Abstract

Background: The COVID-19 pandemic has disrupted clinical medical education, and clinical learning was forced to swiftly adapt by blending offline clinical services with online learning. Changes in the clinical learning environment will have an impact on professional identity. This study aimed to determine the impact of changes in the learning environment on medical students' professional identity. Methods: This was a two-phase mixed-method study with an explanatory sequential design. The PHEEM instrument measured students' perceptions of the learning environment before and during the pandemic, and PIM was used to measure professional identity. Fifty-one clinical students participated in this phase. In the second qualitative phase, fifteen students were selected to join three focus group discussions based on the proportion of sex and completion of the clinical rotation.Results: there was a significant decrease in student perceptions of teaching during the pandemic (p 0.003). This result is in line with qualitative findings, namely a decrease in opportunities for participation and interaction in the social aspects of learning. This lowered self-confidence in clinical competence was supported by a higher correlation between perceptions of the learning environment and professional identity before the pandemic than during the pandemic (respectively r 0.561; 0.554 p < 0.01).Conclusion: During the pandemic, there was a decrease in clinical students' participation opportunities, which resulted in decreased confidence in specific clinical competencies, but decreased participation had no effect on comfort and willingness to be engaged in the medical profession. This situation brings awareness to further improve competence through various training and practices.

Abstract

Background: The implementation of the national licensing examination for medical doctors is expected to have a broad impact on doctors and society. Indonesian Medical Doctor National Competency Examination (IMDNCE; Indonesian: Uji Kompetensi Mahasiswa Program Profesi Dokter or UKMPPD) as an ‘exit exam’ in the medical education process may significantly impact learning and clinical practices. The study’s objective was to explore the impact of IMDNCE on both the learning and clinical practices of doctors.Methods: The study subjects were internship doctors who had passed the IMDNCE. Participants were recruited by purposive sampling based on the ownership status of the institution (state/private) and the accreditation of respective medical schools and participants’ gender. The study participants consisted of 41 internship doctors working in 6 regions from 33 different medical schools out of 75 medical faculties in Indonesia.Results: IMDNCE has a positive influence on learning and clinical practice. The IMDNCE process emphasizes standard competencies that must be mastered and patient management process with current evidence that is not always achieved during clinical education, increasing clinical knowledge and skills, and increasing awareness of professional development. IMDNCE also has very well affected in the development of self-confidence and a sense of responsibility which are essential in clinical practice. In addition to psychological influences, IMDNCE improves competence in clinical skills and promotes good medical practice.

Abstract

ABSTRAK Pendidikan klinis pada profesi dokter dan profesi kesehatan lainnya berlangsung di situasi pelayanan kesehatan otentik. Proses pendidikan yang berjalan paralel dengan pelayanan ini sulit diprediksi dan tidak selalu dapat berjalan sesuai rencana. Keterbatasan waktu, jenis kasus yang tersedia, tuntutan pelayanan dan keterbatasan lingkungan fisik untuk pembelajaran menjadi tantangan yang dihadapi para pendidik klinis. Untuk meningkatkan kualitas pembelajaran klinis, dokter dan profesi kesehatan yang terlibat perlu dibekali dengan kompetensi khusus sebagai pendidik klinis yang tidak didapatkan selama pendidikan mereka sebelumnya, ataupun dari pelatihan khusus untuk dosen seperti PEKERTI dan AA. Kegiatan pengabdian ini bertujuan untuk meningkatkan pengetahuan dokter dan tenaga kesehatan mengenai aspek-aspek mendasar dalam pendidikan klinis. Kegiatan ini dilaksanakan secara daring melalui webinar. Topik yang disampaikan mencakup empat keterampilan dasar bagi pendidik klinis yaitu: menyusun perencanaan, komunikasi dan interaksi, penilaian, dan umpan balik. Dari 98 orang pendaftar, 62.4% adalah dokter, 15.3% bidan, 14.3% perawat, dan 8% tenaga kesehatan lainnya. Partisipan tidak hanya berasal dari institusi pendidikan di Provinsi NTB tetapi juga dari provinsi lainnya. Sebagian besar (75.5%) telah terlibat dalam pendidikan klinis, namun 57.14% belum pernah mengikuti pelatihan khusus. Enam puluh partisipan mengisi tautan presensi dan postes. Dari hasil pre dan postes didapatkan peningkatan rerata skor pengetahuan secara bermakna dari 4.46±2.39 menjadi 5.98±2.55 (p = 0.001, uji Wilcoxon). Seluruh partisipan yang mengisi postes tertarik untuk mengikuti pelatihan lanjutan. Partisipan juga berharap acara semacam ini dilaksanakan secara rutin dan diikuti dengan sesi praktik.

Abstract

Hypoparathyroidism is an endocrine disorder characterized by low or even absent production of parathyroid hormone (PTH). The most common cause of hypoparathyroidism is injury to the parathyroid gland or accidental removal during thyroid surgery. The diagnosis of hypoparathyroidism is made by assessing clinical symptoms, physical examination and laboratory tests. The author wants to know more about how to detect early and diagnose hypoparathyroidism. Abstract

Early marriage is a significant social issue that can have a negative impact on the health and well-being of teenagers, and has the potential to exacerbate the problem of stunting among children. This activity aims to outline community service activities focused on preventing early marriage as a strategy to reduce the prevalence of stunting. One of the institutions that is expected to be a pioneer in implementing activities to prevent early marriage and overcome stunting in West Nusa Tenggara is the Faculty of Maritime Medicine, Mataram University. The activity is aimed at identifying risk factors for early marriage and their impact on the incidence of stunting. This activity was carried out by providing counseling, attended by 40 teenagers who were grade 9 students and 5 teachers at MTs Nurul Huda, Pijot Village, Keruak District, East Lombok Regency, West Nusa Tenggara Province. The results of this service activity show an increase in understanding regarding the risks of early marriage and stunting, as well as a change in attitudes towards education and health. In conclusion, efforts to prevent early marriage have proven effective in supporting efforts to overcome stunting, by having a significant impact on improving the health and welfare conditions of teenagers and children. It is hoped that this program can become a model for similar community service activities in the future. Keywords: Early marriage, stunting

Abstract

Background: Climate change has become a global problem that has a wide impact on human life. It is a moral responsibility for humans to participate in protecting the earth and slowing the destruction of the earth. The role of medical and health profession education is important in producing health workers who are aware of the importance of protecting the environment and have leadership characteristics that are able to drive system change towards environmental improvement. This article is a recommendation from the I AM HPExplore Webinar Series 2022, Webinar #4. This article aims to provide literacy on the importance of eco-ethical leadership in medical and health education and how its development is included in the curriculum.Recommendation: Eco-ethical leadership is needed in the process of health professional education, especially in the process of forming future health workers who have these characteristics, so that they are able to bring health services that are more friendly to planetary health. The process of character building can be carried out at the individual, organizational, and system levels. In educational institutions, this step can be initiated by including it in the formal curriculum by inserting material into the existing curriculum, or by establishing a special learning program. Cultivating behaviors that support planetary health in the hidden curriculum can support character building. Conclusion: Planetary health is a global issue. For keeping the planet healthy, it requires mutual effort from the individual to the system level, multiprofessional and interdisciplinary collaboration, including from the field of education as a form of our moral responsibility. The formation of eco-ethical leadership needs immediate attention in the education of health workers.

Abstract

Antisipasi dalam menghadapi ancaman EREIDs perlu dilakukan sebagai upaya pengurangan dampak buruk bagi penduduk. Sebagai tempat berkumpulnya orang banyak dalam jangka waktu lama, kapal penumpang merupakan salah satu tempat berisiko menjadi hotspot penularan EREIDs yang efektif. Simulasi kesiapsiagaan EREIDs bertujuan untuk penyajian model EREIDs dipelabuhan dan diskusi perbaikan model EREIDs. Pelaksanaan simulasi dilakukan secara hybrid mengundang Pelindo, KSOP, BKK dan operator kapal dengan materi: kesan data kesiapsiagaan EREIDs di Lembar, diskusi tupoksi masing-masing stakeholder dan menggali saran perbaikan kedepannya. Penyajian model EREIDs meliputi: 1) pengendalian teknis (1.1. fasilitas sarpras, 1.2. modifikasi sarpras dan 1.3 penyediaan instrument keselamatan), 2) pengendalian administratif (2.1. penyediaan SOP keselamatan, 2.2 Pelaksana SOP Keselamatan dan 2.3 Pengawasan pelaksanaan SOP keselamatan) dan 3) penggunaan alat pelindung diri (3.1. penyedia APD dan 3.2. Edukasi penggunaan APD). Secara kualitatif didapatkan bahwa peran utama stakeholder masing-masing: 1.1. Pelindo; 1.2. BKK; 1.3. Pelindo; 2.1. BKK; 2.2. Pelindo, Operator Kapal, KSOP; 2.3. BKK, KSOP; 3.1. Pelindo, BKK; 3.2. BKK. Adapun tantangan kesiapsiagaan EREIDs di Pelabuhan dan Kapal adalah koordinasi antar stakeholder, permintaan catatan kejadian, dan rekapitulasi insiden yang masih harus diselaraskan kembali. Dapat disimpulkan bahwa peningkatan kesiapsiagaan EREIDs dipelabuhan dan kapal merupakan penyiapan lintas sector dan lintas bidang intra dan inter stakeholder. Semua stakeholder memiliki peran krusial dalam mencegah masuknya wabah dari luar pulau dan mengirim wabah keluar pulau.

Abstract

Indonesia merupakan negara maritim. Sebagian besar wilayah Indonesia merupakan peraiaran. Hal ini menyebabkan sebagian mata pencaharian masyarakat di daerah pesisir adalah sebagai nelayan. Dalam melakukan akitivitas sehari-hari sebagai nelayan, maka mereka tidak terlepas dari risiko apabila terjadi cedera akibat gigitan/sengatan hewan laut. Penanganan yang tidak tepat dapat memperlambat penyembuhan atau bahkan bisa berakibat fatal. Tujuan kegiatan ini adalah memberikan pembekalan berupa pengetahuan terkait jenis-jenis hewan laut yang berbahaya, mengenal karakteristiknya, serta pertolongan pertama terhadap gigitan/sengatan hewan laut serta melatih keterampilan peserta dalam menangani kasus-kasus sederhana akibat gigitan/sengatan hewan laut sebelum dibawa ke fasilitas kesehatan/ditangani tenaga medis. Kegiatan ini dilaksanakan di Aula Puskesmas Nipah, pada tanggal 4 September jam 09.00-11.00 Wita, dengan jumlah peserta 28 orang, yang merupakan nelayan dan masyarakat sekitar Pantai Nipah. Kegiatan ini meliputi penyuluhan dan pelatihan penanganan awal gigitan/sengatan/tusukan hewan laut. Penyuluhan terdiri dari beberapa tahapan, yaitu: pretest, penyempaian materi penyuluhan, tanya jawab dan diskusi, dan diakhiri dengan postest, sedangkan pelatihan teridiri dari 3 tahap, yaitu peragaan prosedur penanganan awal untuk kasus-kasus tersering akibat gigitan/tusukan/sengatan hewan laut oleh tim, peragaan oleh sasaran kegiatan, dan umpan balik dari tim. Dari hasil kegiatan, diperoleh bahwa terdapat peningkatan pengetahuan sasaran sebelum dan setelah diberikan penyuluhan dan pelatihan, dilihat dari hasil posttest dibanding dengan pretest, yaitu sebesar 68,66% Kegiatan ini perlu terus dilakukan bagi masyarakat pesisir di lokasi yang berbeda sehingga dapat meminimalisir mortalitas dan morbiditas akibat gigitan/sengatan/tusukan hewan laut.

Abstract

Penyakit Tidak Menular saat ini menjadi permasalahan yang menarik perhatian. Penyakit ini tidak dapat ditularkan melalui kontak langsung, makanan, atau air, melainkan gabungan dari faktor genetik, life style, dan lingkungan. PTM lazim terjadi dan memiliki dampak buruk bagi kesehatan masyarakat. Menurut WHO, cardiovascular disease, cancer, diabetes, dan PPOK menjadi penyebab kematian tertinggi pada PTM.  Sebagai rangkaian dari kegiatan UNRAM mengabdi, Program Studi Kedokteran Kelautan berkesempatan memberikan pelayanan kesehatan. Kegiatan ini bertujuan untuk melakukan pemeriksaan dan penapisan sebagai upaya meningkatkan derajat kesehatan masyarakat. Metode dalam kegiatan ini adalah partisipatif, meliputi perencanaan bersama desa, pemeriksaan tekanan darah dan mata, edukasi kesehatan, konsultasi, tindak lanjut melalui rujukan medis, serta pelaporan hasil, guna deteksi dini dan pencegahan hipertensi. Keberlangsungan acara ini dimulai dari proses koordinasi dengan perangkat desa, pemeriksaan kesehatan, serta rencana tindak lanjut. Peserta sebanyak 49 orang terdiri dari 32 orang laki-laki dan 17 orang perempuan. Hasil kegiatan pemeriksaan kesehatan di Balai Desa Sengkol menunjukkan partisipasi 49 orang (65,3% laki-laki, 34,7% perempuan), melebihi target 30 peserta. Ditemukan 33% prahipertensi, 21% hipertensi tahap 1, dan 4% hipertensi tahap 2. Pemeriksaan mata mendeteksi gejala awal katarak serta peningkatan kadar glukosa. Faktor risiko PTM seperti hipertensi dan riwayat kolesterol tinggi teridentifikasi, namun kontrol serta kepatuhan minum obat masih rendah. Edukasi telah diberikan, dan penapisan dini perlu digencarkan untuk mendukung pengendalian PTM secara berkelanjutan.

Abstract

West Nusa Tenggara is an archipelago province with a high potential for marine resources and fisheries, allowing many inhabitants to make a living as fishermen. Some fishermen obtain their livelihood by diving with minimal or inadequate diving equipment. They are known as traditional diver fishermen. Various health risks can be experienced by them, including respiratory system health. This research aims to determine the lung function, including Forced Vital Capacity (FVC), Forced Expiratory Volume in 1 second (FEV1), and the FEV1/FVC ratio in traditional diver fishermen who dive using the breath-hold diving technique, as well as the characteristics of their diving activities. This research is a cross-sectional study with samples obtained through consecutive sampling techniques. The research subjects comprised 23 traditional diver fishermen in Impos Beach, Medana Sub-district, North Lombok District. A descriptive analysis was conducted on the data obtained. The research results showed that the average values of FVC and FEV1 were 3.27 L/minute and 3.004 L/minute, respectively, while the FEV1/FVC ratio obtained was 91.86%. On average, they dive 4.26 times per week, and all of them perform pressure equalization maneuvers to prevent barotrauma. Before diving, 12 subjects (52.17%) performed warm-up exercises, including stretching. After diving, none of the subjects take a hot shower, get a massage, or engage in strenuous activities. The lung function of the research subjects was normal, and they have taken preventive measures to avoid diving-related illnesses.
