Abstract

ABSTRACT Cervical cancer is a type of cancer that develops in the cervix as a result of an infection with high-risk HPV. Cervical cancer patients have varying survival rates. This will be influenced by the patient's prognostic factors such as age, histopathological type, metastases, stage, tumor size, and treatment modalities. This research employs an analytical observational approach and utilizes a retrospective cohort research design. Survival rates will be measured using Kaplan Meier and log-rank based on prognostic factors. The research was carried out at RSUP Prof. Dr. I.G.N.G. Ngoerah, utilizing data from cervical cancer patients in the year 2021. The sample in this study was 245 samples. From 245 samples, 55 patients (22.4%) experienced events (died), 32 patients (13.1%) were uncensored (alive), and 158 patients (64.5%) were censored. The highest one-year survival rate based on prognostic factors was patients with age <40 years (78.8%), histopathological type of SCC (78.1%), no metastases (78.2%), patients with stage I (81%), tumor size < 2 cm (82.9%), and treatment modalities of chemotherapy, radiotherapy, and hysterectomy (90%). The results of the analysis based on the log rank test which reports only the incidence of metastasis factors show significant results on the one-year survival of cervical cancer patients. The overall one-year survival rate for cervical cancer patients is 77.6%. To determine the survival rate for cervical cancer patients over three and five years, further research is required. Abstract

Cervical cancer is a cancer caused by the oncogenic type of Human Papilloma Virus (HPV). This cancer ranks as the second common cancer in Indonesia. Among the 32,469 thousand new cases, there are 18,279 thousand of patients mortality. Cervical cancer can be prevented by early detection. The high prevalence of cervical cancer in Indonesia is due to the lack of public knowledge and awareness. In addition, the level of knowledge also affects the participation of women to conduct cervical cancer early detection. The faculty of medicine is very close to health issues and active to educate public regarding health. Therefore, the entire academic community is expected to be aware of and care about their respective personal health. This study is aimed to determining the correlation between cervical cancer knowledge level with early detection behavior towards employees at Faculty of Medicine, Udayana University. This study was an observational using a cross-sectional method and total sampling method for the data collection. Based on the results of the study, there was a significant relationship between the respondent’s knowledge level with cervical cancer early detection behavior (p-value <0.05). The group of respondents with insufficient knowledge, most of them said that they had never conducted early detection of cervical cancer as many as 8 people (80.0%). Meanwhile, most of the respondents with sufficient knowledge stated that they had conducted early detection of cervical cancer as many as 21 people (61.8%). Abstract

ABSTRAK Pendahuluan: Kanker serviks merupakan jenis tumor ganas yang bisa mengenai lapisan permukaan serviks. HPV memiliki pengaruh besar terhadap kejadian kanker serviks, vaksinasi HPV merupakan pilihan ideal untuk program pencegahan kanker serviks, namun untuk melakukan vaksinasi HPV tentunya dipengaruhi oleh pengetahuan dan sikap. Tujuan dari penelitian ini untuk mengetahui hubungan antara tingkat pengetahuan tentang kanker serviks dengan sikap kesediaan melakukan vaksinasi human papillomavirus pada siswi Sekolah Menengah Atas Negeri 3 Kota Bima. Metode : Penelitian deskriptif analitik ini menggunakan data primer yang di analisis dengan desain cross-sectional terhadap populasi siswa Sekolah Menengah Atas Negeri 3 Kota Bima sebanyak 193 sampel dengan melakukan pengambilan data menggunakan kuesioner. Hasil : Siswi yang memiliki tingkat pengetahuan baik 63,7%, siswi yang memiliki tingkat pengetahuan kurang 36,3%. Siswi yang memberikan sikap mendukung melakukan vaksinasi HPV 66,3%, siswi yang memberikan sikap tidak mendukung melakukan vaksinasi HPV 33,7%. Hubungan pengetahuan dengan sikap kesediaan melakukan vaksinasi HPV dari hasil analisis chi square memiliki hubungan signifikan dengan sikap kesediaan melakukan vaksinasi human papillomavirus (HPV) dengan p = 0,037 (p-value <0,05). Simpulan: Dapat disimpulkan bahwa terdapat hubungan antara tingkat pengetahuan dengan sikap kesediaan melakukan vaksinasi human papillomavirus. Perlu dilakukan penelitian lebih lanjut mengenai faktor lain yang mempengaruhi sikap mendukung terhadap kesediaan melakukan vaksinasi human papillomavirus. Abstract

Kanker serviks merupakan salah satu penyebab kematian terbesar pada wanita di Indonesia. Kanker serviks adalah keadaan tumbuhnya tumor ganas pada sel jaringan serviks. Salah satu pengobatan yang dilakukan sebagai terapi pada kanker serviks adalah radioterapi. Penggunaan radioterapi sendiri tentu memiliki efek samping tersendiri bagi pasien kanker serviks. Tujuan dilakukannya penelitian ini adalah untuk mengetahui profil efek samping radioterapi pada pasien kanker serviks di RSUP Prof. Dr. I.G.N.G. Ngoerah. Penelitian ini menggunakan consecutive sampling sebagai metode pendekatan dengan jumlah minimal 43 sampel. Sample didapatkan berdasarkan pengambilan data dan wawancara kuesioner yang diambil di RSUP Prof. Dr. I.G.N.G. Ngoerah dan data disajikan secara deskriptif. Berdasarkan hasil penelitian, didapatkan sampel sebesar 46 sampel dengan usia pasien berada pada rentang 31-60 tahun (89,1%) dengan persentase stadium tertinggi adalah stadium III-IIIC (58,7%). Efek samping yang dirasakan oleh pasien dengan persentase tertinggi adalah kelelahan (78,3%), diikuti dengan penurunan nafsu makan (63,0%), efek samping lainnya (52,2%), mual (43,5%), diare (43,5%), mulas (37,0%), nyeri BAB dan BAK (32,6%), muntah (8,7%), kerontokan rambut (4,3%), cemas dan perubahan kulit (2,2%). Penggunaan radioterapi sebagai terapi penyembuhan kanker serviks tentu memiliki efek samping bagi penggunanya. Efek samping yang sering dirasakan oleh pasien adalah kelelahan, penurunan nafsu makan, mual, muntah, diare, mulas, nyeri BAB, nyeri BAK, perubahan pada kulit, cemas, kerontokan rambut dan lain-lain.

Abstract

Kanker serviks merupakan penyakit ganas yang terjadi di serviks atau leher rahim dan merupakan salah satu kanker menyebabkan kematian terbesar di dunia. Melakukan aktivitas seksual usia dini merupakan salah satu faktor resiko terjadinya infeksi HPV yang nantinya akan berpeluang untuk berkembang menjadi kanker. Tujuan dari penelitian ini adalah untuk mengetahui hubungan melakukan aktivitas seksual usia dini dengan kanker serviks di Rumah Sakit Umum Pusat Prof. Dr. I.G.N.G Ngoerah. Penelitian yang digunakan adalah studi cross sectional deskriptif retrospektif dengan menggunakan data sekunder data rekam medis pasien kanker serviks. Teknik pengumpulan sampel yang digunakan dalam penelitian ini adalah minimal sampling dengan subjek yang memenuhi seluruh kriteria inklusi minimal sebanyak 43 orang. Distribusi pasien kanker serviks berdasarkan usia pertama kali berhubungan seksual adalah pengidap kanker serviks yang melakukan hubungan seksual dibawah 20 tahun sebanyak 55% dan penyidap kanker serviks yang melakukan hubungan seksual diatas usia 20 tahun sebanyak 45%. Melalui uji Chi Square didapatkan p value 0,001 (significance value <0,05) sehingga dapat disimpulkan bahwa terdapat hubungan signifikan antara melakukan hubungan seksual dibawah umur 20 tahun dengan terjadinya kanker serviks di RSUP Prof. Dr. I.G.N.G Ngoerah. Abstract

Background: Cervical cancer is the main cause of death for women worldwide. 500.000 new cases and 250.000 deaths are reported each year. In Indonesia, women as young as 25 are being diagnosed with this disease. The study will be conducted at Prof.Dr.I.G.N.G Ngoerah Hospital from January to December 2022 to analyze cervical cancer patients under 40. This study aimed to see the profile and the distribution of 40 year-old women with cervical cancer at our hospital. Methods: This descriptive study was conducted using medical records that were centered around female patients who were below 40 years of age and had been diagnosed with cervical cancer in Prof.Dr.I.G.N.G Ngoerah hospital in January - December 2022. Purposive sampling was done, and the samples were selected according to the inclusion criteria. The resulting data was presented in a table format. Results: Fifty-one cervical cancer patients aged under 40 years were included, with most of the age group being 30 – 39 years (88.2%). Twenty-eight patients (54.9%) were in the two-to-three parity group. The most dominant stage of cervical cancer is stage IIIB, with a total of 20 people (39.2%). Non-keratinizing squamous cell carcinoma (NKSCC) was the most common histopathological appearance (64.7%).  Conclusions: The study found that cervical cancer commonly affects those between 30-39 years old, particularly in the form of NKSCC. Many patients are diagnosed in advanced stages of the disease. Early screening, education, and vaccination programs are crucial in preventing cervical cancer.

Abstract

Hysterectomy remains one of the most frequently performed gynecological surgeries worldwide. While extra-fascial hysterectomy is the standard abdominal approach, complex cases involving large fibroids, severe endometriosis, adhesions, or malignancy often require alternative techniques such as the Aldridge method or retrograde hysterectomy. These challenging situations are associated with distorted pelvic anatomy, fibrosis, and adhesions, increasing the risk of complications, including injury to adjacent organs. This article reviews strategies and operative steps in managing complex abdominal hysterectomies, with emphasis on retrograde hysterectomy as a valuable approach when the cervicovaginal junction cannot be clearly identified or when extensive adhesions are present. The surgical technique is described step by step, supported by imaging and intraoperative findings, and highlights methods such as hydrodissection and careful dissection around critical structures to minimize morbidity. Preoperative imaging, intraoperative decision-making, and technical modifications are emphasized as key factors for optimizing patient outcomes. Although this study provides a detailed description of techniques under challenging hysterectomies, its limitation lies in the reliance on schematic illustrations rather than comprehensive intraoperative photographic documentation.

Abstract

Introduction: Breast cancer is the most prevalent malignancy among women and a notable comorbidity during pregnancy. Pregnancy-associated breast cancer (PABC) presents unique challenges due to physiological breast changes that complicate early diagnosis. This case report aimed to present a 31-year-old female with a diagnosis of bilateral breast tumors, pleural effusion, and multiple thoracic soft tissue tumors during her second pregnancy, highlighting the complexities and treatment considerations involved. Case presentation: A 31-year-old female patient, referred to Prof. Dr. I. G.N.G Ngoerah General Hospital, Denpasar, was in her second pregnancy at 20-21 weeks of gestation. She presented with bilateral breast lumps, worsening pain, and shortness of breath. Her obstetric history included a previous cesarean delivery and two abortions. Clinical examination revealed pleural effusion and multiple nodules suggestive of metastases. Biopsies confirmed tubular adenoma in the right breast and invasive breast carcinoma in the left breast. Ultrasound and chest x-ray findings indicated metastases to the liver and lungs. The patient was diagnosed with G4P1021 at 20 weeks of gestation with pregnancy-associated bilateral breast cancer and metastases. Conclusion: PABC diagnosis and treatment are fraught with challenges due to the need to balance maternal and fetal health. Delays in diagnosis, limited antenatal care, and the aggressive nature of PABC contribute to poor outcomes. This case underscores the necessity of early detection, prompt multidisciplinary intervention, and tailored treatment strategies to improve PABC patients' prognoses.
