Abstract

PT. ISP is a manufacturing company that focuses on the production of springs for vehicles in the industrial sector. The products produced include leaf springs and snail springs, which are produced via cold and hot processing methods. PT. ISP collaborates with Mitsubishi Steel Manufacturing, Japan, using a license from that company. One of them is Moveable Driven, which is a type of spiral spring that is produced through a cold process. PT. ISPs face challenges related to defects that arise during the production process. This research aims to assess the level of defects, identify factors that contribute to product defects and suggest improvements. Seventools method for identifying factors that cause defects in products. The research results based on the Pareto diagram show that the most dominant type of defect is Outside Diameter (+) with a total of 3,197 pcs with a percentage of 44.9%, so defects need to be the main focus for improvement. On the P Chart, there is 1 data that is out of control below the Lower Control Limit/LCL limit in the 9th data. After revision 1 there is no data out of control. In this way, the factors causing Outside Diameter (+) defects using fishbone include Man Factor, Measurement Factor, Machine Factor, Process Capability, Method Factor. The condition of internal defects is still within control limits. So improvements need to be made so that defects meet the targets that have been set.

Abstract

Kecelakaan kerja masih menjadi tantangan utama dalam industri manufaktur, terutama di workshop fabrikasi PT. XYZ. Tujuan penelitian ini adalah untuk mengidentifikasi berbagai potensi bahaya, menilai tingkat risiko terkait, dan menetapkan tindakan pengendalian yang paling efektif. Pendekatan yang digunakan meliputi “Job Safety Analysis (JSA)” untuk menelaah setiap tahapan pekerjaan, serta “Hazard Identification, Risk Assessment, and Risk Control (HIRARC)” sebagai metode untuk menilai dan mengendalikan risiko yang teridentifikasi. Hasil analisis menunjukkan bahwa kegiatan seperti welding, cutting, drilling, grinding, dan assembling memiliki beragam potensi bahaya dengan tingkat risiko yang bervariasi, mulai dari kategori rendah hingga ekstrem. Secara lebih rinci, proses welding mengandung dua risiko pada level ekstrem, satu risiko tinggi, serta satu risiko sedang. Aktivitas cutting menunjukkan tiga risiko tinggi dan satu risiko ekstrem. Pada kegiatan drilling, ditemukan satu risiko sedang, satu ekstrem, satu tinggi, dan satu rendah. Proses grinding memiliki dua risiko ekstrem, satu risiko tinggi, serta satu risiko sedang, sedangkan aktivitas assembling mencatat dua risiko ekstrem, satu sedang, dan satu rendah. Berdasarkan hasil tersebut, disarankan penerapan beberapa langkah pengendalian, seperti peningkatan disiplin penggunaan APD, penataan ulang area kerja agar lebih aman, pemeliharaan rutin terhadap peralatan, penerapan prosedur kerja dengan pengawasan yang lebih ketat, serta penyelenggaraan pelatihan keselamatan kerja bagi seluruh pekerja. Hasil penelitian ini menegaskan bahwa penerapan JSA dan HIRARC mampu memberikan pendekatan sistematis dalam pemetaan bahaya serta strategi pencegahan, sehingga dapat menekan angka kecelakaan kerja sekaligus memperkuat budaya K3 di industri fabrikasi.

Abstract

Sektor makanan cepat saji menghadapi risiko operasional tinggi akibat keterlambatan pasokan, fluktuasi permintaan, serta kesalahan manusia yang dapat mengganggu kontinuitas layanan. Namun, penelitian terdahulu umumnya berfokus pada manajemen risiko di industri manufaktur atau distribusi, sementara penerapannya pada restoran cepat saji masih terbatas. Penelitian ini untuk mendeteksi dan meminimalkan risiko operasional dalam rantai pasok restoran Hotway’s Chicken Gresik menggunakan pendekatan Supply Chain Operations Reference (SCOR) dan House of Risk (HOR). Model SCOR digunakan untuk memetakan proses bisnis ke dalam lima tahap plan, source, make, deliver, dan return sedangkan HOR membantu mengenali sumber risiko serta menentukan prioritas mitigasi berdasarkan nilai Aggregate Risk Potential (ARP). Data dikumpulkan melalui wawancara dan kuesioner kepada staf operasional berpengalaman. Hasil penelitian menunjukkan terdapat 21 penyebab risiko dengan A8 (ketidakpatuhan terhadap SOP memasak dan kurangnya pelatihan staf) sebagai risiko paling kritis dengan nilai ARP sebesar 984. Strategi mitigasi utama meliputi penerapan daftar periksa SOP harian, program pelatihan rutin, kontrak pemasok jangka panjang, serta audit kualitas bahan baku. Penelitian ini berkontribusi pada pengembangan model manajemen risiko terintegrasi yang relevan untuk meningkatkan efisiensi dan keandalan operasional restoran cepat saji.

Abstract

PT. BIJ is an analytical laboratory services company, therefore PT. BIJ needs a good warehouse to be able to accommodate the samples it has PT. BIJ experienced problems with sample storage. Therefore, it is necessary to improve the sample storage system to make it easier to find samples to be analyzed. Using the Dedicated Storege and Class Base Storage methods will help provide grouping for each type of sample and place the samples in the right place so that the sample travel distance is shorter. The results of the research carried out can be obtained that the initial travel distance is 262.98 meters, while the proposed dedicated storage distance is 163.19 meters and the proposed basic class storage distance is 149.95 meters, so there is the largest percentage difference of 42.89%. So it can be concluded that using the class base storage method is better than the dedicated storage method.

Abstract

                Office ergonomics is a branch of ergonomics that covers the entire work environment (workstations) and work tools (especially using computers and chairs) in offices. Computer users or office workers often experience health problems related to musculoskeletal disorders (MSDs). The Secretariat and P3T BUMD Gresik Regency use a computer/laptop as the main work tool. Secretarial employees use computers for 8 (eight) hours daily, while P3T employees use computers for about 5 hours daily. The method used in this study was the Rapid Office Strain Assessment (ROSA). Assessment of work posture using the ROSA method on six office employees at BUMD Gresik Regency is divided into three sections. Section 1 assesses the height of the seat, the outside of the backrest seat surface, and the chair’s armrest. Section 2 conducts an assessment of the use of monitors and telephones. Section 3 assesses the use of mice and keyboards. Based on the calculation results using the ROSA method, the results obtained at the risk level value for each employee, namely, four employees have a high-risk level value or are at a dangerous/risky level and need to be repaired immediately. The other two employees indicate a safe / non-risky risk level.

Abstract

XYZ Cafe is an MSME business in Gresik Regency with a target market of teenagers and surrounding office employees. Cafe management evaluates cafe services for consumer complaints, which decreases the number of cafe visitors. Service quality is essential in marketing current products so that products can be well received in the market. The method used to determine the quality of service is service quality while determining the service attributes that need to be improved using the importance-performance analysis method. The quality of service provided by the XYZ Gresik cafe is based on service quality. From the results of the IPA analysis, the results of quadrants I, II, have relatively high demand, and quadrants III, IV, have relatively low demand with a relatively high level of satisfaction, so it is considered excessive. And from the GAP calculation results using the service quality method has a negative value.

Abstract

PT. KQW is related to quality. Some problems occur during the production process and product results, which are associated with the level of defects in the production process. From historical production data for April 2022 – June 2022, glulam wood products contribute the most defects, both congenital physical defects of the material itself and during the production process. This study aims to identify the level of defects in wood products, determine the factors that cause defects in wood products, and determine the efforts made to reduce the level of damage to wood products. The formulation of the problem in this study is how to control the damage/defects of wood products faced by PT. KQW. This research method uses FMEA. This research resulted in defects in the production process of glulam wood cracks, holes and breaks. The highest types of defect are hollow wood. Identification of the causes of every kind of defect, the order of the highest RPN value for each NG defect. There are 3 solution recommendations based on the highest RPN value for each defect. For cracked wood defects, the highest RPN is because the workers are less thorough (100). Then for hollow wood defects, the highest RPN is due to poor raw materials (147). Abstract

PT. XYZ is a service company in the field of construction and fabrication. This study aims to determine the results of work postures in the company and determine the level of risk for operators based on the nordic Body Map (NBM) and provide suggestions for improving work posture to the operator. Ovako Work Posture Analysis System(OWAS) is a method for evaluating and analyzing the work attitude of observed operators, including body movements of the back, shoulders, arms, and legs, including thighs, knees, and ankles. This method is fast in identifying work attitudes that have the potential to cause work accidents. Work accidents that are of concern to this method are the musculoskeletal system. It is known that there are 4 work postures from the company PT XYZ, out of these four postures need improvement as soon as possible. It is recommended to pay more attention to the health and safety of operators, especially during disturbancesMusculoskeletal disorders which are caused by unsuitable work environment conditions and workload which can result in decreased work productivity

Abstract

                PT. XYZ is a service company in the field of construction and fabrication. The purpose of this research is to identify critical components in system failure on the lathe unit, to find out how often a failure on the lathe is likely to occur, to find out the causes of damage to the lathe machine components and to be able to find out the criteria of risk that has the highest risk score and RPN. The analysis process in this study needs to be done to minimize damage to the lathe. If there is damage to the lathe, the production process cannot run smoothly. For the production process to run smoothly, it is necessary to have machine maintenance activities. Machine maintenance activities aim to keep machines in good condition and ready for use in the production process. The method used in this research is FMEA and RCA. The FMEA method can be used as a system to prevent errors that are predicted to occur. This method is carried out by sorting the RPN value from the highest and then suggesting improvements to the lathe. Meanwhile, the RCA method is a structured approach to identifying influencing factors in one or more past events so that it can be used to improve performance. There are various structured evaluation methods to identify the root causes of problems. The results of this study are the two most significant RPN values, namely the Chuck and Gear components. The cause of the failure of these components is for the chuck. When setting, the chuck is not a flashlight, while the machine is not functioning properly for the gear.

Abstract

Dalam dunia usaha kualitas pelayanan kepada pelanggan merupakan salah satu faktor pendukung dalam menjalankan usaha. Penurunan kepercayaan pelanggan di J&T Express Gresik pernah dialami pada tahun 2021, sehingga perlu adanya penelitian terhadap masalah turunnya kepuasan pelanggan terhadap pelayanan. Adapula tujuan dari penelitian ini ialah untuk mencari kesesuaian atribut yang menjadi prioritas utama untuk ditingkatkan, mengetahui kriteria tingkat kepuasan pelanggan, dan kesenjangan antara presepsi dan harapan pelanggan. Penelitian kali ini menggunakan metode servqual, IPA, dan CSI dengan pengumpulan data melalui kuisioner sebanyak 20 atribut pernyataan kepada 50 responden. Tahapan pengolahan data meliputi karakteristik responden, uji kecukupan data, uji validitas dan reliabilitas, analisis servqual, analisis GAP, analisis IPA, analisis CSI. Responden 56% laki-laki dam 44% perempuan, uji kecukupan data minimal 44 responden (valid), semua atribut dinyatakan valid dan reliable, kesenjangan yang menempati peringkat pertama adalah atribut P1 dan kesenjangan dimensi tertinggi ialah emphaty (-0,55). Hasil analisis IPA diketahui bahwa prioritas utama yang harus ditingkatkan ialah pada atribut P4 (Penampilan petugas/karyawan), P10 (Petugas melayani pelanggan yang kesulitan), dan P12 (Petugas mampu melayani pelanggan dengan cepat). Pada hasil perhitungan CSI diketahui sebesar 70,64%, dalam kriteria kepuasan pelanggan termasuk dalam kategori puas, maka dari itu perlu adanya peningkatan prioritas utama pada kuadran pertama.
