Abstract

XYZ is a company operating in the manufacturing and rubber roll service industry located in Indonesia. The frequent issue within the company is the occurrence of product rejects, leading to inefficient production processes. The research is conducted using the Fault Tree Analysis (FTA) method with recommendations for improvement from the Failure Mode and Effect Analysis. This method is employed to identify the causes of product rejection and provide preventive solutions to avoid such rejections. There are three types of rejects at PT. XYZ, namely blisters rejection with a probability of 1.137%, material impurities rejection with a probability of 1.18%, and PU rejection with a probability of 1.152%. The root causes of these rejections include operators rushing, operator fatigue, machines in need of maintenance, very short production deadlines, operator laziness, incomplete environmental cleanliness facilities, and operators joking around while working. As for recommendations to reduce and prevent the recurrence of these three types of rejections, they include regular machine maintenance, providing customers with longer estimated completion times than the actual estimates, and offering rewards to operators who meet targets without making errors.

Abstract

XYZ is a company that operates in the frozen food industry in particular. The company is a producer of high-quality frozen food products for export, so, naturally, other stakeholders in each division of the company need to work together well. Machine maintenance is a key aspect in maintaining the performance and reliability of industrial equipment. To increase machine maintenance efficiency in this factory, a comprehensive and structured approach is needed. Therefore, this research will discuss the machine maintenance strategy implemented in the Sabroe production line. The two methods that will be the main focus of this research are Reliability Centered Maintenance (RCM) and Maintenance Value Stream Mapping (MVSM). The conclusion obtained by examining the highest Risk Priority Number (RPN) for each component in the blanching machine, it can be concluded that the gearbox component has the highest RPN, namely 216, which is the result of multiplying the Severity, Occurrence, and Detection values. During the period of breakdowns and repairs of the blanching machine, the Mean Time to Failure (MTTF) is found to be 242.2538 hours. Meanwhile, the Mean Time to Repair (MTTR) reaches 172.6 minutes or equivalent to 2.8 hours. The gearbox component requires a repair time of 2.8 hours. The inspection interval for the gearbox component is calculated for 302.01 hours, or every 12 days in a month. Efficiency analysis of maintenance using the Maintenance Value Stream Mapping approach indicates that the percentage of maintenance efficiency reaches 73.2%. Activities that add value require a longer time, around 172.6 minutes, while activities that do not add value require about 63.2 minutes.

Abstract

When the MRT was first operated, operations ran smoothly and it is hoped that this condition will continue to be optimal in the long term and have efficient operational costs. As a corporation that offers transportation services, PT MRT XYZ distributes passengers. Fixed expenses, variable costs, and travel costs all have an impact on efficient operational costs. Minimizing operational costs to support other costs, like maintenance costs, is vital for MRT XYZ operations to run as efficiently as possible. The study's goal is to determine the MRT system's operating expenses. The optimal operational costs, which were determined by using the travel matrix computation, were IDR 1,267,110,000,000 based on the analysis and discussion's findings. The factors utilized according to Tyonardo, the MRT station area's function and the costs that affect operations are used to determine the variable number of passengers using financial modeling. Optimizing language costs is anticipated to be the primary determinant in deciding how much profit PT MRT will make.

Abstract

PT. XYZ is a company operating in the food industry, with its main product being instant noodles. In its production process, not all products produced meet the quality standards set by the company. This indicates the presence of defective products, often due to errors in factors such as machinery or human resources, among others. Initial observations reveal that there are still many defects in their products. Quality control at PT. XYZ is done manually by the owner, who supervises each production cycle. Defects in PT. XYZ's products are categorized into three types: torn noodle packaging, crushed noodles, and torn seasoning packaging. Therefore, improvements are needed in the production of instant noodles by PT. XYZ. This research aims to improve the quality of production in the company and to explain the approach to analyzing the defect rate of instant noodles using Six Sigma and Kaizen. The results obtained using the Six Sigma method in the DMAIC analysis (Define, Measure, Analyze, Improve, Control), showed that product quality testing during the instant noodle production process resulted in 3238 failures. The Defects Per Million Opportunities (DPMO) value was 41172.88, which, when converted to a sigma level, was 3.23, indicating that the sigma level was at the industry average in Indonesia. This suggests that there is a potential for 41172.88 defects per one million productions, showing an inconsistent pattern of DPMO and sigma achievement. This indicates that the production process has not been managed accurately and requires improvement, particularly in the quest for zero defects.

Abstract

Agriculture plays an important role in the Indonesian economy, fertilizer is an important element in the development of the agricultural sector, especially urea fertilizer. PT. Damai Sejahtera, as one of the largest fertilizer producers in Indonesia, experienced waste in the urea fertilizer production process. In this study, lean six sigma was utilized. Using a lean approach, a reduction in non-value-added activities formed by the nine types of waste of 1360 to 1276 or 6.17% was achieved. However, using six sigma, it was found that the production process was not satisfactory as it resulted in many product defects with 36,331 DPMO and a sigma level of 3.32. Improvement suggestions to increase the sigma value were obtained through FMEA. Through this research, Process Cycle Efficiency was obtained from 74.85% to 80.35%.

Abstract

For a company, efficient operations hinge on a robust supply chain system. Consequently, it is crucial to manage a well-structured supply chain system effectively. This research examines a manufacturing firm that specializes in producing heavy equipment for industrial applications. The firm adopts a make-to-order (MTO) production strategy, requiring it to order raw materials from suppliers in advance. However, this approach poses multiple risks, including delays or mistakes in raw material deliveries, which directly lead to setbacks in production. In response to these issues, this study seeks to pinpoint potential risks and formulate risk mitigation strategies using the House of Risk (HOR) method. The HOR method is a sophisticated tool tailored for risk mitigation analysis. Through this research, 21 risk events and 15 risk agents were identified, leading to the development of 25 prioritized mitigation strategies to tackle the risks in the company’s supply chain system. These strategies are anticipated to assist the company in reducing supply chain risks, thereby promoting more seamless and effective operational performance.
