Abstract

Patient centered care (PCC) merupakan bentuk pelayanan kesehatanyang menempatkan pasien sebagai center atau pusat pelayanan.PCC sebuah hal baru di dalam sistem pelayanan keperawatan diIndonesia, dan masih terbatas penelitian terkait PCC di ICU.Pengetahuan perawat tentang PCC menjadi komponen pentingdalam implementasi PCC di ICU dengan kompleksitas perawatanpasien. Tujuan penelitian ini adalah untuk mengetahui tingkatpengetahuan perawat tentang PCC di ruang ICU RS di kabupatenkebumen. Desain penelitian ini deskriptif crossectional. Sampelpenelitian ini adalah perawat ruang ICU RS di kabupaten kebumensejumlah 41 responden, dengan tekhnik total sampling. Instrumendalam penelitian ini memodifikasi 8 dimensi PCC menurut HarveyPicker (1988). Tekhnik analisa data yang digunakan yaitu analisisunivariat dengan menampilkan tabel distribusi frekuensi. Hasilpenelitian menunjukkan bahwa sebagian besar perawatberpengetahuan baik tentang PCC (90,2%). Penelitian ini dijadikandata dasar untuk mengetahui tingkat pengetahuan perawat ICUtentang PCC.

Abstract

 Introduction: Recurrence of coronary heart disease (CHD) occurs due to non-compliance patients in managing CHD. One predictor of compliance with CHD management is illness cognition (IC), and IC can be improved through illness stimuli that can be done through health education in this study through workbooks. Purpose: This study aimed to identify the effect of the workbook on illness cognition in CHD patients. Methods. This research was a quasi-experimental with pretest-posttest control design. A pretest was done to the patient who was treated in the Cardiac Intensive Unit; posttest was carried out at the end of the first and second month after the pretest. The population was all post-acute CHD patients who were admitted to the cardiac intensive unit in one of the referral hospital in West Java, Indonesia. Purposive sampling was used and obtained 39 respondents who were divided into control and intervention groups. The instrument used was the illness cognition questionnaire (ICQ). Data were analyzed using the mean, median, percentage, and to estimate the effect of the workbook to the patients' IC used Wilcoxon and Friedman test. Results: The results showed an increase in acceptance and perceived benefits for both groups while in the helplessness showed the decrease scores. The p-value of IC comparison in the pre-test, post-test I, and II between groups for helplessness, acceptance, and perceived benefits were 0.118; 0.376; and 0.424, respectively. Besides, the p-value for differences of IC within control and intervention groups was less than 0.05. Conclusions: The workbook in this study generally did not provide significant IC changes to respondents.

Abstract

The provision of phase I cardiac rehabilitation intervention in patients with cardiovascular disease needs to be given because it can accelerate recovery and determine the needs of the next phase of cardiac rehabilitation, namely phases II, III and IV.  The aim of this study was to find out the impact of phase 1 cardiac rehabilitation on acute coronary syndrome patients. This literature study examined scientific articles published from 2011 to 2021 and in Indonesian and English. Data was obtained from the database includes PubMed, Cinahl Ebscohost and Google Scholar search engine using keywords developed based on population, concept and context namely Cardiac Patients, Phase 1 of Cardiac Rehabilitation, Outcomes, Benefits and Effects. A total of 275,000 articles were obtained and after being selected based on duplication, title, abstract and inclusion criteria were obtained 9 articles that are eligible for analysis. The results of the study found that phase 1 cardiac rehabilitation in SKA patients was shown to have good impact and outcomes, such as improving quality of life and self-efficacy, reducing the need for the second phase of rehabilitation, and reducing the level of anxiety and depression experienced by SKA patients. Therefore, the results of this literature review can be the basis of the next research in the implementation of phase 1 cardiac rehabilitation in Indonesia.

Abstract

There are many cases of Sudden Cardiac Arrest (SCA) in the community that cannot be rescued, because they did not get the proper and fast action due to ignorance and incompetence of the community. The purpose of this training was  increasing the knowledge, attitudes, intentions, and abilities of the community in identifying and handling SCA in Cileles Village, Jatinangor District. The community service activities were integrated with student’s KKN activities. This activity was carried out starting from the initial assessment related to the CPR ability of  the community, socialization to KKN students, preparation of materials and learning media, the implementation of socialization and training activities for the community, and also this evaluation. At the implementation stage, 33 training participants participated. Pre-test and post-test measurements were carried out with the response rate as much as 90.9%. As much as 53.3% of respondents had never received information about cardiac arrest, and 93.3% had never received information about CPR before. After training, the respondents' average knowledge score was 75.7. On the attitude aspect, before training, 90% of respondents supported the implementation of CPR and after training, 96.7% of participants supported the implementation of CPR. Meanwhile, before the training, no participants intended to do CPR if they found victims of SCA, but 100% of participants intended to do CPR after training. There was an increase in knowledge, attitudes, and community intentions related to the handling of cardiac emergencies, especially cardiac arrest. This was an indication that CPR will be carried out by respondents if they found victims of SCA. Similar training needs to be conducted periodically to prevent the decline in knowledge or skills related to CPR actions.Keywords: Pemberdayaan Masyarakat, Kegawatdaruratan, Sudden Cardiac Arrest.

Abstract

Hypertension is a health problem that needs attention in Indonesia. To decrease the incidence of hypertension, we could give health education to the community. Provision of information and education through learning media such as booklet was one effort to empower the community in preventing and performing hypertension treatment at home. Increasing health promotion through the provision of information and education using learning media was expected to improve the knowledge and self-efficacy of the community that will affect the behavior of the community. Pangandaran Regency is one of the target areas of Padjadjaran University where hypertension is one of the highest prevalence diseases. The purpose of this activity was to improve knowledge of community self-efficacy about prevention and treatment of hypertension. This activity was carried out from community surveys, booklet preparation, booklet socialization, and evaluation. At the implementation stage, the activity was followed by 22 people and 13 of them had hypertension. People who had hypertension were given hypertension management booklet then their knowledge and self-efficacy evaluate after one to two weeks. The result showed that there was an increase in the average knowledge and self-efficacy of them after the program. Besides that, there were differences in an average score of public knowledge before and after the giving of booklets, but there was no difference in the average self-efficacy score. This booklet can be used to provide health education to the community related to prevention and treatment of hypertension, so hopefully, there will be changes in behavior to manage hypertension.Keywords: Booklet, learning media, hypertension, knowledge, self-efficacy

Abstract

The high and varied disasters occur in Indonesia, requiring the government and the whole community to be ready for disaster. In the Faculty of Nursing Universitas Padjadjaran (Unpad), there is a group of students who do extracurricular activities to provide health aid called PNC. Training needs to be done to enhance the role of PNC as well as increasing the participation of students in the disaster relief program. This training resulted in increased knowledge and attitudes of PNC members on disaster management, and also increased the knowledge and attitude related to the role and function of students in disaster management effort. In addition, other additional results related to the effectiveness of training methods for PNC members successfully obtained. The methods compared were lecture and discussion (LD) compared with LD plus simulation and FGD methods. It is measured by using the quasi experimental method with pretest-posttest control group design. Data analysis used Mann Whitney test. The result showed that there was no difference in knowledge and attitude (p = 1,000) in both treatment groups (p = 0,424). Which means that there was no difference between LD method and LD plus simulation and FGD method.Keywords: Training, Disaster Management, Padjadjaran Nursing Corps (PNC)

Abstract

Management airways become one important part in an act of care in patients with critical conditions due to disease. One attempt to keep the airway is to intubate, is inserting a pipe into the upper respiratory tract. The main requirement that must be considered is to keep the airway is always free and breath can run smoothly and orderly. The final stage of intubation was extubated. The purpose of this study was to determine what factors are associated with outcome of extubation in ICU Prof. Dr. Margono Soekardjo Purwokerto Hospital. This study is a cohort retrospective with  analytic correlational design.The sample in this study amounted to 96 people. Bivariate test results that there is a relationship between age (p = 0.000), tidal volume (p = 0.001), systolic blood pressure (p = 0.000), FiO2 (p = 0.007) and PEEP (p = 0.014) with outcome of extubation. There is no associated GCS (p = 0.358) with outcome of extubation. Therefore, nurses need to give special attention regarding the condition of the patient, such as age characteristics, the value of tidal volume, systolic blood pressure, FiO2 and PEEP before extubation. Keywords: outcome, extubation, ICU

Abstract

Background: Interprofessional education and collaborative practice (IPE/IPC) are essential for preparing students to work together and respect the unique qualities and abilities of professionals. However, IPE/IPC and its related concepts are highly abstract phenomena and complicated to assess and measure. In consequence, a critical appraisal is needed to evaluate the quality of the instruments. Purpose: This study aimed to critically appraise, compare and summarize the quality of measurement properties of all self-report collaboration questionnaires for health professional students and to provide evidence concerning the psychometric properties of the measurement. Methods: A psychometric review was employed, and the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) approach was applied to assess the methodological quality of the nature of the measurements. Data search using keywords: health professional students, interprofessional, collaboration, teamwork, collaborative, through MEDLINE, EMBASE, and EBSCO-hosted Education Resource Information Centre databases. Results: Seven instruments from 10 reviewed studies were identified. Among them, four instruments targeted attitudes toward collaboration. One instrument focused on students' collaborative learning readiness and had been tested in Hong Kong using English, in Iran using Persian, and in Indonesia using Bahasa Indonesia. One instrument measured perception about IPE, and two studies measured IPE/IPC competencies related to patient-centered, team-based, and collaborative care. The methodological quality assessment indicated that several instruments were less rigorously developed and validated than COSMIN and Quality Criteria of Measurement Properties recommend. Conclusions: The findings of this psychometric review are that the Interprofessional Attitudes Scale is an instrument with adequate content validation and very good structural validity, internal consistency, cross-cultural validity, reliability, measurement error, and criterion validity. It is recommended that the Interprofessional Attitudes Scale be used to measure the interprofessional attitudes of health professional students.

Abstract

Aims: Restless Legs Syndrome (RLS) is a neurological sensorimotor disorder that causes a person to feel the need to move their lower extremities due to uncomfortable sensations. This condition can be monitored to minimize and cure the symptoms. One of the monitoring instrument is The Self-administered version of The International Restless Legs Syndrome Rating Scale (sIRLS). In Indonesia, the sIRLS instrument has not been culturally adapted. This research aims to translate and to test the content validity the Indonesian version of the sIRLS, so that it can further contribute to subsequent psychometric research on this instrument. Methods: The sIRLS were translated through forward translation, synthesized translation, back-translation, synthesized back-translation, and expert committee by five translators and three expert judges. Content validity was tested through CVR, I-CVI, and S-CVI for the essentiality, relevancy, and clarity of each item. Results: The first validity test showed a significant value for essentiality and relevancy of all the items (1,00). However the first test showed an insignificant value for the clarity of the seven items (0,33 and 0,66). Therefore the researcher should revise the seven items based on the suggestions from the expert judges and then carry out a second content validity test. Consequently, the final values of the CVR, I-CVI, and S-CVI showed significant values (1,00) for the essentiality, relevancy, and clarity. Discussions: Many Adaptation and modification on a few terms were required to achieve both meaning and cultural equivalence and also to facilitate understanding of the respondents. Conclusion: The Indonesian Version of self-administered version of The International Restless Legs Syndrome Rating Scale (sIRLS) is validly reviewed from the essentiality, relevancy, and clarity of the items.

Abstract

Intra-Aortic Balloon Pump (IABP) can be used among patients with Acute Coronary Syndrome (ACS) who experience Ventricular Septal Rupture (VSR) as a form of mechanical support for hemodynamics that has worsened due to cardiogenic shock. As for the cases reported, a 61-year-old male patient with ACS STEMI Infero-Anterior Extensive Killip 4 and VSR experienced chest pain accompanied by shortness of breath. Auscultatory examination found a heart murmur on 3/6 without thrills. The patient had received drugs to improve hemodynamics and was reported to have used an IABP machine. IABP uses semi-auto mode, 1:1 frequency with maximum augmentation. The patient's condition worsened, and he lost consciousness. The patient then had an endotracheal tube connected to a ventilator. Keywords: IABP, Intra-aortic-counterpulsation, ventricular septal rupture.
