Abstract

Background: Discharge planning is an important process to ensure continuity of care after discharge from the hospital. This process helps reduce readmission rates, especially in children with chronic illnesses. Discharge planning can be a strategic approach to preventing readmissions by ensuring a smooth transition of care from the hospital to home. Purpose: To determine the effect of discharge planning in reducing readmissions in children with chronic diseases. Method: This study uses a systematic review approach with reference to the PRISMA guidelines. The articles analyzed came from the PubMed, SAGE, and ScienceDirect databases with a Randomized Controlled Trial (RCT) research design. A total of 4 (four) articles were selected based on inclusion criteria, discharge planning topics, focus on children with chronic diseases, and publications in the last 5 (five) years (2020-2024). Results: Comprehensive discharge planning significantly reduces readmission rates in children with chronic illness. Findings include that structured family education improves parental readiness to care for children and that social factors, such as parental literacy and household resource availability, influence discharge planning success. Case management provides superior outcomes in reducing readmission rates and improving patient quality of life. An as-needed follow-up approach is highly efficacious without compromising intervention effectiveness. Conclusion: Discharge planning is an effective intervention in reducing readmission rates and improving the quality of home care for children with chronic illness. Successful implementation requires integrated family education, social support, and case management. Adapting the intervention to account for social and cultural factors is also important to improve success across contexts.   Keyword: Children With Chronic Illness; Discharge Planning; Readmission.   Pendahuluan: Discharge planning adalah proses penting untuk memastikan kelanjutan perawatan pasien setelah keluar dari rumah sakit. Proses ini membantu mengurangi angka readmisi, terutama pada anak dengan penyakit kronis. Discharge planning dapat menjadi pendekatan strategis untuk mencegah readmisi dengan memastikan transisi perawatan yang lancar dari rumah sakit ke rumah. Tujuan: Untuk mengetahui efektivitas discharge planning dalam mengurangi readmisi pada anak dengan penyakit kronis. Metode: Penelitian systematic review mengacu pada panduan PRISMA. Artikel yang dianalisis berasal dari database PubMed, SAGE, dan ScienceDirect dengan desain penelitian Randomized Controlled Trial (RCT). Sebanyak 4 artikel dipilih berdasarkan kriteria inklusi, topik discharge planning, fokus pada anak dengan penyakit kronis, dan publikasi dalam 5 tahun terakhir (2020-2024). Hasil: Discharge planning yang komprehensif secara signifikan menurunkan angka readmisi pada anak dengan penyakit kronis. Temuan mencakup edukasi keluarga terstruktur meningkatkan kesiapan orang tua dalam merawat anak dan faktor sosial, seperti literasi orang tua dan ketersediaan sumber daya rumah tangga memengaruhi keberhasilan discharge planning. Manajemen kasus memberikan hasil superior dalam menurunkan angka rawat inap ulang dan meningkatkan kualitas hidup pasien. Pendekatan follow-up berbasis kebutuhan (as-needed) memiliki efisiensi tinggi tanpa mengurangi efektivitas intervensi. Simpulan: Discharge planning efektif dalam mengurangi angka readmisi dan meningkatkan kualitas perawatan di rumah bagi anak dengan penyakit kronis. Keberhasilan intervensi ini membutuhkan edukasi keluarga, dukungan sosial, dan manajemen kasus yang terintegrasi. Penyesuaian intervensi dengan mempertimbangkan faktor sosial dan kultural juga penting untuk meningkatkan keberhasilan di berbagai konteks.   Kata Kunci: Anak-Anak dengan Penyakit Kronis; Readmisi; Rencana Pemulangan Pasien.

Abstract

Stunting is the main problems for children in Indonesia, children who are already stunted can catch-up growth to minimize the impact of stunting. This literature study aims to identified the factors contribute to catch-up growth in children with stunting. Literature search several online databases with the keyword "Factors AND Stunting Catch-up Growth AND Child *". The selected articles were articles published in 2016-2020 in English and used a sample of children with stunting. The results of the literature study found 555 articles and 9 articles that met the inclusion and exclusion criteria. Several major factors that contribute to the catch-up growth of children with stunting are the level of education and height of the mother, economic status, socio-culture, nutritional supplements and community based intervention. Contributing factors can be used as considerations in providing interventions to support catch-up growth in children with stunting so as to reduce the impact of stunting.

Abstract

Aims: Providing medical equipment intervention increases the risk of MARSI in newborns with laceration and traumatic wounds in the subcutaneous tissue of the skin. The application of an adhesive or a skin barrier is one technique to lower the risk of MARSI. Nonetheless, past research has yielded mixed results in terms of its usefulness in reducing the risk of MARSI. Purpose: This study is to conduct a literature evaluation on the effectiveness of using a skin barrier to reduce the occurrence of MARSI. Methods: A systematic review of four databases was conducted, including Scopus, EBSCOhost, SAGE, and Wiley. A total of 8 publications were reviewed utilizing inclusion and exclusion criteria with a total end of article review. Results: The application of adhesive efficiently and considerably reduces the risk of MARSI, according to 5 of 8 articles. Conclusion: It is recommended that adhesive be used in an integrated manner on a regular basis to prevent the incidence of MARSI in babies.

Abstract

Children under five deaths caused by one of them by pneumonia every year in the world and Indonesia.Pneumonia causes respiratory problems that affect oxygen saturation. If not handled properly causes disabilityand death, then intervention is needed in the form of positioning that can improve children's health status.Objective: to identify research articles on positioning for an improvement in the health status of children withpneumonia. The method is literature review, which analyzes scientific articles from 3 databases: ScienceDirect, Whiley Online and Ebscohost. Keywords pneumonia, position *, and oxygenation. Criteria for inclusionof research articles, 2014-2019 and English. 9 journals obtained using the PICO formulation. Results:Semirecumbent, prone, fowler and lateral position adjustments can increase oxygen saturation, prevent VAPevents and stabilize hemodynamics. Conclusion: proper positioning is semirecumbent, prone, fowler, andlateral positions. Research is needed on the effectiveness of each position on the health status of children withpneumonia that is useful and can be implemented in nursing care in children with pneumonia in the hospital.

Method research used​ is approach qualitative with design phenomenology, which relies on interview deep against 29 participants aged​ between 10 and 18 years old. The results of the study show that condition physical, complaints physical, results laboratory, and conditions genetics become factor key in the early diagnosis of thalassemia. Support social from family and friends peers also play a role important in increase quality life teenager sufferer thalassemia. Findings This support importance approach holistic in management disease, which includes maintenance medical, education, and support psychosocial For increase ability teenager in manage condition they in a way independent. Programs such as Self-care Management of Adolescent Thalassemia expected can give contribution significant to strengthening self-care among teenager sufferer thalassemia in West Java.

Abstract

Ruam popok merupakan reaksi inflamasi akut pada daerah kulit perianal yang umum terjadi pada anak-anak. Tujuan dari studi kasus ini adalah melakukan pemenuhan kebutuhan kenyamanan pada anak yang mengalami gangguan integritas kulit dengan implementasi virgin coconut oil dengan pendekatan Teori Comfort Kolcaba. Hasil implementasi pada lima kasus terpilih menunjukkan bahwa terjadi perbaikan pada masalah keperawatan kerusakan integritas kulit dan nyeri akut yang dialami anak dengan ruam popok. Implementasi virgin coconut oil dapat memperbaiki kerusakan jaringan yang terjadi. Penerapan virgin coconut oil melalui pendekatan Teori Comfort Kolkaba dapat diterapkan pada anak dengan gangguan kenyamanan.

Abstract

An increase in HIV cases in housewives is at risk of increasing the population of children exposed to HIV. Caregivers of children exposed to HIV are at risk of having problems caring for children exposed to HIV. The difficulties experienced by caregivers can be affected by the condition of the child, who may or may not be infected with HIV. This study aims to get an n overview of the emotional experience of caregivers in caring for children underfive who are exposed to HIV. Methods: The research design uses a qualitative method with a phenomenology approach. Data collection was carried out using snowball sampling techniques and obtained from ten participants domiciled in Jakarta and its surroundings. The participants recruited in this study are caregiver of HIV-exposed children. Data collection was carried out using in-depth interview techniques. Data analysis uses thematic analysis using the Colaizzi method. This study obtained ten participants consisting of seven biological mothers, two children’s relatives and one foster parent. There were seven children with HIV-negative and five children with HIV-positive. Two caregivers in this study cared for more than one child exposed to HIV, namely one child with HIV-positive and one child with HIV-negative. The study found three themes: easy versus complex; grace and entrustment; and love and pity. This study concluded the that there are differences in emotional experiences among caregivers in caring for children underfive exposed to HIV influenced by the child’s HIV status. Positive affirmations and emotional support from health professionals adjusted to the values and socio-cultural condition of caregivers will benefit caregivers in providing adequate care for children.

Abstract

Pain is a health problem to consider especially for children and adolescents. Studies showed 15-35% of children in the nonclinical population suffered persistent and chronic pain. This study aims to provide an overview of the fulfillment of comfort needs in children with pain problems through the provision of Virtual Reality interventions. The application of Kolcaba's theory in providing nursing care for children with pain problems can provide holistic benefits and guide nurses in conducting systematic assessments. The application of virtual reality as an intervention in nursing care has also been proven to provide a sense of comfort and reduce pain and anxiety. The method used is a case study with a nursing care approach to 5 patients with pain problems aged > 10 years. The comfort level will be measured using the Comfort Behavior Checklist Scale. Cases were taken in the pediatric infection room of National Referral Hospital DR. Cipto Mangunkusumo.

Abstract

Background: Introduction: Unplanned extubation (UE) is a common adverse event in the Pediatric Intensive Care Unit (PICU) and can lead to clinical deterioration, increased morbidity, and decreased quality of care. Despite various preventative measures, UE remains a challenge in pediatric intensive care. Purpose: To synthesize current evidence on risk factors associated with unplanned extubation in pediatric patients to support the development of effective, evidence-based prevention strategies. Method: A systematic literature search was conducted in Scopus, ProQuest, and PubMed databases for observational studies published in the past 15 years following PRISMA guidelines. The methodological quality of included studies was assessed using the Newcastle–Ottawa Scale (NOS). Narrative synthesis was used to summarize and integrate the study findings. Results: Eleven observational studies met the inclusion criteria, with eight studies demonstrating a low risk of bias. The most consistent risk factors associated with UE were inadequate sedation and patient agitation. Other important risk factors include suboptimal endotracheal tube (ETT) fixation, high nurse-to-patient ratios, night shift care, and limited nursing experience. The relationship between patient age and UE risk has shown varying results across studies. Conclusion: The incidence of unplanned extubation in pediatric patients in the PICU is multifactorial and influenced by patient factors, clinical conditions, the care environment, and equipment and procedures. Modifiable factors, such as sedation management, standardization of ETT fixation, and optimization of nursing resources, are the most potential targets for intervention. UE significantly impacts the quality of care, particularly the quality of nursing care, due to variations in the preventive interventions implemented. Therefore, implementing a best-practice-based prevention bundle is crucial to reducing UE rates in the PICU.   Keywords: Pediatric Intensive Care Unit; Pediatric Patients; Risk Factors; Unplanned Extubation.   Pendahuluan: Unplanned extubation (UE) merupakan kejadian tidak diinginkan yang sering terjadi di Pediatric Intensive Care Unit (PICU) dan dapat menyebabkan penurunan kondisi klinis pasien, peningkatan morbiditas, serta penurunan mutu pelayanan. Meskipun berbagai upaya pencegahan telah dilakukan, kejadian UE masih menjadi tantangan dalam perawatan intensif anak.Tujuan: Untuk mensintesis bukti terkini mengenai faktor-faktor risiko yang berhubungan dengan kejadian unplanned extubation pada pasien anak guna mendukung pengembangan strategi pencegahan yang efektif dan berbasis bukti.Metode: Pencarian literatur sistematis dilakukan pada basis data Scopus, ProQuest, dan PubMed terhadap studi observasional yang dipublikasikan dalam 15 tahun terakhir dengan mengikuti pedoman PRISMA. Kualitas metodologis studi yang diinklusi dinilai menggunakan Newcastle–Ottawa Scale (NOS). Sintesis naratif digunakan untuk merangkum dan mengintegrasikan temuan penelitian.Hasil: Sebanyak 11 studi observasional memenuhi kriteria inklusi, dengan 8 studi menunjukkan risiko bias yang rendah. Faktor risiko yang paling konsisten terkait dengan kejadian UE adalah sedasi yang tidak adekuat dan agitasi pasien. Faktor risiko penting lainnya meliputi fiksasi endotracheal tube (ETT) yang tidak optimal, rasio perawat–pasien yang tinggi, perawatan pada shift malam, serta pengalaman perawat yang terbatas. Hubungan antara usia pasien dan risiko UE menunjukkan hasil yang bervariasi antar penelitian.Simpulan: Kejadian unplanned extubation pada pasien anak di PICU bersifat multifaktorial dan dipengaruhi oleh faktor pasien, kondisi klinis, lingkungan perawatan, serta aspek alat dan prosedur. Faktor-faktor yang dapat dimodifikasi, seperti manajemen sedasi, standarisasi fiksasi ETT, dan optimalisasi sumber daya keperawatan, merupakan target intervensi yang paling potensial. Kejadian UE berdampak signifikan terhadap mutu pelayanan, khususnya mutu asuhan keperawatan, akibat variasi intervensi pencegahan yang diterapkan. Oleh karena itu, implementasi bundle pencegahan berbasis praktik terbaik sangat penting untuk menurunkan angka kejadian UE di PICU.   Kata Kunci: Faktor Risiko; Pasien Anak; Pediatric Intensive Care Unit; Unplanned Extubation.

Abstract

Background: Children undergoing invasive procedures in hospitals, such as IV insertion or injection, often experience significant pain and anxiety, especially in critically ill children. Non-pharmacological interventions such as parental holding have been studied as an effective approach in reducing children's pain responses during procedures. Purpose: To analyze the scientific evidence regarding the role of parental holding in pain management in children during invasive procedures, including in critically ill children. Method: A search was conducted through PubMed, Scopus, and ScienceDirect databases following PRISMA guidelines. Inclusion criteria included RCT design, population of children undergoing invasive procedures, parental holding intervention, and outcomes in the form of pain intensity and/or anxiety. A total of 1,324 articles were identified, while 8 studies met the criteria and were analyzed narratively. Results: Showed that parental holding is effective in reducing children's pain and anxiety, as well as increasing emotional comfort for both children and parents. Despite its proven benefits, implementation of this intervention is still limited in clinical practice and lacks standardized guidelines, especially in pediatric intensive care units. Conclusion: Parental hugs have been shown to be effective in reducing procedural pain in infants and children during invasive procedures.
