Abstract

PT. Sari Agro Tama Persada is a company engaged in the agribusiness industry. In the business world, quality is a determining factor in customer buying interest. Many new business actors, PT Sari Agro Tama Persada (SATP) must be careful in maintaining and even improving the quality of their products. To control the quality, the methods used are six sigma, FMEA and seven tools. The Six Sigma method is a procedure to identify the triggers of defects in products or processes by correcting cases and improving quality with Define, Measure, Analysis, Improve, Control (DMAIC), Failure Mode and Effect Analysis (FMEA) is used to determine the RPN value on the PT SATP problem of 245 in poor quality sacks. Based on the calculation of the Defecr Per Million Opportunity (DPMO) value that has been carried out, the highest value is 2284,595, while the highest defect in torn sacks is 75 sacks with a percentage of 39.89. For the sigma (α) value of 4.97. In the cause-and-effect diagram, there are suggestions for improvements that will minimize defects, namely: carrying out extra supervision of workers, conducting safety briefings/talks before work, checking machines, and prioritizing the availability of spare parts (sewing needles), performing routine calibrations on machines balance. PT. Sari Agro Tama Persada merupakan perusahaan yang bergerak di industri agribisnis. Dalam dunia bisnis, kualitas merupakan faktor penentu pada minat beli customer. Banyaknya pelaku usaha baru , PT Sari Agro Tama Persada (SATP)harus berhati-hati dalam menjaga bahkan meningkatkan kualitas produknya. Untuk mengendalikan kualitas mutu di gunakan metode yaitu six sigma, FMEA dan seven tools. Metode Six Sigma ialah prosedur untuk mengenali pemicu kecacatan pada produk ataupun proses dengan membetulkan kasus serta tingkatkan mutu dengan Define,  Measure, Analysis, Improve, Control (DMAIC), Failure Mode and Effect Analysis (FMEA) digunakan untuk mengetahui nilai RPN pada permasalahan PT SATP sebesar 245 pada kualitas karung yang buruk. Berdasarkan penghitungan nilai Defecr Per Million Opportunity (DPMO) yang telah dilakukan didapat nilai tertinggi 2284.595, sedangkan cacat tertinggi pada kemasan karung sobek sebesar 75 karung dengan persentase 39.89. Untuk nilai sigma(α) sebesar 4.97. Pada diagram sebab-akibat, terdapat usulan perbaikan akan meminimalisir defect  yaitu : melakukan pengawasan ekstra pada pekerja, di lakukan briefieng/safety talk pada saat sebelum bekerja, melakukan pengecekan mesin, serta mementingkan ketersediaan spare part (jarum jahit), melakukan kalibrasi rutin pada mesin timbangan

Abstract

This study aims to determine the performance of the Ammonia Plant of PT Petrokimia Gresik using the Balanced Scorecard method to provide an alternative performance measurement system that can provide an understanding to management about the company's performance appropriately and thoroughly. The Balanced Scorecard method is one of the performance measurement methods that reviews company performance not only from the Financial side, but also Non-Financial. The performance index of the PT Petrokimia Gresik Ammonia Plant in 2020 is 1,891 and in 2021 it is 1,892, this means that overall the PT Petrokimia Gresik Ammonia Plant has decreased by 0.001.

Abstract

Sistem kriogenik memegang peranan vital dalam industri gas karena berfungsi menjaga kestabilan penyimpanan serta distribusi fluida pada suhu dan tekanan ekstrem. Kegagalan peralatan pada sistem ini tidak hanya berpotensi menghambat proses produksi dan menimbulkan kerugian finansial, tetapi juga dapat mengancam keselamatan operator serta lingkungan akibat potensi kebocoran fluida bertekanan tinggi. Salah satu peralatan utama di PT Air Products Indonesia adalah Liquid Oxygen (LOX) tanker fill pump, yang berfungsi untuk memindahkan oksigen cair dari tangki penyimpanan menuju kendaraan distribusi. Unit ini kerap mengalami gangguan pada komponen mechanical seal, yang menjadi penyebab utama kebocoran dan downtime produksi. Penelitian ini menerapkan integrasi metode Failure Mode and Effect Analysis (FMEA) dan Reliability Centered Maintenance (RCM) untuk menyusun strategi pemeliharaan berbasis risiko yang terukur dan efektif. FMEA digunakan untuk mengidentifikasi serta menilai potensi mode kegagalan berdasarkan tingkat keparahan, frekuensi kejadian, dan kemampuan deteksi, sementara RCM digunakan untuk menentukan jenis pemeliharaan paling sesuai berdasarkan konsekuensi kegagalan dan tingkat kritikalitas komponen. Kombinasi kedua metode ini memungkinkan penyusunan kebijakan pemeliharaan yang lebih sistematis, efisien, dan berfokus pada peningkatan keandalan peralatan. Hasil analisis FMEA menunjukkan bahwa komponen mechanical seal memiliki nilai Risk Priority Number (RPN) sangat tinggi sebesar 576, disebabkan oleh kombinasi tingkat keparahan yang tinggi, frekuensi kejadian berulang, serta kesulitan dalam deteksi dini. Faktor penyebab utama mencakup keausan material, kontaminasi fluida pelumas, dan kesalahan dalam proses pemasangan. Berdasarkan logika keputusan RCM, strategi pemeliharaan yang disarankan adalah Condition-Based Maintenance (CBM) dengan interval inspeksi setiap tiga bulan dan penggantian mechanical seal menjadi dry seal yang memiliki umur pakai lebih panjang. Implementasi strategi ini diproyeksikan mampu menurunkan frekuensi kerusakan hingga dua kali per tahun, meningkatkan nilai availability sistem menjadi 99,45%, serta menekan biaya perawatan darurat. Selain itu, peningkatan sistem pemantauan tekanan dan vibrasi secara berkala turut memperkuat aspek keselamatan kerja karena risiko kebocoran oksigen cair bertekanan tinggi dapat diminimalkan. Temuan ini membuktikan bahwa integrasi FMEA dan RCM efektif dalam mengoptimalkan keandalan, efisiensi biaya, dan keselamatan operasional pada sistem kriogenik industri gas modern.

Abstract

General Background: Service quality is a critical determinant of competitiveness in modern retail, particularly as rapid growth in small convenience stores intensifies customer expectations. Specific Background: XYZ Store has experienced declining customer satisfaction, requiring a systematic evaluation of service delivery using established measurement frameworks. Knowledge Gap: Despite the extensive use of SERVQUAL and Importance Performance Analysis (IPA) in service research, limited studies integrate both methods to diagnose priority improvements in small-scale retail settings. Aims: This study aims to measure service quality at XYZ Store, identify performance gaps across SERVQUAL dimensions, and prioritize improvement strategies using IPA. Results: Analysis of 100 respondents showed all SERVQUAL attributes were valid and reliable, with an overall gap score of –0.74 and a service quality index (Q) of 0.84, indicating service performance below expectations. IPA mapping revealed four critical attributes—store layout (T3), complaint handling (RP3), facility conditions (T1), and product availability (RL2)—requiring immediate improvement. Novelty: This research offers an integrated SERVQUAL–IPA approach that produces a precise, actionable prioritization of service weaknesses in a small retail context. Implications: Findings provide managerial guidance for enhancing operational performance, increasing customer satisfaction, and strengthening competitive positioning in the retail sector. Highlights: Identifies critical service gaps with an integrated SERVQUAL–IPA approach. Highlights four priority attributes needing immediate improvement. Abstract

Provision of knowledge and skills for MSME business actors needs to be carried out periodically to add insight into how the times, competitive conditions and changes in consumer behavior. The lack of marketing utilization through online media makes MSME actors do not have good competitiveness. Community service activities with Sidayu's MSME product marketing strategy through online media explain the importance of online marketing. In this activity explained some online media that can be used in marketing such as facebook, Instagram, go food (go biz). From the results of this activity, it is hoped that MSMEs can apply one of these online media as a means to increase marketing in order to increase sales and revenue.

Abstract

Business management management cannot be avoided from risks in the form of pure risks and speculative risks. Risk can be associated with the possibility of losses from unwanted adverse events or unexpected events. The problem of MSMEs, especially in Sidayu District, Gresik Regency, is that many MSME actors have not implemented risk management in their businesses. Therefore, this community service activity is carried out to provide education to Sidayu MSME actors to pay attention to the risks that exist in the process of managing their business. From the results of community service activities, they can implement risk management and improve risk management management in their businesses. With the implementation and improvement, it can be used as a means to improve business management and income.

Abstract

The layout of facilities in the industrial world greatly affects the production process of a product. If the spatial layout used is good and appropriate, it will indirectly save distance, costs, time, and effort. Community service activities are carried out in partnership with SMK Manbaul Ulum. These activities aim to provide learning about facility layout to minimize production process time. The expected outcome of these activities is that students will be able to understand how to reduce excessively long production times and increase the number of products in a business unit, which impacts student productivity. The implementation methods include delivering material, simulating the production process layout, and conducting Q&A discussions. As a result, the activities are able to enhance students' understanding of the importance of layout in production facilities. Students can also simulate the production process to evaluate the existing layout in order to minimize production time. This activity has an impact on decision-making and problem-solving.

Abstract

Proyek konstruksi mempunyai tingkat risiko kecelakaan kerja yang tinggi akibat kompleksitas aktivitas, penggunaan alat berat, serta keterlibatan banyak tenaga kerja. Proyek pembangunan perluasan gedung pabrik PT Sika Indonesia Phase 4 di Gresik merupakan proyek industri berskala besar yang memiliki potensi bahaya signifikan, khususnya pada pekerjaan panas (hot work) dan pekerjaan pengangkatan (lifting work). Studi ini memiliki tujuan untuk mengenali kemungkinan bahaya yang berpotensi terjadi akibat kecelakaan di tempat kerja dan untuk menentukan cara yang sesuai dalam mengendalikan risiko. Hal ini dilakukan dengan menggunakan metode Analisis Keamanan Kerja (JSA) yang dipadukan dengan pendekatan Hierarki Pengendalian. Metode yang diterapkan dalam kajian ini adalah observasi langsung di lokasi kerja. wawancara dengan pihak HSE, serta studi dokumentasi proyek. Temuan riset menggambarkan bahwa terdapat 10 potensi bahaya utama yang teridentifikasi pada aktivitas hot work dan lifting work. Penilaian risiko sebelum pengendalian menunjukkan terdapat 3 risiko dengan kategori ekstrem, 7 risiko tinggi, dan tidak terdapat risiko rendah maupun sedang. Setelah dilakukan pengendalian berdasarkan hirarki pengendalian risiko, jumlah risiko ekstrem berhasil diturunkan menjadi 0, Sebagian besar risiko terletak dalam kategori sedang dan rendah. Implementasi JSA, yang didukung oleh HIRARC, telah terbukti memberikan saran pengendalian yang lebih terstruktur dan efisien untuk mengurangi kemungkinan terjadinya kecelakaan kerja. Riset ini memiliki potensi untuk menjadi rujukan bagi perusahaan dalam meningkatkan pelaksanaan keselamatan dan kesehatan kerja (K3) di proyek konstruksi.

Abstract

This study aims to identify potential occupational hazards and assess the level of occupational health and safety (OHS) risks in the maintenance activities of the fatty acid tank at PT Bramindra Indotama Gresik using an integrated Job Safety Analysis (JSA) and Failure Mode and Effects Analysis (FMEA) approach.This research employs a qualitative descriptive approach, with data collected through direct observation, interviews, and questionnaires administered to maintenance workers and Health, Safety, and Environment (HSE) personnel. JSA was used to identify work steps and potential hazards, while FMEA was applied to evaluate risk levels based on Severity, Occurrence, and Detection, resulting in a Risk Priority Number (RPN). The results indicate that several activities fall into high-risk categories, particularly during Lock Out Tag Out (LOTO), tank cleaning and draining, and inspection of components inside the tank. The highest RPN values were associated with hazards related to residual energy, confined space conditions, and manual handling. The implementation of risk control measures based on the hierarchy of controls is expected to reduce workplace accident risks, improve compliance with safety procedures, and strengthen the OHS culture. Therefore, the integration of JSA and FMEA is proven to be an effective approach for identifying hazards and prioritizing risk control in maintenance activities.

Abstract

Sektor makanan cepat saji menghadapi risiko operasional tinggi akibat keterlambatan pasokan, fluktuasi permintaan, serta kesalahan manusia yang dapat mengganggu kontinuitas layanan. Namun, penelitian terdahulu umumnya berfokus pada manajemen risiko di industri manufaktur atau distribusi, sementara penerapannya pada restoran cepat saji masih terbatas. Penelitian ini untuk mendeteksi dan meminimalkan risiko operasional dalam rantai pasok restoran Hotway’s Chicken Gresik menggunakan pendekatan Supply Chain Operations Reference (SCOR) dan House of Risk (HOR). Model SCOR digunakan untuk memetakan proses bisnis ke dalam lima tahap plan, source, make, deliver, dan return sedangkan HOR membantu mengenali sumber risiko serta menentukan prioritas mitigasi berdasarkan nilai Aggregate Risk Potential (ARP). Data dikumpulkan melalui wawancara dan kuesioner kepada staf operasional berpengalaman. Hasil penelitian menunjukkan terdapat 21 penyebab risiko dengan A8 (ketidakpatuhan terhadap SOP memasak dan kurangnya pelatihan staf) sebagai risiko paling kritis dengan nilai ARP sebesar 984. Strategi mitigasi utama meliputi penerapan daftar periksa SOP harian, program pelatihan rutin, kontrak pemasok jangka panjang, serta audit kualitas bahan baku. Penelitian ini berkontribusi pada pengembangan model manajemen risiko terintegrasi yang relevan untuk meningkatkan efisiensi dan keandalan operasional restoran cepat saji.
