Abstract

Stroke is a disease with high cost, high volume, and high risk. The burden of these costs will increase if you experience complications that often occur in sufferers.  This study aimed to identify a clinical pathway model to prevent complications and improve the quality of life of stroke patients. This study is a quantitative descriptive study with crossectional, with 95 samples of stroke patients who have been treated for at least 1 day, can communicate 2 ways at Hospital dr. Chasbullah Abdulmadjid Bekasi City, and who have studied ethical feasibility. Participants must fill out informed consent. Data were collected using questionnaires and analyzed using univariate analysis. The results showed that the most participants were aged between 45 – 59 years (43.2%), male (61.1%). Hypertension, disease before stroke (71.9%), and 2 attacks (10.5%). (51.6%) were unaware of stroke with symptoms of muscle weakness (87.4%),  (45.3%) difficulty speaking, and (17.2%) blurred vision. (46.9%) complications of paralysis. Activity daily living eating and drinking (55.8%), (58.95) personal hygiene, (62.1%) elimination, (68.4%) ambulation, (65.3%) mobilization, and (54.7%) rest/sleep should be assisted. Treatment was carried out by taking medication (89.5%) and control to health facilities (75.8%), but (75.8%) did not carry out rehabilitation and exercise (54.7%). There are still many participants who do not know about stroke well, which affects their independence in carrying out activities. Therefore, a clinical pathway model is needed that is used as a reference in providing information and education to stroke patients, to prevent complications and improve the quality of stroke patients.

Abstract

Stroke is one of the neurological diseases that often requires complex management and treatment. Making the right decisions in diagnosing and treating stroke patients can have a significant impact on the recovery process and the quality of life for patients. To assist healthcare professionals and patients in making more informed decisions, this research has developed the "StrokeCare Navigator," a web-based Decision Support System (DSS) that utilizes the Decision Tree C4.5 algorithm. This system allows users, including healthcare professionals and patients, to input symptoms and patient medical information, providing an initial diagnosis of the type and severity of the stroke. Additionally, the system offers personalized treatment guidelines, treatment recommendations, and online patient monitoring services. The Decision Tree C4.5 model used in this system was developed through training with relevant patient data. Evaluation results indicate that the system can provide accurate and beneficial recommendations for healthcare professionals and patients. Testing and patient monitoring in clinical practice also demonstrate the potential to enhance stroke patient care. Therefore, StrokeCare Navigator is expected to be a valuable tool in stroke patient management and treatment, ultimately improving the prospects of patient recovery and providing better guidance for healthcare professionals. This research also provides deeper insights into the implementation of the Decision Tree C4.5 algorithm in the context of Decision Support Systems (DSS) in the medical field.

Abstract

Background: Family caregivers play a crucial role in the recovery of stroke patients, especially in community settings. An adequate understanding of the range of motion (ROM) exercises enables caregivers to actively support rehabilitation and prevent complications. This study aimed to analyze the improvement of family caregivers' understanding of ROM exercises after training. Methods: This pre-experimental one-group pretest-posttest study involved 25 families with stroke patients for ≥1 month, selected through consecutive sampling in the working area of Puskesmas Tambora, West Jakarta, in July 2025. Interventions included active and passive ROM exercises based on standard operating procedures (SOPs), also it was used to simulate the manual muscle testing (MMT). Measurement instruments used the validity and reliability questionnaires and MMT observation sheets. Data were analyzed by the Wilcoxon test at a significance level of p<0.05. Results: The results showed a significant increase in family caregiver understanding of ROM (p= 0.000). Before the training, 88% of respondents (22 people) had sufficient knowledge, and 12% (3 people) had good knowledge. After the training, the proportion of good knowledge increased to 56% (14 people), while moderate knowledge decreased to 44% (11 people). Conclusion: Structured training effectively enhanced family caregivers' understanding of ROM exercises, supporting their active participation in patient care. These findings suggest that such education can be integrated into community-based non-digital clinical pathways to prevent complications in stroke patients.

Abstract

Clinical practice is an essential component of nursing education, aimed at developing students’ competencies in real healthcare settings. The role of clinical nurse mentors is a key external factor influencing the success of students in achieving clinical competence. This study aimed to analyze the correlation between nursing students’ perceptions of nurse mentoring and their clinical competence achievement. This type of research is quantitative correlative analytic with a cross-sectional research design, namely by measuring variables at a certain time. The sample consisted of 82 nursing students from University A who had completed their clinical practice, selected through purposive sampling. Research instruments included a questionnaire on students’ perceptions of nurse mentoring and a clinical competence achievement questionnaire. Data analysis was performed using the Spearman correlation test. The results showed that most students had a positive perception of nurse mentoring (86.6%) and achieved clinical competence in the "good" category (93.9%). A significant relationship was found between students’ perceptions of nurse mentoring and their clinical competence (r = 0.698; p < 0.05), with the evaluation domain showing the strongest correlation (r = 0.650). These findings suggest that the more positive students’ perceptions of their clinical mentors, the higher their level of clinical competence. Associating the role of clinical mentors through training and ongoing evaluation is essential to support effective learning outcomes in nursing clinical education.

Abstract

Background: The most common complication after surgery, or postoperative nausea and vomiting (PONV), is nausea and vomiting. Treatment can be provided either pharmacologically with medications or non-pharmacologically through lifestyle modifications, physical activity, and simple therapies to address complaints arising from postoperative side effects. One non-pharmacological treatment that can be provided is acupressure therapy. For patients prone to nausea and vomiting, acupressure therapy, applied by pressing the PC6 and ST36 points, can be used as an alternative to manage PONV. Purpose: To provide knowledge about the application of acupressure therapy to reduce nausea and vomiting in patients who have undergone laparotomy surgery. Method: This activity was conducted in January 2026 at the Iman Sudjudi Pavilion, Gatot Soebroto Army Central Hospital. This qualitative research, using a case study approach, involved nursing care in the form of acupressure for postoperative patients experiencing nausea and vomiting (PONV). The intervention was given in the form of acupressure therapy at the Pericardium point (PC6) located on the inner wrist and the Stomach point (ST36) located below the knee after 2 hours of surgery. Measurements in the assessment used the Rhodes Index of Nausea, Vomiting and Retching (RINVR) which was given before the intervention (pre-test) and 7 hours after the intervention (post-test). Descriptive analysis was conducted to evaluate the effectiveness of the intervention and changes in the level of nausea and vomiting after acupressure therapy. Results: Measurement data using the RINVR instrument showed a decrease in scores from 7 before the intervention to 1 after. Subjective evaluations indicated a decrease in nausea, no vomiting, and an improved appetite. Objectively, respondents were able to correctly perform acupressure at points PC6 and ST36. Conclusion: The application of acupressure therapy at points PC6 and ST36 was highly effective in controlling nausea and reducing the incidence of vomiting in post-laparotomy patients. Acupressure therapy is an alternative solution as an effective non-pharmacological method for post-laparotomy patients, accelerating the recovery of bodily functions, preventing complications, and improving the patient's quality of life. Suggestion: Healthcare workers are encouraged to implement non-pharmacological therapies for patients, particularly post-laparotomy patients, as a complementary approach, and to educate the public about acupressure therapy for managing pain, nausea, and vomiting after laparotomy surgery. Keywords: Acupressure therapy; Health promotion; Nausea and vomiting; Post-laparotomy surgery Pendahuluan: Komplikasi yang paling umum setelah operasi atau postoperative nausea and vomiting (PONV) adalah mual muntah.
