Abstract

The Triple Elimination Program aims to prevent the transmission of HIV, syphilis, and hepatitis B from mother to child. In Rokan Hilir Regency, screening coverage remains low: hepatitis B (42.2%), HIV (38%), and syphilis (48.3%). Bantaian Health Center has the lowest coverage, while Bagan Batu Health Center has the highest. A thorough evaluation is needed to improve program performance using the Malcolm Baldrige approach. This study aims to evaluate the performance of the triple elimination program for pregnant women at the Rokan Hilir District Health Office in 2024, employing a mixed-method explanatory sequential design. The sample consisted of 40 respondents and 9 stakeholders as informants. Triangulation was applied to sources, methods, and data. The results show that most respondents rated program leadership as high (57.5%), while strategic planning (52.5%), patient focus (62.5%), measurement and knowledge management (60%), team focus (55%), process focus (57.5%), and performance results (60%) were rated low. Qualitatively, program leadership demonstrated strong commitment. Program planning is based on technical guidelines and BOK funds, with limitations in APBD budget allocations. Education and collaboration with community leaders have been implemented. However, the program has not focused on patients and is not widely recognized by the community. Staff at Bantaian and Bagan Batu Health Centers enthusiastically implement the program despite limited human resources, introducing innovations and conducting regular evaluations. The program is developed through coordination meetings, data analysis, and cadre training. Key factors for achieving targets include case monitoring, health education, cross-sector collaboration, and periodic evaluations. Health centers need to expand education and socialization on the importance of triple elimination screening, supported by integrated coordination between the Health Office and related agencies (OPD).

Abstract

The largest proportion of health workers in the hospital is nurses which is 50.79%. One of the performance indicators is seen from the provision of nursing care in the hospital, seen from the nursing care documentation for the last 3 years in 2020 by 30%, in 2021 by 40.3% and in 2022 by 50.8%. The purpose of this research was to analyse nurses performance in the aspects of quality, quantity, used of working time, teamwork and incentives. This research used a qualitative research approach method with a case study design. interviews, observation and documentation were conducted on 5 informants to produce accurate and complete data. Data analysis in this research was carried out by triangulation. The results of this research found that nurses' knowledge of SOPs was good. Incomplete documentation of the initial study on medical history was 40% and physical examination was 60%. Nurses’ activity time were still classified as low direct activities by 19.5%, indirect activities by 32.6%, personal activities by 28.9% and non-productive activities by 19.5%. Providing financial incentives in the form of Eid Mubarak THR, while non-financial incentives in the form of awards and praise have not been implemented. Teamwork is less effective due to the lack of pharmacists and nutritionists in collaboration. The performance of nurses in inpatient rooms are less effective because in the implementation of nursing SOPs there are still nurses who are not in accordance with SOPs, incompleteness in filling out the initial nursing review documentation, nursing activity time are still low, financial and non-financial incentives are not implemented and teamwork is less effective due to the lack of involvement between pharmacists and nutrition professionals in collaboration.

Abstract

Adolescents are increasingly at risk of preventable health issues due to unhealthy lifestyle behaviors and limited access to contextualized health education. Addressing this gap, the study aimed to develop and evaluate a School-Based Community Learning Model that enhances health literacy and student engagement through participatory, project-based learning. The intervention was based on contextual teaching, social constructivism, and experiential learning principles. It involved digital health modules, peer discussions, and community action projects delivered over six weeks to 120 senior high school students in Pekanbaru, Indonesia. A quasi-experimental pretest-posttest control group design was employed to assess changes in health literacy and life skills. Statistical analysis showed a significant improvement in both health literacy and life skills in the intervention group (p 0.001). The average N-Gain score was 0.62, indicating moderate-to-high learning gains. Qualitative observations also indicated enhanced collaboration, critical thinking, and student initiative—markers of affective and psychomotor development. These findings suggest that the model effectively fosters essential health competencies and supports holistic student development. By aligning with national education goals, the program provides a viable framework for integrating health education into the standard curriculum. The School-Based Community Learning Model presents a scalable and practical approach to embedding health literacy in secondary education. It reinforces the role of health education as a fundamental aspect of character building and life skills development in schools.

Abstract

Purpose: This study aimed to improve personal hygiene practices and Perilaku Hidup Bersih dan Sehat (PHBS) among elderly individuals through a structured health education program integrated with Indonesia’s national Gerakan Masyarakat Hidup Sehat (GERMAS). The program focused on increasing awareness, encouraging healthy daily habits, and supporting independence among older adults. Methodology/Approach: The program was implemented in Sungai Kamuyang Village, West Sumatra, using a community-based home-visit approach. Health education was delivered through brief counseling and demonstrations of personal hygiene practices, accompanied by the distribution of hygiene kits. Observations of household sanitation conditions and daily hygiene routines were conducted, and data were collected through short interviews and field notes with assistance from local health cadres. Results/Findings: The intervention resulted in positive behavioral changes. Approximately 85% of participants demonstrated improved understanding of PHBS principles, 80% practiced regular personal hygiene routines, and 60% engaged in light physical activity. However, only 40% reported routine health check-ups, and nearly half of the households still lacked adequate sanitation facilities, indicating ongoing environmental and service-related barriers. Conclusions: The program effectively improved elderly hygiene practices and PHBS awareness, though barriers like sanitation and health check-ups remain. Further research is needed. Limitations: The study was limited by a small sample size and a short implementation period, which may restrict the generalizability of the findings. Contributions: This study provides practical evidence for public health practitioners, community health workers, and policymakers on the effectiveness of home-based education integrated with GERMAS in promoting healthy behaviors among the elderly.

Abstract

Digital transformation in healthcare has become a major focus of the Indonesian government through the development of the Mobile JKN application by BPJS Kesehatan. However, at Dumai General Hospital (RSUD Dumai), the utilization rate of this application remains low. This study aims to analyze the factors contributing to the low adoption of Mobile JKN in patient registration using the Innovation Resistance Theory (IRT) by Ram and Sheth (1989), which includes five dimensions: Usage Barrier, Value Barrier, Risk Barrier, Tradition Barrier, and Image Barrier. The research applied a qualitative case study approach involving key informants consisting of both users and non-users of Mobile JKN, as well as hospital registration staff. Data were collected through in-depth interviews and field observations, then analyzed thematically. The findings show that low adoption is influenced by limited digital literacy among elderly patients (Usage Barrier), perceptions that manual registration is faster (Value Barrier), concerns about data security and technical issues (Risk Barrier), strong preference for face-to-face interactions (Tradition Barrier), and the negative image of the application due to system errors and insufficient socialization (Image Barrier).

Abstract

The behavior of pregnant women in conducting ANC visits remains a challenge in the success of the ANC program. The behavior of pregnant women visiting ANC remains a challenge at the Meral Community Health Center, with coverage of K1 (75.3%), K4 (69.3%), and K6 (64.3%) not meeting the 2024 SPM. This study aims to analyze maternal behavioral factors and health promotion strategies in the 2025 ANC program using a mixed method convergent design. The sample consisted of 130 respondents, 5 stakeholders, and 5 postpartum mothers. Quantitative analysis used multiple logistic regression, while qualitative analysis used triangulation of sources, methods, and data. This study found that K1 was 88.5%, K4 was 85.4%, and K6 was 43.8%. There is a significant relationship between knowledge (p=0.018, POR=6.610, CI: 1.383-31.672), attitude (p=0.002, POR=3.550, CI: 1.574-8.008), and husband's support (p=0.003, POR=3.395, CI: 1.558-8.292) with the behavior of pregnant women. Knowledge is the most dominant factor (POR=6.610). Advocacy is carried out through cross-sector workshops and budget submissions to the Regent/BPKAD, although participation is still low. Cultivating an atmosphere supported by facilities such as air conditioning, priority services, and ultrasound, but educational media are not optimal. Empowerment has been carried out, but human resources are limited. Cross-sector partnerships exist, but coordination and understanding are still weak. Meral Health Center needs to develop community-based innovations integrated with education, family support, and service digitalization

Abstract

Abstract

Access to safe household drinking water remains a major environmental health concern in riverine rural communities that rely on untreated groundwater and refill water sources. Buluh Cina Village, located along the Kampar River in Riau Province, depends largely on shallow wells and refilled drinking water stations, many of which lack adequate disinfection and routine monitoring. This study aimed to conduct an integrated environmental health surveillance of household drinking water quality in Buluh Cina Village to identify microbiological and physicochemical risks relevant to local public health action. A cross-sectional surveillance study was conducted by collecting 30 water samples from points of access (supply) and 30 samples from points of use (ready-to-drink) in selected households using stratified random sampling. Laboratory analyses assessed microbiological parameters (Escherichia coli and Total Coliform), physical parameters (turbidity, color, and pH), and chemical parameters (iron, manganese, and residual chlorine), following the Indonesian Ministry of Health Regulation No. 2/2023. Overall, 91.7% of samples did not meet national drinking water quality standards. Microbiological contamination was detected at both sampling points, with Escherichia coli present in 33.3% of PA samples and 46.7% of PU samples, and total coliform contamination in 53.3% of PA samples and 63.3% of PU samples. However, the differences between PA and PU were not statistically significant. Residual chlorine was absent or below the recommended level in 70% of samples, while turbidity, abnormal pH, and elevated iron and manganese were also observed. These findings indicate significant environmental health risks associated with household drinking water in this riverine rural community. Priority public health actions include improving safe household water storage practices, increasing uptake of point-of-use disinfection, strengthening routine monitoring of refill drinking water depots, and integrating drinking water surveillance into primary health care center (puskesmas)-based environmental health workflows. This study provides locally grounded evidence to support targeted interventions for improving drinking water safety in riverine rural settings Keywords:  Buluh Cina Village, Drinking Water Quality, Environmental Health Surveillance, Household Water Safety, Kampar River
