Abstract

Pelaksanaan retensi telah berjalan di Rumah Sakit X Pekanbaru dimana masih perlu ada pembenahan terkait SOP pelaksanaanya, adapun tujuan dalam pengabdian ini mengenai kebijakan sistem retensi, pelaksanaan sistem retensi dan masalah atau hambatan retensi berkas rekam medis di rumah sakit. Pengabdian ini dilakukan dengan cara observasi dan wawancara, dengan hasil bahwa dalam retensi berkas rekam medis Rumah sakit X memiliki kebijakan pelaksanaan retensi berkas rekam medis yaitu, berdasarkan Peraturan Menteri Kesehatan No. 269/Menkes/Per/III/2008, Peraturan Direktur Rumah Sakit dan SOP yang berlaku di rumah sakit. Pelaksanaan retensi berkas rekam medis dengan memindahkan berkas rekam medis aktif ke inaktif walaupun terdapat hambatan yaitu kurangnya petugas untuk melakukan retensi, ataupun kurangnya rak penyimpanan berkas rekam medis. Berdasarkan hasil review artikel penelitian, pelaksanaan retensi berkas rekam medis dirumah sakit pada umumnya telah dilaksanakan berdasarkan kebijakan yang ditetapkan setiap rumah sakit walaupun terdapat masalah atau hambatan seperti kurangnya petugas ataupun tidak adanya jadwal retensi arsip dalam SOP.

Abstract

Inpatient medical records are essential documents that function as clinical, administrative, legal, financial, research, and managerial information sources in hospital health services. The completeness of documentation and the timeliness of returning medical record files are important indicators of service quality, continuity of care, and patient safety. Incomplete records and delayed returns may affect clinical decision-making, reporting accuracy, and hospital performance evaluation. This study aims to analyze the completeness of documentation and the timeliness of returning inpatient medical records at Zainab Mother and Child Hospital, Pekanbaru, in 2025, as well as to identify factors influencing these processes. This study employed a qualitative approach using a case study design. Data were collected through direct observation of inpatient medical record workflows, in-depth interviews with medical record officers, nurses, and physicians, and document review of standard operating procedures and medical record timeliness reports. Data triangulation was applied to ensure the credibility of the findings. The results indicate that incomplete documentation was still found in several important components, particularly in patient progress notes and authentication sections. In addition, delays in returning medical record files beyond the established 2 × 24-hour standard were identified. Contributing factors included limited human resources, high workload, lack of routine monitoring and evaluation, and inadequate supporting facilities. This study concludes that improving the quality of inpatient medical record management requires strengthening human resource competencies, enforcing compliance with standard operating procedures, and enhancing supervision and coordination among related service units.

Abstract

Digital transformation in healthcare has become a major focus of the Indonesian government through the development of the Mobile JKN application by BPJS Kesehatan. However, at Dumai General Hospital (RSUD Dumai), the utilization rate of this application remains low. This study aims to analyze the factors contributing to the low adoption of Mobile JKN in patient registration using the Innovation Resistance Theory (IRT) by Ram and Sheth (1989), which includes five dimensions: Usage Barrier, Value Barrier, Risk Barrier, Tradition Barrier, and Image Barrier. The research applied a qualitative case study approach involving key informants consisting of both users and non-users of Mobile JKN, as well as hospital registration staff. Data were collected through in-depth interviews and field observations, then analyzed thematically. The findings show that low adoption is influenced by limited digital literacy among elderly patients (Usage Barrier), perceptions that manual registration is faster (Value Barrier), concerns about data security and technical issues (Risk Barrier), strong preference for face-to-face interactions (Tradition Barrier), and the negative image of the application due to system errors and insufficient socialization (Image Barrier).

Abstract

Insiden keselamatan pasien merupakan kejadian tidak disengaja maupun kondisi yang berpotensi menimbulkan cedera yang sebenarnya dapat dicegah. Seluruh tenaga perawat dan bidan yang bekerja di rumah sakit tersebut dilibatkan sebagai responden, dengan pengumpulan data menggunakan kuesioner yang telah teruji validitas dan reliabilitasnya. Data yang diperoleh kemudian dianalisis secara bertahap untuk melihat gambaran umum, hubungan antar faktor, serta faktor yang paling berpengaruh terhadap pelaporan insiden keselamatan pasien. Hasil menunjukkan bahwa berbagai faktor seperti sikap, komunikasi, pelatihan, kerja sama tim, supervisi, komitmen manajemen, ketersediaan sarana dan prasarana, serta regulasi memiliki keterkaitan dengan pelaporan insiden. Namun, setelah dilakukan analisis lanjutan, faktor yang paling berpengaruh adalah regulasi dan komitmen manajemen. Hal ini menunjukkan bahwa keberadaan aturan yang jelas dan konsisten serta dukungan penuh dari manajemen menjadi kunci utama dalam mendorong tenaga kesehatan untuk melaporkan insiden. Dengan demikian, penguatan regulasi dan peningkatan komitmen manajemen menjadi langkah strategis dalam membangun budaya keselamatan pasien yang lebih baik di rumah sakit
