Abstract

Qualitative audits are a crucial component in evaluating the quality of healthcare services through the KLPCM system. However, at Regional General Hospital X, the audit process is still performed manually using spreadsheets, which leads to inefficiencies and a high risk of errors. This community service project aimed to design a digital interface for qualitative audits to enhance the effectiveness of audit activities and support integration with Electronic Medical Records (EMR). The method used was Participatory Action Research (PAR), involving observation, interviews, and the collaborative design and validation of a prototype with audit staff and field supervisors. The results showed that the proposed interface design reduced administrative workload, improved data accuracy, and expedited reporting processes. The prototype received positive responses from users and is expected to serve as an initial step toward the digital transformation of hospital quality audit systems

Abstract

Tujuan dari penelitian ini adalah untuk menentukan tingkat kelengkapan pengisian dokumen rekam medis elektronik (RME) pasien yang dirawat inap di rumah sakit.Siti Miriam Lawang. Pendahuluan : Dalam era digitalisasi, pemanfaatan RME menjadi sangat penting untuk meningkatkan efisiensi dan mutu pelayanan kesehatan. Namun, implementasinya masih menghadapi tantangan, seperti sistem hybrid dan belum optimalnya pengisian data oleh tenaga medis. Metode : Metode yang digunakan adalah kuantitatif deskriptif dengan teknik simple random sampling terhadap 56 dokumen RME periode Maret–Mei 2025. Hasil : Hasil menunjukkan bahwa rata-rata kelengkapan pengisian dokumen RME adalah 70%, masih di bawah standar 100% sesuai permenkes No. 129/Menkes/SK/II/2008. Komponen dengan tingkat ketidaklengkapan tertinggi adalah CPPT rawat inap (52%), skrining gizi (56%), dan resume medis (54%). Faktor penyebab utama ketidaklengkapan antara lain tingginya beban kerja dokter, kurangnya pengingat dari perawat, dan belum tersedianya SOP pengisian RME. Simpulan : Solusi yang diusulkan adalah pemanfaatan google spreadsheet dan aplikasi whatsApp sebagai pengingat serta pendukung monitoring. Diharapkan hasil ini menjadi dasar dalam perbaikan sistem pencatatan rekam medis di rumah sakit.

Abstract

Pencatatan penyebab kematian yang akurat merupakan bagian penting dari sistem informasi kesehatan rumah sakit, namun sering kali terkendala oleh kualitas formulir surat kematian yang belum sesuai standar. RSUD Prof. Dr. Soekandar menggunakan formulir surat kematian yang secara struktur dan isi belum mendukung pencatatan penyebab kematian secara runtut dan komprehensif. Penelitian ini bertujuan untuk mendesain ulang (redesain) formulir surat kematian agar sesuai dengan standar WHO serta meningkatkan kualitas pencatatan penyebab kematian. Penelitian ini menggunakan desain deskriptif kualitatif dengan pendekatan observasi, wawancara mendalam, dan dokumentasi. Variabel penelitian meliputi aspek fisik, anatomi, dan isi formulir. Pengumpulan data dilakukan menggunakan panduan observasi, wawancara, dan studi dokumen, sedangkan analisis data menggunakan model interaktif Miles dan Huberman. Hasil penelitian menunjukkan bahwa formulir lama tidak mencantumkan struktur penyebab kematian sesuai kaidah WHO dan menyulitkan proses pengkodean ICD-10. Redesain formulir dilakukan dengan mengacu pada struktur Medical Certificate of Cause of Death (MCCD), meliputi pembagian Part I dan Part II, kolom kode ICD-10, serta informasi waktu perjalanan penyakit. Kesimpulan dari penelitian ini adalah formulir hasil redesain mampu meningkatkan kejelasan, kelengkapan, dan akurasi pencatatan penyebab kematian, serta mendukung pelaporan data mortalitas yang lebih valid dan bermanfaat bagi sistem kesehatan.

Abstract

This study aims to analyze chronic disease patient medical records through the implementation of data mining techniques using various classification methods. This study employs a literature review approach with meta-aggregation methods to synthesize evidence from relevant scientific articles. The article selection process involves three stages: duplicate removal, application of inclusion and exclusion criteria, and checking the relevance of the article content to the research objectives. Out of 411 articles found through Google Scholar, 10 articles met the criteria and were used in the analysis. The results show that data mining techniques are effective in identifying patterns and relationships in medical record data, particularly for chronic disease patients. This study also reveals challenges in implementing these techniques, such as the need for high-quality data and accurate interpretation of results. Recommendations include improving data quality, training healthcare workers, and developing more adaptive classification models. This study contributes theoretically and practically to the field of medical records and health information, particularly in the use of data mining to support clinical decision-making. In conclusion, the implementation of data mining techniques can be a valuable tool in improving chronic disease patient management.

Abstract

Digital transformation is very important in various aspects of life, including the health sector. One of the results of development in the health sector today is the Mobile JKN application. The Mobile JKN application is one of the results of development in the health sector provided by the Ministry of Health through the Health Social Security Administration Agency (BPJS). However, the successful implementation of this application also depends on the level of understanding of users in operating it. So, public knowledge in using it is a factor that needs to be considered. The purpose of this study is to determine the level of public knowledge and identify the use of the Mobile JKN application in Griya Kalimas Indah Housing, Blitar City. The analysis method used is quantitative analytics. The object of the study was 52 respondents from Mobile JKN application users who live in Griya Kalimas Indah Housing, Blitar City. Data collection techniques through filling out questionnaires and observation sheets. The results of this study show that there is a relationship between the level of public knowledge and the use of the Mobile JKN application with the result (P-Value 0.008), the level of knowledge of the people of Griya Kalimas Indah Housing in Blitar City towards the use of the Mobile JKN application is 88.5% well-knowledged. As for the percentage of use of 9.6%, the use is not good. The conclusion is that there is a relationship between the level of public knowledge and the use of the Mobile JKN application in Griya Kalimas Indah Housing, Blitar City.

Abstract

Technological advancements, especially in the field of internet, have driven the development of digital health services such as telemedicine. This service allows users to consult with medical personnel online, thus providing easy access to health services, especially in geographical areas that are difficult to reach. This study aims to identify the level of satisfaction of students of D3 Medical Records and Health Information Study Program ITSK RS.DR. Soepraoen Malang towards the use of telemedicine applications. The method used is End User Computing Satisfaction (EUCS), which includes five indicators: content, accuracy, format, ease of use, and timeliness. This study uses a descriptive quantitative method with a survey approach to 45 respondents. The results of the analysis showed that the majority of respondents were very satisfied with the use of telemedicine applications, with an average satisfaction score of 4.19. The indicator with the highest score was ease of use (4.26), while the content indicator obtained the lowest score (4.12). These results indicate that although the telemedicine application is considered easy to use, there are still aspects of content that need to be improved. This research is expected to be a recommendation for application developers to improve the quality of telemedicine systems and services to increase user satisfaction.

Abstract

This study aims to analyze the effect of the quality of patient registration services on patient satisfaction at the Singosari Health Center. This study uses a quantitative method with an observational analytical approach and a cross-sectional design. The research sample was outpatients who had received services at the Singosari Health Center calculated using the Slovin formula with a total of 98 respondents selected using random sampling techniques. The data was collected using questionnaires and analyzed using the Spearman statistical test. The research results from the Spearman Rank Test obtained a p-value of 0.000 (p<0.05), and Ho was rejected and declared to be related. It shows that there is a significant relationship between the quality of service at the patient registration site and patient satisfaction at the Singosari Health Center. The dimensions of service quality which include tangibles, reliability, responsiveness, assurance, and empathy affect patient satisfaction. The better the quality of service provided, the higher the level of patient satisfaction.

Abstract

Medical audits aim to improve the quality of health services and ensure compliance with standard operating procedures (SOPs). The application of electronic medical records (RME) at Ngudi Waluyo Wlingi Hospital to improve documentation efficiency and reduce recording errors. This study aims to analyze the management of medical audits on electronic medical records at Ngudi Waluyo Wlingi Hospital, focusing on audit procedures, human resource factors, and facilities and infrastructure that support the implementation of medical audits. The method used in this study is qualitative with a descriptive approach, which is carried out through in-depth interviews and observations. The informants in this study consisted of the doctor conducting the audit, the head of the IT unit, and the head of the medical record. The results of the study show that the medical audit procedure has been carried out in accordance with applicable standards, but there are still obstacles in coordination between teams and lack of training for auditors and medical personnel. The human resource factor is the main challenge, where the limited understanding of medical personnel on audit procedures and the use of the RME system has an impact on the effectiveness of audits. To increase the effectiveness of medical audits, it is necessary to increase training for medical personnel and auditors, strengthen coordination between audit teams and medical staff, and optimize the RME system to make it more integrated and accessible. The implementation of audit recommendations in hospital policies must be improved to support continuous service improvement.

Abstract

This study uses a qualitative method, the data collection is in the form of interview and observation techniques on medical record Assembling officers, registration officers, verification officers, distribution officers and nurses at RS "X" Malang City. Thus, the qualitative method in this study was used to find out what factors caused the inaccuracy in the time of providing and distributing medical record files to the assembling section in terms of input, process and output factors. The results showed that the causes of the delay in returning Medical Record Documents (DRM) included patients who registered in 2 polyclinics until no medical records were found, and a missfile where the medical record file was still in inpatient because the data had not been filled out completely by the doctor in charge of the patient and had not been completed and returned to the medical record installation section, so this will slow down in finding the medical record file.

Abstract

The use of electronic medical records (RME) provides various benefits, such as reducing medical errors, improving the quality of insurance claim documents, and lowering the operational costs of health centers. This study aims to evaluate the success of the use of RME in outpatient care in terms of content completeness and identify the challenges faced. The research method used is qualitative research, Qualitative research is research that originates from an inductive mindset. This study raises the topic of evaluating the use of outpatient RME using 5M management elements (Man, Money, Material, Machine, Method). By using the head of medical records as a key informant and conducting in-depth interviews with 6 health center officers. The results of the research are to find problems and improvement efforts so that the quality of service can be maintained. Conclusion The outpatient RME that is currently being carried out is running well and the officers have been able to understand and implement the RME optimally, however, there are still obstacles related to the access rights owned by each officer but the authority owned by each officer is the same because they can access all menus.
