Abstract

Utilization of health services by pregnant and postpartum women can save women from the incidence of maternal death and severe complications during pregnancy and postpartum. Several factors that directly contribute to the utilization of health services are decision making and sources of information obtained by pregnant and postpartum women regarding the utilization of health services. This study aims to analyze gender equality in decision making on the use of health services in urban and rural areas of Indonesia. This study used a mixed-method approach from primary data and a literature review. The target in this article was pregnant women and postpartum mothers who were spread out in several health centers in Surabaya City and Bojonegoro Regency with a total of 175 respondents who were calculated using the proportion estimation formula. Data analysis used univariate and bivariate analysis. The results showed that the wife in terms of the number of frequencies dominated in making decisions on the use of health services (92.5%). In addition, when the decision maker is the husband, all postpartum mothers (100%) will take advantage of health services. Related to the source of information that many respondents chose in providing information related to pregnancy health for the use of health services, it was health workers (86.3%) compared to other media. The wife's decision is a determinant in making decisions about the use of health services. On the other hand, husband’s support as a decision maker can also influence his wife to take advantage of health services.

Abstract

Background:  Opportunity cost is a loss caused by either income or other activities that are not carried out and can potentially be lost due to waiting for prescription services.  In the hospitals in Surabaya, it is known that, the waiting time for patient prescription services that utilize national health insurance in outpatient pharmacies exceeds the minimum service standards. Aims:  to analyse the opportunity loss and cost of patients utilizing the National Health Insurance from prescription service activities in which the recommendation cost control can be designed. Methods: A case study using an observational approach has been applied concerning activity-based management theory for defining non-value-added activities. Opportunity cost is converting the waiting time for prescription services to the income lost while waiting for the prescription. 120 patients who use national health insurance for obtaining prescriptions were recruited. Data were analysed using an observational table and calculated the service process and activity process. Results: Patient's opportunity loss for concoction drugs is 75.01 minutes, chronic non-concoction drugs are 71.14 minutes and non-chronic finished drugs are 46.3 minutes. The patient average opportunity cost based on the type of work and clustered by diseases, namely civil servants (IDR. 125,088 for Concoction Medicine), Pensioners (IDR. 87,800 for all typical medicine), housewives (IDR. 146,500 for all typical medicine), private employees (IDR. 136,878.20 for Concoction Medicine), and self-employed (IDR. 172,624.45 for Concoction Medicine). Conclusions:  The findings suggest the recommendation for cost control through optimizing cost management and procurement of incentive programs if opportunity costs fall. Abstract

Organizational Citizenship Behavior (OCB) is a voluntary act of creativity and innovation designed to enhance an individual's or organization's performance with persistence and enthusiasm, involving taking more responsibility and encouraging others to do the same. One of the challenges that must be faced is low OCB. Instances reflecting low OCB behavior include difficulty finding replacements when colleagues are not available, a lack of commitment to tasks beyond routine work, and a strong sense of indifference. Therefore, an analysis is needed to determine the relationship between tenure, job satisfaction, organizational culture, and OCB at Delta Surya Sidoarjo Hospital. The research method used is descriptive quantitative. The number of randomly selected respondents was 165. There were two variables in this study: the independent variable and the dependent variable. Independent variables include tenure (X1), job satisfaction (X2), and organizational culture (X3). The dependent variable in this study was Organizational Citizenship Behavior (Y). Two data analysis techniques employed in this study: the linear regression test and the chi-square test. Based on the research results, it was found that the measurements of Organizational Citizenship Behavior for Delta Surya Hospital employees on December 19, 2022, showed that 33.33% of employees had high OCB, while 66.66% had low OCB. Using the linear regression test, it was stated that tenure did not affect OCB (sig = 0.396), Organizational Culture affected OCB (sig = 0.000), and job satisfaction influenced OCB (sig = 0.000). Job satisfaction at Delta Surya Hospital is high, while Organizational Culture and Organizational Citizenship Behavior are low. Job Satisfaction and Organizational Culture affect Organizational Citizenship Behavior, while tenure has no effect.
