Abstract

Background: Depression is a significant predictor of the quality of life among patients with coronary heart disease. Therefore, it is essential to explore the factors associated with depression. Illness cognition is considered one of the factors affecting depression. However, the relationship between illness cognition and the incidence of depression among Indonesian patients have not been widely investigated.Objective: This study aimed to investigate the correlation between illness cognition, consisting of the acceptance, perceived benefits, and helplessness variables, and depression in patients with coronary heart disease. Methods: This study employed a correlational research design with a total of 106 patients undergoing treatment at a hospital in West Java, Indonesia, selected using convenience sampling. Data were collected using a demographic questionnaire, Beck-Depression Inventory-II (BDI-II), and ICQ (Illness-Cognition Questionnaire). Data were analyzed using mean (SD), median, frequency distribution, and Spearman-rank. Results: 72% of respondents had no depression. Nevertheless, mild, moderate, and major depression suffered by 15%, 9%, and 4% of respondents, respectively. In terms of illness cognition, patients scored higher within the perceived benefits dimension (mean 20.13, SD 3.05), followed by acceptance (mean 18.22, SD 3.33) and helplessness (mean 13.20, SD 4.77), respectively. Furthermore, helplessness was significantly associated with depression (p <.01) with a positive correlation coefficient (r). Also, all items on the helplessness dimension had a significant correlation (p <.01) with depression accompanied by a positive r-value. Conclusion: Helplessness had a significant relationship with depression. So, cardiovascular nurses can anticipate depression in patients by making nursing interventions that can decrease the patients’ feelings of helplessness. Thus, factors that reduce helplessness need to be explored and taken into accounts in the treatment of patients with coronary heart disease.

Abstract

Faktor risiko Penyakit Jantung Koroner paling umum adalah hipertensi. Hipertensi jarang diketahui gejala awalnya, sehingga hipertensi sering disebut silent disease. Pengobatan yang diberikan pada gejala awal hipertensi akan menurunkan terjadinya komplikasi. Mengidentifikasi tanda gejala hipertensi dan dampak ketidakpatuhan pasien hipertensi terhadap kejadian Coronary Artery Disease menjadi tujuan dari penelitian ini. Penelitian ini  menggunakan desain case report  dengan melakukan pengumpulan data berupa wawancara kepada klien. Laki-laki berusia 54 tahun memiliki riwayat kolesterol tinggi dan hipertensi sejak tahun 2021, tetapi klien mengabaikan dan tetap menjalani gaya hidup yang tidak sehat. Dua tahun kemudian klien mengalami serangan jantung. Hasil fungsi jantung tidak baik yaitu hanya 40%. Setelahnya, klien rutin kontrol ke Poli Jantung untuk persiapan coronary artery bypass grafting , tetapi gaya hidup klien masih tidak berubah. Januari 2024 klien datang ke Instalagi Gawat Darurat dengan keluhan nyeri hilang timbul di dada kiri dengan skala 4/10. Kemudian klien dirawat inap untuk operasi coronary artery bypass grafting. Tanda gejala hipertensi yang paling umum adalah sakit kepala. Ketidakpatuhan dalam pengobatan dan kontrol rutin hipertensi meningkatkan resiko kematian kardiovaskuler. Deteksi dini tanda gejala hipertensi menjadi penentu dalam pengobatan hipertensi. Hipertensi yang diobati pada gejala awal dan patuh pada pengobatan menurunkan resiko komplikasi kardiovaskuler.

Abstract

Cardiovascular disease in Indonesia has undergone a vast epidemiological transition in the last three decades, with an increase in the number of deaths and disabilities, resulting in the disease burden and high healthcare costs. Community empowerment strategies through educational and participatory approaches are part of efforts to prevent and control cardiovascular disease. A health education program called "Quality Life with a Healthy Heart" was held with the aim of improving public’s knowledge, especially in West Java, in recognizing cardiovascular diseases (primarily coronary heart disease) and efforts to prevent them and in providing initial assistance for out-of-hospital cardiovascular emergencies. Up to 148 people participated in the program via online videoconferencing and streaming media platform. Participants' level of knowledge was evaluated before and after participating in the program with an electronic questionnaire consisted of 8 multiple choice inquiries. Data were analyzed from 27 participants who completely filled out the pre-test and post-test questionnaires after received four session health education on webinar using descriptive statistics and the non-parametric Wilcoxon Signed Ranks test. Results showed that as many as 59.3% of participants had good knowledge at the pre-test, while 85.2% of participants had good knowledge at the post-test. There was a significant difference between participants' level of knowledge before and after receiving the health education (Z = -3.933, p<0.001). It can be concluded that the program improved public's knowledge of cardiovascular health. Further educational initiatives are recommended to facilitate community skills in providing first aid for cardiovascular emergencies through Basic Life Support. Optimizing the use of social media can be an option to increase public health literacy. Keywords: Cardiovascular disease, health education, knowledge, public.

Abstract

Pendahuluan: Penyakit katup jantung dapat mengakibatkan penurunan fungsi jantung yang berujung pada gagal jantung. Proses terjadinya penyakit dapat berlangsung bertahun-tahun sehingga klien baru menyadari setelah munculnya tanda gejala. Tujuan dari penulisan kasus ini yaitu menggambarkan gejala yang dirasakan seorang ibu dengan kerusakan katup mitral yang menggunakan katup jantung buatan. Metode: Case report ini disusun melalui pengumpulan data secara langsung dan berdasarkan rekam medis untuk mengumpulkan data terkait tanda gejala yang dirasakan sebelum dan selama pemakaian katup jantung buatan. Deskripsi kasus: Seorang ibu berusia 69 tahun memiliki riwayat penyakit katup jantung sejak tahun 2012. Gejala yang dirasakan pende sebelum pemakaian katup jantung buatan yaitu dispnea, ortopnea, intoleransi aktivitas dan edema pada ekstremitas yang merupakan tanda gagal jantung. Penanganan yang diberikan pada klien yaitu penggantian katup mitral dengan katup jantung sapi yang digunakan oleh klien selama lebih dari 10 tahun. Setelah operasi pemasangan katup jantung buatan, klien tidak mengalami keluhan sesak dan dapat beraktivitas kembali seperti semula. Kesimpulan: Pemakaian katup jantung buatan dapat mengatasi keluhan yang dirasakan oleh klien dan meningkatkan harapan hidup klien. Komplikasi dari penggunaan katup jantung buatan dapat dicegah dengan melakukan pemeriksaan klinis tiap tahun sebagai bentuk deteksi dini apabila terjadi komplikasi pada klien

Abstract

Many emergency situations require immediate first aid. The community plays a crucial role in saving victims of cardiac arrest or traffic accidents, which are among the leading causes of premature death in Indonesia. Unfortunately, the level of public knowledge and preparedness in providing first aid remains low, highlighting the need for improvement through education and training. Community health cadres were chosen as the target group because they are closely connected to the community, easily accessible, and can act as first responders before professional medical personnel arrive. This study aimed to enhance the knowledge and readiness of community cadres in recognizing and providing effective first aid through CPR and first aid training in Pangandaran Village. A pre-experimental one-group pre-test and post-test design was used, involving 33 participants selected through accidental sampling. Data analysis was conducted using a paired sample t-test. The results showed a significant improvement (p<0.05) in the cadres’ knowledge and preparedness after the training. Based on these findings, it is recommended that similar training programs be conducted regularly by health institutions and community organizations to strengthen the capacity of cadres in providing first aid, thereby increasing the survival rate of victims of accidents and cardiac arrest within the community.Keywords: Accident, cardiac aArrest, knowledge, preparedness.

Abstract

  Kanker payudara merupakan jenis kanker dengan kasus terbanyak secara global dan memberikan dampak serius terhadap kesehatan mental serta kepatuhan pasien terhadap terapi. Pasien sering menghadapi kecemasan, depresi, dan gangguan psikologis lain yang dapat menghambat proses pengobatan. Telehealth menawarkan solusi dukungan jarak jauh, namun masih memiliki keterbatasan dalam hal personalisasi dan keberlanjutan layanan. Integrasi kecerdasan buatan (Artificial Intelligence/AI) ke dalam sistem telehealth dinilai memiliki potensi besar untuk meningkatkan efektivitas intervensi melalui deteksi dini, pemantauan real-time, serta pemberian dukungan yang lebih responsif dan individual. Manuskrip ini merupakan protokol dari scoping review yang bertujuan untuk memetakan jenis, konten, dan pendekatan intervensi telehealth, baik yang berbasis AI maupun tidak, dalam mendukung kesehatan mental dan kepatuhan terapi pada pasien kanker payudara. Kajian disusun berdasarkan pedoman PRISMA-ScR dan menggunakan kerangka PCC (Population: pasien perempuan dengan kanker payudara; Concept: intervensi telehealth dan/atau AI; Context: dukungan psikologis dan kepatuhan terapi). Pencarian literatur dilakukan pada delapan basis data utama: PubMed, Scopus, Web of Science, ProQuest, CINAHL, EBSCO, ScienceDirect, dan Google Scholar, dengan rentang publikasi 2015–2025. Temuan dari review ini diharapkan dapat menjadi dasar pengembangan intervensi digital yang lebih efektif, personal, dan berkelanjutan, khususnya di wilayah dengan keterbatasan layanan kesehatan.

Abstract

Tetanus, an infection caused by Clostridium tetani, remains a global health concern due to its high fatality, morbidity, and mortality rates. Although the spores of this bacterium are commonly found in soil and animal waste, contamination of the oral cavity due to poor oral hygiene and inadequate dental care also presents a significant risk. The aim of this study was to describe the role of dental infection in the incidence of tetanus in the intensive care unit. This study used descriptive study design with case report method. A 67-year-old male patient was admitted for 38 days to the intensive care unit of a government hospital in Bandung due to grade III generalized tetanus, with Ablett grade IV classification and Tetanus Severity Score (TSS) of 7. The patient had a history of stab wounds from wood and multiple plaque (+) in the oral cavity due to poor oral hygiene and had a habit of cleaning teeth using sticks that were not clean and not in accordance with recommended dental care tools.. History of tetanus vaccine with incomplete dose. During treatment, the patient required mechanical ventilator assistance with CPAP-PS mode. Conclusion of this research that the route of entry for tetanus-causing bacteria can be through wounds, including wounds in the oral cavity that often go unnoticed. Therefore, it is very important to maintain oral hygiene through the application of proper oral hygiene and eliminate the habit of cleaning teeth with dirty tools.

Abstract

Pulmonary tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. A healthy lifestyle, including adherence to Anti-Tuberculosis Drugs, quitting smoking, a balanced diet, and regular physical activity, is essential for eradicating the bacteria. This study used a case report method focusing on a 55-year-old male patient with MDR-TB and type 2 diabetes mellitus who was admitted to the Medical Intensive Care Unit. The patient presented with complaints of productive cough, fever, and fatigue. His condition worsened until death, with a history of unhealthy lifestyle habits such as smoking, poor diet, lack of physical activity, and non-adherence to Anti-Tuberculosis Drugs. The management of Multi Drug Resistant Tuberculosis requires integration between pharmacological therapy and healthy lifestyle habits. Family education and lifestyle interventions are crucial to improve medication adherence, treatment success, and the recovery process in patients with Multi Drug Resistant Tuberculosis. Adopting a healthy lifestyle can strengthen the immune system, prevent disease progression, and support the recovery of patients with Multi Drug Resistant Tuberculosis and Type 2 Diabetes Mellitus.

Abstract

Introduction: Hyperkalemia often occurs in patients with acute kidney injury due to decreased ability to excrete potassium. This case report aims to describe treating hyperkalemia in patients with acute kidney injury (AKI) after septic shock. Method: Case reports detailed the management of hyperkalemia correction using a combination of insulin with 40% Dextrose solution, Ca Gluconate, and hemodialysis therapy. Results: A 64-year-old male patient was admitted to the GICU due to septic shock and AKI after undergoing surgery to extract the 36th and 38th teeth accompanied by a retropharyngeal abscess. The physical examination results showed palpitations, tachycardia, hypertension, and grade 1 pitting edema in the upper extremities. The laboratory checks showed hyperkalemia, and an ECG showed sinus tachycardia. The nurse carried out collaborative therapy for potassium correction using a combination of 10-unit insulin and 40% Dextrose, as much as 2 flacons, and Ca Gluconate. Conclusion: Correction of hyperkalemia used a combination of 10-unit insulin and D40%, and Ca Gluconate therapy was the most widely used intervention because it had a rapid onset and hypokalemic effect. Unfortunately, this therapy could not overcome refractory hyperkalemia in these cases, so the patient was advised to undergo hemodialysis therapy.

Abstract

Breast cancer is the most commonly diagnosed cancer and the leading cause of cancer death in women, therefore breast cancer is the most common cancer in women in most countries. Providing adequate information needed by patients can reduce anxiety and feelings of insecurity. Most of women with breast cancer stated that the information obtained was unsatisfactory. However, few studies have comprehensively addressed the information needs of women with breast cancer across all stages of the disease. This scoping review aimed to fully understand the unique information needs of breast cancer patients at various stages of the cancer continuum. A scoping review will be conducted following PRISMA guidelines. Relevant studies will be identified from databases such as PubMed, Scopus, and Taylor & Francis, EBSCOhost, and ScienceDirect. In addition, searches via Google Scholar, interpersonal network recommendations, and reference chains will also be conducted. Studies will be selected based on PCC criteria, focusing on breast cancer patients as the population, information needs as content, and at various stages of the cancer continuum as the context. Data extraction and risk of bias assessment will be performed using Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) Checklist. This review is expected to provide comprehensive information needs of breast cancer patients at various stages of the cancer continuum. The findings will be valuable for educational institutions, healthcare facilities, in designing innovative, technology-based educating programs that enhance knowledge retention, practical skills, and psychological preparedness.
