Abstract

Background: Prior vaccination can prevent a COVID-19 patient from falling into moderate, severe, and critical conditions. The effect of vaccination on COVID-19 patients’ recovery has been widely studied. However, its correlation in critically severe COVID-19 patients with comorbidity has not been fully understood yet. This study aims to determine the correlation of vaccination in critically severe COVID-19 patients with comorbidity of hypertension and/or Diabetes Mellitus (DM).Methods: A retrospective cohort study was conducted in critically severe COVID-19 patients with hypertension and/or DM treated in Dr. Moewardi Hospital, Surakarta, Indonesia from March 2021 to September 2021. The data were taken from patients’ medical records. We analyzed all data statistically with Chi-Square and fisher's exact test, and a p-value of < 0.05 was considered significant.Results: There were 489 patients included in our study, 247 patients with hypertension and DM, and 242 patients without comorbidities. Vaccination status was significantly associated with the cure rate of critically severe COVID- 19 patients with hypertension (p=0.018), but not with DM (p=0.606). There was no significant association between age to the cure rate of critically severe COVID-19 patients with hypertension and DM (p=0.953). Vaccination status was related among patients with comorbidities and without comorbidities (p<0.001).Conclusion: Vaccination was significantly correlated the cure rate of moderate to critically severe COVID-19 patients with hypertension and without comorbidities.

Abstract

Background: Bronchoscopy is a minimally invasive procedure used for diagnostic examination and intervention of the airways. Patient comfort and cooperation during bronchoscopy are very important because they affect the success and outcome. The sympathetic anticholinergic effect of ipratropium bromide can improve procedure tolerance and airway visualization. This study was conducted to analyze the effect of inhaled ipratropium bromide as a bronchoscopy premedication for the assessment of dyspnea, cough, and tracheobronchial secretion.Methods: This was a clinical study with a quasi-experimental pretest-posttest control group design in pulmonary patients who underwent bronchoscopy at Dr. Moewardi General Hospital Surakarta in October 2021 using consecutive sampling. The subjects of the study were divided into an intervention group with inhaled ipratropium bromide and a control group without inhaled ipratropium bromide. The Borg scale of dyspnea and the visual analog scale (VAS) score of cough were assessed before and after bronchoscopy in both groups. The grading of tracheobronchial secretion was assessed during bronchoscopy.Results: Thirty-six pulmonary patients who underwent bronchoscopy were included in this study. The intervention group showed a lower Borg scale (0.28±0.57) and VAS score (3.22±8.54), lower tracheobronchial secretion grading, and there was a significant difference compared to the control (P≤0.05).Conclusion: There was a significant difference in the Borg scale of dyspnea, VAS score of cough, and the grading of tracheobronchial secretion in patients undergoing bronchoscopy as an effect of ipratropium bromide inhalation.

Abstract

ackground: Bronchoscopy is a relatively safe diagnostic and therapeutic procedure, but it is often reported as an uncomfortable experience and causes acute procedural anxiety that affects the procedure and the patient and operator's comfort. Anti-anxiety drugs have the risk of causing mild to severe side effects. Therefore, we need premedication with potent anxiolytics with minimal side effects, such as the use of aromatherapy. This study aims to analyze the effectiveness of aromatherapy as an additional premedication to reduce anxiety, respiratory rate, pulse rate, and pain in patients undergoing bronchoscopy. Method: A clinical study with experimental quasi pre-post test control group design using consecutive sampling was performed in pulmonary patients undergoing bronchoscopy in dr. Moewardi Hospital from February to March 2020. The study subjects were randomized into three groups: lavender aromatherapy, orange aromatherapy, and control. Hospital anxiety and depression scale (HADS) score, respiratory rate, pulse, and visual analogue scale (VAS) pain score were measured before and after bronchoscopy.  Results: A total of 45 subjects of lung patients undergoing bronchoscopy participated in this study. Post hoc test differences in the HADS anxiety score of lavender and orange groups showed a significant decrease (P=0.011); (P=0.083), respectively. The decrease in the control group was not significant (P=0.622). There was a significant decrease in the respiratory rate of lavender (P≤0.0001), and orange groups (P=0.001), while the control group did not decrease (P=0.515). There was a significant decrease in pulse rate in the lavender (P=0.004) and orange (P=0.011) groups. The decrease in the control group was not significant (P=0.900). There was a significant decrease in VAS pain scores in the lavender and orange groups with each (P<0.001), whereas, in the control group, there was an increase in VAS pain scores. Conclusion: Aromatherapy effectively controls anxiety, respiration rate, pulse, and pain in bronchoscopy patients.

Abstract

Development of galenic preparation formulations that can be used to make anti-inflammatory cream preparations. Snail seromucoid contains glycoproteins, mannose carbohydrates, glucose, N-acetyl muramic acid and the α-1 globulin-oromucoid fraction which functions as a bioimmunostimulator. Durian peel (Durio zibethinus L.) contains a polar polysaccharide polymer compound such as D-galacturonic acid which has anti-microbial properties. Chitosan is a polymer compound resulting from derivatization of chitin which has antimicrobial polycationic properties; biodegradable, and biocompatible with animal body tissue. The aim of the research was to examine the effectiveness of a galenic anti-inflammatory cream preparation based on snail seromucoid, durian skin extract, and chitosan for healing acute and chronic wounds in vivo. The research method is experimental research, namely the effectiveness of anti-inflammatory galenic cream preparations made from snail seromucoid, durian peel extract and chitosan on various types of wounds in vivo. Analysis of observation data using the one way anova test. A galenic preparation based on a combination of 2% snail seromucoid, 2% durian peel extract polysaccharide and 2% chitosan ratio 1:1:1 is effective in the healing process based on the formation of granulation tissue thickness in incision injuries, hot objects injuries, second degree burns and ulcers. diabetes. The bioformulation galenic preparation of 2% snail seromucoid, 2% polysaccharide of durian peel extract; 2% chitosan and their combination with ratio 1:1:1 is effective as an anti-inflammatory galenic cream preparation so that it can be applied for healing chronic wounds, acute wounds and diabetic ulcers.

Abstract

The host's cellular immune response plays an important role in the process of eliminating microorganisms that cause infection. Substances that can stimulate an increase in the immune response are called immunostimulators. Snail seromucoid contains bioactive compounds such as glycans, peptides, glycopeptides and chondroitin sulfate. Chitosan as an antimicrobial agent can be used in the biomedical field because chitosan has a number of hydroxyl groups (OH) and amine groups (NH2). The research objective was to determine the potency and effectiveness of snail seromucoid and chitosan as bioimmunostimulators. The research method is based on laboratory experimental results with the research stages, namely the analysis of the effectiveness of seromucoid and chitosan on lymphocyte proliferation. The results of the one way ANOVA analysis showed a p value of 0.000 so that there was a significant effect between the treatment groups, namely the effect of giving chitosan 65 ug/ml; snail mucus 65 ug/mL and a combination of chitosan (65 ug/mL) and snail mucus (65 ug/mL) ratio of 1:1, can increase lymphocyte proliferation optimally. This shows that snail mucus, chitosan and their combination in a 1:1 ratio are effective as BRM (Biological Response Modifier). It is hoped that the contribution of the results of this study can be further developed in the bioformulation of snail seromucoid preparations and chitosan as therapeutic agents for infectious diseases, including Acute Respiratory Distress Syndrome (ARDS), Tuberculosis and other diseases.

Abstract

Background: Community-acquired pneumonia (CAP) is a respiratory infectious disease caused by bacteria, viruses, or fungi. Procalcitonin (PCT) levels will rise, especially in bacterial infection. PCT examination in CAP can help to confirm the diagnosis and anticipate complications. CAP is diagnosed by symptoms, vital signs, laboratory tests, and radiographic investigations. Inflammatory biomarkers are required for predicting causative microorganisms, guiding antibiotic therapy, and determining severity. The purpose of this study is to compare NLR, WBC, and TNF-α levels in CAP patients dependent on PCT level.Methods: This was an analytical cross-sectional study involving 43 CAP patients admitted to Universitas Sebelas Maret (UNS) Hospital and Moewardi Regional General Hospital Surakarta between February and March 2023. Patients were separated into two groups based on PCT levels: those with levels ≤0.12 ng/mL and >0.12 ng/mL. All patients provided a blood sample for NLR, WBC, TNF-α, and PCT testing. The independent t-test and Mann-Whitney tests were performed for statistical analysis of two unpaired groups, and the Chi-square or Fisher exact test was utilized for ordinal categorical data. If the value of P<0.05, the result is statistically significant.Results: NLR and TNF-α levels were higher in the PCT group >0.12 ng/mL and statistically significant with a value of P=0.001. WBC levels in the PCT group >0.12 ng/mL were higher, but the difference was not statistically significant (P=0.096).Conclusion: The NLR value, WBC, and TNF-α levels were found to be higher in the group with PCT >0.12 ng/mL.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is the leading cause of morbidity and mortality worldwide. Exposure to cigarette smoke activates alveolar macrophages producing neutrophils and proteases such as MMP-9 and NE that damage the extracellular matrix triggering emphysema as well as spill over into the systemic circulation. This study aimed to determine the correlation of serum MMP-9 level and blood absolute neutrophil count with diffusion capacity and exercise capacity of stable COPD patients.Methods: A cross-sectional observational study was conducted in stable COPD patients visiting the pulmonary outpatient clinic of Universitas Sebelas Maret (UNS) Hospital in October 2022. Consecutive sampling was applied for sample collection. We assessed diffusion capacity and exercise capacity with the DLCO method and 6MWT, respectively. We examined serum MMP-9 level and blood absolute neutrophil count. All data were statistically analyzed with SPSS 22.0 and P<0.05 was considered significant.Results: Thirty stable COPD patients were included in the study comprising 19 males (63.3%), and 11 females (36.7%). With a mean age of 61.90±9.99. Serum MMP-9 level did not correlate with either diffusion capacity and blood absolute neutrophil count (P=0.898 and P=0.589, respectively). However, serum MMP-9 level had a significant correlation with exercise capacity (P=0.014). There was no correlation between blood absolute neutrophil count with diffusion capacity and exercise capacity (P=0.0281 and P=0.592, respectively). Diffusion capacity related to exercise capacity (P=0.001).Conclusion: Serum MMP-9 level is not related to diffusion capacity, but it is related to exercise capacity. Blood absolute neutrophil count is not related to diffusion capacity or exercise capacity. Serum MMP-9 level is not associated with blood absolute neutrophil count. Diffusion capacity is related to exercise capacity.

The examination of IL-6 and neutrophils helps in diagnosis as a marker of inflammation, a predictor of mortality and morbidity, and is useful in evaluating the outcome of treatment. Garlic and its organosulfur content possess anti-inflammatory activity that has the potential to be used as additional therapy in pneumonia patients.Methods: A clinical study with a pretest-posttest quasi-experimental design was conducted on pneumonia patients who were hospitalized at Dr. Moewardi General Hospital, Surakarta from August to October 2022 by consecutive sampling. The treatment group (n=20) received standard therapy plus 3.5 mg of garlic capsules per day for 6 days, while the control group (n=20) received standard therapy. Levels of IL-6 and neutrophil were calculated on the first and sixth days, while the length of hospitalization was calculated from when the patient was admitted until discharge.Results: There was a significant IL-6 difference in the treatment group (P=0.027) and a neutrophil difference in the treatment group (P=0.025) compared to the control group, but there was no significant difference (P=0.876) in the length of stay in the treatment group compared to the control group.Conclusion: Giving garlic as a supplemental therapy to pneumonia patients could significantly reduce IL-6 and neutrophil levels.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is a leading factor in morbidity and mortality worldwide associated with excessive chronic inflammatory response. Guided imagery is a relaxation technique to achieve the desired positive outcome. This study analyzed the effectiveness of guided imagery in stable COPD patients on the value of % FEV1, absolute neutrophils, anxiety, and the standard of living in those with steady COPD.Methods: Experimental analytic research with quasi-experimental, pretest and post-test design. Subjects were outpatient stable COPD patients at the pulmonary polyclinic of UNS Surakarta Hospital in January-June 2022. Subjects were divided into guided imagery intervention groups for 4 weeks and controls. Subjects were then examined for %FEV1 by spirometry, absolute neutrophils, Taylor Minnesota Anxiety Scale (TMAS) questionnaire, and St. George Respiratory Questionnaire (SGRQ) quality of life questionnaire, and re-evaluated after four weeks.Results: There were 32 research subjects. The findings demonstrated that the therapy group did not endure a significant increase in the mean value of %FEV1 (P=0.617). Meanwhile, the mean value of %FEV1 significantly decreased in the control group (P=0.025), given that the control group's value of % FEV1 fell. In contrast, the treatment group's increased, even if the difference was not statistically significant, the use of guided imagery could effectively halt the decline in %FEV1 value. Compared to the control group (P=0.014), the TMAS anxiety score was lower in the treatment group (P≤0.001). The overall SGRQ score (quality of life) considerably decreased in the treatment group (P≤0.001) while significantly increasing in the control group (P=0.014). Absolute neutrophils were found in both the treatment group and the control group (P=0.642; P=0.224, respectively). Absolute neutrophil blood levels in the treatment and control groups did not differ significantly.Conclusion: Guided imagery is effective on %FEV1 values, anxiety, and quality of life in stable COPD patients but not against absolute neutrophils.

Abstract

Synbiotic tempeh extract soyghurt is a Krenova (creative and innovative) food product because it is novelty and unique as healthy and safe food, functional food, fun for diet and one for all. The result of probiotic fermentation of Lactic Acid Bacteria (LAB) and based on synbiotic tempeh extract (rice bran and chitosan) which has multifunctional anti-hyperglycemia and anti-hypercholesterolemia properties. Research objective: to analyze the effectiveness of synbiotic tempeh soyghurt as a functional drink. The research method analyzes the nutritional content and quality of synbiotic tempeh soygurt products which refer to SNI 7552:2009 concerning flavored fermented milk. Data analysis used bivariate and multivariate Anova tests. The Hedonic test results showed a semi-solid texture, white color, synbiotic tempeh extract aroma, sour taste in the formulation with the addition of 15% skim milk, 1% chitosan, 1:1 ratio LAB starter mix and 5% inoculum. The results of physical chemical and mycobiological quality tests show that the synbiotic tempeh extract meets the quality test as a functional drink.
