Abstract

Latar belakang: Kanker serviks merupakan masalah kesehatan global dengan angka morbiditas dan mortalitas yang tinggi, termasuk di Indonesia. Deteksi dini melalui pemeriksaan Pap smear masih menjadi metode yang sering dipakai, namun akurasi hasilnya sangat bergantung pada teknik pengambilan sampel yang adekuat. Tujuan: Pengabdian masyarakat ini bertujuan untuk Meningkatkan pengetahuan dan keterampilan tenaga kesehatan di Puskesmas Ngariboyo dalam teknik pengambilan sediaan Pap smear yang memadai, dan Meningkatkan kesadaran dan pengetahuan masyarakat tentang deteksi dini kanker serviks. Metode: Metode kegiatan terdiri dari dua bagian utama. Pertama, pelatihan bagi tenaga kesehatan yang meliputi webinar dan praktikum offline untuk mengukur adekuasi sediaan berdasarkan Sistem Bethesda. Kedua, penyuluhan dan skrining offline bagi 95 warga dan 101 peserta pemeriksaan Pap smear. Hasil Hasil pelatihan menunjukkan peningkatan signifikan pengetahuan tenaga kesehatan (nilai 80-100 meningkat dari 58,82% menjadi 82,35%). Penyuluhan juga berhasil meningkatkan pengetahuan masyarakat (nilai 80-100 meningkat dari 80% menjadi 94,74%). Hasil skrining menunjukkan 100% sediaan adekuat dan 100% hasil Negative for Intraepithelial Lesion or Malignancy (NILM). Kesimpulan: Disimpulkan bahwa program ini efektif dalam meningkatkan kapasitas tenaga kesehatan dan kesadaran masyarakat, sekaligus menghasilkan data dasar skrining kanker serviks yang berkualitas bagi wilayah Ngariboyo.

Abstract

BackgroundDefect in cell cycle control affect cell proliferation and play an important role in pathogenesis of cancer. Recently, some proteins areknown to have an influence in cell cycle proliferation. P16 and CDK4 may have influenced in laryngeal carcinoma. CDK4, anoncogene, can form cyclin D1-CDK4 complex that inactivates pRb so that the cell cycle goes from G1 phase to S phase. P16, atumor suppressor protein, can prevent cyclin D1-CDK4 complex so that inhibit cell proliferation.MethodsThis research was an observational analysis study with paraffin block samples, consists of 3 samples of stage T1 and each 10samples of stage T2, T3 and T4 in Laboratory of Anatomical Pathology of RSUD dr. Soetomo that was collected from 2013-2015.Samples were stained with antibody of p16 and CDK4. P16 and CDK4 expression were assessed based on the percentage andintensity of tumor cells that were stained. The differences between variables were analyzed by Kruskal-Wallis. The correlationbetween variables was analyzed by Spearman correlation test.ResultsP16 expression of carcinoma larynx T1 (88.33±7.64), T2 (85.00±5.27), T3 (61.00±21.83), and T4 (62.00±25.30). Besides CDK4expression of carcinoma larynx T1 (43.33±15.28), T2 (47.00±14.18), T3 (75.00±8.50), and T4 (70.00±9.43). Statistic analysisshowed significant differences between p16 and CDK4 expression on each stade of carcinoma larynx (p=0.017 and p=0.000). Andthere were corelation between p16 and CDK4 expression on each stage of carcinoma larynx (p=0.000 and p=0.000).ConclusionThe lower p16 expression and the higher CDK4 expression showed the higher stage of carcinoma larynx.

Abstract

World Health Organization histopathological grading of meningioma is associated with recurrence and clinical outcome. Grade II meningioma can be difficult to distinguish with grade I especially in case in which mitosis is not easily identified. MCM-2 and Cyclin D1 play crucial role in cell cycle and have been reported overexpressed in many malignant tumors. The aim of this observational analytic study was to analyze the differences and correlation between MCM-2 and Cyclin D1 in various meningioma grading. Immunohistochemystry with MCM-2 and Cyclin D1 was performed on 25 paraffin blocks of grade I, II and III meningiomas at the Laboratory of Anatomical Pathology, Dr. Soetomo Hospital. The immunoexpression are evaluated using Labelling Index, then analyzed statistically. The results showed significant difference in expression of MCM-2 within various meningioma grading (p=0.000) and significant difference in expression of Cyclin D1 within grade I and II also within grade I and III (p<0.050), but not in grade II and III (p>0.050). There was also positive correlation between MCM-2 and Cyclin D1 expression in various meningioma grading (rs=0.683, p=0.000). MCM-2 may play role in distinguishing various meningioma grading. Cyclin D1 can distinguish grade I and II also grade I and III, but not grade II and III. Overexpression MCM-2 was along with Cyclin D1 in various meningioma gradings.

Abstract

Mammary Serine Protease Inhibitor (maspin) is a tumor suppressor gene, a member of the serine protease inhibitor (serpin) family that works by inhibiting motility of cell movement, invasion and metastasis. Maspin expression is expected to be a prognostic factor as well as a predictive factor in mammary tumors. However, in some recent studies, maspin has a variety of expressions. Although it is known that no maspine appears as an indicator of tumor progression and metastasis, recent study has shown that maspine expression is associated with an aggressive phenotype of breast cancer and with a poor prognosis. Correlations between maspine expression and poor prognosis have also been reported in pancreatic, ovarian, thyroid, bladder and lung cancers. Knowledge of the expression and role of this maspin as well as its relationship with the pathogenesis of breast invasive ductal carcinoma is still small. The aim of this study was to look at differences in maspin expression in breast-invasive ductal carcinoma of stage IIA and stage IIIB groups. This research method used analytic observational research with cross sectional approach. The samples were invasive carcinoma of NST paraffin at the Department of Anatomic Pathology of Dr Soetomo Hospital, Surabaya, from January to December 2015. Thirty samples were divided into two groups, namely stage IIA and stage IIIB groups and immunohistochemical examination with maspin antibody was carried out. The difference of maspin expression in stage IIA and stage IIIB was analyzed using Mann-Whitney statistic test. There were significant differences in maspin expression between stage IIA and stage IIIB groups, where stage IIA has a high maspin expression rather than stage IIIB.

Abstract

Highlights: This study provides updated insights into glioma profiles in Indonesia, focusing on a large patient cohort from a major referral hospital. Most gliomas were large tumors located in the cerebrum, with midline shift and heterogeneous contrast enhancement being the primary radiological features observed. Glioblastoma was the predominant type, yet no significant correlation was found between radiological features and histopathological grading, highlighting a potential gap in predictive diagnostics.   Abstract Introduction: Glioma is a common brain tumor that significantly impacts patients’ quality of life by affecting motor, sensory, cognitive, and emotional functions. This study aimed to provide a holistic and comprehensive analysis of glioma characteristics by examining clinical, radiological, and histopathological data from patients treated at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, over five years from 2018 to 2022. Methods: This retrospective cross-sectional study analyzed records of 204 glioma patients from Dr. Soetomo General Academic Hospital, Surabaya, Indonesia (2018-2022). Data included demographics, clinical symptoms, tumor size and location, radiological features, and histopathological grading. Statistical analysis used Fisher’s exact test (p<0.05), and data were processed using the International Business Machines Corporation (IBM) Statistical Package for the Social Sciences (SPSS) version 23.0. Results: The most affected age group was 30-39 years old (19.1%), and it was dominated by male patients (59.8%). Headache was the most common presenting symptom (33.8%). Tumors were predominantly located in the cerebrum (75.9%), and most lesions measured >4 cm (76.9%). Midline shift was observed in 69.1% of cases. Contrast enhancement was described as heterogeneous in 35.8% and homogeneous in 1.5%, while 62.7% of cases lacked enhancement data. Histopathologically, the World Health Organization (WHO) grade 4 gliomas (glioblastoma) were the most frequent (39.22%). No significant association was found between radiological features (midline shift or contrast enhancement) and histopathological grading (p>0.05). Conclusion: Gliomas were predominantly found in male patients and were most common among individuals in their younger adult years. The primary clinical symptom was headache, with tumors frequently located in the cerebrum and typically presenting as large masses. Histopathologically, glioblastoma represented the most frequent and aggressive type identified.

Abstract

Dysplastic gangliocytoma or Lhermitte-Duclos disease (LDD), is a rare benign cerebellar tumor often presenting with nonspecific symptoms such as intracranial hypertension, cerebellar dysfunction, or cranial nerve deficits, and associated with phacomatosis. The characteristic “tiger stripe” sign on magnetic resonance imaging (MRI) is highly suggestive of LDD, though nodular medulloblastoma remains an important differential diagnosis. We report a 51-year-old woman with arm and leg phacomatosis who presented with a right cerebellar hypodense lesion on computed tomography extending frontally. MRI demonstrated the hallmark “tiger stripe” appearance, and histopathology following surgery confirmed LDD. On follow-up, despite evidence of a recurrent mass on MRI, her cranial nerve paresis improved postoperatively. Imaging revealed hypointense striations on T1 and alternating linear hyperintensities on T2 sequences, obscuring the cerebellar folia, features that distinguish LDD from other cerebellar tumors. This case underscores the diagnostic value of MRI, emphasizing early recognition to prevent misdiagnosis and guide surgical planning.
