Abstract

Swamedikasi merupakan tindakan dalam memilih dan menggunakan obat tanpa resep dokter untuk mengatasi masalah kesehatan atau gejala tertentu yang dirasakan. Tujuan penelitian ini untuk analisis tingkat pengetahuan dan perilaku swamedikasi obat analgesik di Apotek Sumber Urip Jaya. Sampel yang digunakan pada penelitian ini sebanyak 105 responden. Instrumen penelitian berupa kuesioner pengetahuan yang terdiri dari 10 pertanyaan domain kepercayaan swamedikasi, 3 pertanyaan cara mendapatkan obat, 3 pertanyaan cara menggunakan obat, 3 pertanyaan cara penyimpanan obat, dan kuesioner perilaku swamedikasi sebanyak 10 pertanyaan. Penelitian ini dilakukan pada bulan Maret – Mei dan dilakukan di Apotek Sumber Urip Jaya. Analisis data menggunakan SPSS versi 16.0 dengan menggunakan uji chisquare. Hasil penelitian menunjukkan  sebagian besar berusia 36-45 tahun, dengan pendidikan terakhir SMA  dan berprofesi sebagai wiraswasta, mayoritas responden pada indikator kepercayaan swamedikasi (91,42%), cara mendapatkan obat (85,71%), cara penyimpanan obat (86,67%). Nilai signifikansi 0,000 yang artinya terdapat hubungan yang signifikan antara tingkat pengetahuan dan perilaku swamedikasi di apotek Sumber Urip Jaya

Abstract

Treatment of COVID-19 patients is differentiated based on the degree of symptoms. The purpose of this study was to determine the therapeutic profile and appropriateness of prescriptions for outpatient COVID-19 at Wava Husada Hospital for the period January–July 202, based on the COVID-19 Management Protocol Book, Edition 2, 2021 and the Hospital Formulary. The research method was a retrospective descriptive observational study with a purposive sampling technique. The research instruments were medical records from SIMRS and patient prescriptions. The results showed that most patients were male (53%), aged 18-44 years (60%). The treatment profile of COVID-19 patients with asymptomatic and mild symptoms, according to therapeutic class, were vitamins (40.3%), cough and cold (12.7%), antipyretic analgesics (9.1%), supplements (8.2%), and macrolide antibiotics (7.6%). The most common types of drugs were vitamin C 500 mg non-acidic (11.8%), vitamin E 400 IU (9.8%) and vitamin D 5000 IU (8.1%). The prescription compliance is 92% of drugs according to the COVID-19 Management Protocol Book Ed. 2 of 2021 and the rationality compliance is 79%. The prescription compliance according to the Hospital Formulary has been fulfilled 100%.

Abstract

Cerebrovascular accident (CVA) is a major cause of morbidity and mortality, often requiring intensive care unit (ICU) treatment. Patients with CVA are at high risk for secondary infections, which commonly lead to the use of antibiotics. This study aimed to evaluate the patterns and rationality of antibiotic use in CVA patients treated in the ICU of a regional hospital using the Gyssens method. A retrospective observational study was conducted involving 30 CVA patients who received antibiotics during their ICU stay. Data collected included patient demographics, comorbidities, types of antibiotics used, route of administration, combination patterns, and evaluation of prescription rationality based on the Gyssens criteria. The most frequently used antibiotic class was third-generation cephalosporins (60%), with ceftriaxone being the most prescribed (46.7%). All antibiotics were administered intravenously. Combination therapy was used in 20% of cases. Based on Gyssens categorization, only 33.3% of prescriptions were considered rational (category 0). The remaining 66.7% were irrational, including delayed initiation or discontinuation (13.3%), incorrect dosing (16.7%), unnecessarily broad-spectrum (10%), and no indication (6.7%). The study highlights a substantial proportion of irrational antibiotic use in ICU CVA patients, mainly due to suboptimal timing, dosing errors, and unnecessary broad-spectrum antibiotics. These findings underline the importance of antimicrobial stewardship and routine rationality assessments to ensure optimal antibiotic use and patient outcomes.

Abstract

Infeksi saluran kemih berdasarkan pemeriksaan biakan kemih diketahui disebabkan oleh bakteri. Perempuan umumnya empat hingga lima kali lebih mudah mengalami infeksi saluran kemih dibandingkan dengan pria. Antibiotik merupakan terapi utama ISK untuk mencegah infeksi semakin parah. Penata laksana anterapi antibiotik pada ISK didasarkan pada jenis bakteri, tanda dan gejala yang dialami pasien, letak infeksi, dan kondisi klinis infeksi. Tujuan penelitian ini adalah untuk mengetahui rasionalitas penggunaan anti biotic berdasarkan tepat antibiotik, tepat dosis, tepat rute pemberian, tepat interval pemberian dan tepat lama pemberian. Metode penelitian yang digunakan adalah metode deskriptif dan data yang diambil secara retrospektif. Populasi penelitian ini seluruh data pasien rawat inap dengan diagnosis infeksi saluran kemih di Rawat Inap Rumah Sakit Ibu dan Anak Lombok Dua Dua Surabaya Tahun 2023. Sampel penelitian ini adalah seluruh data rekammedis pasien ISK selama bulan Januari – September 2023. Hasil rasionalitas penggunaan antibiotic didapatkan evaluasi Rasionalitas penggunaan anti biotic meliputi tepat diagnosis (75,25%), tepat pasien (100%), tepat antibiotik (77,84%), tepat dosis (72,5%), tepat rute pemberian (100%), tepat interval pemberian (72,36%) dan tepat lama pemberian (74,02%). Rata – rata rasionalitas penggunaan antibiotik sebesar 91,70%, sedangkan 46,56% masih belum rasional dikarenakan pemberian antibiotik masih belum memenuhi aspek ketepatan diagnosis, pasien, antibiotik, dosis, rute, interval dan lama pemberian kepada pasien.

Abstract

The emergence of the SARS-CoV-2 virus in 2020 led to the onset of the COVID-19 pandemic, posing significant challenges globally. As of December 19, 2021, the worldwide confirmed cases surpassed 273 million, with a devastating death toll exceeding 5.3 million. Indonesia reported its first case on March 2, 2020, and has since faced escalating numbers, necessitating urgent attention.Chronic kidney disease (CKD) has emerged as a commonly occurring comorbidity among COVID-19 patients, drawing attention due to the vulnerability of CKD patients to medical complications. Understanding the use of antiviral drugs in CKD patients with COVID-19 is essential, considering the risks of drug toxicity and interactions. This study aims to investigate the frequency of antiviral drug usage in COVID-19 patients with CKD, providing insights into treatment strategies and guiding medical practitioners. Conducted at Dr. Soedomo Trenggalek Regional Hospital, the research utilizes a cross-sectional, non-experimental design and purposive sampling method. Data collection involves patients' medical records, focusing on demographics, antiviral drug usage, disease severity, treatment duration, mortality rates, and potential side effects. The demographic analysis reveals that the majority of COVID-19 patients with CKD are aged 50 and above, with males comprising 74% of the cohort. Age and gender influence disease severity, with older individuals and males at higher risk. Antiviral drug usage predominantly includes favipiravir (64%) and remdesivir (36%), underscoring their importance in treatment protocols. Decisions regarding antiviral therapy in CKD patients with COVID-19 require careful consideration of benefits, risks, and patient characteristics.

Abstract

This study aims to evaluate the cost-effectiveness of the immunosuppressant drug methylprednisolone at Gembleb Farma Pharmacy in Trenggalek, East Java, Indonesia. The research was conducted on May 30, 2025, using a descriptive quantitative approach supported by interviews with the responsible pharmacist. Data collection involved daily sales records of methylprednisolone and clinical response outcomes among patients. Each patient typically received five tablets per treatment, and the pharmacy recorded an average daily sale of 250 tablets. Therefore, it was estimated that approximately 50 patients were served per day. From those, 45 patients—equivalent to 90%—experienced favorable clinical responses to methylprednisolone therapy. The cost per tablet was IDR 500, resulting in a total daily cost of IDR 125,000. The calculated Cost-Effectiveness Ratio (CER) per day was IDR 2,777.78 per effectively treated patient. This value remained consistent when scaled to monthly (IDR 3,750,000 for 1,350 effective patients) and annual estimates (IDR 45,625,000 for 16,425 effective patients), yielding the same CER value of IDR 2,777.78. The findings indicate that methylprednisolone provides a stable and predictable cost-effectiveness profile in outpatient pharmacy settings. These results may support decision-making in therapeutic planning, particularly in community pharmacies that aim to deliver effective yet affordable immunosuppressive therapies. Additionally, this cost-efficiency aligns with the goals of rational drug use and sustainable pharmaceutical services at the primary healthcare level.

Abstract

Upaya untuk mendeteksi penyakit pada masyarakat, kegiatan bakti sosial merupakan kegiatan yang akan sangat membantu, dan kegiatan ini dapat meningkatkan kepedulian terhadap kesehatan diri. Pelaksanaan kegiatan Baksos di Desa Jongbiru Kabupaten Kediri bertujuan untuk memberikan pelayanan pemeriksaan dan pengobatan gratis kepada masyarakat. Kemudian memberikan informasi kesehatan yang baik dan benar kepada masyarakat sehingga dapat membentuk persepsi kesehatan yang positif dan pola hidup sehat. Kegiatan ini diikuti oleh 31 orang dengan usia rata-rata 18-67 tahun. Tahapan selama kegiatan berlangsung yaitu: perencanaan dan penyusunan proposal kegiatan. Observasi dan survey lokasi. Sosialisasi dan persiapan kegiatan. Pengumpulan masyarakat desa Jongbiru. Melakukan pemeriksaan kesehatan pada masyarakat. Konsultasi dengan apoteker dan pemberian informasi serta edukasi kepada masyarakat desa Jongbiru. Dengan demikian, Baksos sangat penting dalam mendeteksi penyakit sejak dini, meningkatkan kepedulian masyarakat terhadap kesehatannya, serta menyediakan akses konsultasi dan pemeriksaan kesehatan yang diperlukan. Hal ini penting untuk mencegah penyakit lebih lanjut dan meningkatkan kualitas hidup masyarakat.

Abstract

Leukemia remains a significant global health challenge, necessitating the exploration of novel therapeutic agents with improved efficacy and safety profiles. Elephantopus scaber (commonly known as Tapak Liman) is a medicinal plant traditionally used for various ailments, including cancer. Scabertopin, a sesquiterpene lactone isolated from E. scaber, has attracted scientific interest due to its potential antileukemic properties. This study aims to predict the antileukemic activity of Scabertopin through an in silico approach. The compound's chemical structure was retrieved in SMILES format from the PubChem database and analyzed using the PASS (Prediction of Activity Spectra for Substances) online tool. The prediction results indicated a high probability of antileukemic activity, with a Pa (probability of activity) value of 0.822 and a Pi (probability of inactivity) value of 0.004. These findings suggest a strong likelihood that Scabertopin exhibits antileukemic effects, supporting its potential as a candidate for further pharmacological and clinical investigations. The use of computational prediction tools provides an efficient and cost-effective preliminary screening method for identifying promising bioactive compounds from natural sources. This study contributes valuable insights into the therapeutic potential of Scabertopin and highlights the importance of traditional medicinal plants in drug discovery for leukemia treatment.

Abstract

Hipertensi merupakan penyakit tidak menular yang prevalensinya mengalami peningkatan. Hipertensi adalah keadaan dimana pada tekanan sistolik darah > 140mmHg sedangkan pada tekanan diastolik darah >90mmHg yang dilakukan pemeriksaan berulang. Prinsip penatalaksanaan terapi untuk hipertensi yaitu terapi farmakologi dan nonfarmakologi. Salah satu terapi nonfarmakologi yaitu pemanfaatan herbal jus untuk terapi alternatif menurunkan tekanan darah pada penderita hipertensi. Tujuan edukasi terapi hipertensi dan pemanfaatan herbal jus ini untuk menurunkan tekanan darah pada penderita hipertensi dan melatih masyarakat untuk menurunkan tekanan darah melalui pembuatan herbal jus secara mandiri di rumah. Bahan yang digunakan untuk pembuatan herbal jus meliputi : buah melon, brokoli hijau, blimbing wuluh, jahe, serai dan madu. Kegiatan yang dilakukan meliputi pemberian edukasi melalui pembagian leaflet yang berisi definisi, klasifikasi, penyebab serta penatalaksanaan farmakologi dan nonfarmakologi berupa terapi komplementer sebagai upaya mengatasi hipertensi dan mencegah komplikasi hipertensi dan demosntrasi pemanfaatan herbal jus untuk terapi alternatif hipertensi. Jumlah peserta kegiatan ini 25 orang. Kegiatan ini dimulai dengan pembukaan, pemberian edukasi melalui pembagian leaflet, demontrasi pembuatan jus herbal, tanya jawab dan feedback dari peserta. Masyarakat tampak antusias dan mampu mempraktikkan membuat jus herbal. Kegiatan tentang demonstrasi terapi nonfarmakologi untuk menurunkan tekanan darah dapat dilaksanakan rutin dirumah dengan alat dan bahan yang mudah di dapatkan.

Abstract

Methadone is a synthetic drug belonging to the opiate class that is given in oral dosage form to patients as a substitute therapy for opioid addiction. Methadone was chosen as the main substitution therapy because it has an effect resembling morphine and cocaine with a longer working life so that it can be given once a day which is used by drinking. This study aims to determine the factors that affect the retention value of drug addict patients in methadone maintenance therapy at Bangil Health Center. This study was conducted retrospectively, taking medical record data of patients who followed methadone maintenance therapy selected by consecutive sampling. Data obtained as many as 35 patients. The results of demographic data showed the most male sex 33 people (94.3%), aged 26-35 years 22 (62.9%), SMA 29 (82.9%), working 35 (100%), married 27 (77.1%), opium 35 (100%), amphetamines 26 (74.29%), alcohol 33 (94.29%) and tobacco 33 (94.29%). The highest initial dose was 30-50mg, 23 people (65.7%), the smallest maintenance dose <60mg, 33 people (94.3%) and the largest maintenance dose >120mg, 24 people (68.6%). The retention value of patients < 6 months is 7 people (20%) and > 6 months 28 people (80%)
