Abstract

Medical records are written evidence of the healthcare services provided by doctors and medical professionals to patients. They include patient social data, medical history, examinations, diagnoses, as well as treatments and medical procedures. One of the indicators of healthcare service quality is the availability of complete and accurate medical records. At RSUD Madani Pekanbaru, there is still a lack of understanding regarding medical record management. Patients often show low cooperation in providing personal data and medical information, which affects the quality of services. To address this issue, education was provided to the general public and healthcare workers about the importance of medical records in patient safety. The success of improving medical record management depends on the readiness of hospital facilities and the active participation of both patients and healthcare professionals. The method used was counseling/lectures, with the results showing that the activity was successful. This was reflected in the participants’ improved competence, active interaction, and high enthusiasm. Full support from the hospital, especially from the head of medical records, played a crucial role in the success of this activity, helping participants better understand standard medical record management.   Abstrak Rekam medis merupakan bukti tertulis dari proses pelayanan kesehatan yang diberikan oleh dokter dan tenaga kesehatan kepada pasien. Rekam medis mencakup data sosial pasien, riwayat penyakit, pemeriksaan, diagnosis, serta pengobatan dan tindakan medis. Salah satu indikator mutu pelayanan kesehatan adalah ketersediaan rekam medis yang lengkap dan akurat. Di RSUD Madani Pekanbaru, masih ditemukan rendahnya pemahaman tentang manajemen rekam medis. Pasien sering kurang kooperatif dalam memberikan data pribadi dan informasi medis, yang berdampak pada rendahnya mutu pelayanan. Untuk mengatasi hal ini, dilakukan edukasi kepada masyarakat umum dan petugas kesehatan mengenai pentingnya rekam medis dalam keselamatan pasien. Keberhasilan implementasi peningkatan manajemen rekam medis bergantung pada kesiapan fasilitas rumah sakit serta partisipasi aktif dari pasien dan tenaga kesehatan. Metode yang digunakan adalah penyuluhan/ceramah dengan hasil Kegiatan penyuluhan berjalan sukses dengan peningkatan kompetensi peserta yang terlihat dari interaksi aktif dan antusiasme tinggi. Dukungan penuh dari pihak rumah sakit, khususnya kepala rekam medis, menjadi faktor penting dalam keberhasilan kegiatan ini, membantu peserta lebih memahami manajemen rekam medis sesuai standar.

Abstract

Penyimpanan berkas rekam medis harus sesuai fungsi manajemen rekam medis seperti perencanaan (planning), pengorganisasian (organizing), pelaksanaan (actuating), serta pengawasan (controlling), maka kegiatan pelayanan terutamanya rekam medis dapat berjalan secara terencana, sistematis, berkesinambungan dan mencapai tujuan yang telah diciptakan. Berdasarka observasi ditemukannya fungsi perencanaan, pengorganisasian, pelaksanaan, dan pengawasan yang belum terjalankan dengan baik dikarenakan adanya kendala dalam fungsi manajemen tersebut. Tujuan penelitian ini adalah untuk mengetahui penyelenggaraan fungsi manajemen di bagian penyimpanan berkas (filing) unit rekam medis di Rumah Sakit Umum Daerah Petala Bumi Provinsi Riau Tahun 2024. Metode penelitian yang digunakan adalah kualitatif dengan jumlah informan 3 orang. Lokasi penelitian di Rumah Sakit Umum Daerah Petala Bumi Bagian Filing pada bulan Juni�Juli 2024. Pengumpulan data menggunakan pedoman wawancara, observasi, dokumentasi, alat rekam. Pengolahan data menggunakan triangulasi data, sumber dan metode. Analisis data yang digunakan adalah deskriftif. Fungsi manajemen untuk perencanaan (planning) sudah ada tetapi tidak semuanya didapatkan dan juga ada yang harus dihilangkan atau dihapuskan dalam perencanaan tersebut agar anggaran tercukupi untuk dibagian filing rekam medis. Untuk pengorganisasian (organizing) bagian filing memiliki 3 orang petugas dengan 1 orang adalah petugas pembantu. Untuk pelaksanaan (actuating) bagian filing masih memiliki kendala, salah satunya seperti tidak memiliki tangga untuk mengambil berkas yang berada di rak tinggi. Untuk pengawasan (controlling) bagian filing dilakuakn oleh kepala ruangan rekam medis dan penanggung jawab dengan waktu yang tidak menentu. Pada perencanaan (planning) masih ada perencanan yang belum terealisasikan. Pengorganisasian (organizing) masih memerlukan petugas untuk bagian filing. Pelaksanaan (aktuasi) ada kendala mengenai berkas rekam medis yang lama balik ke rungan, tidak adanya tangga sehingga memperlambat pengambilan berkas di rak paling atas. Pengawasan (controlling) tidak memiliki waktu yang pasti dalam melakukan pengawasan. Untuk perencanaan perlu merencanakan anggaran untuk memenuhi kebutuhan ruang filing. Organisasi perlu penambahan petugas filing. Pelaksanaan perlu memperhatikan berkas yang lama kembali ke ruang filing. Dan pengawasan perlunya dibuatkan jadwal pengawasan.

Abstract

The application of artificial intelligence technology in occupational safety monitoring systems within healthcare facilities has become an urgent necessity, particularly to support compliance with Occupational Safety and Health (OSH) standards in hospitals. This study aims to develop an automated detection model based on YOLOv12 to identify visual OSH elements in hospital archive rooms, such as APAR, evacuation signs, windows, and Personal Protective Equipment (PPE) including masks, gloves, and shoes. The initial dataset consisted of 2,866 documented images, which were expanded through augmentation to 6,886 images to increase data diversity and prevent overfitting. The YOLOv12 model was trained over 100 epochs using SGD as the optimization technique. The dataset was divided into three subsets training, validation, and testing in a proportional manner. Model evaluation employed metrics such as precision, recall, mAP@0.5, and mAP@0.5–0.95, supported by visualizations including the confusion matrix, F1-confidence curve, and precision-recall curve. One of the key advantages of YOLOv12 lies in its architectural efficiency and enhanced generalization capability, enabled by the integration of R-ELAN, Area Attention Mechanism, and FlashAttention. These components allow for broader receptive field processing with reduced computational complexity. Furthermore, the removal of positional encoding and adjustment of the MLP ratio make the model lighter and faster without compromising accuracy. Compared to previous versions (YOLOv8–YOLOv11), YOLOv12 demonstrates more stable and accurate performance in detecting complex OSH objects in indoor environments. The system was also implemented in a real-time user interface using Streamlit, automatically displaying personnel PPE completeness and room safety compliance status. In conclusion, the optimized YOLOv12 model has proven effective for real-time visual detection in OSH contexts. Future studies are recommended to incorporate data balancing approaches, spatial segmentation, and IoT sensor integration to expand the system’s coverage and resilience across diverse workplace conditions.

Abstract

Pasien rawat jalan sering mengeluhkan waktu tunggu di RSUD Rokan Hulu, khususnya di Poli TB-DOTS yang mencapai 1 jam 29 menit 45 detik. Keluhan tinggi terkait waktu tunggu berpotensi memengaruhi kepuasan pasien. Tujuan penelitian untuk mengetahui hubungan  lama waktu tunggu dengan kepuasan pasien terhadap pelayanan di Poli TB- DOTS RSUD Rokan Hulu Tahun 2023. Keluhan yang tinggi terhadap waktu tunggu berdampak pada kepuasan pasien. Metode penelitian analitik dengan pendekatan cross sectional.  Variabel independen lama waktu tunggu pelayanan, variabel dependen kepuasan pasien. Populasi sebanyak 176 pasien dengan sampel 64 responden.Teknik sampling Purposive Sampling, data penelitian menggunakan data primer, alat pengumpulan data berupa kuisioner survei kepuasan masyarakat dari Permenpan No. 14 Tahun 2017. Analis data yang digunakan univariat dan bivarat dengan uji chi-Square., Hasil uji statistik diperoleh ada hubungan lama waktu tunggu dengan kepuasan pasien terhadap pelayanan di Poli TB- DOTS (P-value 0,006). Manajemen dapat melakukan perencanaan yang lebih efektif terkait penempatan SDM, terutama Dokter Spesialis Penyakit Dalam di Poli TB-DOTS dan poli Paru. Petugas perlu meningkatkan efisiensi dan ketepatan dalam mengikuti SOP yang telah ditetapkan untuk mengurangi waktu tunggu pasien.

Abstract

ABSTRAK: Capaian pelaksanaan vaksinasi COVID-19 pada ibu hamil di Wilayah Kerja Puskesmas Rantau Kopar masih rendah yaitu 15% dari sasaran ibu hamil berjumlah 15.893 hanya 146 ibu hamil yang divaksinasi. Tujuan penelitian untuk memperoleh informasi mendalam tentang pelaksanaan vaksinasi COVID-19 pada ibu hamil di wilayah kerja Puskesmas Rantau Kopar Kabupaten Rokan Hilir. Metode penelitian yang digunakan kualitatif desain penelitian studi kasus dengan wawancara mendalam, telaah dokumen dan observasi. Jumlah informan utama 5 orang ibu hamil yang berada di wilayah kerja Puskesmas Rantau Kopar dan informan pendukung terdiri 1 orang pengelola program vaksinasi COVID-19, 5 orang suami ibu hamil dan 4 orang bidan desa. Hasil penelitian didapatkan bahwa terdapat kecenderungan persepsi negative, dukungan suami, sosialisasi penyuluhan vaksinasi, akses informasi yang diterima oleh ibu hamil terhadap kegiatan vaksinasi COVID-19 bagi ibu hamil diwilayah kerja puskesmas Rantau Kopar. Dapat disimpulkan bahwa perlu adanya strategi, advokasi, pemberdayaan masyarakat dan bina suasana oleh Puskesmas Rantau Kopar. Kata Kunci : Vaksinasi COVID-19, Ibu Hamil, Rantau Kopar

Abstract

Tuberculosis masih menjadi masalah kesehatan yang utama di Indonesia. Puskesmas Alahair Kabupaten Kepulauan Meranti mempunyai peran yang sangat penting dalam mensukseskan program ini, namun cakupan pelayanan kesehatan orang terduga Tuberkulosis masih rendah hanya 76% di tahun2023 dan tidak mencapai target SPM sebesar 100%. Tujuan  penelitian ini untuk menganalisis upaya pelayanan kesehatan orang terduga Tuberkulosis di Puskesmas Alahair Kabupaten Kepulauan Meranti. Jenis penelitian adalah kualitatif  Phenomenology dengan pendekatan evaluatif. Informan berjumlah 7 orang yaitu PJ program P2P, PJ program TB, PJ program promosi kesehatan, pasien TBC 2 orang, Tokoh masyarakat 2 orang. Informan dipilih menggunakan teknik  purposive sampling. Metode pengumpulan data wawancara mendalam dan telaah dokumen. Analisa data menggunakan teknik problem solving cyle meliputi analisa situasi, identifikasi masalah, prioritas masalah dan menentukan alternatif masalah dengan menggunakan Fishbone analysis dan analisis SWOT. Hasil penelitian ini rendahnya kesadaran masyarakat terhadap upaya pencegahan dan pengobatan Tuberkulosis, rendahnya partisipasi dalam skrining, dan stigma terhadap penderita Tuberkulosis. Kesimpulan Tantangan utama meeliputi reendahnya keesadaran masyarakat teerhadap upaya peenceegahan dan peengobatan Tubeerkulosis, reendahnya partisipasi dalam skrining, dan stigma teerhadap peendeerita Tubeerkulosis

Abstract

Dental and oral health is an integral part of overall body health. Having healthy teeth and mouth significantly helps in activities such as speaking, eating, and socializing without being disturbed by pain, discomfort, or embarrassment. However, the reality is that the level of dental and oral health in Indonesia is still relatively low. This study aims to analyze the dental and oral health of elementary school students at the Tembilahan Hulu Community Health Center, Indragiri Hilir Regency. This research employs a qualitative method through in-depth interviews, observations, and document review. The research informants were the Head of the Community Health Center and the dentist. Observations were made by directly observing the planning, implementation, and reporting processes, as well as documentation review. The results of the study identified the priority problem, which was the high number of dental caries cases in school-age children at the Tembilahan Hulu Community Health Center. The causes were limited human resources for implementing the dental and oral health program, non-functioning counseling and mass tooth brushing, lack of educational media, incomplete School Dental Health (UKGS) KIT, no Health Operational Assistance funds for the UKGS program, lack of knowledge and awareness of the importance of dental and oral health, and the family environment having incorrect habits such as improper tooth brushing. It is recommended for the Community Health Center to attend dental and oral health training, create educational videos on Instagram, and optimize the budget for activities such as counseling and mass tooth brushing.

Abstract

The Triple Elimination Program aims to prevent the transmission of HIV, syphilis, and hepatitis B from mother to child. In Rokan Hilir Regency, screening coverage remains low: hepatitis B (42.2%), HIV (38%), and syphilis (48.3%). Bantaian Health Center has the lowest coverage, while Bagan Batu Health Center has the highest. A thorough evaluation is needed to improve program performance using the Malcolm Baldrige approach. This study aims to evaluate the performance of the triple elimination program for pregnant women at the Rokan Hilir District Health Office in 2024, employing a mixed-method explanatory sequential design. The sample consisted of 40 respondents and 9 stakeholders as informants. Triangulation was applied to sources, methods, and data. The results show that most respondents rated program leadership as high (57.5%), while strategic planning (52.5%), patient focus (62.5%), measurement and knowledge management (60%), team focus (55%), process focus (57.5%), and performance results (60%) were rated low. Qualitatively, program leadership demonstrated strong commitment. Program planning is based on technical guidelines and BOK funds, with limitations in APBD budget allocations. Education and collaboration with community leaders have been implemented. However, the program has not focused on patients and is not widely recognized by the community. Staff at Bantaian and Bagan Batu Health Centers enthusiastically implement the program despite limited human resources, introducing innovations and conducting regular evaluations. The program is developed through coordination meetings, data analysis, and cadre training. Key factors for achieving targets include case monitoring, health education, cross-sector collaboration, and periodic evaluations. Health centers need to expand education and socialization on the importance of triple elimination screening, supported by integrated coordination between the Health Office and related agencies (OPD).

Abstract

ABSTRAK Peningkatan mutu pelayanan seharusnya tercermin dalam setiap kegiatan dirumah sakit (RS). Untuk menjaminnya, RS wajib memiliki sistem pengawasan mutu yakni dengan penerapan standar yang terdapat pada sistem akreditasi rumah sakit untuk mencapai Indikator Nasional Mutu (INM) dari 13 Indikator Nasional Mutu yang harus dilakukan RS. Hasil survei awal yang dilakukan ada 5 indikator yang belum mencapai standar akan berdampak terhadap hasil mutu pelayanan. Penelitian ini bertujuan diperolehnya informasi mendalam tentang kecenderungan penerapan Indikator Nasional Mutu (INM) peningkatan mutu pelayanan dalam pencapaian standar akreditasi rumah sakit. Penelitian menggunakan metode kualitatif desain fenomenologi melalui wawancara mendalam, observasi, dan telaah dokumen. Komponen yang diteliti yaitu kepatuhan kebersihan tangan, kepatuhan penggunaan APD, waktu operasi sectio caesarean emergency, waktu tunggu rawat jalan, dan kepatuhan pengisian alur klinis (clinical pathway). Dari penelitian ini didapatkan hasil bahwa kepatuhan kebersihan tangan, kepatuhan penggunaan APD, waktu operasi sectio caesarean emergency, waktu tunggu rawat jalan, dan kepatuhan pengisian alur klinis (clinical pathway) belum sesuai standar. Penerapan Indikator Nasional Mutu (INM) yang dilakukan melalui peningkatan mutu pelayanan dalam pencapaian Standar Akreditasi Rumah Sakit di RSUD Bangkinang tahun 2023 masih belum sesuai standar disebabkan oleh beberapa faktor, antara lain belum ada budaya organisasi, belum ada komitmen dari petugas. Belum optimalnya penggunaan sistem aplikasi pendaftaran online, belum optimalnya penggunaan aplikasi E-RM dan dukungan dari manajemen. Disarankan agar pihak rumah sakit dalam pencapaian indikator nasional perlu menambahkan poin penilaian kepatuhan pelaksanaan penerapan Indikator Nasional Mutu (INM) di Lembar Penilaian Kinerja dan memberlakukan sistem reward dan punishment. Kata Kunci: Indikator Nasional Mutu (INM), Peningkatan Mutu Pelayanan,   Standar Akreditasi Rumah Sakit  ABSTRACT  Service quality enhancement should appear in hospital activities. Each hospital is required to have a quality control system, which is designed to ensure that the level of performance of a system remains stable, or in control. For example, to achieve 13 National Health Care Quality Indicators, some standards can be applied in the hospital accreditation system. However, we still found that there were 5 indicators that have not reached the standard. Thus, these could have a significant impact on service quality results. The purpose of this study is to obtain more in-depth information about the tendency of National Health Care Quality Indicators implementation in order to improve service quality on hospital accreditation achievement. Phenomenological Qualitative Method was used in this study. The data were collected through interviews, observation, and secondary data. The component of health care quality; such as, hand hygiene compliance, PPE compliance, timing of emergency caesarean section, patient waiting time at outpatient department, and clinical pathway compliance, were analyzed in this study. The results confirm that the five components of health care quality; such as, hand hygiene compliance, PPE compliance, timing of emergency caesarean section, patient waiting time at outpatient department, and clinical pathway compliance; had not met the standard yet. Implementation of the National Health Care Quality Indicators for improving service quality to achieve hospital accreditation in RSUD Bangkinang (2023) has not met the standard yet. It happened because of some factors; such as, there is no organizational culture yet, there are no commitments from employees yet, the online registration application system and the E-RM application, and management support have not run properly. Therefore, the researcher hopes that the hospital is necessary to add the assessment of compliance with the implementation of National Health Care Quality Indicators in performance appraisal form, and to design reward as well as punishment systems. Keywords: National Health Care Quality Indicators, Service quality improvement, Accreditation Standards for Hospital

Abstract

Patient safety stands as a vital benchmark of healthcare quality, yet reporting incidents remains a challenge influenced by multiple human and organizational factors. This study explores the elements that shape nurses’ performance in reporting patient safety incidents at Permata Hati Duri Hospital in 2025. Using a quantitative analytical observational approach with a cross-sectional design, the research involved 85 inpatient nurses. Data were analyzed through univariate, bivariate (Fisher’s Exact test), and multivariate (logistic regression) techniques. Most participants were young adults aged 20–30 years (32.9%), predominantly female (91.8%), highly educated (84.7% D4/S1/Nursing), and experienced (80.0% with 6–>10 years). Findings revealed that most nurses demonstrated strong perceptions, positive attitudes, high motivation, and favorable views of leadership. Although bivariate analysis showed no significant association between independent variables and incident reporting, multivariate results indicated that work experience played the most dominant role (OR = 3.821; 95% CI = 0.010–1.025). The study concludes that enhancing reporting behavior requires consistent training, improved digital systems, and a reinforced culture of safety across all levels of hospital practice.
