Abstract

Interprofessional Education (IPE) is becoming a worldwide trend to be applied to the education of health workers. IPE is believed to be able to prepare students to be more ready to collaborate with various health professions in the world of work later. However, the challenge of implementing an ideal education and assessment system is still being debated. Evaluate the education system and IPE assessment of multicenter (3 universities) in achieving 4 domains of IPE competency. Furthermore, the results are used as a base for recommendations to improve the education system and student assessment. Cross sectional study to evaluate education process and IPE assessment take part into three health institute centers. The institutions are a part of JOGLOSEMAR (Jogjakarta, Solo and Semarang City). Research intended to evaluate competence achievement on 4 IPE domains following IPEC 2016. A multicenter survey involved students (n= 500), focused group discussion with academic lecturer (n=10 and focused group discussion with student representatives (n=10) established to evaluate the education system and IPE assessment. A qualitative descriptive data analysis performed by 2 independent researchers. Multicenter survey indicate that students have receiving positive impact from the IPE program to achieve 4 designated competence. FGD shows that challenge for the program occurred from activity scheduling as it necessary to accommodate the whole professions participation. Besides, challenge also coming from the ability of academic lecturer to provide sufficient supervision, availability of learning resources e.g. guide book and assignments, transparent and reliable evaluation items and last distinct guideline of student activity and any kind of violation. IPE Program from 3 academic center shows similar result, indicating that the program provides positive result to achieve student competencies. Challenges among the students and academic lecturer require appropriate management in order to organize a responsible study activity.

Abstract

Introduction: Tuberculosis (TB) carries a high morbidity and mortality, however, its diagnosis can be difficult due to low concentration of bacteria in the cerebrospinal fluid (CSF). Although increased CSF protein, decreased CSF glucose; cerebrospinal fluid (CSF) pleocytosis with mononuclear cells predominance are commonly used to diagnose TB infection, they show inconsistent results. Objectives: To analyze the relationship between several characteristics of the CSF and TB meningitis. Methods: This is an observational research, with a cross sectional design, 27 patients with TB meningitis and or meningoencephalitis with Thwaites diagnostic score ≤ 4 obtained from mecidal records. The definitive diagnosis of TB meningitis was made using the Ziehl Neelsen (ZN) staining and or culture TB, or polymerase chain reaction (PCR) amplification with the target in IS6110, Ziehl-Neelsen staining (ZN) culture method. Characteristics of CSF including increased CSF protein, decreased CSF glucose, cerebrospinal fluid (CSF) pleocytosis with mononuclear cells predominance and the CSF abnormalities were evaluated in the laboratory of molecular microbiology of Medical Faculty, Diponegoro University/dr Kariadi General Hospital, Semarang. The statistical analysis used was prevalent ratio (RP).Results: Over 27 patients, 12 (44.4%) were diagnosed with definite TB meningitis. Statistical analysis showed that prevalence ratio of the increased levels of protein CSF was 3.143 (95% CI: 0.502 to 19.692), decreased glucose levels was 1.750 (95% CI: 0.501 to 6.112), pleocytosis with a predominance of mononuclear cells was 2.5 (CI 95% : 1.238 to 5.048), whereas the CFS abnormality was RP 2.2 (95% CI: 1.087 to 4.454).Conclusion: This study has shown that pleocytosis with mononuclear cells predominance and CFS abnormalities have a positive relationship with definite TB meningitis.

Abstract

Abstract

Background: Diabetic ulcer is one of the most feared chronic infections due to Diabetes Mellitus because it can lead to amputation and death. Aim: To prove the effectiveness of sucralfate and platelet-rich plasma (PRP) combination for Vascular Endothelial Growth Factor (VEGF) expression in diabetic ulcer healing. Methods: This research is an experimental study of Phase I Clinical Trial with post-test only group design. There were 20 patients with diabetic ulcers divided into two groups, namely the treated group that was given sucralfate and PRP therapy and the control group was given standard therapy of normal saline drainage and gauze covered. Parameters were VEGF expression levels, wound area after being given therapy, and side effect from the treatment. Data on VEGF expression levels were obtained by means of examination with the Quantikine Human VEGF-ELISA Quantikine, R&D System, Inc, Minneapolis. The measurement of the wound area was assessed based on several criteria, namely grade 0 (no change), grade 1 (wound size reduced to less than of the previous wound), grade 2 (wound size was reduced to less than of the previous wound, but granulation was visible), and grade 3 (wound has closed completely). Result: In unpaired t-test, the mean VEGF expression was 98.18+10.96 in the treatment group and 66.69+23.79 in the control group which showed significant difference in VEGF expression levels (p = 0.003). In Mann-Whitney test, the mean wound area was 0.68+0.40 in the treatment group and 0.77+0.67 in the control group which showed that there was not any significant difference in wound area (p = 0.152). There were no side effects in both study group. Conclusions: The combination of sucralfate and PRP can increase VEGF levels significantly in diabetic ulcer patients but does not show a different effect in reducing wound area compared to standard treatment. The combination did not cause any side effects in the study subjects, as well as those using standard treatment.
