Abstract

Imiquimod induces activation of Th17 cells and dendritic cells that play a role in psoriasis. Ciplukan extract ( Physalis angulata L.) contains steroid compounds, flavonoids, alkaloids, and saponins such as Physalin B, Physalin F, Physalin G which have anti-inflammatory activity in psoriasis pathophysiology. This research is an experimental research with post test only with control group design. Thirty five female mice were divided into 7 groups. Group A: negative control, group B: positive control, group C induced by imiquimod and given with ciplukan extract 400 mg/kg BW for 7 days, group D induced by imiquimod and given with ciplukan extract 800 mg/kg BW for 7 days, group E induced imiquimod and given with ciplukan extract 1,200 mg/kg BW for 7 days, group F induced by imiquimod and given with methotrexate 1 mg/kg BW for 7 days, and group G induced by imiquimod and given with a combination of ciplukan extract 1,200 mg/kgBW and methotrexate 1mg/kgBW for 7 days. 7 days. The number of fibroblast cells was counted on the fifteenth day by taken a sample of mouse skin and made histological preparations and then counted manually used a microscope. The mean number of fibroblast cells in groups A, B, C, D, E, F, and G were 21.6±2.3, respectively; 39.2±5.5; 30.6±1.3; 24.0±2.8; 24.8±2.9; 28,4±3,0;28,2±3,2 . The results of the test One Way ANOVA showed a value of p = 0.000 (p < 0.05), so that the results are significant. Keywords: ciplukan extract, fibroblast, methotrexate , Physalis angulata

Abstract

The pandemic due to the COVID-19 virus has had a significant impact on the online learning process. The difficulty of adapting learning methods for learning outcomes at the Faculty of Medicine requires that several methods be carried out offline, taking into account the safety and security of students during the pandemic. Therefore, this activity aims to design a prevention program for offline learning within the General Sudirman University Faculty of Medicine during the COVID-19 pandemic. This activity was carried out for 112 students and staff involved in the learning process. Activities were carried out by collecting data and filling out questionnaires, screening for the use of Genose, then followed by evaluation of vaccination using a rapid antibody test. Final detection was carried out using RT-PCR. The results of our activities succeeded in detecting 15 reactive students using genose, 5 reactive using a rapid test, and 1 confirmed positive for a PCR swab, so that confirmed participants could be given health assistance and break the spread of the COVID-19 virus. ABSTRAK Pandemi akibat dari virus COVID-19 memberikan dampak yang besar terhadap proses pembelajaran secara online. Sulitnya adaptasi metode pembelajaran untuk capaian pembelajaran di Fakultas Kedokteran mengharuskan beberapa metode dilakukan secara offline, dengan mempertimbangkan keamanan dan keselamatan peserta didik selama masa pandemi. Oleh karena itu, Tujuan dari kegiatan ini untuk mendesain program pencegahan untuk pembelajaran secara luring di lingkungan Fakultas Kedokteran Universitas Jenderal Soedirman selama masa pandemi COVID-19. Kegiatan ini dilakukan kepada mahasiswa dan tendik yang terlibat dalam proses pembelajaran sebanyak 112 orang. Kegiatan dilakukaan dengan pendataan dan pengisian kuisioner, skrining penggunaan Genose, kemudian dilanjutkan dengan evaluasi dari vaksinasi menggunakan rapid test antibody dan penegakan deteksi akhir dilakukan dengan menggunakan RT-PCR. Hasil dari kegiatan kami berhasil mendeteksi 15 mahasiswa yang reaktif menggunakan genose, 5 reaktif menggunakan rapid test dan 1 yang terkonfirmasi positif swab PCR, sehingga peserta yang terkonfirmasi dapat diberikan pendampingan kesehatan dan pemutusan rantai penyebaran virus COVID-19.

Abstract

Latar Belakang : Hidrosalfing adalah suatu kondisi medis dengan peran prognostik penting dalam reproduksi. Insiden hidrosalfing pada wanita infertil adalah 30%. Hidrosalfing dikaitkan dengan tingkat kehamilan dan kelahiran hidup yang rendah. Selain itu dapat meningkatkan risiko keguguran biokimia dan tiga kali lipat risiko kehamilan ektopik. Penelitian ini bertujuan mengetahui gambaran dan distribusi pasien hidrosalfing di RSUD Prof. dr Margono Soekarjo Purwokerto periode Januari – Desember 2022. Metode : Rancangan deskriptif observasional dengan sampel penelitian adalah semua data rekam medis pasien hidrosalfing di RSUD Prof. dr Margono Soekarjo Purwokerto periode.Januari – Desember 2022. Sampel dipilih dengan cara purposive sampling yang memenuhi kriteria inklusi dan data disajikan dalam bentuk tabel. Hasil : Dari hasil penelitian didapatkan pasien hidrosalfing selama periode Januari – Desember 2022 sebanyak 53 dengan keluhan infertilitas sebanyak 49 orang (92,4%), nyeri panggul 18 orang (33,9%) dan keputihan berulang sebanyak 12 orang (22,6%). Yang mengalami keluhan infertilitas dan nyeri panggul sebanyak 13 orang (24,5%), infertilitas dan keputihan berulang sebanyak 11 orang (20,7%). Sedang yang mengeluh infertilitas, nyeri panggul dan keputihan berulang sebanyak 1 orang (1,8%). Kelompok terbanyak hidrosalfing terjadi pada usia < 35 tahun (66%). Hidrosalfing unilateral sebanyak 23 orang (43,4%) dan bilateral sebanyak 30 orang (56,6%). Terdapat 4 orang (7,5%) penderita hidrosalfing dengan riwayat keguguran berulang. Simpulan : Sebagian besar pasien penderita hidrosalfing mempunyai keluhan utama infertilitas serta dapat diikuti dengan nyeri panggul dan atau keputihan berulang.

Abstract

Background: Radiotherapy is generally used to treat head and neck malignancy through high radiation, focusing on killing cancer cells. However, some adverse effects in oral mucosal tissue, including cell morphology changes and apoptosis, are commonly found. This study aims to determine the morphological changes and apoptosis of buccal mucosa epithelium after radiotherapy in head and neck cancer (HNC) patients.Methods: This study involved 8 subjects of patients diagnosed with HNC. Buccal mucosal smear samples were collected using cytobrush two times, before and after radiotherapy, with a dose of 70 Gy in 7 weeks. The specimens were prepared and stained using Feulgen and Rosenbeck technique. Observations were made using a light microscope with a count per 1000 epithelial cells. Statistical analysis was performed using statistical software with Pearson’s correlation test and significance t-test (p < 0.05) between irradiated and non-irradiated samples.Results: Data analysis showed significant changes in cell morphological damage and apoptosis in patients before and after radiotherapy. It increases in the number of micronuclei (p = 0.001), broken egg (p = 0.001), binuclei (p = 0.003), pyknosis (p = 0.033), karyorrhexis (p = 0.020), and karyolysis (p = 0,004). Conclusions: The effects of radiation reflect morphological changes and apoptosis in the buccal mucosa basal epithelium in HNC patients.

Abstract

Colorectal carcinogenesis induced by cigarette smoke requires at least 30-40 years. This long time duration causes an animal research conducted becomes relevant. This research was carried out to observe colorectal carcinogenesis due to cigarette smoke exposure in Wistar Rat. The observations focused on changes in epithelial morphology and expression of APC, MSH2 and MLH1. Twenty male Wistar rats inbreed strain were randomly allocated into control group and experimental group exposure to cigarettes smoke for 14 weeks and 28 weeks sequentially.  Colorectal epithelial morphology was assessed on the histopathology examination, whereas the expression of APC, MSH2 and MLH1 was assessed on aspect of immunohistochemistry. The comparative analysis between the two groups was performed using non-parametric Mann-Whitney U test. Histology of colorectal epithelium showed pattern of colitis associated cancer that was significant both in 14 weeks and 28 weeks of treatment.  This research indicated negative expression of APC, MSH2 and MLH1 in the colorectal cancer that were significant at 28 weeks of exposure. This research implies that chronic exposure to cigarette smoke can induce colitis associated colorectal cancer through decreased expression of APC, MSH2 and MLH1.

Abstract

Background: Endometrial cancer ranks 15th with the highest number of new cases in 2020 in the world. There are still differences in research results regarding the relationship between age and number of parities with endometrial carcinome. Aim: To determine the relationship between age and parity with type of endometrial carcinoma at RSUD Prof. Dr. Margono Soekarjo. Method: This research is an observational analytic study with a cross sectional study. The sampling technique uses total sampling. Total subjects were 129 endometrial carcinoma patients at RSUD Prof. Dr. Margono Soekarjo in 2017-2022. The data taken is secondary data from the medical records. The variable age and number of parities are the data listed in the medical record document. Data were analyzed using the chi-square statistical test. Result: The total sample obtained was 129 patients. The most common age was found at the age of 51-60 years (41.1%) and the least at the age of <40 years (7.8%). The highest number of parities found was parity ≥2 (55.8%) and the least was parity 1 (15.5%). The results of the chi-square statistical test for age with endometrial carcinoma type showed a value of p = 0.159 (p>0.05). Based on the results of the analysis, there was no significant relationship between age and the type of endometrial cancer. The results of the chi-square statistical test for the number of endometrial carcinoma types showed a value of p = 0.02 (p <0.05). Based on the results of the analysis, it showed that there was a significant relationship between the number of parities and the type of endometrial cancer. Conclusion: There was no significant relationship between age and the type of endometrial carcinome and there was a significant relationship between the number of parities and the type of endometrial carcinome at RSUD Prof. Dr. Margono Soekarjo.

Abstract

Background: Periodontitis is a chronic multifactorial disease caused by microorganisms such as G-anaerobes in the periodontal tissues. It activates host defense cells and releases inflammatory mediators such as tumor necrosis factor-alpha (TNF-α) and prostaglandin E2 (PGE2). Ganoderma lucidum is a traditional medicinal mushroom with anti-inflammatory effects against various diseases. Biologically, different levels of its active constituents, such as triterpenoids and phenolic compounds, reduce inflammation with various pathways. Furthermore, the constituents inhibit toll-like receptor 4, MyD88 receptors, and the activities of nuclear factor-kappa B, which synthesize TNF-α and PGE2. Purpose: This study aimed to analyze the extent to which G. lucidum extract can reduce TNF-α and PGE2 levels in periodontitis model Sprague Dawley rats. Methods: Thirty Sprague Dawley rats were randomly divided into six groups of five rats. Periodontitis inflammation was induced by the injection of Porphyromonas gingivalis bacteria into intrasulcular gingival incisors in the lower jaw labial section. Grouping was as follows: Group K1 (healthy control); Group K2 (negative control); Group K3 (positive control with doxycycline dose 0.27 mg/kg BW); Group P1 (G. lucidum extract dose 5 mg/kg BW); Group P2 (G. lucidum extract dose 10 mg/kg BW); and Group P3 (G. lucidum extract dose 20 mg/kg BW). Samples were taken from rat gingival tissue and the levels of TNF-α and PGE2 were examined using the enzyme-linked immunosorbent assay method. Data analysis was performed using one-way analysis of variance (ANOVA) with a confidence level of 95% (p < 0.05). Results: The levels of TNF-α and PGE2 were the highest in the K2 group and the lowest in the K1 group. One-way ANOVA showed no significant difference in TNF-α and PGE2 levels between P3 and K1 group. Conclusion: G. lucidum extract can reduce TNF-α and PGE2 levels in Sprague Dawley rats with periodontitis.

Abstract

Latar Belakang: Limfoma non-Hodgkin (LNH) dibedakan menjadi dua kelompok utama, yaitu limfoma sel B dan limfoma sel T/NK. Tipe LNH paling banyak di dunia adalah LNH sel B. Salah satu cara mengelompokkan LNH sel B yaitu dengan menggunakan pemeriksaan histopatologi untuk membedakan grading menjadi low grade dan high grade. Tingkat kejadian LNH sel B meningkat bertahap berdasarkan usia dan lebih banyak terjadi pada laki-laki. Penelitian ini bertujuan untuk mengetahui apakah terdapat hubungan antara usia dan jenis kelamin dengan grading LNH sel B. Metode: Penelitian ini merupakan penelitian analitik observasional dengan pendekatan cross sectional yang dilakukan pada pasien limfoma non-Hodgkin sel B di RSUD Prof. Dr. Margono Soekarjo Purwokerto pada tahun 2019-2021. Analisis data menggunakan uji Chi-square. Hasil: Total sampel yang didapatkan adalah 75 pasien LNH sel B. Sebanyak 47 (62,7%) pasien berusia <60 tahun dan 28 (37,3%) pasien berusia ⩾60 tahun. Pasien laki-laki berjumlah 40 (53,3%) pasien, lebih banyak daripada perempuan yang berjumlah 35%. Hasil analisis menunjukkan bahwa tidak terdapat hubungan antara usia dengan grading (p=0,617), serta jenis kelamin dengan grading (p=0,366). Kesimpulan: Tidak terdapat hubungan antara usia dengan grading LNH sel B. Tidak terdapat hubungan antara jenis kelamin dengan grading LNH sel B.

Abstract

Introduction: Traumatic brain injury (TBI) represents a major global health burden, because it is the foremost causes of death, particularly in young adults. Elevated IL-6 and IL-10 serum levels in severe TBI have been identified as an early indicator of unfavorable clinical outcomes. Pegagan (Centella asiatica (L.)) has antioxidant, neuroprotective, neurogenerative, anti-inflammatory properties. Our goal of this study was to find out what happened to IL-6 and IL-10 levels in blood of the TBI rat model when they were given pegagan extract (Centella asiatica (L.)). Subject and Method: experimental research design. For 3 days, there were 4 groups of male Wistar strain rats (Rattus norvegicus) subjects, consisting of three treatment groups (pegagan extract doses of 300 mg/kgBW, 500 mg/kgBW, and 1000 mg/kgBW) and one control group. On the 4th day, blood was taken, and we continued to examine IL-6 and IL-10 serum levels by ELISA. Result: A dose-dependent and significant increase of IL-6 and IL-10 serum levels was observed in the treatment groups compared with control group. Conclusion: In TBI rat model, administration of pegagan (Centella asiatica (L.)) extract increases IL-6 and IL-10 levels.

Abstract

The growing global interest in herbal medicines underscores the importance of safety assessments for traditional plants. Physalis angulata L. (Ciplukan) demonstrates therapeutic potential, yet data on Indonesian extracts remain limited. This study evaluated the acute oral toxicity and determined the median lethal dose (LD50) of Indonesian Ciplukan extract. Twenty-five female Sprague-Dawley rats were divided into five groups (n=5): control (aqueous), and four groups that received extract at 100, 400, 800, or 1200 mg/kg body weight. Over 14 days, researchers monitored clinical signs, body weight, food intake, and organ weights in accordance with Acute Oral Toxicity (OECD Test Guideline 425). Major organs were examined histopathological using the modified International Harmonization of Nomenclature and Diagnostic Criteria. No mortality or significant behavioral changes were observed at any dose. Rats maintained normal body weight gain and food intake. The LD50 was determined to be exceeded 1200 mg/kg, indicating favourable acute safety. However, histopathological analysis revealed significant dose-dependent necrotic changes (p < 0.05) in the lungs, kidneys, liver, and spleen at doses of 400 mg/kg and above. Liver and spleen damage were detected first at this threshold. These findings indicate an LD50 above 1200 mg/kg body weight for Indonesian Ciplukan extract, supporting its acute safety. Nonetheless, subclinical organ toxicity occurred at doses of 400 mg/kg or higher, emphasizing the need for dose optimization in phytomedicine. The study provides regulatory-grade toxicological data to support evidence-based standardization of Indonesian herbal medicines and highlights the necessity for further research on sub chronic and chronic toxicity to establish safe therapeutic doses.
