Abstract

Abstract Background Coronary Artery Bypass Graft (CABG) is a surgical procedure with a fairly high number of cases and risks. To handle the procedure, the implementation of Clinical Pathway is required. The purpose of this study was to analyze the effect of Clinical Pathway compliance on the quality and cost of CABG procedures at Dr. Soetomo Hospital, Surabaya. Method Quantitative with an observational analytical approach, cross-sectional design. The population size was 99 medical records, with a total sampling technique of 95 medical records obtained according to the inclusion criteria, descriptive data analysis. Secondary data were obtained from medical records, incident data from the PPI Committee and the hospital's financial report in 2023. Result The average age of patients was 53 years with male gender of 80%. Compliance of officers in implementing the Clinical Pathway for CABG procedures with a compliant category 14.7%, a less compliant category 78.9% and a non-compliant category of 6.3%. There were no incidents of SSI for CABG procedures (0%). LOS of CABG procedures with length of stay ≤10 days were 70.5% and >10 days were 29.5%. Costs for CABG procedures with large Disparity (+) were 1.1%, small Disparity (+) were 54.7%, large Disparity (-) were 13.7% and small Disparity (-) were 30.5%. Conclusion There is an inverse effect between Clinical Pathway compliance and Length Of Stay (LOS), there is no effect of Clinical Pathway compliance on surgical wound infection rates and there is an effect of Clinical Pathway compliance on cost disparities.

Abstract

This study focuses on assessing the effectiveness of digital marketing applications at X Hospital Mojokerto by reviewing patient demographics, online behavior, and perceptions of digital media. A quantitative approach using a survey method was used, with a questionnaire consisting of several questions, to collect data from new patients who access health information through the hospital's digital media. The results indicate that patients aged 26–35 years dominate, and TikTok and Instagram are shown as the primary platforms used for accessing health information. Nonetheless, only Facebook indicated a statistically meaningful result on patients' choices in seeking hospital care. The remaining variables, such as demographic factors and digital media perception, demonstrated no significant impact. The study implies that digital platforms' usage behavior and content strategy bring a greater impact than demographic characteristics. Recommendations cover maximizing Facebook performance, improving content interaction on Instagram and TikTok, and implementing communication strategies tailored to user behavior to strengthen communication outcomes and conversion.

Abstract

Effective nursing management is a crucial factor in improving the quality of healthcare services in hospitals. One approach that can be applied in nursing management is the PDCA (Plan-Do-Check-Act) cycle, which aims to enhance efficiency and effectiveness in the care process. This article aims to analyze the impact of PDCA cycle implementation on nursing management in hospitals. The method used in this study is a systematic literature review with a PICOS approach, utilizing databases from PubMed and ScienceDirect. Out of the 26 articles obtained based on the keywords “PDCA Cycle” and “nursing management”, 8 articles met the inclusion criteria: (1) implementation of the PDCA method in nursing management; (2) a quantitative study with an intervention; (3) articles published no earlier than 2020; and (4) international publications. The results indicate that the implementation of the PDCA cycle on nursing management has a positive impact on both patients and nurses. For patients, outcomes include reduced complications and infection rates, shorter hospital stays, fewer side effects, and improved satisfaction. For nurses, benefits include enhanced compliance with procedures, better infection control practices, reduced workload and errors, and increased professional competency and safety. Therefore, the PDCA cycle can be considered an effective approach to improving the quality of nursing management.

Abstract

Hospitals have been competing to provide the best services in order to gain patient loyalty. Patient loyalty can be assessed from patient satisfaction and experience during hospitalization. This study aims to analyse the influence of patient experience and patient satisfaction on patient loyalty in the Outpatient Installation of Darmo Hospital. An analytical observational study using a cross-sectional approach was used in this study. The sample used in this study was 181 patients who were treated in the outpatient installation of Darmo Hospital Surabaya, selected using proportionate stratified random sampling technique. The independent variables in this study were patient experience and satisfaction, while the dependent variable was patient loyalty. Data was collected using a questionnaire and analyzed using logistic regression. The results showed a significant influence of patient experience on satisfaction (0.0001), patient experience on loyalty (0.0001), and satisfaction on loyalty (0.0001). Patient loyalty can be achieved by improving and enhancing patient experience, as better patient experience leads to increased patient satisfaction and loyalty towards the hospital. To gain patient loyalty, hospital services must be improved based on feedback in accordance with patients' wishes.

Abstract

The patient discharge process at Jemursari Islamic Hospital, Surabaya for the four types of patient insurance, namely BPJS, General, Institution, and Private Insurance, does not currently meet the standard operating procedures of a 2-hour discharge time. This study was observational using a quantitative analysis approach with the Six Sigma methodology. The study measured the time variation and sigma level of each process and sub-process. Qualitative analysis was conducted through a Focus Group Discussion to identify activities. The sample size was 100 respondents, including 70 patients with BPJS insurance, 10 patients with general insurance, 8 patients with institutional insurance, and 12 patients with private insurance. The findings showed that the average total time for the patient discharge process was 166.61 minutes for patients with BPJS insurance, 252.60 minutes for patients with general insurance, 258.16 minutes for patients with institutional insurance, and 229.69 minutes for patients with private insurance. The sigma level of the sub-processes for discharging inpatient patients guaranteed by BPJS was near the standard level in two sub-processes. The sigma level of the sub-processes for discharging inpatient patients with general and institutional insurance in inpatient care was near the standard level. The sigma level of the sub-processes for discharging inpatient patients with private insurance was near the standard level in one sub-process.

Abstract

The current study investigates the comparison between Personal Resources (PR) and Work Engagement (WE) based on Length of Service. This is a quantitative study conducted using a cross-sectional approach. The sample contained 257 employees of three hospitals in South Sulawesi who were selected by simple random sampling and then analyzed using an ANOVA test on SPSS 24. The results of the analysis demonstrated that there was a statistically significant relationship (p < 0.05) between PR and WE based on the employees' length of service within the hospital. Length of Service has an important role in increasing the influence of personal resources and work engagement.

Abstract

Patient safety culture is paramount in healthcare, and fostering a just culture is essential for promoting incident reporting as a means of collective learning rather than blame. This study aims to understand the process of establishing a just culture in healthcare organizations, its influencing factors, and associated challenges. Following PRISMA 2020 guidelines, a systematic search of electronic databases from 2019 to 2023 was conducted. The focus was on quantitative, qualitative, and mixed-method studies exploring just culture, patient safety, healthcare organizations, influencing factors, and challenges. STROBE criteria were used for quality assessment, and data extraction included authors, publication year, participants, interventions, and outcomes. To establish a just culture emphasizing accountability and transparency, distinguishing human errors from risky or reckless behaviors is crucial. Building a just culture encourages error reporting while maintaining a balance between a blame-free and punitive environment. Key influencing factors include organizational characteristics (safety climate and just culture) and personal factors (patient safety knowledge), particularly among nurses. The challenges faced include privacy-protecting policies that may hinder open communication and trust factors, as emotional consequences can arise from safety incidents. Fostering a just culture is vital for enhancing patient safety. Five influential factors emerged: Awareness, Empowerment, Organizational Factors, Leadership, and Communication. Challenges include privacy-protecting policies hindering open communication and trust issues, potentially leading to emotional consequences from safety incidents. Further research is needed to explore implementation processes and assess their impact on patient safety culture, incident reporting, system improvements, and responsible behaviors in healthcare organizations

Abstract

This study aims to determine how the influence of organizational culture and organizational commitment on the work motivation of employees of Surabaya Gotong Royong Hospital. The population of this study was 143 respondents, and the sample used was 60 respondents. The analysis used in this study uses descriptive and multiple linear regression analysis, namely to see the influence, organizational culture and organizational commitment to work motivation. The results of this study show that organizational culture variables and organizational commitment have a positive effect on the work motivation of Surabaya Gotong Royong Hospital employees, both partially and simultaneously.

Abstract

Medical equipment that have been suggested in planning for 2021 and have been calibrated at the Primary Health Center in the Pasuruan District Health Office's work area total 10.02% and 21.17% respectively. The goal of this study was to examine how teamwork participation affected the calibration performance outcomes at Pasuruan District Health Center. This kind of study involves 86 informants and analytical observation using a cross-sectional research methodology. The data were examined using a linear regression test.The performance of the medical device calibration team at the Primary Health Center was poor (36.4%), and the findings demonstrated that they were at a teamwork engagement level of commitment. The analysis's findings indicate that the management of writing medical device calibration application letters is significantly impacted by teamwork participation. Additionally, the research of the impact of teamwork engagement on performance outcomes for medical device calibration revealed a substantial impact.Conclusion: There is an influence of team work engagement showing a significant effect on the performance results of medical device calibration. Recommendations for improving the results of medical device calibration performance at the primary health center in Pasuruan Regency were carried out through discussion and research by researchers.

Abstract

ABSTRAKLatar Belakang: Data Laporan Pemakaian dan Lembar Permintaan Obat (LPLPO) di Kabupaten Sampang menunjukkan bahwa seluruh puskesmas di Kabupaten Sampang mengalami stagnan obat dengan rata-rata sebanyak 43% di seluruh puskesmas pada bulan Januari sampai bulan Oktober tahun 2019.Tujuan: Tujuan penelitian ini adalah menganalisis proses manajemen logistik obat di puskesmas di Kabupaten Sampang dan melihat kesesuaiannya dengan Peraturan Menteri Kesehatan Nomor 74 Tahun 2016.Metode: Penelitian ini berjenis observasional dengan rancang bangun cross sectional. Penelitian ini dilakukan pada Juli 2020 dengan metode wawancara, observasi, dan studi dokumen. Sampel dipilih menggunakan metode purposive sampling yaitu sebanyak sepuluh puskesmas yang terdiri atas lima puskesmas dengan angka stagnan tinggi dan lima lainnya rendah. Keseluruhan proses manajemen logistik obat di puskesmas akan dibandingkan dengan Peraturan Menteri Kesehatan Nomor 74 Tahun 2016 untuk dilihat kesesuaiannya.Hasil: Hasil dari penelitian ini menunjukkan bahwa pada proses pencatatan, pelaporan, dan pengarsipan obat 20% puskesmas dengan angka stagnant tinggi belum sesuai dengan Peraturan Menteri Kesehatan Nomor 74 Tahun 2016. Selanjutnya, pada proses pemantauan dan evaluasi seluruh puskesmas baik puskesmas dengan angka stagnant tinggi maupun puskesmas dengan angka stagnant rendah belum sesuai dengan Peraturan Menteri Kesehatan Nomor 74 Tahun 2016.Kesimpulan: Terdapat dua proses yang belum sesuai dengan Peraturan Menteri Kesehatan Nomor 74 Tahun 2016 yaitu proses pencatatan, pelaporan, dan pengarsipan obat serta proses pemantauan dan evaluasi obat. Oleh karena itu, perlu dilakukan monitoring dan evaluasi manajemen logistik secara berkala agar dapat dilakukan tindakan segera apabila terdapat hal yang tidak sesuai dengan rencana awal.Kata kunci: evaluasi, manajemen logistik, obat, puskesmas
