Abstract

In an effort to improve performance achievements, organizations need to analyze factors that influence performance so that they can determine effective strategies. One theory of performance factors that can be used is the ACHIEVE model, which stands for ability, clarity, help, incentive, evaluation, validity and environment. This study aimed to analyze the ACHIEVE factor on the performance of Bojonegoro Community Health Center employees. This research design was cross-sectional, involving 27 employees selected using total sampling technique. Data collection was carried out using a questionnaire in the form of a Google form. Descriptive data analysis was carried out to get an overview of each variable and multiple linear regression tests to test the significance of the influence of each factor. The results of the analysis showed that ability, clarity, help and validity have a p value <0.05; while incentive, evaluation and environment have a p value >0.05. It was concluded that employee performance at the Bojonegoro Community Health Center was influenced by the factors of ability, clarity, help and validity.Keywords: health center; employee; performance; abilities; clarity; help; incentives, evaluation; validity; environment ABSTRAK Dalam upaya meningkatkan capaian kinerja, organisasi perlu melakukan analisis faktor yang berpengaruh terhadap kinerja sehingga dapat menentukan strategi yang efektif. Salah satu teori faktor kinerja yang dapat digunakan adalah model ACHIEVE yaitu singkatan dari ability, clarity, help, incentive, evaluation, validity, dan environment. Penelitian ini bertujuan untuk menganalisis faktor ACHIEVE pada kinerja pegawai Puskesmas Bojonegoro. Rancangan penelitian ini adalah cross-sectional, yang melibatkan 27 pegawai yang dipilih dengan teknik total sampling. Pengumpulan data dilakukan menggunakan kuesioner dalam bentuk Google form. Analisis data secara deskriptif dilakukan untuk mendapatkan gambaran masing-masing variabel dan uji regresi linier berganda untuk menguji signifikansi pengaruh dari masing-masing faktor. Hasil analisis menunjukkan bahwa ability, clarity, help dan validity memiliki nilai p <0,05; sedangkan incentive, evaluation dan environment memiliki nilai p >0,05. Disimpulkan bahwa kinerja pegawai di Puskesmas Bojonegoro dipengaruhi oleh faktor ability, clarity, help dan validity.Kata kunci: puskesmas; pegawai; kinerja; ability; clarity; help; incentive, evaluation; validity; environment

Abstract

Kabupaten Jember merupakan salah satu kabupaten  yang memiliki jumlah kasus TBC (TB) termasuk kasus TB anak tertinggi di Jawa Timur. Skrining TB merupakan salah satu strategi penanggulangan TB anak yang dapat dilakukan secara aktif, intensif dan massif. Skrining TB anak secara aktif masif dapat dilaksanakan oleh petugas kesehatan dan juga masyarakat dalam hal ini kader. Belum memadainya tata laksana TB yang sesuai standar dan belum optimalnya peran kader kesehatan manjadikan penemuan kasus TB di Kabupaten Jember masih rendah. Selain itu, masih terdapat kelemahan dan keterbatasan dalam pelaksanaan skrining karena masih dilakukan secara manual sehingga peru digitalisasi dalam pelaksanaan skrining TB. Metode yang digunakan dalam pengabdian kepada masyarakat ini adalah ceramah, diskusi, tanya jawab, dan pelatihan kasus kepada kader dan tenaga kesehatan di Kabupaten Jember sebanyak 43 orang. Kegiatan capacity building dilaksanakan pada tanggal 15 Juli 2023 di Aula Dinas Kesehatan Kabupaten Jember. Kegiatan ini diawali dengan pre-test, pemberian materi, pelatihan, serta post-test. Didapatkan hasil bahwa terdapat peningkatan kemampuan kader dan tenaga kesehatan dari nilai rata-rata pre-test 85,5 meningkat menjadi 91,6 pada post-test. Diharapkan kegiatan skrining TB anak di Kabupaten Jember dapat dilakukan secara lebih aktif dan masif sehingga cakupan penemuan kasus meningkat yang dilanjutkan dengan treatment yang juga meningkat.

Abstract

Five focus group discussions (FGDs) with 61 pregnant women were conducted in June and July 2019 at primary health care (PHC) services within five urban areas of Surabaya, Indonesia. In addition, five semi-structured interviews with five midwives were carried out to explore the experiences of pregnant women accessing Antenatal Care (ANC) and the factors shaping uptake of ANC services. Data were audio-recorded, transcribed, and translated into English, and analyzed using thematic analysis. Findings from focus group discussions suggested that fears of negative diagnosis before initial ANC appointment and personal beliefs and myths surrounding pregnancy may delay uptake of ANC. Further, the influence of husbands, family, and friends and long waiting times with overcrowding leading to limited seating shaped timely access and return visits. In addition, feeling comfortable with the quality of the service and receiving a friendly service from the practitioners assisted women in feeling comfortable to return. Finally, midwives acknowledged feeling afraid of being referred to a hospital if deemed a high-risk pregnancy-shaped return ANC visits. The findings highlighted several factors needing to be addressed to increase the promptness of first ANC visits and ensure return visits to achieve great ANC coverage.

Abstract

Latar belakang: Tuberkulosis (TB) anak di Indonesia terutama di Kecamatan Pabean Cantian Surabaya memiliki tingkat kesakitan yang tinggi, meskipun begitu, banyak kasus TB yang tidak terdiagnosa dan tidak dilaporkan. Untuk mengatasi masalah tersebut, Geliat Unair, Unicef, dan Dinas Kesehatan Kota Surabaya bekerjasama dalam program pelatihan untuk meningkatkan kemampuan kader dan guru dalam penyuluhan dan skrining TB anak serta mendukung eliminasi TB anak. Tujuan: Penelitian ini bertujuan untuk menganalisis perbedaan pengetahuan kader dan guru saat sebelum dan sesudah mendapatkan pelatihan TB anak. Metode: Metode penelitian menggunakan kuantitatif observasional. Teknik pengambilan sampel adalah total sampling yakni seluruh peserta pelatihan kader dan guru yang berjumlah 60 orang yang merupakan kader kesehatan dan guru (KB, PAUD, TK, SD, SMP, dan guru pondok pesantren) di wilayah Kecamatan Pabean Cantian Surabaya. Pengukuran pengetahuan dalam kegiatan ini diperoleh dari hasil pre-test dan post-test menggunakan kuesioner. Analisis data dilakukan menggunakan uji statistik Paired T-test. Hasil: Hasil statistik menunjukkan p-values: 0,00 (p < ?) sehingga ada perbedaan pengetahuan sebelum dan sesudah pelatihan. Kesimpulan: Terdapat perbedaan pengetahuan kader dan guru saat sebelum dan setelah diberikan pelatihan TB anak.

Abstract

Background: It is important to evaluates the readiness of hospitals in response to health emergencies, focusing on their ability to manage crises effectively. The objective of this study was to assess the hospitals' readiness across multiple key components as defined by the WHO hospital readiness checklist. Method: A cross-sectional study design was used to assess the readiness of 59 hospitals over 2021 and 2022. Data were collected through the WHO checklist, evaluating 12 components such as leadership, communication, surveillance, and resource management. The hospitals were evaluated quarterly to track changes in readiness levels over time. Descriptive statistics were used to analyze the overall readiness scores for each hospital. Results: Leadership, incident management, and infection prevention and control remained consistently strong during the study period. However, notable weaknesses were found in occupational health, mental health support, and the continuity of essential services, which scored the lowest. Although there were minor improvements in maintaining essential services in 2022, these areas still need considerable improvement. Conclusions: The study concludes that, despite strong leadership and infection control measures, hospitals in the region need targeted interventions to address gaps in mental health support and service continuity. Improving these areas is critical for achieving comprehensive disaster readiness and enhancing hospital resilience to future health crises and natural disasters.

Abstract

Background: Decreasing Infant Mortality Rate (AKB) and improving nutritional status are indicators of health development in Indonesia. This is closely related to exclusive breastfeeding of infants. In Tuban Regency, the achievement of exclusive breastfeeding decreased from 2019 to 2020, with an achievement of 79.5% to 76.7%. Success in increasing the achievement of exclusive breastfeeding involves several factors, namely individual factors and community factors in the mother's neighborhood. Objectives: Analyzed the influence of variables of maternal characteristics, attitudes, beliefs, social norms, stigma, and culture in the mother's living environment with variables of exclusive breastfeeding actions on infants in the Working Area of Puskesmas X Tuban. Methods: Conducted a cross-sectional design observational study. The sample for this study consisted of 110 mothers with babies aged 7-12 months. Individual factors and community factors served as independent variables, while the act of exclusive breastfeeding in infants was the dependent variable. The test utilized was a binary logistic regression test. Results: The result revealed several factors that were pertinent to the provision of exclusive breastfeeding. In individual factors, there was a significant association with age subvariable (0,005); expenses (0,038); and knowledge (0,006). Meanwhile, community factors showed a significant association with cultural subvariables (0,006). Exclusice breastfeeding by the baby was found to be correlated with the mother’s condition and the environment around the mother. Inadequate conditions, such as iuffcient family knowledge and income, had an impact on exclusive breastfeeding. Additionally, the culture in the surrounding environment posed as an obstacle for mothers in providing excluuve breastfeeding to ther babies. Conclusions: The practice of exclusive breasfeeding by mothers to babies is closely associated with various factor, including individual factors and community factors.

Abstract

Dukungan lebih lanjut terhadap deteksi kasus dan pengobatan ibu hamil diperlukan untuk dapat meningkatkan pencapaian program menurunkan AKI (Angka Kematian Ibu) di Kabupaten Kediri. Pada tahun 2022 cakupan K1 dan K4 di Kabupaten Kediri berada di bawah rata-rata provinsi serta cakupan pertolongan persalinan oleh tenaga kesehatan Kabupaten Kediri berada pada peringkat terendah kedua di Provinsi Jawa Timur dengan cakupan hanya mencapai angka 81%. Untuk itu peran kader dan volunteer dalam pendampingan ibu hamil dan nifas sangat diperlukan. Tujuan pengabdian kepada masyarakat ini adalah untuk meningkatkan kemampuan yang meliputi pengetahuan, keterampilan, serta pemahaman kader dan volunteer mengenai pelaksanaan pendampingan ibu dan anak di Kabupaten Kediri. Metode yang digunakan dalam pengabdian kepada masyarakat ini adalah ceramah, diskusi, tanya jawab, dan roleplay kepada kader dan volunteer pendampingan. Kegiatan capacity building dilaksanakan secara hybrid pada tanggal 11-12 Juli 2024 di Auditorium RSUD Simpang Lima Gumul Kediri dan Zoom meeting. Kegiatan ini diawali dengan pre-test, pemberian materi, roleplay, serta post-test. Hasil dari kegiatan didapatkan peningkatan kemampuan kader dan volunteer dari nilai rata-rata pre-test 56,71 meningkat menjadi 79,14 pada post-test, artinya terdapat peningkatan pengetahuan sebesar 22,43% setelah peserta diberikan materi serta pelatihan KAP.

Abstract

Introduction: Reducing the Maternal Mortality Rate (MMR) to <70 per 100,000 live births is a global SDG target. Despite a decline in maternal deaths, infant mortality in East Java remains high. The Geliat Airlangga–UNICEF program applies a Penta Helix approach through continuity of care to improve maternal and child health outcomes. Objective: To analyze the effect of continuity of care for pregnant women on newborn birth weight, immunization, and postpartum contraceptive use in Jember District. Method: This observational analytic study involved 100 pregnant women supported by volunteers from December 2024 to June 2025 in three health centers. Data were collected from logbooks and analyzed using the Spearman correlation test. Result and Discussion: Most respondents (90%) were aged 20–35 years, 70% were primigravida, and 86% had MUAC >23.5 cm. A weak positive correlation was found between support frequency and newborn birth weight (r=0.185; p<0.05). No significant correlation was found with immunization status (r=0.163; p>0.05), while a weak negative correlation existed with postpartum contraception (r=-0.227; p<0.05). Continuous support improved maternal awareness and early detection but had varied effects on postpartum practices. Conclusions: Continuity of care positively influences pregnancy readiness and newborn outcomes. Strengthening community-based, volunteer-supported maternal care can contribute to reducing maternal and infant mortality in Jember District.
