Abstract

Penyakit hipertensi dan diabetes melitus termasuk penyakit terbanyak pada kelompok lanjut usia di Indonesia, yaitu sebesar 57,6% pada penyakit hipertensi dan 4,8% pada penyakit diabetes melitus. Dalam upaya pengelolaan penyakit kronis yang cenderung meningkat. BPJS Kesehatan menyelenggarakan program untuk menangani masalah penyakit kronis seperti hipertensi dan diabetes melitus tipe 2 yang bertujuan mendorong peserta penyandang penyakit kronis mencapai kualitas hidup optimal sehingga dapat mencegah timbulnya komplikasi penyakit. Survei awal yang dilakukan peneliti pada bulan Januari tahun 2019 terhadap penderita penyakit kronis di Puskesmas Darussalam menunjukkan bahwa rendahnya kunjungan masyarakat terhadap Program Pengelolaan Pengelolaan Penyakit Kronis (Prolanis) hal ini disebabkan oleh kurangnya dukungan keluarga dimana beberapa pasien datang berkunjung tanpa ada pendamping/keluarga. Tujuan penelitian ini adalah untuk mengetahui faktor-faktor yang mempengaruhi pemanfaatan Prolanis pada lansia di Puskesmas Darussalam Medan. Penelitian ini merupakan jenis penelitian survey analitik dengan rancangan cross sectional. Populasi dalam penelitian ini adalah seluruh penderita hipertensi dan diabetes mellitus baik yang mengikuti kegiatan Prolanis maupun tidak mengikuti kegiatan Prolanis di Puskesmas Darussalam dengan teknik pengambilan sampel secara purposive sampling dan diperoleh besar sampel yaitu sebanyak 92 responden. Data diuji dengan Chi-Square. Hasil penelitian menunjukkan pengaruh yang signifikan antara pengetahuan (0,003) dan dukungan keluarga (0,001) terhadap pemanfaatan Prolanis. Dapat disimpulkan pemanfaatan Prolanis di Puskesmas Darussalam Medan dipengaruhi oleh pengetahuan dan dukungan keluarga responden, sedangkan peran petugas dan kebutuhan akan pelayanan tidak berpengaruh signifikan.

Abstract

Background:Tuberculosis (TB) patients with HIV/AIDS experience higher mortality rates, lower cure rates, and poorer treatment adherence compared to TB patients without HIV. As a result, TB can exacerbate HIV progression and lead to increased mortality. This study aims to investigate and summarize the determinants of TB incidence among people living with HIV/AIDS. Objective:This study employs a scoping review approach to explore the key factors associated with TB occurrence in HIV/AIDS patients. Methods:A literature search was conducted across indexed databases, including Scopus, PubMed, Google Scholar, and Crossref. Studies published between 2013 and 2021 were selected based on article titles, study design, population, intervention, and results. Fourteen relevant articles were included. Data were processed by identifying key sentences relevant to the coding framework, highlighting important findings, and organizing a narrative review framework. Qualitative data analysis was performed using NVIVO-12 Plus, with data presentation and report compilation. Results:The study identifies age, gender, education and knowledge level, nutritional status, use of antiretroviral therapy (ART), and cluster of differentiation 4 (CD4) cell count as the dominant factors influencing TB incidence among people living with HIV/AIDS. Conclusion:HIV infection weakens the immune system by attacking lymphocytes, leaving individuals with HIV/AIDS highly susceptible to Mycobacterium tuberculosis infection. Abstract

Human resources in a hospital are expected to always provide maximum and optimum performance in providing services to their patients. Nurses dominate the number of health officers in a hospital and hold an important role because they serve patients 24 hours. This study aims to determine the effect of training, incentive provision, work promotion, and participation in policymaking on the performance of nurses in Royal Prima Marelan General Hospital. This is a quantitative study with a descriptive-analytic design. The population consisted of all nurse executives in the Royal Prima Marelan General Hospital. Two hundred and twenty-four samples were taken using the total sampling technique. Data were analyzed with univariate, bivariate, and multivariate analysis. Results showed that age, education, and years of service affect the performance of nurses in Royal Prima Marelan General Hospital. However, gender did not affect the performance. Training affected nurse performance with a p-value of 0.05. Work promotion and participation in policymaking also affected the performance of nurses with a p-value of 0.05 on both aspects. Nurses’ characteristics, training, incentive provision, work promotion, and participation in policymaking simultaneously affect the performance of nurses in Royal Prima Marelan General Hospital. The most influential independent variable was participation in policymaking.

Abstract

Latar Belakang: Studi mengenai gangguan muskuloskeletal yang diakibatkan oleh aktivitas mendayung perahu atau sampan pada atlit atau nelayan telah banyak dilakukan. Namun, studi yang berfokus pada pendayung sampan untuk jasa transportasi masih terbatas. Aktivitas mendayung yang berat dapat mengakibatkan gangguan muskuloskeletal sehingga menurunkan kualitas hidup pekerja dan merusak kesehatan fisik dan mental mereka.Tujuan: Penelitian ini bertujuan untuk menganalisis hubungan usia, durasi kerja dan masa kerja dengan gangguan muskuloskeletal pada pendayung sampan.Metode: Penelitian ini menggunakan desain cross-sectional yang dilakukan selama bulan Januari 2021. Sebanyak 30 orang disertakan dalam pengukuran dan pengumpulan data. Pengumpulan data menggunakan kuesioner Nordic Body Map dan selanjutnya dianalisis secara statistik menggunakan uji Chi Square.Hasil: Hasil analisis menunjukkan sebanyak 12 pendayung sampan transportasi (40%) mengalami gangguan muskuloskeletal. Keluhan yang timbul pada pendayung adalah pada bagian punggung, pinggang, bahu, lengan atas, lengan bawah, paha, dan pergelangan kaki. Dari hasil uji Chi Square terlihat bahwa keseluruhan faktor risiko seperti usia (p value =0,044), durasi kerja (p value=0,025), dan masa kerja (p value=0,016) berpengaruh terhadap gangguan muskuloskeletal.Kesimpulan: Pendayung sampan berisiko mengalami gangguan muskuloskeletal, sehingga perlu adanya penambahan jam istirahat dan melakukan peregangan otot.

Abstract

Latar Belakang: Kepatuhan tenaga kesehatan dalam menerapkan protokol kesehatan COVID-19 sangat penting meminimalkan risiko infeksi COVID-19. Persepsi individu tentang ancaman/ keparahan penyakit akan mendorong seseorang dalam melakukan tindakan pencegahan. Tujuan: Penelitian ini bertujuan untuk mengetahui pengaruh persepsi dengan kepatuhan tenaga kesehatan terhadap protokol kesehatan COVID-19. Metode: Penelitian ini bersifat survei analitik dengan pendekatan cross sectional. Populasi pada penelitian ini yaitu tenaga kesehatan di Puskesmas Rambung. Teknik pengambilan sampel menggunakan total sampling (n= 43). Variabel dependen pada penelitian ini adalah kepatuhan tenaga kesehatan. Variabel independen yaitu persepsi penggunaan APD, persepsi physical distancing, persepsi hand hygiene. Pengumpulan data dalam penelitian ini dilakukan melalui wawancara dengan menggunakan kuesioner (persepsi penggunaan APD= 11 pernyataan, persepsi physical distancing= 5 pernyataan, persepsi hand hygiene= 5 pernyataan, dan kepatuhan= 14 pernyataan). Penelitian ini menggunakan uji statistik Chi-square (Confidence Interval (CI) 95%; α = 0,05). Analisis multivariat menggunakan regresi logistik. Hasil: Persepsi penggunaan APD (p= 0,001; PR= 15,2; 95% CI= 3,680-62,780), physical distancing (p= 0,047; PR= 6,667; 95% CI= 1,955-22,731), dan hand hygiene (p= 0,002; PR= 12,333; 95% CI= 2,859-53,206) memiliki pengaruh dengan kepatuhan tenaga kesehatan terhadap protokol kesehatan COVID-19. Variabel independen yang dominan adalah persepsi penggunaan APD (p=0,027; PR= 41,329; 95% CI= 1,54-1.109,178). Kesimpulan: Persepsi tenaga kesehatan terhadap penggunaan APD, physical distancing, dan hand hygiene di Puskesmas Rambung Kota Binjai memiliki persepsi baik dan tenaga kesehatan sebagian besar sudah patuh terhadap protokol kesehatan COVID-19. Variabel independen yang dominan yaitu persepsi penggunaan APD.

Abstract

Background: Controlling hypertension through pharmacological treatment is effective but can potentially lead to long-term side effects. Thus, more patients with hypertension have turned to herbal plants for treatment. The objective of this study was to examine the effectiveness of soursop leaf tea in reducing blood pressure in patients with hypertension.Methods: This study employed a quasi-experimental approach with a non-randomized pretest-posttest control group design, involving 30 participants in the intervention group and 30 participants in the control group. Data was analyzed using the Wilcoxon test to assess the disparity in mean blood pressure levels before and after consumption of soursop leaf tea. The Mann-Whitney test was used to examine the distinction in mean blood pressure between the intervention group and the control group.Results: In the intervention and control groups, most respondents were aged 56–65 years (60% and 66.7%), male (70% and 56.7%) and had suffered from hypertension for more than 5 years (70% and 56.7%). There was a significant difference in systolic blood pressure (p<0.001) and diastolic blood pressure (p<0.001) before and after the administration of soursop leaf tea. The mean systolic and diastolic blood pressure in the intervention group decreased significantly compared to the control group (p<0.001).Conclusions: Consumption of soursop leaf tea can reduce systolic and diastolic blood pressure, suggesting that this local herb has a potential effect on individuals diagnosed with hypertension.

Abstract

Background: The industrial sector is a vulnerable place for diseases, especially lung diseases such as asbestosis. Continuous exposure of workers to asbestos has caused the incidence rate to increase over time.Objective: This review aimed to explore the factors influencing the increased incidence of asbestosis in workers in the industrial sectoreMethods: The investigation involved analyzing the results of 17 relevant previous studies using qualitative analysis procedures, variables, themes and factors were extracted from the studies.Results: The analysis revealed eight main categories: age, gender, occupation, industry type, length of exposure, education, income and smoking history. Age, gender and length of exposure were found to be the most important risk factors for asbestosis in workers at an average age of 54 years in men who had been exposed for 21 years.Conclusion: In conclusion, with the increasing incidence of asbestosis from past exposure, this study is needed as an evaluation and consideration in reducing asbestosis in workers.

Abstract

Exclusive breastfeeding for the first six months are crucial for infant nutrition. The condition of mothers as workers is reported to be one of the factors that hinders exclusive breastfeeding, while the number of working mothers continues to increase every year. This study aimed to evaluate the level of exclusive breastfeeding and identify factors associated with the cessation of exclusive breastfeeding among working mothers. This research employed an analytical observational study. Data was collected from five health center areas in Langkat district, involving 165 working mothers. Samples were taken using a purposive sampling technique. Data analysis included univariate, bivariate (chi-square), and multivariate (logistic regression) analyses. This study reported low levels of exclusive breastfeeding among working mothers. Several factors were associated with breastfeeding, namely age (OR=2.13, 95% CI, 1.07-4.24); p=0.044), education (OR=2.40, 95% CI, 11.24-4.64; p = 0.009), knowledge (OR= 27.08, 95% CI, 10.88-67.37; p<0.001), attitude (OR=18.80, 95% CI,8.33-42.43; p<0.001) and husband's support (OR=8.14, CI 95%, 3.92-16.87; p<0.001). This study concluded that adequate knowledge, a positive attitude towards breastfeeding, and a husband's support are key factors for the success of exclusive breastfeeding for working mothers. Therefore, health workers must pay special attention to these aspects by providing education on the importance of breastfeeding for pregnant women and husbands by providing education of exclusive breastfeeding. It is suggested that future research design a model of public health education to increase the quality and quantity of exclusive breastfeeding among employed women.

Abstract

Latar belakang: Paparan polutan seperti emisi gas kendaraan dan kebisingan yang dihasilkan oleh aktivitas pengujian kendaraan bermotor dapat membahayakan kesehatan petugas. Berbagai literatur telah mengaitkan peningkatan tekanan darah yang dialami pekerja dengan paparan polutan di lingkungan kerja. Penelitian ini bertujuan untuk mengetahui apakah ada hubungan antara emisi gas kendaraan dan kebisingan terhadap tekanan darah pada petugas pengujian kendaraan bermotor.Metode: Penelitian ini menggunakan pendekatan kuantitatif dengan desain cross sectional. Penelitian dilakukan mulai dari Desember 2020 sampai Juni 2021. Populasi pada penelitian ini adalah 54 petugas pengujian kendaraan bermotor Dinas Perhubungan Kabupaten Deli Serdang dan keseluruhan populasi dijadikan sampel (total sampling). Pengukuran paparan karbon monoksida dan hidrokarbon menggunakan emission analyzer AET-2000S. Sedangkan pengukuran kebisingan menggunakan sound level meter. Data dianalisis dengan menggunakan uji Chi Square (α=0,05).Hasil: Hasil penelitian menunjukkan sebanyak 13 petugas terpapar karbon monoksida dengan paparan di atas nilai ambang batas. 25 petugas terpapar hidrokarbon yang melebihi nilai ambang batas. Sebanyak 21 petugas terpapar kebisingan melebihi ambang batas selama bekerja. Dari hasil pengukuran tekanan darah terlihat bahwa 20 orang memiliki tekanan darah di atas 140/90 mmHg. Uji statistik menunjukkan paparan karbon monoksida (0,006), hidrokarbon (0,014) dan kebisingan (0,001) berhubungan signifikan dengan peningkatan tekanan darah pada petugas pengujian kendaraan bermotor.Simpulan: Tindakan pencegahan yang disarankan berupa penggunaan alat pelindung diri selama melakukan pengujian kendaraan bermotor. Selain itu, perlu dilakukan pembatasan jam kerja untuk meminimalisir durasi paparan polutan pada petugas. ABSTRACT Title: Exposure to Vehicle Emissions and Noise in Relation to Blood Pressure Among Motor Vehicle Testing Officers Background: Exposure to pollutants, such as vehicle gas emissions and noise generated by motor vehicle testing activities, can jeopardize officers' health. Various studies have linked the increase in blood pressure experienced by workers with exposure to pollutants in the work environment. This study aimed to determine whether there is a relationship between vehicle gas emissions and noise on blood pressure among motor vehicle testing officers.Method: This study used a quantitative approach with a cross-sectional design. This study was conducted from December 2020 to June 2021. The population in this study was 54 motor vehicle testing officers of the Deli Serdang Regency Transportation Office, and the entire population was sampled (total sampling). Carbon monoxide and hydrocarbon exposure was measured using an AET-2000S emission analyzer. Noise measurements were performed using a sound-level meter. Data were analyzed using the chi-square test (α=0.05).Result: The results showed that 13 officers were exposed to carbon monoxide with exposure above the threshold value. 25 officers were exposed to hydrocarbons that exceeded the threshold value. A total of 21 officers were exposed to noise exceeding the threshold during work. Blood pressure measurements showed that 20 people had a blood pressure above 140/90 mmHg. Statistical tests showed that exposure to carbon monoxide (0.006), hydrocarbons (0.014), and noise (0.001) were significantly associated with increased blood pressure in motor vehicle testing officers. Conclusion: To mitigate the risks associated with pollutant exposure, it is recommended that workers engaged in motor vehicle testing wear personal protective equipment (PPE) and that working hours are limited.

Abstract

Background: Pregnant women with hypertension face elevated risks of complications, including preeclampsia and maternal mortality. Despite the increasing prevalence of gestational hypertension and its associated burdens, limited research—particularly at the local level—hinders effective prevention and management strategies. Purpose: This research aimed to investigate the risk factors associated with hypertension in pregnant women in Labuhan Batu Regency. Methods: This cross-sectional study involved 80 pregnant women from Labuhanbatu Regency, North Sumatra, Indonesia. Data on obesity, age, physical activity, and secondhand smoke exposure were collected through measurements and questionnaires. Obesity was assessed using mid-upper arm circumference (MUAC), while other variables were determined through interviews. Data were analyzed descriptively, and Fisher’s exact test was employed to compare risk factors between hypertensive and non-hypertensive pregnant women. Results: Obesity (p=0.29) and age (p=0.84) were not significantly associated with hypertension. However, pregnant women with light physical activity had a lower risk of hypertension compared to those with heavy physical activity (p=0.02). There was no significant association between exposure to cigarette smoke and hypertension (p=0.20). Conclusion: The findings suggest that light physical activity might be protective against hypertension during pregnancy among homemakers in Labuhanbatu regency.
