Abstract

Abstract

The COVID-19 pandemic has unleashed a profound psychological crisis, markedly increasing rates of depression, anxiety, loneliness, and PTSD, especially among young adults, women, racialized communities, and individuals with lower education and income. Effective coping strategies like positive reframing, humor, and acceptance have been linked to improved mental health outcomes, while maladaptive coping correlates with worsening psychosocial symptoms. This study explores the biopsychosocial factors that shape adaptive coping mechanisms in COVID-19 survivors. Using a cross-sectional design and quantitative approach, data were analyzed using logistic regression analysis. The study involved 378 COVID-19 survivors from Surakarta, who completed both online (via Telegram and WhatsApp) and offline questionnaires between November 2022 and January 2023. The sample predominantly consisted of women (73%, n=276) aged 18-40 years (89.7%, n=339), with a high prevalence of adaptive coping mechanisms (97.1%, n=367). Results revealed that gender (p=0.029, B=-1.497), history of mental disorders (p=0.039, B=-2.002), and alcohol/substance use (p=0.027, B=-2.572) had a significant influence on adaptive coping. These findings suggest that gender, mental health history, and substance use as crucial factors influencing adaptive coping among COVID-19 survivors. Abstract

Breast cancer is a malignancy originating from breast cells, ducts, and tissues that can occur due to genetic mutations due to DNA damage in normal cells. The PI3K/Akt/mTOR pathway is an intracellular pathway that plays an important role in regulating basic cell functions such as cell growth, movement, survival, metabolism, and angiogenesis. Some examples of PI3K/mTOR pathway inhibitor drugs on the market and often used are dactolisib, samotolisib, voxtalisib, and so on. These drugs are more effective than single-pathway inhibitors. This article was prepared using the literature review method. The data used in this article were obtained from the PubMed and Google Scholar using the keywords breast cancer, PI3K/Akt/mTOR inhibitor, and PI3K/AKT/mTOR signaling pathway. Several studies suggest that the PI3K-AKT-mTOR pathway is critical in mediating the proliferation and migration of breast cancer cells. Because of the importance of the PI3K-AKT-mTOR pathway in the pathogenesis of breast cancer, it is often used as a target for breast cancer therapy. PI3K/mTOR dual ATP competitive inhibitors can directly act on the main target proteins of PI3K and mTOR, thereby more efficiently inhibiting the PI3K/AKT/mTOR signaling pathway and effectively preventing other factors that can activate the PI3K pathway, and reducing the resistance and side effects produced by single inhibitors. Abstract

Antibiotic use has increased in both developing countries, such as Indonesia, and developed countries, such as Japan and Australia. This trend may lead to a rise in antibiotic resistance if antibiotics are used irrationally. This study aimed to evaluate antibiotic prescribing patterns using the Gyssens method among adult outpatients in the Internal Medicine Clinic at Persahabatan General Hospital during the period of July–September 2023. This was a descriptive study with a cross-sectional design. Data were collected retrospectively from the medical records of adult outpatients attending the Internal Medicine Clinic during the study period. Of the 30 patient records that met the inclusion and exclusion criteria, the age group 51–60 years represented the largest proportion of antibiotic prescriptions (46.67%). Levofloxacin was the most frequently prescribed antibiotic (30.3%). Based on the Gyssens method, 88.8% of prescriptions were classified as rational, while 11.1% were irrational. In conclusion, 88.8% of antibiotics prescribed for adult outpatients in the Internal Medicine Clinic at Persahabatan General Hospital during July–September 2023 were deemed rational according to the Gyssens method. Abstract

Tension-type headache (TTH) is the most common form of primary headache in the general population, with a one-year prevalence ranging from 38 to 78%. The most common triggers for TTH are stress and emotional conflict. Nurses are among the workers exposed to high work stressors, placing them at risk of developing TTH, which is often triggered by chronic occupational stress known as burnout syndrome. This study aims to determine the correlation between burnout syndrome and tension-type headaches in nurses. This research employed an observational cross-sectional design conducted on 87 nurses at Roemani Muhammadiyah Hospital in Semarang, using proportional random sampling. The research instruments included a tension-type headache questionnaire and the Maslach Burnout Inventory (MBI). The statistical test used is the chi-square test. Among the 87 respondents, mild burnout syndrome was present in 64.4%, moderate burnout syndrome in 24.1%, and severe burnout syndrome in 11.5%. Respondents reporting tension-type headache constituted 26.4%, compared to 73.6% without this condition. The majority of respondents with severe burnout syndrome also experienced tension-type headaches (70%). There was a significant correlation between burnout syndrome and tension-type headaches among nurses at Roemani Muhammadiyah Hospital, Semarang, with a p-value of less than 0.005. Abstract

The renoprotective effect in chronic kidney disease (CKD) patients can be induced by antihypertensive therapy, which provides a vasodilatory effect on the blood vessels leading to the kidneys. This resulting renoprotective function is considered to slow the decrease in kidney function in CKD patients. The purpose of this study was to examine the effectiveness of antihypertensive therapy as renoprotective agents through the analysis of kidney function parameters in CKD patients. A descriptive design with a cross-sectional method was used, and the study sample used data from patient medical records, specifically focusing on kidney function parameters of CKD patients during the January-June 2023 treatment period at RSUD Sultan Abdul Aziz Syah Peureulak. Of the 134 respondents, there were two groups of patients receiving antihypertensive therapy: ACE-Inhibitors (ACEIs) and (angiotensin receptor blockers) ARBs. After conducting the Wilcoxon test, there were significant changes in kidney function parameters (Urea, creatinine, and eGFR) between before and after antihypertensive therapy, with a p value <0.05. In the ACEI group, the mean urea level was 72.4 ± 48.3 mg/dL, creatinine 4.8 ± 4.3 mg/dL, and eGFR 30.6 ± 30.8 mL/min. In the ARB group, mean urea was were 85.2 ± 48.7 mg/dL, creatinine 6.1 ± 4.2 mg/dL, and eGFR 21.6 ± 26.7 mL/min. These findings indicate that ACEI and ARB antihypertensive therapies are effective as renoprotective agents in CKD patients.
