Abstract

Thalassemia major (TM) is an inherited disease caused by defective or absent of hemoglobin chain synthesis. Regular chelation therapy is necessary to reduce excess iron in several organs of TM patients. The most commonly used chelating agents are deferasirox and deferiprone. However, information regarding their effectiveness and side effects in Indonesian children population with TM were limited. This study was conducted to assess the effectiveness and side effects of deferasirox and deferiprone in pediatric patients with TM. This was an observational study with prospective analysis which was conducted during April-August 2015. We included pediatric patients with TM who visited a hospital in Bandung, Indonesia, using consecutive sampling method. Thirty two subjects were divided into two groups, i.e., deferasirox and deferiprone group. Review of medical records and interview were performed for each participants. Effectiveness was defined as reduction in ferritin level. Side effects were assessed using Naranjo scale. Data were analyzed using Mann-Whitney test, Wiloxon test and Chi square test. P value < 0.05 defined statistical significance. We found that deferasirox was more effective than deferiprone for the treatment of TM in pediatric patiens, with less side effects. The use of deferasirox as iron chelating agent is recommended for patients with TM.Keywords: deferasirox, deferiprone, ferritin, thalassemia major

Abstract

Chronic renal failure (CRF) is one the most prevalent health problems among the elderly. Onthe fifth stage of CRF, the patient becomes eligible to hemodialysis. CRF-induced anemiais commonly treated with Erythropoietin (Epo). Information regarding the effectiveness ofEpo in hypertensive hemodialysis patients was limited. Therefore, this study was conductedto evaluate the effectiveness of Epo in hypertensive hemodialysis patients. This study usedan observational case-control analytic method. Data were retrieved from the medical recordsof hemodialysis patients during March-May 2014. A total of 54 participants were included.The increase of haemoglobin (Hb) in controlled hypertension was 0.6257 g/dl, while the inuncontrolled hypertension group, there was a decrease in Hb (-0.1590 g/dl). The use of Epowas more effective in hemodialysis patients with controlled hypertension.Keywords: chronic kidney disease, erythropoietin, hypertension

Abstract

Kecombrang (Etlingera elatior) merupakan tanaman suku Zingiberaceae yang memiliki potensi sebagai antioksidan. Penelitian ini bertujuan untuk mengetahui aktivitas antioksidan dan sitotoksik terhadap larva udang dari ekstrak daun, bunga, dan rimpang kecombrang, serta menetapkan kadar senyawa fenol totalnya. Sampel daun, bunga, dan rimpang kecombrang dimaserasi dengan pelarut etanol 70% selama 3x24 jam dalam suhu ruangan. Ekstrak yang didapat selanjutnya dievaporasi pada suhu 50 0C sehingga menghasilkan ekstrak kental. Uji aktivitas antioksidan dilakukan menggunakan metode peredaman radikal bebas 1,1-difenil-2-pikrihidrazil (DPPH), dan uji sitotoksik menggunakan metode BSLT (Brine Scrimp Lethality Test). Penetapan kadar fenol total dengan standar asam galat menggunakan reagen Folin Ciocalteu yang diukur dengan spektrofotometer pada panjang gelombang 765 nm. Aktivitas antioksidan dari ekstrak daun, bunga, dan rimpang kecombrang ditunjukkan oleh nilai IC50 masing-masing 52,05; 457,54; dan 310,69 ppm. Aktivitas sitotosik yang dinyatakan dalam nilai LC50 dari ketiga bahan tersebut berturut-turut adalah 859,039; 418,022; dan 1261,202 ppm. Kadar senyawa fenol total ekstrak daun, bunga, dan rimpang kecombrang dihitung sebagai asam galat ekuivalen berturut-turut sebesar 0,339% ± 0,006; 0,144% ± 0,024; dan 0,074% ± 0,018. Ekstrak daun kecombrang menunjukkan aktivitas antioksidan yang paling baik dan kadar fenol total paling tinggi dibandingkan ekstrak bunga dan rimpang kecombrang. Ekstrak daun dan bunga kecombrang menunjukkan potensi sebagai agen antikanker karena menunjukkan nilai LC50  1000 ppm.

Abstract

Pharmacy counseling, which provides quality drug information, improves patient safety. However, this service is not optimally provided in Indonesia. A new model, modified pharmacy counseling (MPC), was developed to make it easier for service providers. We aimed to measure MPC effectiveness in improving short-term antibiotic compliance. This community trial (ISRCTN82062391) involved patients (age, ≥18 years) recruited from selected pharmacies between December 2020 and February 2021 in two Bali Province districts. At baseline, data regarding sociodemographic characteristics, knowledge levels, and attitudes toward antibiotic use were collected. MPC effectiveness was assessed using Morisky medication adherence scale-8. Of 300 recruited patients, 290 participated (response rate, 96.67%). The baseline study revealed that knowledge regarding short-term antibiotic use is lacking; however, awareness regarding antimicrobial resistance is high. MPC significantly improved short-term antibiotic compliance (risk ratio, 2.849; 95% confidence interval, 1.904-6.640; p<0.001). The mean rank significantly differed between the intervention (61.05) and control groups (37.95). The proportions of compliant patients in the intervention and control groups were 35.6% and 12.5%, respectively. The main reason for low compliance was forgetting and not being made aware by pharmacy workers in the intervention (46.8%) and control (55.6%) groups, respectively. These findings provide insights for improving pharmaceutical care.

Abstract

Etlingera is one of the plant genera from the Zingiberaceae family and is widely distributed in Asia to the Pacific Islands, with various species ranging from 150 to 200 species. The Etlingera are commonly used as spices, vegetables, and traditional medicines. Many pharmacological activities have been reported from this genus, including antioxidants, antibacterials, cholesterol-lowering, anticancer, and others. The phytochemical content of Etlingera reported are phenolics, diarylheptanoids, flavonoids, steroids, alkaloids, and terpenoids. Eight species from Etlingera, namely Etlingera elatior, E. pavieana, E. brevilabrum, E. pyramidosphaera, E. megalocheilos, E. coccinea, E. fimbriobracteata, and E. corneri are reported provided anticancer activity. This review article aims to review the phytochemical screening from Etlingera genus, and its anticancer activity.

Abstract

Etlingera alba (EA) is one of the endemic plants grown in Sulawesi. We determined the cytotoxic and anti-metastatic activity of isolates obtained from EA rhizome extract. IC50 value for 1,7-diphenyl-6-heptene-3-one, sitostenone, sinapyl alcohol diacetate, and sinapyl alcohol acetate were 134.59; 170 ; 128.11; and 161.96 μg/mL, respectively. The compounds also had anti-migration activity. 200-μg/mL Sitostenon exhibited the lowest migration rate (3.32%). The free bond energies of sitostenon, 1,7-diphenyl-6-heptene-3-one, sinapyl alcohol acetate, and sinapyl alcohol diacetate were -11.81; -8.25; -6.64; and -6.28 kcal/mol, respectively. All compounds were more effective in stabilizing the Matrix metalloproteinase 1 complex. Isolation from EA rhizomes have the potential to be developed as anti-metastatic for Triple-Negative Breast Cancer.

Abstract

Catechol-O-Methyltransferase (COMT) gene is located at chromosome 22q11 on a location with a high risk of the occurrence of schizophrenia that has a role in the degradation and inactivation of dopamine neurotransmitters. The COMT Val158Met gene polymorphism has enzymatic activity variation that can affect the availability of COMT enzyme, balance of dopamine level, and availability of dopamine receptors so it can influence the effectiveness of risperidone therapy that varies. The aim of this study was to observe the genetic profile of COMT Val158Met gene polymorphism and its correlation with effectiveness of risperidone therapy either single or combination in schizophrenia patients at Mental Hospital of Prof. Dr. Soerojo Magelang. This study was an observational descriptive study with a cross-sectional plan. Data collection was carried out prospectively through assessment of risperidone therapy effectiveness based on PANSS-EC scores in medical record, whole blood sampling, and identification of COMT Val158Met gene polymorphism using Polymerase Chain Reaction-Amplification Refractory Mutation System (PCR-ARMS) method. The result of study showed that all schizophrenia patients at Mental Hospital of Prof. Dr. Soerojo Magelang had heterozygote Val/Met (GA) genotype. Combination risperidone therapy was the most widely used (90.79%) rather than single with the result that 44 patients of 69 patients (63.77%) responded to the combination risperidone. Combined risperidone clozapine therapy was the most widely used (57.97%) with the result that 23 patients out of 40 (57.50%) responded to the combination therapy.

Abstract

Kemoterapi berbasis platinum merupakan regimen kemoterapi lini pertama untuk pasien kanker paru jenis karsinoma bukan sel kecil (KPKBSK) pada stadium lanjut dan non-operasi tanpa mutasi gen Epidermal growth factor receptor (EGFR). Excision repair cross-complementation group 1 (ERCC1) adalah enzim yang mengeksekusi insisi dari untai DNA yang rusak dan menghilangkan aduksi DNA yang diinduksi oleh platinum. Tujuan penelitian ini adalah untuk mengetahui korelasi polimorfisme gen ERCC1 rs11615 dengan efektivitas kemoterapi berbasis-platinum pada pasien KPKBSK di RS Paru dr. H. A. Rotinsulu Bandung pada bulan November 2019 sampai dengan Maret 2020. Penelitian ini merupakan observasional analitik dengan desain penelitian kohort prospektif terhadap 28 pasien. Polimorfisme gen ERCC1 rs11615 diidentifikasi dengan metode DNA squencing, sedangkan untuk efektivitas kemoterapi menggunakan metode response evaluation criteria in solid tumour (RECIST) dan analisis Cytokeratin 19 fragments (CYFRA 21-1). Hasil penelitian menunjukkan adanya frekuensi alel gen ERCC1 rs11615 masing-masing 50% (C/C), 35,7% (C/T), dan 14,3% (T/T). Tidak terdapat hubungan yang signifikan antara polimorfisme gen ERCC1 rs11615 dengan nilai RECIST (OR = 0,417; 95% confidence interval (CI): 0,091-1,908, P=0,256) atau dengan nilai CYFRA 21-1 (OR = 0,417; 95% confidence interval (CI): 0,063-2,768, P=0,357). Kami tidak menemukan bukti yang mendukung penggunaan gen ERCC1 rs11615 sebagai prediktor untuk menentukan efektivitas kemoterapi berbasis platinum. Dibutuhkan sampel yang memadai untuk mengkonfirmasi secara statistik korelasi polimorfisme gen ERCC1 rs11615 terhadap efektivitas kemoterapi.

Abstract

Sel punca mesenkimal (SPM) telah banyak digunakan dalam uji klinis maupun pre klinis dan menunjukkan hasil yang cukup menjanjikan sebagai alternatif terapi pengobatan penyakit degeneratif. Efektivitas terapi SPM bukan hanya dipengaruhi oleh sel yang ditransplantasikan, tetapi juga oleh efek parakrin SPM melalui sekresi molekul sinyaling berupa protein baik secara in vivo maupun in vitro. Efek parakrin terjadi saat sel menerima signal kerusakan, yang secara in vitro dapat dikondisikan dengan pengaturan lingkungan hipoksia bagi pertumbuhan sel. Penelitian ini membandingkan total protein yang dihasilkan oleh SPM dari dua macam sumber, yaitu jaringan tali pusat dan lemak, yang dikultur dalam kondisi hipoksia dan normoksia selama 24 dan 48 jam. Hasil penelitian menunjukkan bahwa protein yang dihasilkan oleh SPM dari jaringan lemak menghasilkan protein dalam jumlah yang lebih banyak dibandingkan dengan SPM dari tali pusat. Sedangkan jika dilihat dari parameter waktu perlakuan, protein yang dihasilkan setelah perlakuan 48 jam lebih banyak dibandingkan protein yang dihasilkan oleh perlakuan selama 24 jam.

Abstract

ABSTRAKKanker merupakan penyakit yang mengancam jiwa yang sangat serius bagi semua manusia. Menurut data GLOBOCAN 2018 kasus populasi kanker masih meningkat menjadi 18,1 juta dan 9,6 juta dan kematian dan hampir dari setengah populasi terjadi di Asia. Ekstrak, fraksi hingga isolat Garcinia Celebica (manggis hutan) dilaporkan mempunyai aktivitas sitoktosik yang dapat menghambat pertumbuhan dalam berbagai sel kanker diantaranya MCF-7 (kanker payudara), A549 (kanker paru),  Hela (kanker servik),  KB (karsinoma epidermoid), B16F10 (melanoma), DBTRG (glioma).
