Abstract

This study aims to analyze the adaptation strategies of Ananda Sehat Karangsono Clinic in facing the competition in healthcare services in the era of the National Health Insurance (JKN). The background of this research is based on the increasingly fierce competition between healthcare facilities, both clinics and hospitals, in providing fast, efficient, and quality services according to JKN standards. The presence of the JKN program, which demands integrated, transparent, and technology-based services, encourages clinics to innovate to remain competitive and maintain operational sustainability. The research method used is descriptive qualitative with data collection techniques through in-depth interviews, participatory observation, and documentation studies. Research informants included clinic leaders, healthcare workers, and administrative staff, thus providing a comprehensive perspective on the implemented adaptation strategies. The results show that Ananda Sehat Karangsono Clinic has integrated digital technologies, such as the Mobile JKN application and WhatsApp, to speed up the registration process, verify patient data, and facilitate communication. This innovation can improve the efficiency of service flows, reduce queues, and create a better service experience for patients. In addition, the clinic emphasizes the importance of patient satisfaction by improving service quality, speed, and friendliness of healthcare workers and support staff. The adaptation strategy implemented was not limited to technological aspects, but also encompassed operational efficiency and humanistic interpersonal services. Overall, the clinic's adaptation strategy was flexible and holistic, encompassing digitalization, strengthening internal management, and developing positive patient relationships. These findings confirm that the clinic was able to maintain competitiveness and service sustainability amidst the dynamics of the National Health Insurance (JKN) and the digitalization of the healthcare system.

Abstract

Penelitian ini menganalisis perbedaan efektivitas biaya dan lama masa rawat antara metode operasi sesar Enhanced Recovery After Caesarean Surgery (ERACS) dan konvensional (non-ERACS) di Rumah Sakit Bhayangkara Brimob Depok. Menggunakan pendekatan kuantitatif komparatif dengan desain retrospektif, data rekam medis pasien operasi sesar tahun 2023 dikumpulkan. Analisis Independent Sample t-Test dan Chi-square digunakan untuk menguji perbedaan biaya dan lama rawat. Hasil menunjukkan perbedaan signifikan pada total biaya perawatan (p=0.020), dengan ERACS rata-rata Rp 10.089.544 dan non-ERACS Rp 9.706.049. Komponen biaya seperti akomodasi (p=0.033), obat dan BMHP (p=0.000), visite (p=0.021), serta pemeriksaan penunjang (p=0.000) menunjukkan perbedaan signifikan, meskipun biaya tindakan medis dan operasi tidak berbeda signifikan. Lama masa rawat juga berbeda signifikan (p=0.000), di mana pasien ERACS pulih lebih cepat (45.7% keluar <3 hari) dibandingkan non-ERACS. ERACS terbukti lebih efektif dalam efisiensi klinis dan durasi rawat, menjadikannya alternatif unggul dalam pelayanan obstetri berbasis efisiensi dan mutu.

Abstract

This study is a systematic literature review of 25 studies discussing hospital management in Indonesia during the National Health Insurance (JKN) era and collaboration with the Social Security Administering Body (BPJS) Health. The focus of the review includes financial management, human resource management (HRM), business strategies, service quality, and claim issues. The synthesis of results shows that in the financial aspect, cost efficiency strategies, receivables optimization, and service diversification contribute to increased revenue and cash flow stability; however, discrepancies in INA-CBGs tariffs increase the risk of bankruptcy, especially for small hospitals. In the HRM aspect, continuous training, transformational leadership, and adaptation to BPJS regulations have proven to reduce patient complaints and improve employee performance. The implementation of JKN has increased patient volume and service accessibility but has pressured profit margins and revenue per patient. Effective business strategies include service diversification, innovation, process digitalization, and strengthening partnerships with BPJS. The main challenges lie in claim issues due to incomplete documentation, slow verification, and differences in diagnosis codes, which negatively affect hospital cash flow. This review confirms that the integration of financial and HRM management, supported by integrated information technology, is key to the sustainability of hospital operations in the JKN era.

Abstract

Hospitals as healthcare institutions, are required to deliver high-quality, efficient, and sustainable services through proper strategic management. Dr. H. Yuliddin Away Regional General Hospital, a type B referral hospital in Aceh, faces challenges such as limited resources, increasing demands for service quality, and regulatory changes in the healthcare sector. This study aims to analyze the implementation of the BSC (Balanced Scorecard) (BSC) as a strategic management tool to improve hospital performance. The BSC is employed because it integrates financial and non-financial indicators through four perspectives: financial, customer, internal business processes, and learning and growth. This research applies a descriptive qualitative approach using a case study at Dr. H. Yuliddin Away Hospital. Data were collected through interviews, observations, and documentation, and analyzed using content analysis. The findings show that the hospital has adopted the BSC since 2021 with key performance indicators aligned to its vision and mission. From the financial perspective, the operational cost ratio achieved the target (65% against ≤70%). The customer and internal process perspectives did not fully meet the targets, as patient satisfaction (4.1 vs. ≥4.2) and average service time (62 minutes vs. ≤60 minutes) revealed small gaps. Conversely, the learning and growth perspective exceeded expectations with six staff training sessions per year. Overall, the implementation of BSC has had a positive impact on transparency, accountability, and interdepartmental coordination, while also fostering service innovation. However, challenges remain, including limited managerial understanding, budget constraints, and organizational resistance to change. The study concludes that BSC is an effective tool for improving hospital performance, though further efforts are needed to enhance patient satisfaction and service efficiency.

Abstract

The management of primary clinics, as providers of both primary and secondary healthcare services, faces challenges in achieving operational efficiency, inter-unit coordination, and service quality. This article aims to analyze the implementation of organizational theory in managing primary clinics using a multidimensional approach, encompassing classical, neoclassical, and modern theories. The study examines how classical theory contributes to the establishment of structure and work efficiency, how neoclassical theory enhances interpersonal relationships and employee motivation, and how modern theory, through a systems-based approach, supports the integration of technology and management innovation. This article offers practical recommendations for optimizing primary clinic management through the holistic application of organizational theory.

Abstract

Telemedicine is a breakthrough in health services that allows interaction between medical personnel and doctors without meeting in person. The presence of telemedicine in health services is very beneficial during the COVID-19 pandemic when the physical distancing protocol is implemented. Telemedicine in the regulations set by the Indonesian government allows the implementation of telemedicine services that include consultation, diagnosis, therapy, and even monitoring. Telemedicine has the potential to be a solution to problems in Indonesia as a country with an uneven distribution of medical personnel and a doctor-to-community ratio that is still insufficient, so it is considered a good strategy for health services. The purpose of this study was to determine the benefits of telemedicine as a health service strategy during the pandemic and post-pandemic through a literature review. This study is a literature review obtained from the digital databases PubMed, Google Scholar, and Neliti with the keywords "telemedicine", "telemedicine", "e-health", "teleconsultation", "teleconsultation", "telehealth", and "Indonesia" which are adjusted to the inclusion and exclusion criteria. Based on the results of the study, 10 research articles from 2014-2024 were included. Telemedicine is a health service solution during the COVID-19 pandemic. In the post-pandemic era, telemedicine can still be an option for health services because it is still considered to have meaningful benefits

Abstract

Kesehatan merupakan aspek yang sangat penting dalam kehidupan manusia dan merupakan hak mendasar manusia. Hak tersebut diwujudkan dalam bentuk penyediaan fasilitas kesehatan. Klinik dapat dikembangkan menjadi rumah sakit dengan menggunakan beberapa rancangan strategis yang mencakup beberapa hal, seperti : kerjasama, pembangunan, modal, sumber daya manusia, organisasi, promosi, program, inovasi, dan birokrasi. Rancangan tersebut dapat menjadi panduan bagi klinik dalam menentukan apakah dapat dikembangkan menjadi rumah sakit. Literature review ini bertujuan untuk mengetahui dasar - dasar pertimbangan dalam penentuan pengembangan klinik menjadi rumah sakit. Studi ini dilakukan dengan metode literature review dan mengikutsertakan publikasi dari tahun 2020 - 2024. Pencarian literatur dilakukan menggunakan Google Scolar. Artikel ini menganalisa 5 studi yang memenuhi kriteria inklusi yang telah ditetapkan. Hasil Penelitian ini adalah terdapat beberapa strategi yang dapat dilakukan dalam pengembangan klinik menjadi rumah sakit, salah satunya yaitu product development, yaitu melakukan pengembangan produk dengan cara melakukan penambahan ruang perawatan, seperti kamar operasi, ruang icu dan ruang picu/nicu, melakukan penambahan peralatan medis, seperti inkubator untuk bayi dan ventilator, dan melakukan pengembangan pelayanan khusus, contohnya seperti pelayanan fertilitas. Selain itu juga terdapat berbagai pertimbangan yang dijadikan dasar dalam pengembangan klinik menjadi rumah sakit, antara lain kesulitan dalam mengakses pelayanan kesehatan tingkat lanjut, upaya pemerintah dan legislatif untuk meningkatkan kesejahteraan penduduk, kondisi demografi dan ekonomi penduduk, serta kemampuan sumber daya manusia dan keuangan. Klinik harus memperhatikan faktor internal dan eksternal dan saat hendak melakukan pengembangan menjadi rumah sakit. Kesimpulan penelitian ini adalah proses pengembangan klinik menjadi suatu rumah sakit bukanlah suatu proses yang mudah. Dalam proses perubahan ini membutuhkan kesiapan dan rencana strategis yang baik, baik secara internal seperti organisasi, sistem informasi, anggaran, sarana prasarana dan sumber daya manusia yang kompeten, maupun eksternal seperti geografi, demografi, aspek pemerintah, dan aspek pasar

Abstract

Pengelolaan keuangan rumah sakit menjadi krusial dalam memastikan efisiensi operasional dan keberlanjutan layanan kesehatan, terutama di tengah tantangan seperti peningkatan biaya dan kompleksitas regulasi. Penelitian ini bertujuan mengkaji strategi pengelolaan keuangan rumah sakit yang dapat meningkatkan efisiensi berdasarkan tinjauan literatur lima tahun terakhir (2019–2024). Metode systematic literature review dengan pendekatan PRISMA digunakan untuk menyaring dan menganalisis berbagai literature yang relevan. Hasil kajian menunjukkan bahwa penerapan system informasi manajemen keuangan, pengelolaan berbasis data, serta model pembayaran berbasis nilai berkontribusi positif terhadap efisiensi keuangan. Inovasi lain seperti digitalisasi proses keuangan dan manajemen risiko keuangan juga memainkan peran penting dalam meningkatkan stabilitas keuangan rumah sakit. Penelitian ini menyoroti pentingnya strategi pengelolaan keuangan yang tepat untuk menghadapi tantangan finansial di masa mendatang, dan diharapkan dapat menjadi panduan bagi manajemen rumah sakit dalam merancang kebijakan keuangan yang lebih efisien.

Abstract

Penelitian ini bertujuan untuk menganalisis perkembangan inovasi dalam manajemen sumber daya manusia (SDM) di rumah sakit dalam kurun waktu 2019-2024, dengan fokus pada peran kepemimpinan dan pengembangan karyawan. Metode penelitian yang digunakan adalah analisis literatur sistematis dengan pendekatan kualitatif. Temuan menunjukkan bahwa rumah sakit telah mengadopsi berbagai inisiatif inovatif, seperti pemanfaatan teknologi canggih, program pelatihan berbasis kompetensi, dan transformasi budaya organisasi. Kepemimpinan transformasional dan program pengembangan karyawan yang komprehensif terbukti menjadi faktor kunci yang mendorong inovasi manajemen SDM. Selain itu, penelitian ini juga mengidentifikasi faktor-faktor keberhasilan implementasi, seperti dukungan kepemimpinan, keterlibatan karyawan, dan budaya organisasi yang kondusif. Hasil studi ini memberikan implikasi teoritis dan praktis bagi akademisi serta manajer SDM dan pembuat kebijakan di sektor kesehatan.

Abstract

Research Objectives: This study aims to analyze the implementation of strategic management in enhancing hospital competitiveness in the digital era, identify critical success factors, and explore the challenges faced by hospitals in their digital transformation process. Research Methods: The study employs a mixed-method approach, combining quantitative methods through surveys of 200 middle and senior managers from 50 hospitals in Indonesia, and qualitative methods through in-depth interviews with 20 hospital executives. Data analysis involves structural equation modeling (SEM) for quantitative data and thematic analysis for qualitative data. Research Results: Effective implementation of digital strategic management has been shown to improve hospital competitiveness through increased service innovation, improved quality of care, and enhanced patient satisfaction. Critical success factors include strong top management support, technological readiness, and a culture of innovation. Key challenges include resistance to change, budget constraints, and digital competency gaps. The study proposes a "Dynamic Digital Capability Framework for Hospitals" (DDCFH) as an adaptive model for digital transformation in healthcare settings. Conclusion: Successful digital transformation in hospitals requires a holistic approach that aligns digital strategies with organizational capabilities and environmental demands. The study emphasizes the importance of digital leadership, change management, and strategic partnerships in navigating the complexities of digital transformation in the healthcare sector.
