Abstract

This study aimed to analyse the influence of organizational factors on patient safety culture at the Nganjuk General District Hospital. This was an analytical study with a cross-sectional design conducted from February 15 to March 14, 2020. Primary data were collected using a questionnaire distributed to 276 staff from 22 work units that provided direct services to patients. Secondary data were obtained from the profile and the Quality and Patient Safety Improvement Report of the Nganjuk General District Hospital in 2019. A cross-tabulation table was employed to analyze the influence of independent and dependent variables on patient safety culture with a percentage difference of less than 20%. The results showed that the majority of respondents were women (68.8%), had an associate degree III or equivalent education (57.6%), and had received training on patient safety (97.8%). The results of the cross-tabulation showed that organizational factors had an influence on patient safety culture at the Hospital. It is recommended that the Hospital pay attention to aspects that may affect patient safety culture since several work units still had a poor implementation of patient safety culture. Abstrak: Tujuan penelitian ini adalah menganalisis pengaruh faktor organisasi terhadap budaya keselamatan di Rumah Sakit Umum Daerah Nganjuk. Penelitian ini merupakan penelitian analitik dengan rancang bangun cross sectional pada 15 Februari sampai dengan 14 Maret 2020. Data primer dikumpulkan dengan panduan kuisioner pada 276 staf dari 22unit kerja yang memberikan pelayanan langsung kepada pasien.Data sekunder diperoleh dari profil RSUD Nganjuk dan laporan Komite PMKP RSUD Nganjuk tahun 2019. Analisis pengaruh menggunakan tabel tabulasi silang (cross tabulation) dengan selisih persentase kurang dari 20% antara variabel independen dan dependen.Hasil penelitian didapatkan bahwa mayoritas responden adalah perempuan (68,8%), memiliki pendidikan D3/sederajat (57,6%), dan telah mendapatkan pelatihan tentang keselamatan pasien (97,8%). Hasil tabulasi silang menunjukkan adanya pengaruh antara faktor organisasi terhadap budaya keselamatan pasien di RSUD Nganjuk.Disarankan RSUD Nganjuk kembali memperhatikan aspek-aspek yang dapat mempengaruhi budaya keselamatan pasien, mengingat masih terdapat beberapa unit kerja yang memiliki penilaian kurang terkait dengan aspek yang mempengaruhi budaya keselamatan pasien.

Abstract

Hospital service quality should be developed based on customer needs and satisfaction. Long service time at Bhayangkara Kediri Hospital Outpatient Unit (267,62 ± 136,41 min) becomes a problem for management. The process is an asset for an organization, especially the core process should be well managed. Business process management has been viewed as an effective concept to improve processes continuously. It is an operational research combining document review, stakeholder interview, focused group discussion, quantitative and qualitative process analyze, prioritization, and root cause analysis to give process redesign recommendations. There are five core processes in the outpatient unit, and we focus our research on the dispensing process in pharmacy. Then we made a process model based on evidence and interview-based discovery. The cycle time efficiency of the prescription analysis process was the worst with a value of 0.31. With non-value activities reach to 40% of all activities with the waste are over processing, waiting, and transportation. The selected problem was the disposition of prescriptions to the dispensing unit as waste. Based on the root cause analysis, we suggest two recommendations. For the short-term redesign, we support the use of a basket to transport the prescription embedded in the operational procedures. And for the long-term redesign, we suggest the implementation of electronic prescribing. These redesigns are assumed to reduce waiting time by 2.5 – 16.2 % of service time.

Abstract

Payment for health services with national health insurance applies a prospective method. Hospitals must make efforts so that health services can run efficiently and not cause losses. The problem in this research is the negative difference between the Indonesian Case Based Group (INA-CBG) rates and service costs based on actual patient rates with national health insurance. This study aimed to develop a clinical pathway on 10 top CBG codes with the largest negative differences between INA-CBG and hospital rate. It also evaluated its pilot implementation impact on financial outcomes. Action research was conducted on a sample of the top 10 Case Based Group (CBG) codes with the largest negative difference between INA-CBG rates. One thousand ninety-nine (1099) medical records were analyzed and taken randomly using simple random sampling. Most of the variants found were the provision of consumable medical materials that exceeded standards and the administration of patent medicines outside the national formulary without written medical indications. The value of compliance with the clinical pathway was still below the expected standard (< 80%), especially in medical consumables. Researchers carried out a three-month intervention of a new clinical pathway implementation. The results showed increased compliance with clinical pathways and reduced negative differences between INA-CBG and hospital rates. Clinical pathway audits could reduce the negative difference between INA-CBG and hospital rates.

Abstract

This study aimed to analyze the effect of organizational and individual factors on the level of compliance of nurses and midwives in phlebitis observation. This research is an observational analytic study with a cross-sectional design. Respondents of this study were nurses and midwives (n =48) at Inpatient Installation. Data were obtained from questionnaires and secondary data from the IV Usage Surveillance form. The results showed that policy, guidelines, knowledge, and motivation had a positive effect. In contrast, aspects of standard operating procedures and attitudes had an inverse effect on the compliance of nurses and midwives in observing phlebitis. Some nurses and midwives have the weak motivation and the assessment of nurses and midwives that phlebitis observation activities are not supported by the existence of standard operating procedures related to identifying phlebitis signs/scales. The conclusion of this study is the need for an organizational approach to motivate nurses and midwives and provide clear standard operating procedures so that they can change attitudes to become more obedient.

Abstract

Based on data from PPI Hospital X in Surabaya there are still nosocomial infections. If the incidence of this infection continues to recur, then the hospital's image will be bad and can cause hospital bed occupational rates to decrease. The purpose of this study was to determine the relationship between the characteristics of health workers (level of knowledge, level of education, age, and length of work) with the level of compliance of health workers doing hand washing at the "X" Hospital in Surabaya. This research is a descriptive correlation study. The sample in this study was 150 health workers working at X Hospital Surabaya, using proportional random sampling stratification technique. Data were collected using a questionnaire instrument. Data analysis using Pearson correlation test. The results revealed a significant relationship between the level of knowledge about handwashing (p = 0.001), an education level (p = 0.004), age (p = 0,000) and years of service (p = 0,000) to the level of compliance of health workers doing hand washing at the "X" Hospital in Surabaya. Surabaya "X" Hospital health workers have a level of compliance with handwashing with a maximum compliance category >75% (76%).

Abstract

COVID-19 (coronavirus2019) is a viral infection of concern globally. Based on data released by the National Disaster Management Agency in July 2020, there was a twenty-six COVID-19 hospital cluster. This study aimed to analyze determinant factors for hospital staff adherence to the use of personal protective equipment in the care of COVID-19 patients. This Study is observational research with a cross-sectional design. The sample was selected using a simple random sampling technique with a total sample of 53 respondents. Data was collected using a questionnaire and analyzed using linear regression statistical analysis. The result of the study illustrated that only 43.40% of the hospital staff were compliant to use PPE. The respondents have characteristics as follows: good knowledge (52.83%), good risk perception (50.94%), low barriers to implementation (71.70%), good safety climate (90.57%), and good information and training (94.34%). There were correlation between knowledge (p-value = 0.000, β = 0.880), risk perception (p-value = 0.000, β = 0.846), barriers to implementation (p-value = 0.000, β = 0.579), safety climate (p-value = 0.005, β = 0.646), information and training (p-value = 0.034, β = 0.620) with hospital staff using PPE. Knowledge and risk perception has a very strong correlation with hospital staff adherence.

Abstract

This study aimed at analyzing correlation between digital marketing and customer satisfaction and loyalty at general hospitals during the COVID-19 pandemic era. This study was a quantitative study with an observational design and a cross-sectional approach. The population of this study was the hospital’s customers with a total sample of 100 customers that had already received hospital services during the COVID-19 pandemic era and taken using a simple random sampling technique. Data analysis was done using Pearson’s correlation test. Digital marketing was perceived well by 84% of the respondents. Further, for customer satisfaction, it was shown that most respondents had a high satisfaction of 77%. For customer loyalty, it was shown that most respondents had a high loyalty of 75%. The result of data processing for correlation between digital marketing and the respondent’s satisfaction showed a significance level of 0.000 (p-value < 0.05) with an r-value of 0.827. Meanwhile, correlation between digital marketing and the respondent’s loyalty showed a significance level of 0.000 (p-value < 0.05) with an r-value of 0.655. Digital marketing and customer satisfaction and loyalty at general hospitals are correlated and it’s an effective way to improve hospital customer satisfaction and loyalty during the COVID-19 pandemic era.

Abstract

Patient safety culture should be applied by all nurses in healthcare facilities. Yet, most of the time, nurses are not aware of its importance, resulting in patients’ poor perception of the nurses’ performance. This study identified the effect of patient safety culture on nurses’ performance at Bakti Mulia General Hospital. This was a correlation-descriptive study, which selected all nurses as many as 28 at Bakti Mulia General Hospital by using simple random sampling. Data were collected through questionnaires and analyzed using univariate and bivariate analysis with Pearson Correlation test. The majority of the respondents had poor patient safety culture (46%), and some had high patient safety culture (54%). The majority of the respondents showed had poor performance (68%), and some others had high performance (32%). There was a significant effect on patient safety culture on nurses' performance at Bakti Mulia General Hospital.

Abstract

One issue in patient safety still a concern is the timeliness of reporting patient safety incidents. This study aims to analyze the relationship between the group, organizational, and mediator factors on misbehavior performance in implementing patient safety incident reporting in hospitals. The study was a descriptive study with a cross-sectional design conducted on all units in the hospital involving 174 respondents. Data was collected using a questionnaire that measured all variables on group factors, organizational factors, mediator factors, and misbehavior performance. Cross tabulation is used to analysis with a percentage difference of more than 20%. The results showed several groups and organizational factor namely cohesiveness, leadership, constructive type organizational culture, organizational climate, and control systems related to the ability to internalize organizational values and norms, belief in deviant behavior, and intentions to engage in deviant behavior. There is a correlation between mediator factors consisting of normative force, instrumental force, and intention to misbehave with deviant behavior in reporting patient safety incidents. Group and organizational factors are related to mediator factors, including internalization of organizational values and norms, beliefs of deviant behavior, and intentions. In addition, there is also a relationship between mediator factors and deviant behavior in reporting patient safety incidents.

Abstract

One indicator of a nurse’s performance evaluation regarding implementing patient safety programs in hospitals is the risk of a patient falling. An audit of fall risk prevention at Siloam Hospitals in Surabaya was used to evaluate nurses’ performance in preventing patient falls. The impact of personality and situation awareness characteristics on the nurse’s output performance in preventing fall risk was examined. A cross-sectional research design was used in the observational analytic research method. Forty-five nurses made up the total sample. The EPPS and SEAFAP questionnaire were the instruments used in the data analysis, which applied the linear regression test. The following personal characteristics have a p-value of 0.05 or less: Order (p-value 0.016), Autonomy (p-value 0.019), Affiliate (p-value 0.012), Succorance (p-value 0.012), and Nurturance (p-value 0.009). With a p-value of 0.040, situation awareness impacts output performance. Both overall personality and situation awareness have a p-value of 0.006 and influence output performance, respectively. Situation awareness and personality-based needs have an impact on output performance. To provide specific solutions to improve the probability of patients falling, the recommendations for enhancing the fall risk prevention programs can be carried out through discussion and interviews.
