Abstract

Around 30% of hospital-acquired pressure injuries are caused by the use of medical devices. Children who are treated in intensive care units are at risk for medical device-related pressure injuries (MDRPI). This incident needs to be prevented considering the impact it has, namely prolonging hospitalization, increasing pain, increasing treatment costs, and requiring further care after discharge from the hospital. This review aims to identify risk factors that contribute to MDRPI in children who are in intensive care. A systematic review was carried out through a search strategy on an electronic database. A total of eight articles were selected and quality was assessed using the JBI critical appraisal guidelines. The face is the most frequently affected location for pressure injuries (54.5%), followed by the occiput (18.2%). Factors that contribute to MDRPI in children treated in the intensive care unit are the length of stay in the hospital, age of the respondent, knowledge of the nurse, attitude of the nurse, work experience of the nurse in the intensive care unit, leakage of the NIV mask, duration of device use, and gestational age. It is expected that nurses can carry out accurate risk assessments and apply appropriate interventions to prevent MDRPI events.

Abstract

Stunting is a chronic nutritional problem facing the world. Parental empowerment is necessary for stunting prevention. The main goal of this paper is to systematically review the literature to identify parental empowerment interventions and interventions to improve nutritional status in children, especially stunted children. This Systematic Review uses the PRISMA method, identifies articles published in English, in the last five years (2017 to 2022) through data based on ClinicalKey, ProQuest, SAGE Journal, Scopus, Science Direct, JSTOR, and EBSCO Host, and produces eight relevant literature. The results show a significant relationship between parental empowerment and the nutritional status of children. Parental empowerment is an effort to improve knowledge and skills as well as parents' self-confidence about the care, upbringing, and development of children. Indicators of parental empowerment are assessed based on education, employment, income, and parents' ability to make decisions. Interventions to improve the ability of mothers in the care of stunted children can be carried out through the assistance of health workers in the parenting process. Interventions to improve children's nutritional status through education to mothers about nutrition and nutrition delivery strategies, independently and across sectors. Z score from WHO is the most relevant nutritional status assessment indicator used. Abstrak: Stunting merupakan permasalahan gizi kronik yang dihadapi dunia. Pemberdayaan orangtua diperlukan untuk pencegahan stunting. Penulisan artikel bertujuan menganalisis dengan systematic review untuk mengidenfikasi intervensi pemberdayaan orangtua dan intervensi untuk memperbaiki status nutrisi pada anak, terutama anak stunting. Systematic Review ini menggunakan metode PRISMA, mengidentifikasi artikel yang diterbitkan dalam bahasa inggris, kurun waktu 5 tahun terkahir (2017 sd 2022) melalui data based ClinicalKey, ProQuest, SAGE Journal, Scopus, Science Direct, JSTOR dan EBSCO Host, dan menghasilkan 8 literatur yang relevan. Hasil dari analisis yang dilakukan mengidentifikasi ada hubungan yang signifikan antara pemberdayaan orangtua dengan status gizi anak. Pemberdayaan orangtua merupakan upaya untuk meningkatkan pengetahuan dan ketrampilan serta rasa percaya diri orangtua tentang perawatan, pengasuhan dan perkembangan anak. Indikator pemberdayaan orangtua dinilai berdasarkan pendidikan, pekerjaan, penghasilan dan kemampuan orangtua dalam pengambilan keputusan. Intervensi untuk meningkatkan kemampuan ibu dalam pengasuhan anak stunting dapat dilakukan melalui pendampingan petugas kesehatan dalam proses pengasuhan. Intervensi untuk memperbaiki status nutrisi anak melaui pendidikan pada ibu tentang nutrisi dan strategi pemberian nutrisi, secara mandiri maupun lintas sectoral. Z score dari WHO merupakan indicator penilaian status gizi yang paling relevan digunakan.

Abstract

This study aims to determine the effect of non-pharmacological therapy of watching cartoons in treating children's pain during infusion and venipuncture. The method used is a systematic review using guidelines from the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA). The research results showed ten articles selected after selection and quality assessment using guidance from the JBI critical appraisal tool. Watching videos effectively reduces pain in children during infusion and venipuncture with p<0.05. In conclusion, the videos watched by children are adjusted to the child's preferences and age. Nurses are expected to be able to use non-pharmacological pain management strategies to provide the best care for pediatric patients. Keywords: Watching Cartoons, Pain, Pediatrics, Venipuncture

Abstract

Stunting is a problem of chronic malnutrition in young children. Some of the causes of stunting were inappropriate of complementary feeding practices and lack of support from health cadres. One of the stunting locus areas in Indonesia is East Lombok Regency. An education programme needs to be conducted for stunting prevention, such as education of complementary feeding practices and socialisation of the book called "Rempah Lombok". The book was prepared as a guideline for cadres and mothers to provide recommended complementary feeding with local Lombok recipes. The indicator of the success of this activity was done by giving a question-and-answer quiz directly before and after the education process related to complementary feeding practices. The existence of programmes was expected to guide the cadres and mothers more easily and confidently in providing appropriate complementary feeding practices and improving the nutritional status of under-five children in East Lombok.

Abstract

Latar belakang: Demam pada anak  menyebabkan orangtua membawa anaknya ke pelayanan kesehatan dan menjadi penyebab utama anak dirawat di rumah sakit. Warm water sponge adalah satu mekanisme menurunkan demam pada anak dimana keberhasilannya bisa mencapai 100 %, namun penggunaannya di Indonesia baru sebatas kompres dengan teknik tepid water sponge. Oleh karena itu tujuan dari study ini adalah untuk melihat efektifitas kompres menggunakan teknik warm water sponge pada anak dengan peningkatan suhu tubuh. Metode: Desain yang digunakan adalah quasi eksperimental pre-posttest with control group design. Responden berjumlah 36 pasien anak yang mengalami demam yang dipilih menggunakan teknik consecutive sampling, terdiri dari 18 pasien anak menerima perlakuan warm water sponge dan 18 pasien anak menerima perlakuan kompres sesuai standar rumah sakit. Analisis yang dilakukan adalah uji T Independent. Hasil: Intervensi warm water sponge efektif menurunkan suhu tubuh anak yang mengalami demam dengan hasil uji T Dependent diperoleh  Pvalue =0,000 < α (0,05). Kesimpulan: Warm water sponge efektif menurunkan suhu tubuh pasien anak yang demam dan pasien anak lebih merasa nyaman dibandingkan dengan terapi penurunan panas yang biasa dilakukan di rumah sakit.

Abstract

Latar Belakang: Masalah pemenuhan kebutuhan oksigenasi pada anak sering disebabkan oleh ketidakpatenan jalan napas sehingga modifikasi posisi tubuh salah satunya posisi lateral untuk meningkatkan kelancaran oksigenasi pada anak merupakan intervensi yang efektif. Study ini bertujuan untuk mengidentifikasi pengaruh pemberian posisi lateral terhadap perubahan saturasi oksigen dan frekuensi pernapasan pada anak dengan gangguan pemenuhan kebutuhan oksigenasi di ruang rawat infeksi anak. Metode: Desain kuasi eksperimen. Responden dalam study ini berjumlah 36 orang yang dibagi ke dalam kelompok pemberian posisi lateral dan kelompok yang diberikan posisi sesuai standar perawatan rumah sakit sebagai kontrol, masing-masing 18 responden. Pengumpulan data dilakukan dengan mengukur saturasi oksigen dan frekuensi pernapasan sebelum dan sesudah diberikan intervensi. Analisis data menggunakan uji paired t-test dan Wilcoxon untuk menilai adanya perbedaan saturasi oksigen dan frekuensi pernapasan anak sebelum dan sesudah intervensi. Uji independent t-test dan Mann Whitney digunakan untuk menganalisis perbedaan saturasi oksigen dan frekuensi pernapasan antar kelompok. Hasil: analisis statistik menunjukkan terdapat perbedaan signifikan antara saturasi oksigen (p value < 0,05) dan frekuensi pernapasan (p value < 0,05) sebelum dan sesudah diberikan posisi lateral. Kesimpulan: posisi lateral dapat dijadikan salah satu intervensi keperawatan mandiri yang dapat diterapkan dalam memberikan asuhan pada anak dengan masalah pemenuhan kebutuhan oksigenasi di ruang rawat.

Abstract

Latar belakang: Keseimbangan cairan sangat penting untuk kelangsungan hidup  sehingga pengawasan cairan perlu diperhatikan. Pencatatan keseimbangan cairan diperlukan untuk menjadi dasar penilaian yang akurat dari status keseimbangan cairan pasien anak dan menjadi dasar pertimbangan pemberian terapi selanjutnya. Untuk memudahkan perawat melakukan pencatatan keseimbangan cairan yang akurat diperlukan inovasi menggunakan teknologi berbasis smartphone. Penelitian ini bertujuan untuk mengetahui perbedaan efektifitas pengawasan cairan menggunakan lembar observasi dengan aplikasi smartphone. Metode: Penelitian ini diawali dengan pengembangan aplikasi smartphone bernama SIMBaCaAn berdasarkan tinjauan pustaka yang dilakukan menggunakan pendekatan PICO (Population, Intervention, Comparison,Outcome). Uji efektifitas aplikasi SIMBaCaAn dilakukan pada 48 pasien anak yang dirawat di salah satu rumah sakit rujukan di DKI Jakarta. Analisis data dilakukan menggunakan uji statistik Kappa Test. Hasil: Pengawasan keseimbangan cairan yang menggunakan lembar observasi menghasilkan pencatatan data tidak lengkap sebesar 14,6%, data lengkap tidak tepat 20,8% dan data lengkap tepat sebanyak 64,4 %. Sedangkan pengawasan keseimbangan cairan menggunakan aplikasi SIMBaCaAn didapatkan hasil data tidak lengkap 18,7% dan hasil data lengkap tepat 81,3%. Dari hasil uji statistik, ditemukan perbedaan yang signifikan antara pengawasan keseimbangan cairan menggunakan aplikasi dan lembar observasi. Kesimpulan: Aplikasi smartphone lebih efektif terhadap pencatatan keseimbangan cairan secara lengkap tepat dibandingkan dengan lembar observasi manual.

Abstract

Background: Acute pain during suction in pediatric patients in the intensive care unit is unavoidable. However, it is necessary to provide non-pharmacological interventions that nurses can guide. One of the interventions that can applied in that situation is maternal voice. That intervention giving the opportunity to the children hearing the voice of their mother via voice recorder.Objective: This study was conducted to determine the effect of maternal voice on acute pain experienced by critically ill children during suctioning procedure.Methods: The research method used was quasi experiment pretest posttest without control group design. The study was conducted in May-June 2022 in the PICU in one of the hospitals in Jakarta. The research sample was selected by purposive sampling, 21 respondents assessed at the time of pretest and post-test (5 minutes before suctioning procedure, during procedure, and 5 minutes after procedure). The pain of the critically ill children was observed by FLACC (Face, Leg, Activity, Cry, Consolability) Scale.Results: The Saphiro Wilk test of normality data was carried out for all the data, either pretest or post-test data. Bivariate tests were analysed using Wilcoxon Signed Rank Test (for the abnormal data) and Paired T-Test (for the normal data) with the results of pain scores before and after being given maternal voice (when starting suction, p value = 0.001; after starting suction, p value = 0.001; 5 minutes after suction, p value = 0.002).Conclusions: PICU nurses should practice maternal voice along with suction to reduce the pain sensation received by children.

Abstract

Background: Family-centered care has evolved into patient and family-centered care. Although this is not a new concept; however, its application to nursing practice is very challenging among nurses due to its ambiguity. Objective: This study aimed to clarify the concept of patient and family-centered care for children. Methods: Walker and Avant’s concept analysis method was used. A literature search was also done using the following databases: Google Scholar, ProQuest, ScienceDirect, and Scopus, for articles published from 2011 to 2021.  Results: The defining attributes of patient and family-centered care are partnership, communication, respect, and compassion. Antecedents include patient and family involvement, readiness to collaborate and participate, competency and desire of the care professional team, supportive environment, and policies. Consequences of the patient and family-centered care include improved child outcomes and quality of life, promotion of patient safety, increased patient and family satisfaction, enhancement of humanistic values, reduction of hospitalization cost and length of stay, and decreased stress, anxiety, and depression in family members. Conclusion: Four attributes of patient and family-centered care, its antecedents, and consequences may aid researchers in better understanding the concept and its application in nursing practice. This concept can also be used to establish quality care delivery strategies and promote professional relationships between nurses, patients, and families in clinical settings.

Abstract

Background: Parents of newly diagnosed cancer children often experience significant concerns about the implications of the diagnosis. They find themselves in challenging decision-making situations, navigating through uncertain conditions following the disclosure. Uncertainty is a prevailing issue in healthcare, yet its interpretation and application within the context of health and nursing practice exhibit multiple meanings and variations.   Objective: This study aimed to clarify the concept of parental uncertainty arising from having children with cancer.                                                              Methods: The concept analysis followed the steps outlined by Walker and Avant. A thorough literature search was conducted using the Embase, CINAHL, and MEDLINE databases to identify articles published between 1980 and 2022. Results: The literature search yielded 147 articles from various disciplines, including health, nursing, medicine, psycho-oncology, and management. After analysis, 20 articles were selected for inclusion. This study identified four key attributes of parental uncertainty associated with having a child with cancer: (1) emotional disturbance, (2) unpredictability, (3) insufficient information, and (4) problems with decision-making. The antecedents of parental uncertainty encompassed the complexity of the disease, the diagnostic and treatment processes, the hospital environment, communication, and support from credible authorities. The consequences of parental uncertainty included the pursuit of new information, parental adaptability to their role, and parental distress. Conclusion: Understanding the four attributes of parental uncertainty, along with their antecedents and consequences, can facilitate the future development and operational translation of the concept of uncertainty in nursing practice. This concept is valuable in the evidence-based provision of nursing care to parents of children with cancer, enabling comprehensive recognition and assessment of parental uncertainty and the implementation of appropriate interventions that are sensitive to parental experiences. Ultimately, this approach promotes the adaptation of both the child and the parent.
