Abstract

Background: Acceptance of the Covid-19 vaccine is a challenge in the success of the Covid-19 vaccination program. Objective: This study aims to achieve herd immunity and to analyze predisposing factors (age, gender, knowledge, and belief in the Covid-19 vaccine) in the community in the Alang Alang Lebar Community Health Center Work Area. Methods: This type of research is a quantitative study using a cross-sectional study. The population in this study is people who live in Talang Ke-lapa Village and Alang Alang Lebar Village aged 18-60 years, a sample of 211 people with the determination of the sample using the Proportionate Stratified Random Sampling technique, the data were analyzed univariately and bivariate. Results: The research conducted showed that there was a significant relationship between knowledge (p-value = 0.041) and trust in the Covid-19 vaccine (p-value = 0.000) and acceptance of the Covid-19 vaccination program. Meanwhile, the age variable (p=0.739) and the gender variable (p=0.838) had no relationship with the acceptance of the Covid-19 vaccination program. Conclusion: Efforts are needed to increase access to information related to Covid-19 vaccination so that people believe in the Covid-19 vaccine so that public acceptance of the Covid-19 vaccination program increases in the Alang Alang Lebar Health Center Work Area.

Abstract

ABSTRACTBackground: Informal sector workers still dominate the workforce in Indonesia. BPS data per August 2012 stated that 60.14% of workers in Indonesia’s status as workers in the informal sector. The informal workers is potential members of national health insurance. In achieving Universal Health Coverage, the health BPJS extend its scope beginning in January 2014 to include these workers. Method: This study aimed to evaluate the participation of the informal sector in health insurance scheme pre- and post the national health insurance. The study was conducted with quantitative and qualitative methods. Quantitative studies using secondary data, participation of workers in the informal sector BPJS 2014 program and qualitative study was obtained through a review of several studies related to worker’s health insurance and literature studies related health insurance policy. Result: The result shows that pre JKN the coverage is still low. This is motivated because the workers are not fully be covered in the social security program and they have not been covered by the Law No. 3 1992. Jamkesmas Program only able to cover poor informal sector workers. However, with the enactment of the National Health Insurance program organized by the health BPJS there is an increase in membership. The first 4 months of  monitoring the implementation of national health insurance showed a sizeable increase in registration. In addition, an increasing number of utilization is dominated by those who are sick. This indicates the occurrence of adverse selection. Based on the policy aspects, their participation has been stated in Presidential Decree No. 111 in 2013, but the rules do not explicitly describe the efforts that must be made in extending coverage and maintaining the continuity of these workers to remain a participant of JKN. This study implies that the policy makers need to create a scheme as effort to ensure the sustainability of the expansion of the membership and participation of the informal sector in health BPJS ABSTRAKLatar belakang: Pekerja sektor informal atau dikenal dengan istilah pekerja bukan penerima upah masih mendominasi angkatan kerja di Indonesia. Data BPS per Agustus 2012 menyebutkan bahwa 60,14% pekerja di Indonesia berstatus sebagai pekerja di sektor informal. Jumlah pekerja informal yang cukup besar merupakan potensi kepesertaan jaminan kesehatan nasional. Dalam upaya pencapaian Universal Health Coverage (UHC), pihak BPJS kesehatan memperluas cakupannya mulai Januari 2014 termasuk pada pekerja ini. Kajian ini bertujuan untuk mengevaluasi kepesertaan pekerja sektor informal dalam skema jaminan kesehatan pra dan pasca JKN. Metode: Kajian ini dilakukan dengan metode kuantitatif dan kualitatif. Studi Kuantitatif menggunakan data sekunder berupa data kepesertaan pekerja sektor informal pada program BPJS tahun 2014 dan studi kualitatif diperoleh melalui review beberapa hasil penelitian terkait jaminan kesehatan pekerja ini dan studi literatur terkait kebijakan jaminan kesehatan. Hasil: Cakupan kepesertaan pra JKN masih rendah. Hal ini dimotivasi karena pekerja ini belum sepenuhnya ter-cover dalam program jamsostek & keberadaan mereka belum dicakup dalamUU No. 3/1992. Program jamkesmas hanya mampu mengcover pekerja sektor informal miskin. Namun, dengan berlakunya program Jaminan kesehatan Nasional yang diselenggarakan oleh BPJS kesehatan menunjukkan peningkatan kepesertaan. Monitoring 4 bulan pertama pelaksanaan jaminan kesehatannasional menunjukkan adanya kenaikan registrasi yang cukup besar. Selain itu, peningkatan jumlah utilisasi pekerja ini didominasi oleh mereka yang sakit. Hal ini menunjukkan terjadinya adverse selection. Berdasarkan aspek kebijakan,kepesertaan mereka telah tercantum dalam Perpres No. 111 tahun 2013 namun kebijakan ini belum secara eksplisitmenerangkan upaya yang harus dilakukan dalam memperluas kepesertaan dan menjaga kesinambungan pekerja ini untuk tetap menjadi peserta BPJS. Studi ini memberikan implikasi agar pihak pengambil kebijakan jaminan kesehatan nasional perlu membuat skema dalam upaya perluasan kepesertaan dan menjamin keberlangsungan kepesertaan pekerja sektor informal dalam BPJS kesehatan.

Abstract

Primary dentists at the era of the National Health Insurance are only paid IDR2,000 each patient. The Capitation funds can not cover the cost of services that must be spent. The payment system must be improved because it is related to the quality of service. The aims of this study to analyze the management and utilization of  dental capitation funds including the bottlenecks and to generate solutions in the implementation of JKN. This research was descriptive with qualitative approach. The informants of the study were 16 (sixteen) dentists as an independent practitioners and pratama clinics in Palembang and Lubuklinggau with highest and lowest capitation coverage. Data were analyzed by data reduction, data presentation, conclusion drawing and verification. The result showed that there was a disparity in capitation fund income received by dentists both as an independent practitioners and pratama clinic. Not all dentists get the ideal number of participants 1: 10,000. Dentists with low capitation norms have difficulty to set their operational funds. The budget for operational cost is bigger than services cost. Revenues compared to capitation funds are still lacking. Most capitation funds for primary dentists are insufficient and still rely on fee for service patients. The government  should set dental capitation norm and regulation for National Health Insurance era.Key words: dentist, capitation, clinic, utilization, regulation

Abstract

Background: Hand hygiene is the simplest practical method of preventing cross-infection. Knowledge of hand hygiene plays a vital role in determining adherence to hand hygiene. Nosocomial infection is an infection that occurs in patients in hospital.Aims: This study aims to analyze the knowledge of health workers regarding hand hygiene in Palembang hospitals during Covid-19 era.Methods: This study used a cross-sectional design with 300 health workers who worked in hospitals in Palembang. They are selected by simple random sampling. Data were collected using online questionnaire during October 2020.Results: The majority of respondents were female (88.67%), with the majority age group 30-39 years (43.67%), the majority of respondent came from nurses (63%), work in inpatient rooms (27.33%). A total of 80.67% of respondents have received formal training related to hand hygiene. 96% used alcohol-based hand rub. About 15.33% of respondents indicated good knowledge, moderate knowledge (80.33%), and less knowledge (4.3%) regarding hand hygiene. The difference in health workers' profession was significantly related to the level of knowledge about hand hygiene (p <0.05).Conclusion: There are differences in the level of knowledge about hand hygiene in each type of health worker professionKeywords: Hand Hygiene; Knowledge; Health Workers; Online

Abstract

Background: The requirement for malaria elimination to be recognized is to prove API lower than 1 per 1000 in the population, together with no indigenous case. Lahat Regency is an area of low-endemicity for malaria, where the target for elimination is projected for 2024. Since 2019, Lahat Regency has received socialization about E-SISMAL, which was developed as a web-based basis to speed up the reporting process. However, there are still delays in reporting malaria on E-SISMAL. Methods: Descriptive research with a qualitative approach was designed. The study subjects were the malaria person in charge, the head of seven selected community health centers (Puskesmas) and the Lahat District Health Office. Primary data came from observation, focus group discussion (FGD) and in-depth interviews with eighteen informants selected by the purposive sampling method. Results: The available inputs are inadequate, such as the human resource education was not up to standard, the internet network was not smooth, and there were no guidelines on E-SISMAL in the health centers. Malaria person in charge does not understand how to use E-SISMAL. In the process, there was the involvement of cross-sectoral and networked establishments. The output shows that the report indicators on E-SISMAL are not up to standard with less than 80%. Conclusions: The use of E-SISMAL in the Lahat district has not been maximized. It has an impact on delays in reporting. It is recommended that training and practice of E-SISMAL occur to improve officers' ability through socialization and that the Lahat Health Office establishes partnerships with telecommunications companies to ensure the smooth running of the network. Developing specific guidelines on E-SISMAL and a mobile phone-based malaria surveillance information system to speed up the malaria reporting process in the Lahat district is also advisable.

Abstract

The low utilization of IPV immunization services in Palembang in 2017 was 66,7% while the national target was 95%. The purpose of this study was to determine the factors associated with the utilization of IPV immunization services in Palembang. The study was conducted in September-October 2018, the population was 3570 mothers who had babies who were targeted by IPV immunization, 400 samples of mothers who have babies are targeted by IPV immunization in 5 health centers in Palembang by random sampling. Research using quantitative methods with cross-sectional design. Data collection instruments were questionnaires based on the concept of health trust models, interviews, and observations, the data were analyzed using multiple logistic regression. The results showed that 84.5% of mothers used IPV immunization services in Palembang. The results of multiple logistic regression showed that the family support variable (PR=2,015), knowledge of the mother variable (PR=2,164), the availability of health care facility variables (PR=2,376), and variable support for health workers (PR=3,601) affect the utilization of IPV immunization services in Palembang City. Conclusion: variable support for health workers is the variable that has the greatest influence on the utilization of IPV immunization services in Palembang.

Abstract

Leprosy Hospital of dr. Rivai Abdullah Palembang in the last 3 (three) years found data on visitors who came to the Outpatient Polyclinic showed a downward trend every year since 2015. This study aims to determine the influence of patient perceptions of the marketing mix to the patient’s loyalty at Outpatient Polyclinic of Leprosy Hospital of dr. Rivai Abdullah Palembang. The study design used a cross-sectional approach, where the population in this study was outpatients in 2017. Data collection using a questionnaire that had previously been tested for validity in 30 respondents on inpatients of RSK. dr. Rivai Abdullah Palembang, samples were taken accidentally as many as 152 respondents in September - October 2018 that met the inclusion and exclusion criteria at Outpatient Polyclinic of RSK of dr. Rivai Abdullah Palembang. Data analysis used chi-square and multiple logistic regression. The results of the study showed that the 7 variables (product, price, promotion, place, people, process and physical evidence) there were 3 variables that were significant relationship with patient loyalty, namely promotion (p-value=0.005, OR=6.491), process (p-value=0,000, OR=8,796), and physical evidence (p-value=0,000, OR=12). While the product (p-value=0.335, OR=4,556), price (p-value=1, OR=0,807), place (p-value=1, OR=0,815), people (p-value=1, OR=0.815) showed there was no significant relationship with patient loyalty. The most dominant variable relationship with outpatient loyalty at Leprosy Hospital of dr. Rivai Abdullah Palembang was a physical evidence variable (OR=19,867, 95% CI 4,010 98,429).

Abstract

The purpose of this study was to determine the performance or success rate of stunting prevention program implementation related to the characteristics of the institution structure, network and communication, community culture, community needs, and external networks at the Air Beliti Community Health Center, Musi Rawas Regency. The method of this research is quantitative research. The research was conducted at the Air Beliti Community Health Center, Tuah Negri District, Musi Rawas Regency. It was carried out from March to May 2021. The number of samples of the stunting program working team at the Air Beliti Community Health Center was 43. The results of research on factors related to the success of the stunting program at the Air Beliti Community Health Center, Musi Rawas Regency, were the characteristic variables of the institution's structure, network and communication, and community needs. The factors that are not related to the success of the stunting program are organizational culture and external networks. The conclusion of the implementation of the stunting prevention program at the Air Beliti Community Health Center, Musi Rawas Regency in general, has been going well, with positive community responses. Stunting program implementation factors that can affect the success of the stunting program are the characteristics of the institution structure, network and communication, and community needs.

Abstract

Latar Belakang: Kejadian stunting di negara berkembang terjadi pada sepertiga anak di bawah 5 tahun, dan 14% kematian anak-anak disebabkan oleh stunting. Menurut Unicef (2003) stunting sangat berhubungan dengan kemiskinan. Tingkat kemiskinan di Kota Palembang pada Maret 2018 tercatat sebesar 12,18 persen (BPS, 2018). Dari aspek biologis, tinggi badan ibu menjadi penting untuk diteliti karena orang tua tentunya dapat menurunkan stunting dari gen pembawa kromosom sebagai penentu fisik anak, ditambah tidak adekuatnya asupan gizi (Kuku, 2011). Dari aspek psikologis, stunting erat kaitannya dengan pola asuh yang tidak memadai. Menurut Hurlock (1999) pola asuh dapat dipengaruhi oleh kepribadian ibu. Aspek psikologis lainnya yaitu, usia persalinan (Candra, 2011) dan peran ganda (Andiani 2013).Tujuan: Penelitian ini bertujuan untuk melihat  hubungan antara aspek biologis yakni dari tinggi badan ibu dan aspek psikologis yakni usia persalinan, peran ganda dan kepribadian ibu terhadap stunting.Metode: Penelitian ini berjenis kuantitatif dengan pendekatan cross sectional yang dilakukan di kota Palembang pada 110 ibu dari keluarga miskin yang memiliki balita usia 24-59 bulan yang dipilih secara simple random sampling. Pengumpulan data usia, peran ganda, dan keperibadian ibu dengan menggunakan kuisioner, untuk variabel tinggi badan ibu dan stunting dengan pengukuran menggunakan microtoise.Hasil: Didapatkan bahwa p value tiggi badan ibu adalah  0,006 (p<0,05) dengan OR 3,606 (CI 95: 1,517-8,572) dan kepribadian ibu dengan nilai p value 0.035 (p<0,05) OR: 0,27 (CI 95:0,086-0,849). Sementara itu, usia persalinan terhadap stunting didapatkan p value 0,99 (p>0,005) dan peran ganda terhadap stunting didapatkan p value 0,464 (p>0,005).Kesimpulan: Terdapat hubungan yang signifikan antara tinggi badan ibu dan tipe kepribadian ibu terhadap stunting, sementara itu untuk usia persalinan dan peran ganda diketahui tidak berhubungan terhadap kejadian stunting. Disarankan dapat dilakukan modifikasi dalam penyuluhan dan pemberian informasi kepada masyarakat menurut tipe kepribadian ibu.

Abstract

Latar Belakang: Usia yang rawan terkena penyakit terjadi pada anak-anak dan remaja. Pemerintah berusaha menciptakan lingkungan sehat, menyebarkan pengetahuan kesehatan dan menyediakan fasilitas layanan kesehatan melalui UKS. Di daerah endemik malaria, anak sekolah dasar rentan mengalami anemia. Kondisi lingkungan endemik dan kurangnya asupan nutrisi meningkatkan terjadinya anemia. Di Kabupaten Seluma tahun 2019 didapatkan 21,08% dari 137 anak sekolah dasar menderita anemia.Tujuan: Melakukan analisis pemanfaatan UKS dalam pencegahan anemia pada anak sekolah dasar di daerah endemik malaria.Metode: Penelitian kualitatif, pemilihan informan secara purposive. Pengambilan data dengan wawancara mendalam kepada 10 informan, observasi dan telaah dokumen. Triangulasi sumber, metode dan data. Analisis data dengan cara mereduksi, menyajikan, menarik kesimpulan dan memverifikasi.Hasil: Pemanfaatan UKS melalui pendidikan kesehatan dilakukan dengan memberikan informasi kesehatan oleh guru dan petugas kesehatan. Pemanfaatan UKS dalam pelayanan kesehatan untuk mencegah anemia dengan memantau status gizi anak sekolah dasar dan pemeriksaan konjungtiva, namun belum maksimal karena hanya dilakukan sekali setahun dan belum ada pengecekan kadar hemoglobin. Sarana dan prasarana program UKS untuk pemantauan anemia belum lengkap dan pemanfaatannya terbatas.Kesimpulan: Pemanfaatan UKS dalam pencegahan anemia belum optimal. Sekolah diharapkan meningkatkan pemahaman guru tentang pencegahan anemia melalui kerjasama dengan Puskesmas.
