Abstract

Latar belakang: Keselamatan pasien merupakan masalah kesehatan masyarakat yang serius. Berbagai studi melaporkan masalah ketidakamanan dalam sistem pelayanan kesehatan di berbagai negara. Dampak yang ditimbulkan pun beragam. Mulai dari kesakitan ringan, kecacatan, kematian hingga berdampak pada besarnya biaya pelayanan. Tulisan ini mencoba mengkaji budaya keselamatan pasien dari beberapa penelitian dan mengidentifikasi faktor yang mempengaruhinya.Metode: Studi ini dilakukan dengan cara pemetaan sistematis terkait budaya keselamatan pasien. Sejumlah 40 artikel penelitian dievaluasi dari berbagai sumber informasi online: Pubmed, MEDLINE, Web of science dan google scholar. Penelusuran dilakukan dengan memasukan kata kunci sesuai dengan topik yakni budaya keselamatan pasien dan faktor yang berpengaruh. Dari hasil yang diperoleh, dianalisis dan dibahas hingga menghasilkan kesimpulan.Hasil Penelitian: Masalah Kejadian yang Tidak Diinginkan (KTD) masih sering terjadi. Tenaga kesehatan memiliki peran dalam menciptakan pelayanan yang aman dan bermutu. Salah satunya melalui budaya keselamatan pasien. Beberapa faktor yang mendukung budaya keselamatan pasien terdiri dari kepemimpinan, kerjasama tim, perawatan pasien berdasarkan bukti, komunikasi, pembelajaran, tepat dan berfokus padapasien.Kesimpulan: Keselamatan pasien belum menjadi budaya dalam organisasi layanan kesehatan. Kejadian yang Tidak Diinginkan (KTD) seperti fenomena gunung es. Upaya pengembangan faktor budaya keselamatan pasien perlu terus digalakkan oleh organisasi pemberi pelayanan kesehatan.

Abstract

Latar Belakang: Proporsi sektor informal mendominasi angkatan kerja di Indonesia. Namun keberadaan mereka belum sepenuhnya ter-cover dalam sistem jaminan kesehatan nasional. Kebijakan jaminan kesehatan di Indonesia terus mengalami perubahan. Dalam rangka upaya pencapaian universal health coverage, maka perlu dilakukan kajian terhadap kebijakan jaminan kesehatan. Studi ini bertujuan untuk menganalisis kebijakan publik jaminan kesehatan pada sektor informal.Metode: Kami melakukan telaah dokumen kebijakan jaminan kesehatan & studi literatur empiris yang terkait sektor informal. Analisis kebijakan menggunakan triangle analysis Hasil Penelitian: Hasil analisis triangle menunjukkan bahwa berdasarkan aktor, peraturan JKN yang ada telah melibatkan peran pelaku kebijakan resmi sedangkan kelompok berkepentingan (serikat pekerja sector informal dan paguyuban sektor informal) belum optimal diikutsertakan. Kebijakan pun belum mampu membiayai pelayanan yang membutuhkan biaya pengobatan yang besar misalnya penyakit langka. Secara proses, lahirnya kebijakan yang ada melalui proses top down dan dipengaruhi berbagai faktor ekonomi, politik dan internasional.Kesimpulan: Pelibatan sektor informal dalam penyusunan kebijakan jaminan kesehatan masih minim. Kebijakan tersebut perlu dikembangkan dengan memperhatikan konteks sektor informal sehingga nantinya diharapkan mampu mencapai universal health coverage.

Abstract

Background: World Health Organization (WHO) showed two of 10.000 women in Indonesia suffer from cervical cancer and an estimated 26 women die each day because cervical cancer. The low early detection performed by productive women lead to cervical cancer incidence is increasing each years. One of prevention cervical cancer is early detection through examination of visual inspection with Acetic Acid (VIA). This study aimed to determinant analyze on productive women towards participation in the VIA in Palembang.Methods: The research method used cross-sectional design, the sampling technique using proportional accidental sampling consisted of 165 respondents which on the inclusion and exclusion criteria . The data collection was done by using a questionnaire home visit CPC-28 Questionare. The data analysis using chi-square and multiple logistic regression. Results: Results showed the proportion of productive women who participated inspection IVA is only 7.9%. Based on the results obtained by multiple logistic regression analysis found significant relationship between education (p value = 0.037; OR = 0.27) and perceived barriers (p value = 0.015; OR = 13) against participation in the IVA examination in productive women in Palembang. Multivariate analysis showed that the perception of barriers is the dominant factor affecting participation in VIA examination in productive age women in Palembang. It is therefore necessary strategies to overcome perceived barriers by women as socialization.Conclusions: Multiple logistic regression analysis found a significant relationship between education and perceived barriers against participation in the IVA examination in productive age women in Palembang.

Abstract

Background: Child’s health becomes the government concern in developing countries. Improving child’s health is a challenge for developing countries due to poor health care. Children tend to need more health services as they are more susceptible to various diseases. This study was aimed to determine the determinants influencing outpatient services utilization on children in Indonesia. Method: This research was a descriptive with cross-sectional design research using quantitative approach. The data were obtained from secondary data of the Indonesian Family Life Survey (IFLS) 2014. The dependent variable was the number of health service visits on children. In this study, the samples were 2,472 0-14 years old individuals selected according to inclusion and exclusion criteria. The data were analyzed using binomial negative regression. Result: The study found out that child using health services at least one outpatient visit in the last 4 weeks was 75.77%. Woman, perception of ill, parental formal employment, parental education (high), economic status were statistically related to utilization of health service for outpatient Conclusion: The perception of ill greatly affects the children in outpatient health service utilization, it is expected that government gives policy by family approach as the effort of family member caring, health protection and improvement especially for child.

Abstract

Customer satisfaction becomes very important to determine the success of service companies. One way to survive in the healthcare industry today is to create customer satisfaction through improved service quality. Hospital cost is an indicator for patients to utilize health services. This study aims to analyze the relationship of hospital cost and service quality with patient satisfaction. This study used analytic survey with a cross-sectional study design. The sample of this study is the general patient who used inpatient and outpatient installation of Bhayangkara Hospital in Palembang as many as 117 patients. The sampling technique used systematic random sampling. Analysis of statistical data used the t-test. The results showed that the quality of service has a significant relationship with patient satisfaction (p-value 0,001). While the hospital cost does not have a significant relationship with satisfaction patients (p-value 0,368). Service quality has relationship with patient satisfaction. The hospital should improve dimension of service quality in order to improve patient satisfaction.

Abstract

The use of contraceptives in Indonesia among married women has tended to be stagnant in the last 3 years. Indonesia's 2017 demographic and health survey report reports that the prevalence of modern contraceptive use was lower among women in urban areas (59%) than in rural areas (55%). This study aims to analyze the determinant of modern contraceptive use in married women in Indonesian urban. This study is a quantitative study with a cross-sectional design. We used secondary data from Indonesia Health and Demographics Survey 2017. The population of this study was married women. We got a sample was 15,520 respondents. We analyze the predictor of modern contraceptive use by using logistic regression. Respondents using modern contraceptives were 53.5%.  Age, education level, number of living children, number of ideal children, employment status, economic status, and region have a significant association with modern contraceptive use in Indonesian urban. The most dominant factor associated with modern contraceptive use is the number of living children >2 children. Proper dissemination of information on the use of modern contraceptives needs to be expanded to increase contraceptive use.

Abstract

The current community service focused on the use medicinal plants which seemed to increase among families. This program was supported by the 2018 Riskesdas report, which showed that 38.7% of Indonesia’s population had used traditional medicine for treatments. This program sought to discover health workers’ knowledge of family medicinal plant training. This community service was carried out through the mentoring method and involved 25 public health center employees representing each primary healthcare in Ogan Ilir Regency. We selected the sample using the total sampling method. The mentoring session was organized in the form of training related to the concepts of family medicinal plants and how to use them. This training involved a number of lectures and presentations of a model of family medicinal plants. The differences in knowledge of the training participants were measured using a pretest and posttest. The data were analyzed using the t-test. The results showed a difference in health workers’ knowledge before and after the training (p <0.05). There is an increase in the knowledge of primary healthcare workers regarding family medicinal plants.

Abstract

Penderita hipertensi diperkirakan di dunia mencapai 1 milyar dan dua pertiga diantaranya berada di negara berkembang. Hipertensi sebagai penyebab kematian nomor tiga setelah stroke dan tubercolosis, jumlahnya mencapai 6,8% dari proporsi penyebab kematian pada semua umur di indonesia. Hipertensi di Kota Batam selalu menduduki urutan pertama dari 17 penyakit tidak menular selama 5 tahun terakhir dan menempati urutan pertama sebesar 17% pada tahun 2016 di PT. Batamec Kota Batam. Kebisingan akibat kerja dapat berhubungan dengan peningkatan tekanan darah. Penelitian ini menggunakan survei observasional dengan pendekatan cross sectional. Populasi dalam penelitian ini adalah seluruh pekerja pria di bagian welding dan mechanical. Pengambilan sampel menggunakan total sampling dengan jumlah 120 pekerja. Analisis data menggunakan chi square dan logistic regression. Analisis t-test menghasilkan tekanan darah sistolik dan diastolik, sesudah dan sebelum bekerja menunjukkan adanya perbedaan yang signifikan yaitu nilai p value = 0,000. Uji korelasi umur memiliki hubungan yang bersifat lemah (p value=0,007; r=0,244). Masa kerja memiliki hubungan hubungan yang bersifat lemah (p value = 0,022; r=0,206). Kebisingan memiliki hubungan yang bersifat lemah (p value=0,048; r=0,181). Hasil penelitian menunjukkan bahwa variabel yang berhubungan dengan peningkatan tekanan darah adalah Umur (p value=0,000; OR=0,062 95% CI 0,025-0,156). Tingkat pendidikan (p value=0,015; OR=0,357 95% CI 0,279-0,456). Masa kerja (p value=0,000; OR=15,138 95% CI 5,330-42,998). Kebisingan memiliki hubungan yang signifikan dengan peningkatan tekanan darah yang menunjukkan nilai p value=0,003; OR=0,202 95% CI 0,071-0,573. Direkomendasikan untuk penggunaan dan pengawasan Alat Pelindung Diri kepada pekerja yang terpapar kebisingan.

Abstract

This study aims to evaluate the input of the puskesmas, including personnel resources (Head of City Health Office, Head of P2P Dinkes, Head of Health Center, COVID manager of Puskesmas, Puskesmas staff, COVID tracer, Community) facilities, funds and methods. The method used is descriptive qualitative. The results showed that the COVID-19 surveillance staff aspect was not appropriate for educational qualifications; the method used was following the guidebook such as the availability of prevention and evaluation guidelines, the available facilities/infrastructure in terms of quantity or quality, the communication tools were adequate and the equipment was out of stock medical use is also good, but PPE clothes are limited. The method for achieving the target of finding suspected COVID-19 is suitable. Funds used in the implementation of surveillance activities are sourced from APBD funds and the amount of the budget follows the needs, for funds for equipment are dropped from the health office. The implementation of case findings has been carried out according to procedures, and supervision of the increase in cases has also been carried out correctly. In conclusion, indicators of the success of implementing COVID-19 epidemiological surveillance at the Jambi City Health Center can be seen from the achievement of the set targets and the achievement of testing targets for case finding. The reason for not achieving the targets in testing and tracing according to informants' statements is because of many rejections in the Community. Keywords: COVID-19, Evaluation, Surveillance

Abstract

Background: Hypertension is a worldwide public health problem, mainly due to its high frequency and risks leading to cardiovascular diseases. The prevalence of hypertension in the Indonesian population aged > 18 years in 2018 was 34.11%. The unmet need for healthcare has generally been explored in most empirical studies concerning people with hypertension. Aims: This study investigated the determinants of unmet needs for healthcare among people with hypertension. Methods: The design of this study was cross-sectional on data from the Indonesian Family Life Survey wave 5 (IFLS-5). The survey sample was 6,302 adults aged > 40 years stratified by rural/urban residence status. A three-level multilevel analysis was performed to estimate the individual, household, and community-level determinants of unmet needs for hypertension care. Results: As many as 78.4% of the respondents with hypertension reported unmet needs for healthcare. Age, female, single, income, having insurance, urban residence, and the number of health-integrated posts for the elderly (Posyandu Lansia) were significantly associated with umeet needs for healthcare utilization among people with hypertension, while education and employment status showed no association with these variables. Conclusion: Improvement in access to healthcare and reduction in health inequality is required to address this problem.
