Abstract

 Abstract Background One of the government's efforts to increase the achievement of exclusive breastfeeding is the 10 LMKM program. Since 1991 in Indonesia, it has been introduced, but in its implementation it is not yet known by all health facilities even though several existing steps have been implemented, but the 10 LMKM policy has not been known by providers.Objectives Describe implementation and barriers to providers in program implementation, identify compliance and barriers to mothers as program recipients. Methods The research used descriptive qualitative research. The sampling technique used was purposive sampling to explore the implementation of 10 LMKM in Tanah Kalikedinding Health Center. Informants in accordance with the inclusion criteria 7 providers and 8 mothers as program recipients. Researchers interacted offline and online via video calls while adhering to health protocols. Results The implementation of 10 LMKM has been running with the internal policies of the Puskesmas referring to the Permenkes, carrying out tasks according to the SOP. Several providers have attended training and disseminated it to staff. Monitoring and evaluation is carried out through the credential team at the Puskesmas. Officers in implementing 10 LMKM to support the achievement of exclusive breastfeeding are committed to complying with the SOP. Provider barriers from external factors are the lack of health workers, especially midwives on duty, so that KIE regarding breastfeeding and others is not optimal. Mothers as program recipients when ANC is already in IEC, obediently follow the services provided and have not been fostered or referred to breastfeeding support groups. Barriers to exclusive breastfeeding in program implementation are influenced by external factors of working mothers. Conclusion The implementation of the 10 LMKM program at the Tanah Kalikedinding heath center has not been maximized.

Abstract

AbstractBackground: The government’s effort to reduce measles and rubella transmission are by increasing MR immunization coverage. The immunization coverage was influenced by parent’s decisions. There were factors that related with parent’s decisions such as knowledge, perceived susceptibility, seriousness, benefit, barrier, self-efficacy, and cues to action. This study aimed to analyze factors  related with parent’s decisions in giving MR immunization. Methods: This was a cross sectional study using questionnaire among parents of children aged 10-30 months at working area of Kenjeran public health center, Surabaya. Data of knowledge, perceived susceptibility, seriousness, benefit, barrier, self-efficacy, and cues to action regarding MR immunization then analyzed using chi-square and logistic regression. Result: From 206 parents, 56.3% were giving MR immunization to their children. Parent’s knowledge (p=0.023; OR=18.08), perceived barrier (p=0.002; OR=46.79), and self-efficacy (p=0.033; OR=7.66) were related with parent’s decisions. Conclusion: Parent’s decisions in giving MR immunization was related with knowledge, perceived barrier, and self-efficacy.

Abstract

AbstractBackground : Picky eating is a condition when children refuse to eat certain foods. Picky eating usually stops within 2 years as part of the growth process, those who stop picky eating after more than 2 years tend to be more difficult to accept new types of food. This may be unfavorable for a child's growth and nutritional status. The study was conducted to analyze the relationship between picky eating with nutritional status in preschool children. Method : This research method was observational analytic quantitative with cross sectional research design. Sampling used a total sampling technique. Data was collected in 3 preschool in October 2019 using primary and secondary data. Primary data was collected using a questionnaire given to parents to measure picky eating used Child Eating Behavior Questionnaire (CEBQ). Nutritional status was measured bytaking anthropometric measurements of children. Data analysis used fisher exact correlation tests. Results : The results obtained a sample of 78 respondents from 3 preschool. The average age of children was 48,37 months with a median was 49,00. The result showed 62,8% of children were female, 55,1% were second or more children, 88,5% had routine growth monitoring, 53,8% were in extended families, and 73,1% of children were taken care by their parents. The prevalence of children with picky eating was 70,5%. Nutritional status measurements found 1.3% of children included in underweight category and 83,3% normal. Results of the analysis of the relationship between picky eating and nutritional status showed p value = 0,819. Conclusion : There was no relationship between picky eating with nutritional status in preschool children.

Abstract

ABSTRACTBackground: Dysmenorrhea is a common gynecological problem related to the menstrual cycle. Risk factors for dysmenorrhea are age of menarche, length of menstrual cycle, duration and menstrual volume. Teenagers who experienced dysmenorrhea report the negative impact of dysmenorrhea on a social life. This study aimed to determine the relationship between menstrual cycle characteristics with dysmenorrhea and adolescent social life. Method: The study design used in this research was Cross Sectional with statistical analysis of Rank Spearman test and Chi Square and a significance limit of 5%. The population in this study were 449 students with the inclusion criteria of female students aged 14-16 who had menstruated in the last 3 months. Data was collected using primary data. Sample of 151 students were obtained using total sampling techniques. Results: The percentage of respondents who experienced dysmenorrhea in this study were 90.7% and 78.8% of them stated that dysmenorrhea disrupted their social life. There was a relationship between duration (p=0.006) and menstrual volume (p=0.004) with dysmenorrhea. The relationship also found between menstrual volume (p=0.033) and dysmenorrhea (p=<0.001) with adolescent social life. Conclusion: The characteristics of the menstrual cycle that were associated with dysmenorrhea were duration and menstrual volume and the social life of adolescents were significantly correlated with dysmenorrhea.

Abstract

Abstract Background: Most of the deaths of children under five in the world occur due to respiratory tract infections, especially in the lower respiratory tract. Pneumonia is one of the biggest causes of infant mortality in the world. Objective: The purpose of this study was to determine several factors that influence acute respiratory tract infections with pneumonia in children under five. Methods: Case control research design was used with statistical analysis of Chi Square test bivariate and multivariate logistic regression analysis which had a significance of 5%. Data collection using simple random sampling technique. Results: The variables that have the greatest significant value are immunization status and exclusive breastfeeding status. Toddlers with incomplete immunization have a 2.876-fold risk of being infected with pneumonia compared to toddlers who are fully immunized. Breastfeeding that is not exclusive is at risk of infection with pneumonia 3.111 times that of toddlers who are given exclusive breastfeeding. Conclusion: Exclusive breastfeeding status and immunization status are the most influential variables on acute respiratory tract infection with pneumonia.   Keywords: pneumonia, exclusive breastfeeding, immunization

Abstract

ABSTRACT In Indonesia, dengue virus infection has a high mortality rate. Hemoglobin, hematocrit, leukocyte, differential count, and thrombocyte are laboratory parameters frequently used to monitor patient progress. To study the pattern of hemoglobin, hematocrit, leukocyte, differential count, and thrombocyte based on defervescence in pediatric patient with viral dengue infections. This was a descriptive observational study using medical records at General Hospital Dr. Soetomo Surabaya from February to August 2012. The total sample were 77 pediatric patients in the aged range of 1-18 years old with DF or DHF and according to inclusion and exclusion criteria. The average hemoglobin and hematocrit levels of DHF were higher than DF, both of increased in D0. The average leukocyte count of DF was lower than DHF, both of decreased on D-0. The average lymphocyte count of DHF decreased on D-3, conversely DF continued to increase. The average monocyte count of DHF increased in D-1, then decreased with DF to D+4. The average basophil count of DHF increased in D-0, while DF increased in D+2. The average eosinophil count of DHF increased in D-3. The average neutrophil count of DHF continued to increase in D-3, but was lower in D0, while DD decreased in D+3. The average thrombocyte count of DHF and DD were the lowest on D+3 and D+1. The average hemoglobin and hematocrit levels of DF and DHF increased on D0. The average thrombocyte count of DHF was lower than DF especially on D-1 to D+1. The average leukocyte count of DF and DHF decreased on D-1. The average monocyte, basophil, and eosinophil count of DHF were higher than DF on D0. Hemoglobin, hematokrit, leukosit, hitung jenis leukosit, dan trombosit sering digunakan untuk memantau perkembangan pasien. Mengetahui pola hemoglobin, hematokrit, leukosit, hitung jenis leukosit, dan trombosit berdasarkan waktu defervescence pada pasien anak infeksi virus dengue. Penelitian deskriptif observasional menggunakan data rekam medik pasien di Instalasi Rawat Inap SMF Anak RSUD Dr. Soetomo Surabaya antara Februari-Agustus tahun 2012. Total sampel adalah 77 pasien anak usia 1-18 tahun dengan DD atau DBD serta memenuhi kriteria inklusi dan eksklusi. Rerata kadar hemoglobin dan hematokrit DBD lebih tinggi dibandingkan DD, keduanya meningkat pada H0. Jumlah leukosit DD lebih rendah dibandingkan DBD, keduanya menurun pada H-1. Rerata jumlah limfosit DBD menurun pada H-3, sebaliknya DD terus meningkat. Rerata jumlah monosit DBD meningkat pada H+1 kemudian menurun dengan DD sampai H+4. Rerata jumlah basofil DBD meningkat pada H0, sebaliknya DD meningkat pada H+2. Rerata jumlah eosinofil DBD meningkat sejak awal demam. Rerata jumlah neutrofil DBD terus meningkat namun lebih rendah pada H0, sedangkan DD menurun pada H+3. Rerata jumlah trombosit DBD dan DD terendah pada H+1 dan H-1. Rerata kadar hemoglobin dan hematokrit DD dan DBD meningkat pada H0. Rerata jumlah trombosit DBD lebih rendah dibandingkan DD terutama pada H-1 sampai H+1. Rerata jumlah leukosit DD dan DBD menurun pada H-1. Rerata jumlah monosit, basofil, dan eosinofil DBD lebih tinggi dibandingkan DD pada H0. sebaliknya jumlah monosit dan neutrofil DBD lebih rendah dibandingkan DD pada H0. Kata Kunci: Dengue, Anak, Defervescence, Trombosit, Leukosit, Hemoglobin, Hematokrit

Abstract

Central nervous system (CNS) infection is a major cause of morbidity and mortality in children. The causes of CNS infections that are often found in children include bacteria, viruses, fungi, and parasites. Immunization is one way that has been proven safe and effective in preventing CNS infections in children. However, the coverage of child protection in Sumenep Regency still has not reached the target. The purpose of this community service activity is to increase the knowledge of health workers and cadres in the Pasongsongan Health Center area, Sumenep Regency about the dangers of CNS infections in children and their prevention. The selected partners are the Pasongsongan Community Health Center which covers the working area of 10 villages in Pasongsongan District, Sumenep Regency. Activities carried out using the counseling method through media zoom, in the form of lectures and questions and answers about the introduction and prevention of CNS infections. Pre-test and post-test assessments of extension activities were carried out to assess the increase in the participants' level of knowledge. There were 72 participants with varied backgrounds consisting of doctors, nurses, midwives, health workers and health cadres. As many as 68% of health workers had received socialization about CNS infection in children before during their education or continuing medical education. The level of knowledge of the participants was assessed from the pre and post tests which consisted of 10 multiple choice questions on the topic of CNS infection in children. From the evaluation results, the average pre-test score was 68.4 and the post-test was 100. An increase in knowledge was obtained with an average increase of 36.48% with the highest increase of 70 points and the smallest increase of 10 points. Community service activities in the form of counseling on CNS infections in children can increase the knowledge of health workers and cadres in the Pasongsongan Health Center, Sumenep Regency, about children's SSP infections.

Abstract

Introduction: The involvement and ability of health workers in Mandangin Island in early managing acute diarrhea in children to reduce infant mortality due to diarrhea are very important. This study aimed to determine the effect of community development Faculty of Medicine Universitas Airlangga - Dr. Soetomo General Academic Hospital Surabaya in a seminar on Early Management of Acute Diarrhea in Children using video conference to educate health care workers (HCWs) at the Mandangin Community Health Center. Material and Methods: This cross-sectional study was designed on 20 HCWs at the Mandangin community health center. Subjects were evaluated for demographic data (gender, age, occupation, last education), and the knowledge was assessed using a pre-test before the seminar was given. We used questionnaire about acute diarrhea containing 10 questions with maximum score 100. The seminar was given through video conferences with form classical lecturer and a case study on acute diarrhea, and subjects attended the seminar in Community Health Center’s Hall. We assessed knowledge after the seminar using the same questionnaire with randomization in question and answer and compared the pre-and post-test using the Wilcoxon Rank sum test. Results: Most of the subjects were midwifery, with a mean age of 32.4 ± 5.75 years old, 45% with D-3 last education and 80% were female. There was a significant difference between pre-and post-test in acute diarrhea in children (47.89±20.7 vs 64.74±22.94) (p=0.008). Conclusion: Video conference education effectively increased health care workers' knowledge about acute diarrhea in children

Abstract

Functional gastrointestinal disorders are problems that are often experienced by children, especially in isolated areas. Mandangin Island is among the secluded islands located in Sampang district.. The Department of Child Health, Faculty of Medicine, Universitas Airlangga - Dr. Soetomo General Academic Hospital held a community development that aimed to increase public knowledge about functional gastrointestinal disorders in children on Mandangin Island, Sampang District. The community service was conducted using a hybrid seminar approach, where speakers participated remotely via Zoom while the local community gathered at the Village Hall on Mandangin Island.. The speakers explained the definition, important signs and symptoms, early management of Functional Gastrointestinal Disorders. All subjects who took part in community service activities were evaluated for knowledge based on pre- and post-tests, each of which consisted of 5 questions with maximum score 100. Subject’s knowledge was considered good if the score was ≥ 60. Based on our data, there were 29 people who participated in community development with the most working as fishermen and entrepreneurs. We found that 69% of the subjects had an increase in the pre-test score. At the time the pre-test was distributed, only 10.4% had good knowledge scores, while in the post-test there were 51.7% who had good knowledge. We also succeeded in making 3 health cadres caring in childhood functional gastrointestinal disorders. Community development in the form of health education for functional gastrointestinal disorders in children using video conference (ZOOM) on Mandangin Island could increase public knowledge and this activity can be implemented on other islands.

Abstract

Highlights Dengue virus infections exhibit a spectrum of clinical manifestations, ranging from asymptomatic cases to severe disease, with the potential for fatalities if not managed effectively. Hematology factors significantly contribute to the severity of dengue virus infection.   Abstract Background: The escalating incidence of dengue cases in Surabaya, Indonesia, underscores the imperative to comprehend the hematology profiles and disease severity in pediatric patients affected by dengue virus infections (DVI). As the prevalence of DVI continues to surge, understanding the nuanced clinical manifestations becomes paramount for effective management and mitigation of the disease burden. Objective: This study aimed to characterize the hematology profiles and the disease severity of dengue virus infections (DVI) among pediatric patients hospitalized at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia throughout 2019. Material and Method: A retrospective descriptive cross-sectional study was conducted using secondary data from medical records. Pediatric patients aged six months to 18 years were enrolled. A total sampling method comprised 67 patients meeting the inclusion criteria. Result: Severe thrombocytopenia was most commonly observed in dengue hemorrhagic fever (DHF) III cases (36.4%), while leukopenia was predominant in DF cases (42.2%). High hematocrit levels were more prevalent in DHF III cases (27.3%), and high hemoglobin levels were most frequently identified in DHF II and DHF III cases (33% in each case). Significant differences in DVI severity were observed in platelets and hemoglobin levels (p=0.0002 and p=0.0066, respectively) but not in leukocyte and hematocrit levels. Conclusion: Mild thrombocytopenia was prevalent in Dengue Fever (DF), while severe thrombocytopenia was most prevalent in Dengue Hemorrhagic Fever (DHF) grade III. Leukopenia was prominent in DF patients, and platelets and hemoglobin levels varied across severity of DVI. These findings provide insights for improved clinical management and diagnostic criteria refinement.
