Abstract

Health workers have the highest risk exposure that can increase the pathogen infection risk during the Covid-19 pandemic. The use of Personal Protective Equipment (PPE) is the most significant prevention, but mistakes while using PPE may occur in the donning (putting on) and doffing (taking off) process. This study compares the practice of using PPE among groups of health providers and hospital service units. The study design was a cross-sectional study conducted on non-infectious hospital employees in one specific hospital in Surabaya, totaling 102 employees. The Infection Prevention and Control Link Nurse (IPCLN) of the hospital conducted observations and analyses of health personnel in inpatient and triage units, treatment units, and support units. Based on the job roles, the employees were divided into health providers, healthcare support workers, and other health workers. Work units were categorized into inpatient units, outpatient and triage units, treatment units, and support units (cashier and registration areas). The results of the assessment were measured using One-Way ANOVA. Based on the job role, significant errors occurred when removing masks (p=0.048) made by health workers (38%). Meanwhile, in the hospital unit category, significant errors occurred in removing masks (p=0.000) and the use of gloves (p=0.002). Most mistakes were made in the outpatient and triage units, 92% and 42%, respectively. The complexity of wearing and removing PPE requires increased awareness and updated training on the correct use of PPE for all health workers who work in hospitals based on WHO recommendations during the Covid-19 pandemic.

Abstract

The doctor's profession is noble and tied up with quite strict rules, both in terms of ethics as well as discipline. Naturally, there is a problem of asymmetrical information between doctors and patients, often leading to misunderstandings. The purpose of this review is to map the available evidence related to the challenges and difficulties faced by doctors in the era of disruption. The evidence indicate that disruption of health services has both positive and negative effects. Many aspects related to the use of technology in the medical practices including innovation, cost-effectiveness, and quality improvement. However, psychologically, the doctors often get frustrated by internal or external triggers. The external factors, ranges from the equipment and work procedures, for instance, using high technology, communication with management, inter-professional relationships, patients, and their families. Volatility, uncertainty, complexity, and ambiguity (VUCA) could cause stress and burnout. In this case, doctors are highly vulnerable, and consequently, have the potential to make mistakes. Therefore, the adversity faced by doctors ought to be mapped. Resilience is a barrier against stress and burnout and the ability to adapt in resilience among doctors is important factor in dealing with the disruption era.

Abstract

This study aims to determine the effect of the remuneration system on nurses' work motivation and nurse performance in East Java Private Hospitals. The research method used is an observational method with a cross-sectional design. The result of this research is that the remuneration system on motivation shows a significance value of 0.022 (a = 0.05). The reason for performance offers a significance value of 0.001 (a=0.05). Meanwhile, the remuneration system has no significant effect on performance, with a significance value of 0.487. In conclusion, the remuneration system has a significant influence on motivation. Motivation has a significant impact on the performance of nurses in East Java Private Hospitals so that the remuneration system can affect motivation. Abstract

This study aimed to analyze comorbid diseases as a risk factor for death due to COVID-19 at Bhakti Dharma Husada Hospital Surabaya. This research method is an observational analytic study with a retrospective study design. The results showed that 358 patients were infected with COVID-19 and confirmed with nose and throat swabs. Sixty-six patients (18%) died from COVID-19. 60.6% were male (OR 1.87, P 0.041), 22.7% aged> 64 years (OR 2.097, P 0.041), and 83.3% of them were co-risk factors. Diabetes mellitus (30.3%) (OR 4,348, P 0,000), and cardiovascular disease (10.6%) (OR 4,319, P 0.016) were the highest risk factors for death in COVID-19. In conclusion, men, old age, diabetes, and hypertension are risk factors for death in COVID-19. Keywords: COVID-19, Diabetes, Hypertension, Comorbid, Elderly, Mortality

Abstract

This study aims to examine the effect of the COVID-19 pandemic on the psychological condition of health workers. The research design used is a systematic search on several databases, including DOAJ, Sage, Proquest, Medline, Google Scholar, Science Direct, from February to June 2020. There is Rational pre-clinical evidence regarding mental or psychological health risks from several articles as a result of Covid-19. Feelings of anxiety, stress, depression or fear, and even panic due to Covid-19 trigger increased psychological or mental health risks faced by health workers so that therapy or group or individual activities need to be carried out. In conclusion, there is reasonable pre-clinical evidence regarding the mental or psychological health risks due to corona. Abstract

This study aims to determine the effectiveness of advanced midwifery training in reducing maternal mortality. The method used is using training, discussion, and demonstration. This type of research is descriptive using secondary data on birth data and maternal mortality. Continuous training carried out during 2016 and 2017 was proven to reduce maternal mortality in Surabaya. The MMR in 10 sub-districts trained in 2015 was 113.7 per 100,000 births; this figure is still above the MDGs standard. After the MMR training was carried out in 10 sub-districts that received training in 2016, it was 81.8 per 100,000 births, and in 2017 it was 66.5 per 100,000 births. The results of the simple T-test showed a significant reduction in maternal mortality with p=0.04 (p<0.05). In conclusion, continuous training for midwives reduces maternal mortality in Surabaya. Abstract

Hospitals have been competing to provide the best services in order to gain patient loyalty. Patient loyalty can be assessed from patient satisfaction and experience during hospitalization. This study aims to analyse the influence of patient experience and patient satisfaction on patient loyalty in the Outpatient Installation of Darmo Hospital. An analytical observational study using a cross-sectional approach was used in this study. The sample used in this study was 181 patients who were treated in the outpatient installation of Darmo Hospital Surabaya, selected using proportionate stratified random sampling technique. The independent variables in this study were patient experience and satisfaction, while the dependent variable was patient loyalty. Data was collected using a questionnaire and analyzed using logistic regression. The results showed a significant influence of patient experience on satisfaction (0.0001), patient experience on loyalty (0.0001), and satisfaction on loyalty (0.0001). Patient loyalty can be achieved by improving and enhancing patient experience, as better patient experience leads to increased patient satisfaction and loyalty towards the hospital. To gain patient loyalty, hospital services must be improved based on feedback in accordance with patients' wishes.

Abstract

The patient discharge process at Jemursari Islamic Hospital, Surabaya for the four types of patient insurance, namely BPJS, General, Institution, and Private Insurance, does not currently meet the standard operating procedures of a 2-hour discharge time. This study was observational using a quantitative analysis approach with the Six Sigma methodology. The study measured the time variation and sigma level of each process and sub-process. Qualitative analysis was conducted through a Focus Group Discussion to identify activities. The sample size was 100 respondents, including 70 patients with BPJS insurance, 10 patients with general insurance, 8 patients with institutional insurance, and 12 patients with private insurance. The findings showed that the average total time for the patient discharge process was 166.61 minutes for patients with BPJS insurance, 252.60 minutes for patients with general insurance, 258.16 minutes for patients with institutional insurance, and 229.69 minutes for patients with private insurance. The sigma level of the sub-processes for discharging inpatient patients guaranteed by BPJS was near the standard level in two sub-processes. The sigma level of the sub-processes for discharging inpatient patients with general and institutional insurance in inpatient care was near the standard level. The sigma level of the sub-processes for discharging inpatient patients with private insurance was near the standard level in one sub-process.

Abstract

Currently, the number of drug raids is increasing day by day. Many factors can cause teenagers to become addicted to drugs. The aim of this study was to investigate factors influencing drug addiction in adolescents through a systematic literature review. The research variables were factors that influence drug addiction among teenagers, including: age, personality, family, friends. Literature sources were journal articles indexed in Google Scholar. The selection results obtained 5 literature that met the inclusion criteria. The results of the review show that the factors that influence drug addiction in teenagers were the age at which teenagers tend to try new things, personal and lifestyle factors that have an influence in determining a teenager to use drugs, and people who experience conflict will experience frustration. Individuals who are not used to solving problems tend to use drugs. One of the factors that influence drug addicts is an imperfect family, which affects children's psychology, so that children are easily influenced by negative things; personality factors; lack of self-confidence, which is associated with the emotional instability of young people; the friendship factor between users is also a threat. It was concluded that age, family, personality and friends influence drug use in adolescents.Keywords: drugs; addicted; teenager ABSTRAK Saat ini, jumlah razia narkoba semakin meningkat dari hari ke hari. Banyak faktor yang dapat menyebabkan remaja menjadi kecanduan narkoba. Tujuan dari penelitian ini adalah untuk menyelidiki faktor-faktor yang mempengaruhi kecanduan narkoba pada remaja melalui tinjauan literatur sistematis. Variabel penelitian adalah faktor-faktor yang mempengaruhi kecanduan narkoba di kalangan remaja, meliputi: usia, kepribadian, keluarga, teman. Sumber literatur adalah artikel-artikel jurnal yang terindeks di Google Scholar. Hasil seleksi mendapatkan 5 literatur yang memenuhi kriteria inklusi. Hasil tinjauan menunjukkan bahwa faktor yang mempengaruhi kecanduan narkoba pada remaja adalah usia di mana remaja cenderung mencoba hal baru,  faktor pribadi dan gaya hidup yang memiliki pengaruh dalam menetukan sorang remaja untuk menggunakan narkoba, serta orang yang mengalami konflik akan mengalami frustrasi. Individu yang tidak biasa dalam menghadapi penyelesaian masalah cenderung menggunakan narkoba. Faktor yang mempengaruhi pecandu narkoba salah satunya adalah keluarga yang tidak sempurna, yang mempengaruhi psikologi anak, sehingga anak mudah terpengaruh hal-hal negatif; faktor kepribadian; kurangnya kepercayaan diri, yang terkait dengan ketidakstabilan emosi anak muda; faktor pertemanan antar pengguna juga merupakan ancaman. Disimpulkan bahwa usia, keluarga, kepribadian dan teman berpengaruh terhadap penggunaan narkoba pada remaja.Kata kunci: narkoba; kecanduan; remaja

Abstract

Pada tahun 2010, organisasi kesehatan dunia telah merilis daftar periksa bedah untuk meningkatkan sikap keselamatan, termasuk komunikasi dan kerja sama tim. Namun, sepertiga dari komunikasi dalam pembedahan gagal menyampaikan informasi yang relevan. Studi ini bertujuan untuk menentukan persyaratan penerapan daftar periksa bedah organisasi kesehatan dunia terhadap komunikasi dan kerja sama tim di ruang operasi. Protokol Prospero dan diagram alur Prisma digunakan untuk memperoleh artikel yang relevan dari empat basis data elektronik. Penyaringan artikel dibatasi pada studi yang menggunakan daftar periksa bedah organisasi kesehatan dunia atau modifikasinya. Informasi tambahan khususnya mengenai desain penelitian, alat instrumen, dan hasil diekstraksi dan disintesis. Membandingkan dan mengontraskan artikel terpilih dilakukan untuk meringkas tinjauan. Dua puluh satu artikel dipilih dari total 3.806 publikasi. Mereka menyimpulkan bahwa daftar periksa bedah organisasi kesehatan dunia dapat meningkatkan komunikasi di ruang operasi jika diikuti dengan pelatihan komunikasi, mengurangi perbedaan persepsi, menghilangkan hierarki di seluruh tim bedah, dan memodifikasi daftar periksa. Itu juga dapat meningkatkan kerja sama tim jika dikejar dengan pelatihan kerja tim dan dukungan kepemimpinan. Beberapa persyaratan substansial untuk daftar periksa bedah organisasi kesehatan dunia harus dipenuhi dan diselesaikan untuk meningkatkan komunikasi dan kerja sama tim di antara tim bedah di ruang operasi.
