Abstract

ABSTRACTBackground: Pre-eclampsia is a major cause of maternal and neonatal mortality and morbidity. The pathogenesismechanism of pre-eclampsia involves the interaction of genetic and environmental factors. Genetic variation in thegene MTHFR C677T (C> T transition) alter activity of MTHFR enzyme and predictrisk for pre-eclampsia but withconflicting results in worldwide population. This study aimed to investigate the association between gene variationof MTHFR C677T with pre- eclampsia in Jambi malay population.Method: This study was a case-control design. We compared the two subject groups; 30 pregnant women withpre-eclampsia and 30 normal pregnant women. Pre-eclampsia was diagnosed by elevated blood pressure (systolicbloodpressure ≥140 mmHg or diastolic blood pressure ≥90 mmHg) after 20 weeks of gestation.All samples wereethnic malay living in Jambi. The two groups were matched by age, gravida and parity. The Amplification RefractoryMutation System - Polymerase Chain Reaction was used for genotyping.Result and Conclusion: Association between the gene variation of MTHFR C667T and preeclampsia wasassessed using bivariate analysis with p<0,05. Statistical analysis of the additive model showed that the frequencyof TT genotype in thepre-eclamptic group was higher than normal pregnant women but did not show a significantdifference (OR = 0,833; 95% CI = 0,170 – 4,088; p= 0,568). Our study resultsuggest that the MTHFR C677T genevariation is not a risk factor for pre-eclampsia in theJambi malay population.Keywords: gene variation, Methylenetetrahydrofolate Reductase gene, MTHFR C677T, preeclampsiaABSTRAKPendahuluan: Mekanisme patogenesis preeklampsia melibatkan interaksi faktorgenetik dan lingkungan. Variasigenetik pada gen Methylenetetrahydrofolate Reductase (MTHFR) C677T (C>T transisition) menyebabkanpenurunan aktivitas enzim MTHFR yang berpengaruh pada peningkatan homosistein dan berkaitan dengankejadian preeklampsia. Penelitian ini bertujuan untuk mengetahui asosiasi antara variasi gen MTHFR C667Tdengan preeklampsia pada populasi melayu Jambi.Metode: Penelitian ini menggunakan rancangan kasus-kontrol. Kamimembandingkan dua subyek grup; 30 wanitahamil dengan preeklampsia dan 30 wanita hamil normal. Diagnosis preeklampsia ditentukan dengan adanyapeningkatan tekanan darah (tekanan darah sistolik ≥140 mmHg atau tekanan darah diastolik ≥90mmHg) setelah 20minggu kehamilan. Seluruh sampel merupakan etnis melayu yang berdomisili di provinsi Jambi. Kedua grup telahdicocokan berdasarkan usia ibu, gravida dan paritas. Pemeriksaan genotyping menggunakan metodeAmplification Refractory Mutation System - Polymerase Chain Reaction.Hasil: Asosiasiantara variasi gen MTHFR C677T dengan preeklampsia dinilai menggunakan analisis bivariatdengan p<0,05. Analisis statistik model additive menunjukkan frekuensi genotype TT pada kelompok hamil denganpreeklampsia lebih tinggi dibandingkan wanita hamil normal namun tidak menunjukkan perbedaan bermakna(OR=0,833; 95% CI=0,170 – 4,088; p=0,568).Kesimpulan: Hasil penelitian memberikan kesan variasi gen MTHFR C677T bukan sebagai faktor risiko terhadapkejadian preeklampsia pada populasi melayu Jambi.Kata kunci: variasi genetik, gen Methylenetetrahydrofolate Reductase, MTHFR C677T, preeklampsia

Abstract

ABSTRACT Background: eNOS play role in preeclampsia pathophysiology due to disturbing spiral arteries re-modelling and enhanced maternal vasoconstriction. A genetic variant of NOS3 associated with eNOS level. In addition, the previous study reported a genetic variant of NOS3 rs1799983 as a risk of preeclampsia with conflicting results beyond the population. To the best of our literature search, this genetic variant as preeclampsia risk has never been reported in the Jambi Malay population. This study aims to determine the association of the NOS3 genetic variant with preeclampsia in the Jambi Malay population. Methods: This study was a cross-sectional study that involved 43 pregnant women suffering preeclampsia and 53 normotensive pregnant women. All the subject resides in Jambi Province and was Jambi Malays. Genotyping was performed with tetra ARMS-PCR. Bivariate statistically analysis was performed. Result: The frequency of GT genotype was lower in the preeclamptic group and GG genotype was higher in the preeclamptic group than the TT genotype, but the difference was not statistically different for NOS3 rs1799983. Conclusion: Our findings indicate that the genetic variant of NOS3 rs1799983 is not associated with preeclampsia in the Jambi Malay population. Varian gen NOS3 berkaitan engan peningkatan kadar eNOS. Penelitian sebelumnya melaporkan hasil yang bertentangan pada berbagai populasi terkait hubungan antara varian gen NOS3 rs1799983 dengan risiko kejadian preeklamsia. Sejauh pencarian literatur, varian gen NOS3 rs1799983 sebagai risiko preeklamsia belum pernah dilaporkan pada populasi Melayu Jambi. Penelitian ini bertujuan mengetahui hubungan varian gen NOS3 dengan preeklamsia pada populasi Melayu Jambi. Metode: Penelitian ini menggunakan desain potong lintang, melibatkan 43 orang ibu hamil dengan preeklamsia dan 53 orang ibu hamil normotensif. Semua subjek berdomisili di Provinsi Jambi dan merupakan orang Melayu Jambi. Pemeriksaan genotip dilakukan dengan metode tetra ARMS-PCR. Analisis statistik dilakukan meggunakan uji bivariat. Hasil: Hasil analisis didapatkan frekuensi genotip GT lebih rendah pada kelompok preeklampsia dan genotip GG lebih tinggi pada kelompok preeklampsia dibandingkan genotip TT, namun perbedaan tersebut tidak berbeda bermakna secara statistik. Kesimpulan: Hasil penelitian ini mengesankan bahwa varian genetik NOS3 rs1799983 tidak meningkatkan risiko preeklampsia pada populasi Melayu Jambi. Abstract

ABSTRACT Background: Beyond controlling blood pressure and blood glucose, a genetic factor may play role in diabetic nephropathy (DN) progression in Type II Diabetes Mellitus (T2DM). Previous studies reported a genetic variant of Angiotensin rs699 (M235T) associated with diabetic nephropathy, with conflicting results across populations worldwide. To the best of our knowledge association of this genetic variant has never been conducted in the Jambi Malay population. Aims The Aims of this study is to the reveal association of the genetic variant of Angiotensin rs699 with diabetic nephropathy in the Jambi Malay population. Methods: This study design was cross-sectional. Totally 70 T2DM patients aged 22-67 years old were participated, as many 41 patients have aged older than 50 years old. Genotyping was performed using tetra ARMS PCR specific for rs699. Result: Patients aged older than 50 who have CTTT genotype have lower risk for suffering DN than CC genotype (p-value 0.029; OR (95% CI) 0.233 (0.06-0.88)), the statistic significant persist in multivariate analysis (p-value 0.030; OR (95% CI) 0.19 (0.04-0.86)). Conclusion: CTTT genotype was protective for DN in patients older than 50 years old. Further study with larger sample size, multi-centre and measuring confounding factors was needed. Studi sebelumnya melaporkan varian genetik Angiotensin rs699 (M235T) yang terkait dengan DN, dengan hasil yang berbeda antar populasi. Sejauh studi literatur yang dilakukan, varian genetik ini belum pernah diteliti pada populasi Melayu Jambi. Tujuan dari penelitian ini adalah untuk mengetahui hubungan varian genetik Angiotensin rs699 dengan nefropati diabetik pada populasi Melayu Jambi. Metode: penelitian ini merupakan penelitian potong lintang. Sebanyak 70 pasien T2DM berusia 22-67 tahun berpartisipasi, 41 diantaranya berusia lebih dari 50 tahun. Genotyping dilakukan dengan menggunakan tetra arms PCR spesifik untuk rs699. Hasil: Pasien berusia lebih dari 50 tahun yang memiliki genotipe CTTT memiliki risiko lebih rendah untuk menderita DN daripada genotipe CC (nilai p 0,029; OR (95% CI) 0,233 (0,06-0,88)), analisis multivariat juga menunjukkan hasil serupa (nilai p 0,030; ATAU (95% CI) 0,19 (0,04-0,86)). Kesimpulan: CTTT genotipe adalah pelindung untuk DN pada pasien yang lebih tua dari 50 tahun. Studi lebih lanjut dengan ukuran sampel yang lebih besar, berbagai pusat studi dan mengukur faktor risiko genetik dan non-genetik lain perlu dilakukan. Abstract

Introduction: Vitamin D have played role as nutrition for reproductive age healthy. Indonesia is tropicalcountry which got sunshine along the year but previous study reported high prevalence of deficiency and insufficiency of Vitamin D. Factor which influence Vitamin D level is complex and may specific for the population.Objective: This study aimed to determined Vitamin D plasma level and factors influenced in it.Methods: This cross-sectional study attended by 23 non pregnant women in reproductive age between 20-45 years old. Plasma level of Vitamin D measure with CLIA. Risk factor assessed with questionnaire.Results: This study reported all the women have deficient and insufficient. Factor which statisticallysignificant associated with higher Vitamin D plasma level were older age, higher sun exposure score, history for delivery the baby, not applied sunscreen and Vitamin D supplement intake.Conclusion: There are several factors influence plasma vitamin D level in women at reproductive age other than sun exposure.Key words: sun exposure, Vitamin D, women in reproductive age

Abstract

Background: Hypoxic conditions in preeclampsia cause oxidative stress in the placenta. Oxidative stress triggers endothelial dysfunction resulting in changes in placental parenchyma and vasculature, turn leads to clinical manifestations in maternal and neonatal. Previous studies reported that preeclampsia placental tissue increased the expression of the oxidative stress marker 8OH2dG.Objective: This study aimed to observe the expression of the marker 8OH2dG on the incidence ofpreeclampsia and its association with neonatal outcome.Methods: This study design was case-control, a number of 44 pregnant women participated in this study. The case group was preeclampsia women based on American College of Obstetricians and Gynecologist (ACOG) 2013 criteria and normotensive women as control group. The immunohistochemical was perform in placental tissue for expression of 8OH2dG in trophoblast nuclei. The results were positive when the trophoblast cell nuclei were stained ≥5% with moderate and strong intensity; and negative if stained weakly <5%. Bivariate analysis was performed to determine the association of 8OH2dG expression with preeclampsia and neonatal outcome.Results: This study reported the frequency of trophoblast nuclei with 8OH2dG positive staining was higher in the preeclampsia group than the control group. Expression of 8OH2dG positive staining also increased the risk of low birth weight and asphyxia.Conclusion: The results of this study indicate that the marker 8OH2dG is associated with preeclampsia and adversely neonatal outcome.Keywords: 8OH2dG, preeclampsia, oxidative stress, neonatal outcome

Abstract

ABSTRACT Background: Bone markings must be studied and mastered in learning skeletal system anatomy. The short and interesting material in the pocketbook helps students identify preparations during the practicum, increasing their understanding and gaining practical grades. Objective: This research aims to determine the effectiveness of using the Skeletal System pocketbook on grades practical anatomy block of the Integumentary System and Locomotion for medical students in the first semester of the Universitas Jambi in 2023. Method: This research used a quasi-experimental method with a pre-test and post-test control group. The 106 sample students were from the Medical Study Program at Universitas Jambi class of 2023. Result: The mean pre-test and post-test grades for students with pocketbooks were 61.86 and 78.04, while those without pocketbooks were 63.74 and 71.35. In the mean pre-test grade, there was no significant difference between the experimental and control groups' mean grades (p-value = 0.488). Meanwhile, in the mean post-test grades, there was a significant difference between the two groups (p-Value = 0.005), where the experimental group had a higher mean grade. Conclusion: There is a significant difference in using the skeletal system anatomy pocketbook on the grade of anatomy practicum for medical students at Universitas Jambi. Abstract

Background: Central obesity is a major risk factor for cardiovascular diseases, influenced by both genetic and lifestyle factors. The rs9939609 variant of Fat Mass and Obesity-Associated (FTO) gene has been associated with increased risk of central obesity, potentially modulated by dietary intake and physical activity. Objective: This study aimed to investigate the association between the FTO rs9939609 variant and central obesity, and its interaction with dietary intake and physical activity among adults in Jambi, Indonesia. Method: A cross-sectional study was conducted on 175 adults. Central obesity was defined using NCEP ATP III criteria. Dietary intake and physical activity were assessed using 24-hour food recall and the IPAQ, respectively. Genotyping was performed using the Tetra ARMS-PCR. Bivariate analysis was used to assess associations. Results: Individuals with the AT genotype had a significantly increased risk of central obesity (OR= 3.29; 95% CI: 1.30–8.35; p= 0.01). Under the dominant model (AAAT), the association remained significant (OR= 2.90; 95% CI: 1.17-7.16; p= 0.02). The risk was higher among individuals with high caloric intake (OR= 4.64) and low to moderate physical activity (OR= 4.77). Conclusion: The FTO rs9939609 variant is associated with increased central obesity risk, and in the presence of obesogenic lifestyle factors.

Abstract

Background: The PE is leading cause of maternal and neonatal mortality and morbidity worldwide. The etiology of PE remains not yet clearly identified, genetics contributed 20-40% of PE. The variation of the rs4343 gene was significant as a risk factor for PE and the severity of PE with effect sizes varying between studies. To the best of our knowledge, the association of rs4343 in the ACE gene for PE risk has not been reported to the population of Indonesia, especially the Jambi Malays. Aims: This study aims to reveal the association of Genetic Variation of ACE gene rs4343 with preeclampsia. Methods: This recent study was case-control, 78 pregnant women matching based on age and pregnancy number participated in this study. The preeclampsia diagnosis is based on the criteria of the American College of Obstetricians and Gynecologists. The genotyping method was tetra ARMS-PCR. Bivariate statistical analysis was performed. Results: The proportion of the AA genotype and AG genotype was slightly different between the two groups, but all GG genotypes suffered preeclampsia. The recessive model reported the GG/AG genotype has a higher risk of PE than the AA genotype (p-value 0.784; OR 1.16, 95% CI (0.40-3.41)). The allele model reported that the G allele (p-value 0.338; OR 1.59, 95% CI (0.62-4.14)) had a higher risk than the A allele. However, all those differences do not reach statistically significant. Conclusion: Genetic Variation of ACE gene rs4343 Has Lack Association with Pre-eclampsia.

Abstract

ABSTRACT Background: Medical students need to have good anatomy knowledge. FKIK UNJA changed the learning system to hybrid in response to the COVID-19 pandemic. An update on the skeletal system anatomy practicum guidebook is needed to improve student’s understanding. Objective: This study aims to determine the effect of using the skeletal system anatomy practicum guidebook on the anatomy grade of first-semester students Faculty of Medicine and Health Science Jambi University. Methods: This study applied a quasi-experimental method with a pretest-posttest control group design. The samples were 120 first-semester Jambi University medical students year 2022/2023 who participated in the axial skeleton anatomy practicum. Results: Students with the practicum guidebook pre-test mean grades were (40,58±11,36) and post-test mean grades were (75,74±7,26). Students without practicum guidebook pre-test mean grades were (27,49±13,89) and post-test mean grades were (72,92±5,13). There was a significant difference between the students with and without the practicum guidebook (p-Grades 0,030). Conclusion: The use of the skeletal system anatomy practicum guidebook significantly improves first-semester medical students’ anatomy grades. Abstract

Background: Tutorial discussions are a form of implementing Problem-Based Learning (PBL) in medical education. Students' cognitive, motivational, and demotivating aspects determine the effectiveness of tutorial discussions. The COVID-19 pandemic has changed learning methods to online learning, and the implementation of online tutorial discussions can affect the effectiveness of discussions. Objective: This study aims to determine the effectiveness of online PBL tutorials at Fakultas Kedokteran dan Ilmu Kesehatan Universitas Jambi (FKIK UNJA) Method: This is an observational study with a cross-sectional design, 260 students became respondents. Data was collected using a modified Tutorial Group Effectiveness Instrument (TGEI) Results: As many as 51.5% of students rated the implementation of online tutorial discussions as quite effective. Based on cognitive and motivational aspects, > 60% of students rated tutorial discussions as effective, while from demotivation aspects, 85% of students rated discussions as quite effective. Conclusion: The implementation of online tutorial discussions at FKIK UNJA was quite effective. In online tutorial discussions, positive elaborations are still formed among students. Students agree that the success of tutorial discussions is students' responsibility, and irresponsible group members when doing group assignments can reduce the effectiveness of discussions.
