Abstract

Organizations exist because of human needs through relationships, interactions, communications, socialization to one another to meet common goals. Organizational culture becomes an important part of this influence on organizational effectiveness (a term for the fulfillment of organizational goals). This study aims to determine how the organizational culture conditions of the Kodiklatal Health Unit by using indicators according to the Denison organizational culture model, namely involvement, consistency, adaptability and mission. In addition, a theoretical study on the importance of organizational culture from the background for this research is because the Kodiklatal Satkes is a work unit under the Kodiklatal, which has duties other than carrying out health services for all military and civil servants who serve in Kodiklatal, all students who are carrying out education at Kodiklatal and Family members of military and civil servants Kodiklatal, also carry out health support for every military activity / training carried out by students and members of the Kodiklatal military. The research method is a quantitative method by collecting data using a questionnaire with a sample population of 50 employees of the Kodiklatal Health Unit, processed using descriptive statistics. The results showed that the average overall organizational culture was in the very weak category with the strongest score on the mission sub-variable and the weakest on the involvement sub-variable. Abstract

ABSTRACT The hospital is a health service business, one of which is based on the principle of trust so that service quality problems, patient satisfaction and patient loyalty are factors that determine its success. Various ways are used by organizations to create competitive advantage. One of the most important aspects in creating competitive advantage is increasing the quality of human resources owned. Quality of Work Life is a concept or management philosophy in the fieldgka improvement in the quality of human resources that has been known since the seventies. By realizing and maintaining a healthy QoWL, it will increase employee job satisfaction, reduce the number of employees leaving, increase productivity and improve nursing care outcomes. Analysis of the QoWL factor was carried out using questionnaire data for 120 nurses in the ER department, outpatient and inpatient care. Analysis of QoWL results for satisfaction is in the dimensions of Control at Work and Stress at Work and for employee dissatisfaction is in the dimensions. between employees. From this analysis, recommendations for improvement and enhancement of the quality of a good work life can be made through stress management, existing workforce analysis. Abstract

Implementasi Badan Penyelenggara Jaminan Sosial Kesehatan menyebabkan peningkatan jumlah pasien.. Jika diasumsikan seseorang sudah sakit sehingga tidak memerlukan program pemberdayaan, kelangsungan program pemberdayaan masyarakat akan terancam. Tujuan penelitian adalah menentukan tingkat urgensi pemberdayan masyarakat di bidang kesehatan dalam era Jaminan Kesehatan Nasional menurut pengelola program pemberdayaan masyarakat di level desa. Penelitian ini merupakan penelitian observasional yang dilakukan secara cross sectional yang di lokasi Kabupaten Mojokerto, Jawa Timur Indonesia. Sampel penelitian ini adalah 20 desa di lima wilayah kecamatan. Responden dalam penelitian ini di masing-masing desa adalah 5 orang kader kesehatan,  sehingga ada 100 orang kader.  Pengumpulan data menggunakan metode wawancara dengan menggunakan kuesioner. Analisis data dilakukan secara deskriptif dan analisis korelasi. Adapun teknik sample  yang  digunakan  dalam  penelitian ini  adalah menggunakan teknik purposive sampling disesuaikan dengan tujuan melakukan penelitian. Hasil penelitian menunjukkan pemahaman kader tentang jaminan kesehatan masyarakat kategori cukup (73,0%), Penilaian kader tentang pentingnya kegiatan pemberdayaan masyarakat di bidang kesehatan menilai penting (78,0%). Variabel lama kerja dan pemahaman berhubungan signifikan dengan penilaian urgensi pemberdayaan masyarakat. Kesimpulan Apabila ditelusuri lebih lanjut ternyata yang belum dipahami oleh sebagian besar responden kader adalah dasar hukum JKN, asas sistem JKN, manfaat yang diterima oleh peserta JKN, kepesertaan JKN, pelayanan kesehatan rujukan, fasilitas kesehatan tingkat pertama, dan jenis layanan yang tidak bisa dicover. Kondisi tersebut tentunya perlu diperbaiki agar semua kader kesehatan mempunyai pemahaman yang baik tentang jaminan kesehaan masyarakat karena kader kesehatan adalah ujung tombak kegiatan pemberdayaan masyarakat di level desa.Kata Kunci: Jaminan Kesehatan; Sosial; PemberdayaanAbstractThe implementation of the Health Social Security Administering Body has led to an increase in the number of patients. The existence of an existing community empowerment program will be threatened if there is an assumption that someone is already suffering from illness, so there is no need for an empowerment program. The purpose of the study was to determine the level of urgency of community empowerment in the health sector in the era of National Health Insurance according to the manager of the community empowerment program at the village level. This research is an observational study conducted in a cross-sectional manner in Mojokerto Regency, East Java, Indonesia. The sample of this study was 20 villages in five sub-districts. Respondents in this study in each village were 5 health cadres, so there were 100 cadres. Collecting data using the interview method using a questionnaire. Data analysis was done descriptively and correlation analysis. The results showed that the understanding of cadres about community health insurance was in the sufficient category (73.0%), cadres' assessment of the importance of community empowerment activities in the health sector was considered important (78.0%). The variable length of work and understanding were significantly related to the assessment of the urgency of community empowerment. Conclusions (1) respondents' understanding of Community Health Insurance is quite good (2) community empowerment programs in the health sector are still considered important (3) there is a relationship between age, length of work and understanding of National Health Insurance with an assessment of the importance of community empowerment in the health sector in the era of insurance National Health.Keywords: Insurance, Health, Social, Empowerment

Abstract

Introduction: Regarding pre-hospital pain management focus on the emergency department, safety of analgesics used is one of the concerned areas to be developed clearly. However, the current level of evidence in pre-hospital pain management of injured patients, focusing on the safety and effect of pre-hospital analgesia has not been explored. The aim of this study was to analyze the level of evidence in pre-hospital pain management of injured patients, focusing on the safety and effect of pre-hospital analgesia. Methods: A systematic review was performed in this study with inclusion criteria adult-elderly injured patients in pre-hospital setting and used analgesic agents. Articles were sourced from electronic database Scopus, PubMed, Science Direct, Springer Link and ProQuest, which were searched to identify all the relevant articles published over the last five years in English. Fifteen studies were included are qualified as articles assessment with Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist. Results: Nine studies were Randomized Control Trial, two studies cross-sectional and retrospective, and one study prospective descriptive and cohort. The result showed that many types of analgesic are used or planned in emergency areas, such as morphine, ketamine, fentanyl, NSAID with different route intravenous, intramuscular and subcutaneous with safety and monitoring continuously by healthcare professionals. Conclusion: The use of pre-hospital analgesia for injured patient in pre-hospital setting is commonly used by health workers according to pain management standard and guidelines; planning developments are needed for safety and management as a concern for focus on quality of pain management status.

Abstract

This study aimed to analyze the effect of organizational and individual factors on the level of compliance of nurses and midwives in phlebitis observation. This research is an observational analytic study with a cross-sectional design. Respondents of this study were nurses and midwives (n =48) at Inpatient Installation. Data were obtained from questionnaires and secondary data from the IV Usage Surveillance form. The results showed that policy, guidelines, knowledge, and motivation had a positive effect. In contrast, aspects of standard operating procedures and attitudes had an inverse effect on the compliance of nurses and midwives in observing phlebitis. Some nurses and midwives have the weak motivation and the assessment of nurses and midwives that phlebitis observation activities are not supported by the existence of standard operating procedures related to identifying phlebitis signs/scales. The conclusion of this study is the need for an organizational approach to motivate nurses and midwives and provide clear standard operating procedures so that they can change attitudes to become more obedient.

Abstract

Introduction: The target of new KB participants for the Long-Term Contraception Method (MKJP) that was not achieved and the decline in the achievement of the new KB MKJP is the responsibility of PKB. This is due to the role of PKB as the spearhead of the success of the family planning program in the field. The purpose of this study was to determine the factors of family planning counselor teamwork related to the achievements of the new MKJP family planning participants. Methods: This research is a descriptive quantitative study, using a cross-sectional design. Result: The results of this study found that 14 of the 20 PKB teams with good leadership teams experienced a low decline and the remaining six teams experienced a high decline. The category of decreasing participants is divided into two, namely the low decline is the team that has decreased from the previous year of 11.77% - 27.54%, while the high decline category is the team with a decrease from the previous year of 27.55% - 43.31%. Conclusion: This happens when the team implements mutual performance monitoring, backup behavior, adaptability, and loop communication properly, so the decrease in the achievement of new MKJP KB participants will be lower, and vice versa. When the workforce is able to work well together, by paying attention to the work of one team and trying to help each other in achieving the agreed goals, it will indirectly improve performance so that it will easily reach the target.

Abstract

The report on the average achievement of nurses’ compliance with the reassessment of patients’ fall risk from January to June 2020 was 54.75% and did not meet the average hospital standard of 100%. This study aims to analyze the influence of organizational and individual factors on nurses’ compliance with the re-assessment of fall risk at the inpatient installation of RSPAL dr. Ramelan Surabaya. This was an analytical observational study with a cross- sectional design. 162 people were taken as the respondents using a proportionate simple random sampling technique. The results of strategic issues and Focus Group Discussion (FGD) showed that nurse peer support in the fairly supportive category (75.9%). The result of measuring the nurses’ compliance with performing the re-assessment of fall risk is in the High category (89.5%). The factors, including supervision, information, the proximity of authority, the location status, personal responsibility, the legitimacy of authority figure, the status of authority figure, and peer support, significantly affect the nurses’ compliance with performing the reassessment of patients’ fall risk. The increase in the nurses’ compliance level in performing the re-assessment of fall risk correctly can be achieved by conducting an outreach to all nurses regarding the effects of performing the re-assessment of fall risk incorrectly, and conducting a regular outreach to heads of nursing room to actively evaluate and motor the implementation of the re-assessment of fall risk. Abstrak: Laporan pencapaian rata-rata kepatuhan perawat terhadap asesmen ulang pasien risiko jatuh periode Januari sampai Juni 2020 sebesar 54,75% dan tidak memenuhi standar rata-rata rumah sakit sebesar 100%. Penelitian ini bertujuan untuk menganalisis pengaruh faktor organisasi dan faktor individu terhadap kepatuhan perawat pada penilaian ulang risiko jatuh di instalasi rawat inap RSPAL dr. Ramlan Surabaya. Penelitian ini merupakan penelitian observasional analitik dengan desain cross-sectional. Sebanyak 162 orang diambil sebagai responden dengan menggunakan teknik proportional simple random sampling. Hasil strategic issues dan Focus Group Discussion (FGD) menunjukkan bahwa peer support perawat dalam kategori cukup mendukung (75,9%). Hasil pengukuran kepatuhan perawat dalam melakukan asesmen ulang risiko jatuh berada pada kategori Tinggi (89,5%). Faktor-faktor yang meliputi supervisi, informasi, kedekatan otoritas, status lokasi, tanggung jawab pribadi, legitimasi figur otoritas, status figur otoritas, dan dukungan sebaya berpengaruh signifikan terhadap kepatuhan perawat dalam melakukan asesmen ulang pasien jatuh. mempertaruhkan. Abstract

The rate of blindness caused by cataracts in Indonesia is the highest in Southeast Asia and ranks third in the world at 1.47%. Cataract treatment can only be cured by surgery. The shape of the IOL (lens) is also increasingly diverse to meet the needs of cataract sufferers. Along with the development of this field, different product offerings can become another value as competitiveness as owned by KMU Gresik Eye Clinic. This research aims to develop IOL Premium service products as a solution for patients with cataracts and refractive errors. This study was observational analysis because there was no intervention on the respondents during the data collection process. Data collection was carried out through filling out questionnaires on 100 respondents based on needs and evaluating services that have been running. The results showed that in general as many as 72% of respondents still experienced poor vision, while 85% of respondents had experiences in the poor category of vision, so 89% of respondents experienced vision difficulties. Klinik Mata KMU Gresik needs to carry out product development, such as IOL Premium, VIP services, and teleconsultation to increase its competitiveness in overcoming emerging problems and meeting the demands and needs of sufferers. Abstrak: Tingkat kebutaan yang diakibatkan katarak di Indonesia merupakan yang tertinggi di Asia Tenggara dan berada di urutan ketiga di dunia yaitu sebesar 1,47%. Pengobatan katarak hanya bisa disembuhkan dengan cara operasi. Bentuk IOL (lensa) juga semakin beragam untuk memenuhi kebutuhan penderita katarak. Seiring dengan berkembangnya bidang ini, penawaran produk yang berbeda dapat menjadi value lain sebagai daya saing seperti yang dimiliki oleh Klinik Mata KMU Gresik. Penelitian ini bertujuan melakukan pengembangan produk layanan IOL Premium sebagai solusi untuk penderita katarak dan kelainan refraksi. Penelitian ini bersifat observasional analiyik karena tidak adanya intervensi terhadap reponden selama proses pengumpulan data. Pengumpulan data dilakukan melalui pengisian kuisioner pada 100 responden berasarkan kebutuhan dan evaluasi layanan yang sudah berjalan. Hasil menunjukkan bahwa secara umum sebanyak 72% responden masih mengalami penglihatan yang buruk, sedangkan 85% responden memiliki pengalaman dalam kategori buruk dalam penglihatan, sehingga 89% responden mengalami kesulitan penglihatan. Klinik Mata KMU perlu melakukan pengembangan produk, seperti IOL Premium, layanan VIP, dan telekonsultasi untuk meningkatkan daya saingnya dalam mengatasi masalah yang muncul dan memenuhi permintaan dan kebutuhan penderita.

Abstract

This study aimed to analyse the influence of organizational factors on patient safety culture at the Nganjuk General District Hospital. This was an analytical study with a cross-sectional design conducted from February 15 to March 14, 2020. Primary data were collected using a questionnaire distributed to 276 staff from 22 work units that provided direct services to patients. Secondary data were obtained from the profile and the Quality and Patient Safety Improvement Report of the Nganjuk General District Hospital in 2019. A cross-tabulation table was employed to analyze the influence of independent and dependent variables on patient safety culture with a percentage difference of less than 20%. The results showed that the majority of respondents were women (68.8%), had an associate degree III or equivalent education (57.6%), and had received training on patient safety (97.8%). The results of the cross-tabulation showed that organizational factors had an influence on patient safety culture at the Hospital. It is recommended that the Hospital pay attention to aspects that may affect patient safety culture since several work units still had a poor implementation of patient safety culture. Abstrak: Tujuan penelitian ini adalah menganalisis pengaruh faktor organisasi terhadap budaya keselamatan di Rumah Sakit Umum Daerah Nganjuk. Penelitian ini merupakan penelitian analitik dengan rancang bangun cross sectional pada 15 Februari sampai dengan 14 Maret 2020. Data primer dikumpulkan dengan panduan kuisioner pada 276 staf dari 22unit kerja yang memberikan pelayanan langsung kepada pasien.Data sekunder diperoleh dari profil RSUD Nganjuk dan laporan Komite PMKP RSUD Nganjuk tahun 2019. Analisis pengaruh menggunakan tabel tabulasi silang (cross tabulation) dengan selisih persentase kurang dari 20% antara variabel independen dan dependen.Hasil penelitian didapatkan bahwa mayoritas responden adalah perempuan (68,8%), memiliki pendidikan D3/sederajat (57,6%), dan telah mendapatkan pelatihan tentang keselamatan pasien (97,8%). Hasil tabulasi silang menunjukkan adanya pengaruh antara faktor organisasi terhadap budaya keselamatan pasien di RSUD Nganjuk.Disarankan RSUD Nganjuk kembali memperhatikan aspek-aspek yang dapat mempengaruhi budaya keselamatan pasien, mengingat masih terdapat beberapa unit kerja yang memiliki penilaian kurang terkait dengan aspek yang mempengaruhi budaya keselamatan pasien.

Abstract

Hospital service quality should be developed based on customer needs and satisfaction. Long service time at Bhayangkara Kediri Hospital Outpatient Unit (267,62 ± 136,41 min) becomes a problem for management. The process is an asset for an organization, especially the core process should be well managed. Business process management has been viewed as an effective concept to improve processes continuously. It is an operational research combining document review, stakeholder interview, focused group discussion, quantitative and qualitative process analyze, prioritization, and root cause analysis to give process redesign recommendations. There are five core processes in the outpatient unit, and we focus our research on the dispensing process in pharmacy. Then we made a process model based on evidence and interview-based discovery. The cycle time efficiency of the prescription analysis process was the worst with a value of 0.31. With non-value activities reach to 40% of all activities with the waste are over processing, waiting, and transportation. The selected problem was the disposition of prescriptions to the dispensing unit as waste. Based on the root cause analysis, we suggest two recommendations. For the short-term redesign, we support the use of a basket to transport the prescription embedded in the operational procedures. And for the long-term redesign, we suggest the implementation of electronic prescribing. These redesigns are assumed to reduce waiting time by 2.5 – 16.2 % of service time.
