Abstract

Background: Stroke is the third most common disease after heart disease and cancer and a leading cause of disability worldwide. The most common type of stroke is non-hemorrhagic stroke (SNH) or Acute Ischemic Stroke, which accounts for about 85-87% of all cases. The purpose of this case report is to examine the benefits of citicoline in a patient with SNH. Case Presentation: In this case, a 63-year-old man was admitted to the Emergency Room of the Agung Mulia Inpatient Clinic on February 25, 2023 with complaints of inability to move his right arm and leg since 5 days before entering the clinic. The complaint was felt suddenly when the patient woke up in the morning. The patient also complained of weakness and difficulty speaking and slurred speech accompanied by difficulty swallowing. The voice could still speak but was unclear but the patient understood what was being said to him. Other complaints such as headaches, nausea, vomiting, bowel and urination disorders, and fainting were denied by the patient. The patient was treated at a hospital near his house on February 21, 2023 and underwent a CT scan of the head and was treated as an outpatient. The results of the CT scan on February 21, 2023, axial section of the head without intravenous contrast, obtained the following results: Infraterritorial cerebellum and 4th ventricle appear normal, Supreterritorial hypodense lesions appear on the left lateral periventricular and in the left parietal, No midline shift, Cortical sulci and prominent ventricular system. It was concluded that the left lateral periventricular and left parietal cerebral infarction were in accordance with Non-Hemorrhagic Stroke (SNH). Senile cerebral atrophy. Based on anamnesis, physical examination, and supporting examinations. The clinical diagnosis in this case was right hemiparesis and paresis of nerves VII, IX, and X, et causa non-hemorrhagic stroke (SNH) with electrolyte disorders (Electrolyte Imbalance), given general and specific management. Results: For general management that can be given is semi-fowler position, oxygen administration, administration of antihypertensive drugs and maintaining fluid and nutritional intake. While specific management with pharmacology by administering citicoline and clopidogrel is given to this patient. Conclusion: Citicoline works to prevent brain damage (neuroprotection) and helps the formation of cell membranes in the brain (neurorepair). The benefits of citicoline in this patient are seen in motor and neurological improvements based on NIHSS assessments.

Abstract

Cardiac Arrest is the leading cause of death in the world including Indonesia, which can occur anytime and anywhere. The causes vary, ranging from fatigue, and underlying health conditions, to traffic accidents. Basic Life Support (BLS) is the initial step that can be taken to restore respiratory and circulatory function in individuals experiencing respiratory or cardiac arrest. It is important for online motorcycle taxi drivers to possess these skills, as they often spend time on the road and interact with various people in different situations. Perhimpunan Dokter Spesialis Anesthesiology dan Terapi Intensif (PERDATIN) North Sumatra Branch concluded a series of Kursus Penyegar dan Penambah Ilmu Anestesia (KPPIA) by providing basic life support training to 521 public transport or online motorcycle taxi drivers in the city of Medan on September 29, 2024. Held at the Gedung Serba Guna of the North Sumatra Provincial Government, the participants were divided into 52 groups guided directly by anesthesiologist specialists as instructors. By providing this education and training, they can be better prepared to face emergency situations on the road, thereby enhancing safety and service for passengers

Abstract

Background: Wounds are skin disorders in the form of damage to continuity due to a pathological process, if not treated properly it can cause serious complications such as failure to heal and chronic problems due to untreated wound infections, especially post-operative infections and scar tissue. The next problem is that excessive antibiotics can cause side effects and increase the risk of antimicrobial resistance and delay wound healing. So a new strategy is needed with herbal-based therapy that has the effect of treating infections and does not cause side effects in patients. Subjects and Method: The research was conducted with a post test only randomized control group design to test macroscopic observations of the average size of the wound length, hyperemia and swelling in 30 white Wistar rats (Rattus norvegicus) after receiving ethanol extract of rosella petals (Hibiscus sabdarifa L.). The dependent variables of the study were wound closure time, hyperemia and wound swelling. The independent variable of the research was varying doses of ethanol extract of rosella flower petals (Hibiscus sabdarifa L.) 6%, 12% and 15%. The parameters assessed are the length of the wound and the macroscopic assessment of the wound. Data were analyzed using the Kruskal-Wallis test, p<0.05 was considered statistically significant. Results: Administration of Hibiscus sabdariffa L. 6%, 12% and 15% ethanol extract gel on the 12th day resulted in a significant acceleration of the wound healing process (p=0.024). There was a significant difference between negative control (Mean=3,281 mm ; SD=1,681 mm), positive control (Mean=2,813 mm ; SD=1,106 mm), 6% Rosella flower petal extract gel (Mean=2,916 mm ; SD=1,224 mm ), 12% (Mean=1,290 mm ; SD=1,098 mm), and 15% (Mean=1,035 mm ; SD=0.952 mm) (p=0.024), in the macroscopic assessment there was no significant difference in the wound healing process on day 3, 6, 9, 12 and 15 except for signs of swelling on days 6 (p=0.038) and 9 (p=0.038) which had a significant difference compared to controls. Conclusion: Administration of Hibiscus sabdariffa L. 15% ethanol extract gel had the fastest time in wound closure and healing (Mean=1.035 mm ; SD=0.952 mm) for 12 days (p=0.024), the longest time in wound closure and healing was the negative control (gel placebo) (Mean=0.976 mm ; SD=1.103 mm) namely for 15 days (p=0.158). Abstract

Background: Diabetes mellitus (DM) is a significant global health challenge with a rapidly increasing prevalence. Although synthetic antidiabetic drugs remain the standard treatment, long-term use often leads to adverse side effects, prompting exploration of natural alternatives. This study investigated the therapeutic potential of ethanol extract of Peperomia pellucida (L.) Kunth leaves in diabetes management through comprehensive assessment of glycemic control, inflammatory marker (IL-1β), and pancreatic tissue histopathology. Subjects and Method: Thirty male Wistar rats were randomized into five groups (n=6): normal control, negative control (STZ-induced), positive control (STZ+metformin 50 mg/kg BW), and two treatment groups receiving Peperomia pellucida extract at doses of 40 mg/kg BW and 80 mg/kg BW, respectively. After 7 days of treatment, blood glucose levels, serum IL-1β concentrations, and pancreatic histopathology were evaluated. Phytochemical screening was conducted to identify bioactive compounds in the extract. Results: Phytochemical analysis confirmed the presence of alkaloids, flavonoids, tannins, saponins, and phenolics. Blood glucose measurements showed significant intergroup differences on day 0 (p<0.001) and day 7 (p<0.001). The 80 mg/kg BW extract group showed a reduction in blood glucose from day 0 (Mean=312.83; SD=40.92) to day 7 (Mean=281.00; SD=34.86). IL-1β serum analysis revealed significant differences among groups (p<0.001), with lower IL-1β levels observed in extract-treated groups compared to the negative control. Histopathological examination demonstrated improved pancreatic tissue structure in extract-treated groups, especially at higher doses. Conclusion: Ethanol extract of Peperomia pellucida (L.) Kunth leaves exhibits promising antidiabetic and anti-inflammatory properties in STZ-induced diabetic rats, as evidenced by reduced blood glucose, decreased IL-1β levels, and improved pancreatic histology. These findings suggest that Peperomia pellucida (L.) Kunth is a potential candidate for the development of natural therapies for diabetes management.

Abstract

Background: Type 2 diabetes mellitus (T2DM) is a complex metabolic disorder associated with chronic inflammation and male reproductive dysfunction. The ethanol extract of turmeric (Curcuma longa) has demonstrated potential as an alternative therapeutic agent due to its anti-inflammatory and reproductive restorative properties. This study aims to analyze the anti-inflammatory and anti-infertility effects of the ethanol extract of Curcuma longa in male Wistar rats with T2DM induced by a streptozotocin-nicotinamide-high fat diet (STZ-NA-HFD). Subjects and Method: Thirty male Wistar rats were randomized into six groups (n=5): normal control (K1), diabetic control (K2), positive control (T2DM + metformin 45 mg/kgBW, K3), and three treatment groups receiving Curcuma longa extract at doses of 50 mg/kgBW (K4), 100 mg/kgBW (K5), and 150 mg/kgBW (K6). Diabetes was induced with STZ (55 mg/kgBW) and nicotinamide (120 mg/kgBW). Blood glucose, IL-6, testosterone levels, sperm count, and sperm motility were evaluated after 21 days of treatment. Results: The turmeric extract significantly reduced blood glucose levels (p<0.001) and IL-6 concentrations from 574.06±68.99 to 214.06±78.71 ng/mL at a dose of 150 mg/kgBW (p<0.001). Normal sperm motility increased significantly at 150 mg/kgBW (20.0±2.55%) compared to the diabetic control (8.40±2.07%) (p=0.008). Conclusion: The ethanol extract of Curcuma longa demonstrated significant anti-inflammatory activity by reducing IL-6 levels and improving glycemic control. Its anti-infertility effect was reflected by enhanced sperm motility, particularly at the 150 mg/kgBW dose. These findings indicate that Curcuma longa possesses potential as an adjuvant therapy for the management of T2DM and its associated complications.
