Abstract

Introduction. Spiritual care is a vital component of nursing that meets patients' spiritual needs, particularly in palliative care. However, spiritual care education should be included in nursing curricula, especially in culturally diverse settings like Indonesia.Objective. This study aimed to evaluate the effectiveness of spiritual care education on nursing students' clinical competence, self-confidence, and caring attitudes during a four-week clinical rotation in a palliative care unit in Indonesia.Methods. A sequential explanatory design was used, combining quantitative and qualitative approaches. The quantitative phase involved 127 nursing students in a quasi-experimental design to assess changes in competencies. Data collection included questionnaires measuring self-efficacy, caring attitudes, clinical competency, and clinical judgment. The qualitative phase involved focus group discussions with ten purposively selected students to explore experiences and challenges in providing spiritual care. Quantitative data were analyzed using the Wilcoxon test, while qualitative data were analyzed through thematic analysis.Results. Quantitative findings significantly improved students' self-efficacy, caring attitudes, and clinical competence. Qualitative results revealed barriers to spiritual care, including communication difficulties, fear of errors, and cultural sensitivity.Conclusion. Integrating spiritual care into nursing education enhances nursing students' competence and caring attitudes. Addressing cultural and communication barriers is crucial for improving holistic patient care in diverse contexts

Abstract

ABSTRACT Hemodialysis is one of the common treatments performed by patients with CKD in Indonesia. The physical consequences of hemodialysis can affect the patient's psychology which ultimately results in non-compliance with the hemodialysis treatment program. This requires management to improve patient adherence to the hemodialysis program, namely multifaceted interventions in the form of education and cognitive behavioral therapy. This study aims to apply and prove a Multifaceted Intervention in increasing adherence of hemodialysis patients. This study was carried out on 12 hemodialysis patients intervention group and 12 patient control group. The level of adherence was measured using The End-Stage Renal Disease Adherence Questionnaire. A multifaceted intervention was provided over 3 weeks. The results of the application of EBN obtained an average pre-intervention score of 792 with a standard deviation of 126, a post-intervention score of 846 with a standard deviation of 127. Intradialytic weight gain pre-intervention 5.96 decreased to 5.39 in post-intervention, the p value of compliance was 0.002 and the p value of IDWG was 0.009. Every nursing action must be based on existing evidence so that it can improve the quality of nursing services, so it is suggested that multifaceted interventions can be used as non-pharmacological interventions for nurses to help increase adherence and reduce IDWG in hemodialysis patients. Keywords: Adherence, Cognitive Behavioral Therapy, Hemodialysis, Multifaset Intervention, Intradialytic Wight Gain.  ABSTRAK  Hemodialisis menjadi salah satu penatalaksanaan yang umum dilakukan oleh pasien dengan penyakit ginjal kronis di Indonesia. Konsekuensi fisik akibat hemodialisis ini dapat mempengaruhi  psikologis pasien yang akhirnya berdampak pada ketidakpatuhan terhadap program pengobatan hemodialisis. Hal tersebut membutuhkan manajemen dalam peningkatan kepatuhan pasien terhadap program hemodialisis yaitu intervensi multifaset berupa edukasi dan cognitive behavioral therapy. Penelitian ini bertujuan untuk mengetahui  Intervensi Multifaset dalam kepatuhan pasien hemodialisis dan intradialytic wight gain (IDWG). Penelitian menggunakan desain quasi experiment terhadap 12 orang intervensi dan 12 kontrol pasien hemodialisis. Tingkat kepatuhan diukur menggunakan The End-Stage Renal Disease Adherence Questionnaire  . Intervensi multifaset diberikan selama 3 minggu. Hasil didapatkan rerata skor pre intervensi (Mean ± SD) 792±126, skor post intervensi (Mean ± SD) 846±127. Intradialytic wight gain (IDWG) pre intervensi 5,96 turun menjadi 5,39 pada post intervensi, dengan uji Wilcoxon p value IDWG  yaitu 0,009 dan uji T-Test p value kepatuhan yaitu 0,002 dan Intervensi multifacet terbuti menurunkan IDWG dan meningkatkan kepatuhan  pada pasien yang menjalani HD Kata Kunci: Cognitive Behavioral Therapy, Hemodialisis, Intervensi Multifaset, Kepatuhan, Intradialytic Wight Gain.

Abstract

Hypertension is a leading global health burden, affecting over a billion adults, yet fewer than a quarter achieve adequate control. In Indonesia, hypertension is a major contributor to cardiovascular morbidity and mortality. Patients often struggle with sustained self-care. This study explored self-care practices and barriers to hypertension management among patients, caregivers, and nurses in a primary healthcare in Bandung. A qualitative phenomenological approach was used. Twenty participants were purposively recruited, comprising seven patients, seven family members, and six nurses. Data were collected through semi-structured interviews, transcribed verbatim, and analyzed using Colaizzi’s method. Trustworthiness was ensured through credibility, dependability, confirmability, and transferability criteria. Two themes emerged. Self-care practices included medication adherence, dietary modification, physical activity, stress management, blood pressure monitoring, and smoking cessation, though these were inconsistent. Barriers included cultural dietary norms, misconceptions about medication, limited knowledge and motivation, financial constraints, and fear of complications. Hypertension self-care in Indonesia is shaped by complex cultural, social, and economic challenges. Culturally sensitive, family-centered, and nurse-led interventions are needed to strengthen self-care and improve outcomes.

Abstract

Diabetes Mellitus (DM) is closely linked to unhealthy dietary habits and sedentary lifestyles. Despite the efforts of the Indonesian government to manage DM through its chronic disease management program, many patients continue to experience poor glycemic control. Families, particularly primary caregivers, play a critical role in supporting patients with DM. This study aimed to analyze the correlation between family members' knowledge and attitudes toward family health tasks and glycemic control (fasting blood glucose levels) in DM patients participating in the Chronic Disease Management Program (CDMP) at a public health center in Bandung, Indonesia. A cross-sectional correlational study was conducted using a quantitative design. The study involved 65 family members who served as primary caregivers for DM patients, selected through a total sampling technique. Data were gathered using structured questionnaires to assess family members' knowledge and attitudes, along with clinical data on the patients' glycemic control. The relationship between variables was analyzed using the Spearman Rank Correlation test. The findings revealed no significant relationship between family members' knowledge and attitudes regarding caregiving tasks and the glycemic control of DM patients. Specifically, family knowledge and attitudes were not significantly associated with patients' glycemic control (p > 0.05). This study concluded that family members' knowledge and attitudes do not significantly influence glycemic control in patients with DM. These findings suggest that other factors may affect blood sugar levels, indicating the need for further research on family-based interventions and additional variables that could impact glycemic management.

Abstract

Background: Needlestick injury (NSI) remains a persistent occupational safety risk for nursing students during clinical practice, with implications for worker safety, patient care, and healthcare safety systems. Understanding preventive safety behavior within clinical training environments is essential for improving workplace safety in healthcare settings. Methods: A cross-sectional study was conducted among nursing students undertaking hospital-based clinical placements. Using a purposive sampling, participants completed a structured self-administered questionnaire assessing NSI exposure, preventive safety behaviors, clinical supervision, safety training, and perceived safety within the clinical learning environment. Data was analyzed to identify patterns of NSI risk and associated preventive behaviors. Results: NSI exposure during clinical practice was frequently reported, with variability in adherence to preventive safety behaviors. Preventive behaviors were associated with the quality of clinical supervision, participation in safety training, and the perceived safety climate. Gaps between expected safety standards and actual clinical practices indicated weaknesses in workplace safety systems. Conclusion: NSI risk among nursing students is influenced by both individual behavior and organizational safety conditions. Strengthening supervision, safety training, and safety-oriented clinical systems is essential to promote consistent preventive practices and improve occupational safety during clinical education.

Abstract

Shivering pasca anestesi spinal dapat meningkatkan kebutuhan metabolik, konsumsi oksigen, ketidaknyamanan, dan risiko keterlambatan pemulihan. Penelitian ini bertujuan menganalisis efektivitas kombinasi blanket warmer dan terapi cairan infus hangat terhadap penurunan derajat shivering pada pasien pasca anestesi spinal.Penelitian menggunakan desain pre-eksperimental one-group pretest-posttest pada 35 pasien di ruang pemulihan RSUD Cicalengka. Sampel direkrut shivering menggunakan teknik sampling konsekutif hingga jumlah sampel terpenuhi. Blanket warmer dan cairan infus hangat diberikan secara bersamaan selama 45 menit. Derajat dinilai menggunakan skala Crossley dan Mahajan pada sebelum intervensi serta menit ke-15, ke-30, dan ke-45; suhu tubuh diukur sebelum dan sesudah intervensi. Analisis menggunakan uji Wilcoxon dan Friedman. Sebelum intervensi, derajat shivering terbanyak adalah derajat 3 (54,3%) dan derajat 4 (31,4%). Pada menit ke-30, 80,0% pasien mencapai derajat 0, dan pada menit ke-45 seluruh pasien (100%) tidak mengalami shivering. Suhu tubuh meningkat dari 35,27 ± 0,24°C menjadi 36,07 ± 0,23°C (p<0,001). Uji Friedman menunjukkan perubahan derajat shivering yang bermakna antarwaktu pengukuran (χ²=100,361; df=3; p<0,001). Kombinasi blanket warmer dan terapi cairan infus hangat diikuti penurunan progresif derajat shivering dan peningkatan suhu tubuh pada pasien pasca anestesi spinal. Temuan perlu dikonfirmasi melalui penelitian dengan kelompok kontrol dan pengendalian faktor perancu
