bakteri. Metode : Studi literatur ini dilakukan pada database PubMed, ScienceDirect dan Cochrane Hasil : Peneliti mengidentifikasi 156 artikel yang dipublikasikan dari tahun 2012 ? 2017. Dari 156 artikel didapatkan 11 artikel yang memenuhi kriteria dan hasil penelitian menunjukkan bahwa sabun antiseptik memiliki efek mikrobicida yang kuat sehingga mampu menghambat pertumbuhan mikroorganisme pathogen dan efektif menurunkan jumlah koloni bakteri. Kesimpulan : sabun antiseptik efektif mengurangi jumlah koloni bakteri dibandingkan dengan sabun biasa dan memiliki daya hambat yang paling kuat terhadap pertumbuhan bakteri sthapylococcus aureus dan eschericia coli.

Abstract

Every wound in diabetics has the potential to experience a setback in healing into a chronic wound so that the wound also has the potential to form into biofilm. One of the modern dressings commonly used to suppress the growth of biofilm is Cadexomer Iodine Powder. However, there is still limited research that addresses directly related to the effectivity of Cadexomer Iodine Powder towards wound healing. Therefore, this report aims to evaluate effectiveness Cadexomer Iodine Powder on wound healing with biofilm problems at Griya Afiat Makassar's wound care clinic. The study aimed to identify the effectiveness of Cadexomer Iodine Powder in minimizing biofilms in diabetic foot ulcer (DFU) patients. This study was conducted on one patient who was followed prospectively for 32 days from 10 July to 10 August 2019 with 10 treatments. The dressing used for wound care is Cadexomer Iodine Powder. To evaluate the reduction in the number of biofilms performed by the expert wound when treatment continues. Meanwhile, to evaluate the wound healing process, the modified Bates-Jensen Wound Assessment was used. This report shows a decrease in biofilm from the first treatment to the last treatment with the number of biofilms in the first treatment (+++) to a few (+) in the 10th treatment. The treatment also took a short period of time which is 32 days with a wound condition at the beginning of treatment which is 30% of slough and 70% of granulation experiencing a very swift healing in the 10th treatment to 40% of epithelium and 60% of granulation. Cadexomer Iodine Powder is very effective in the process of wound treatment which can be seen through the reduction of biofilms. In addition, wound healing is much better with shorter treatment time as a proof.

Abstract

Treatments of diabetic ulcer are very essential to prevent further ulceration damage. Insufficient treatment of the wound can consequence in an infection that will have an impact upon the amputation. One of the antimicrobial wound dressing in the treatment of ulcus diabetic is chitosan. Therefore, this report aims to assess the effectivity of the usage of chitosan gel as a primary dressing for epithelial progression in diabetic ulcer at the wound care clinic of ETN Center, Makassar. Methods, this study was conducted on one patient, a female, 38 years old with random blood glucose 239 mg/dl. The wound occurred in April 2019, the wound has been experienced about three months and then followed prospectively starting 8th July until 1st August 2019 with 8 treatments. Treating the wound uses chitosan gel as a primary dressing and looks at the development process of the wound, especially the progress of the epithelium using the modified Bates-Jansen Wound Assessment Tool. Results, the comparison of treatment results using chitosan in the first and eighth treatments (25 days) respectively slough 10% and 5%, granulation 75% and 60%, and epithelialization 15% and 35%. While the size of the wound 41 x 17 cm to 32 x 13 cm. Conclusion, chitosan gel as a primary dressing plays an important role in helping the wound healing process as indicated by an increase in epithelium and reduction in wound size.

Abstract

The use of old modern dressings cost a lot, so wound practitioners use alternative dressings (sanitary napkins). Identify the use of alternative dressings for chlorine residues and the process of healing diabetic foot injury (LKD). Quasi experimental research design with one group pre and post approach, 10 samples were intervened with alternative pads in the form of sanitary napkins used after wound care. Sanitary pads are used on one wound for a type of dressing based on the area of ​​the wound. Intervention for 12 days with dressing changes per 2 days. Examination of residual chlorine (Spectrophotometry UV-Vis) based on the wound with swab techniques on days 1, 6 and 12, while the healing process with a standard instrument that is Diabetis Foot Ulcer Assessment Scale (DFUAS). Chlorine residues of days 1, 6 and 12 were 0.6837, 2.4477 and 2.7927 ppm with p = 0.001. The average DFUAS scores of days 1, 6 and 12 are 29.6, 27.7 and 24.6 with p = 0.005. Thus a significant increase in residual chlorine at the wound bed but DFUAS scores continued to decline. An increase in chlorine residue on days 6 and 12 of treatment using alternative dressings of sanitary napkins and DFUAS scores decreased. It is recommended for wound practitioners to evaluate Diabetic Foot Injuries when using alternative dressings (sanitary napkins).

Abstract

Stroke dan penyakit gangguan fungsi neurologis lainnya akan mengakibatkan berbagai macam gangguan fungsi tubuh seperti gangguan fungsi kognitif, sirkulasi, kekuatan otot, fungsi perifer, fisiologis yang akan berpengaruh pada sistem sensorik dan motorik penderitanya. Hal ini membuat pasien mengalami immobilisasi. Immobilisasi yang lama akan menimbulkan resiko kerusakan pada permukaan tubuh yang tertekan yang disebut dengan dekubitus terutama pada daerah-daerah yang menonjol. Oleh karena dilakukan pemberian tindakan reposisi dan massage untuk menurunkan derajat dekubitus pasien imobilisasi dengan gangguan neurologis. Penelitian ini merupakan penerapan Evidence Based Nursing Practice (EBNP) dengan pendekatan studi kasus yang melihat perubahan pada luka dekubitus yang terjadi setiap hari. Populasi dalam penerapan EBNP ini adalah seluruh pasien yang dirawat di Ruang perawatan Lontara 3 Neuro RS dr. Wahidin Sudirohusodo Makassar. Teknik sampling yang digunakan adalah purposive sampling, sampel ditentukan berdasarkan Number Needed to Treat (NNT) dari jurnal utama yang digunakan sebagai pedoman penerapan EBNP. Hasil implementasi EBNP ditemukan bahwa pemberian reposisi dan massage selama 7 hari dapat menurunkan ukuran diameter luka, memberikan perubahan warna mendekati warna kulit di sekitar, lebih cepat menurunkan derajat luka pada dekubitus derajat 1, dan lebih cepat menurunkan derajat dekubitus pada bagian Heel. Disimpulkan bahwa reposisi dan massage efektif dalam menurunkan dekubitus derajat 1 dan 2 pada pasien imobilisasi dengan gangguan neurologis di Ruang Lontara 3 Neuro RS dr. Wahidin Sudirohusodo Makassar. Sehingga perawat sebaiknya melakukan screening dekubitus dan jika ditemukan tanda-tanda dekubitus derajat 1 dan 2 sebaiknya dilakukan kombinasi reposisi tiap 2 jam dan massage 2 kali sehari minimal selama 7 hari. Stroke and other neurological dysfunction diseases will result in various kinds of bodily disorders such as cognitive impairment, circulation, muscle strength, peripheral function, physiological effects that will affect the sensory and motor systems. This makes the patient immobilized. Prolonged immobilization will pose a risk of damage to depressed body surfaces called pressure ulcers, especially on prominent areas. Therefore, the administration of repositioning and massage measures to reduce the degree of pressure ulcers of immobilized patients with neurological disorders. This research is the application of Evidence Based Nursing Practice (EBNP) with a case study approach that looks at changes in pressure ulcers that occur every day. The population in the application of EBNP is all patients treated in the Lontara 3 room Neuro at Hospital Dr. Wahidin Sudirohusodo Makassar. The sampling technique used is purposive sampling, the sample is determined based on the Number Needed to Treat (NNT) from the main journal that is used as a guideline for the implementation of EBNP. The results of the EBNP implementation found that the administration of repositioning and massage for 7 days can reduce the size of the wound diameter, provide color changes close to the color of the surrounding skin, more quickly reduce the degree of injury in pressure ulcerss 1 degree, and more quickly reduce the degree of pressure ulcers on the heel. It was concluded that repositioning and massage are effective in reducing pressure ulcers 1 and 2 in immobilized patients with neurological disorders in the Lontara 3 room Neuro Hospital Dr. Wahidin Sudirohusodo Makassar. So that nurses should do dekubituss and if there are signs of pressure ulcers 1 and 2 should be a combination of repositioning every 2 hours and massage twice a day for at least 7 days.
