Abstract

Background: Primitive reflexes affect emotional development, because in a healthy and well-developed brain, the baby will slowly integrate primitive reflexes naturally into a mature developed reflex pattern. In early childhood, children have begun to enter early childhood education, and the occurrence of postural disorders can occur due to the use of the wrong backpack, various types of movement behavior, physical activity and sports including genetic factors. Purpose: Analyze the effect of ATNR primitive reflex on postural control and focus of children aged 5-8 years at Touch Care physiotherapy clinic. Methods: Quantitative design with a cross sectional approach. This study was conducted at the Touchcare Mataram Physiotherapy clinic with a population of children aged 5-8 years who came to the TouchCare physiotherapy clinic to examine / consult. Sample determination using purposive sampling with the results of 102 children. Results: The results of the study based on hypothesis testing were 0.011 for postural control, which means that there is an effect of ATNR primitive reflexes on postural control in children aged 5-8 years. The focus variable has a significance level of 0.647, which means that the ATNR variable has no effect on the focus of children aged 5-8 years. The posture variable has a significance level of 0.155, which means that the ATNR primitive reflex variable has no effect on the posture of children aged 5-8 years. Conclusion: There is an influence between ATNR primitive reflex with postural control, posture and focus of children aged 5-8 years at Touchcare Mataram Physiotherapy clinic.

Abstract

Background: Currently, public service agencies, both private and government-owned, are demanding excellent service. Quality customer service is the key to success and the basis for building customer success and trust. Purpose: This research was conducted to explore the evaluation of the implementation of the Regional Public Service Agency at Sultan Imanuddin Pangkalan Bun Hospital. Methods: The design of this research is qualitative research with a phenomenological approach with the focus of the research directed at exploring the evaluation of the implementation of the Regional Public Service Agency at Sultan Imanuddin Pangkalan Bun Hospital. Apart from that, with a qualitative approach. The informants in this research were the owners or people in charge of the clinic, a total of 5 (Five) people with triangulation of sources. Results: The implementation of the Regional Public Service Agency (BLUD) at Sultan Imanuddin Hospital Pangkalan Bun provides benefits in financial management, human resources and decision making. Even though it requires socialization, technology, training and commitment from all parties, the implementation of BLUD still increases efficiency in managing health services. Obstacles that arise, such as ineffective management, policy changes, and lack of resources, are still overcome by increasing transparency and efficiency. Conclusion: The implementation of BLUD at Sultan Imanuddin Pangkalan Bun Hospital is beneficial in financial management and health services, although there are obstacles such as less effective management.

Abstract

IGD patients’ dissatisfaction can occur if the service received does not meet patients’ expectations. Communication style and response time from medical and non-medical personnel are factors that can affect patients’ satisfaction. Objective: The objectives of this study are: 1) to determine the response time carried out by all officers in the Emergency Installation (IGD) at the Tugu Koja Regional General Hospital (RSUD) North Jakarta. 2) to determine the classification of response time of all officers in the Emergency Installation (IGD) at the RSUD Tugu Koja North Jakarta. 3) to find out the correlation of response time and patients’ satisfaction level in IGD at RSUD Tugu Koja North Jakarta. 4) to find out the communication style or method of all officers in IGD at RSUD North Jakarta. 5) to find out the correlation of the communication style of all officers in IGD and patients’ satisfaction level at RSUD Tugu Koja North Jakarta. Method: The subjects for this research are 175 IGD patients at RSUD Tugu Koja, North Jakarta. The sampling technique used is simple random sampling. The data collection method used is a questionnaire method aimed at IGD patients and medical and non-medical personnel. The validity test shows that all questionnaire items (Statements 1–4) have a sig. (2-tailed) value < 0.05, confirming their validity. The reliability test yields a Cronbach’s Alpha of 1.000, indicating excellent consistency. The data analysis technique used is Rank Spearman correlation. Results; Based on the results of the Spearman Rank test, it is found that there is a correlation between Response Time (X1) and Patients’ Satisfaction Level (Y). This is known from the sig. (2- tailed) value of 0.000 less than 0.05. However, Communication Style (X2) has no correlation with Patients’ Satisfaction Level (Y). This is known from the sig. (2-tailed) value of 0.222 which is more than 0.005. Conclusion: The study concludes that Response Time (X1) is significantly correlated with Patients' Satisfaction Level (Y), while Communication Style (X2) is not.

Abstract

Strengthening primary health care services is essential to improve the achievement of minimum health service standards. Primary Care Integration aims to reorganize existing primary health care services, enabling them to serve the entire Indonesian population with comprehensive and quality services by coordinating primary health care services based on the life cycle and no longer on programs. This study aims to explore the readiness of human resources, organization and governance, and technology in health centers to implement PCI. This descriptive study was conducted using a qualitative approach with 15 respondents from various job backgrounds. The research was conducted at Ipuh Bangun Jaya Health Center from August to October 2024. In-depth interview results were processed and validated through field observations, resulting in 18 sub-themes. Sub-themes 1-3 are about understanding PCI; sub-themes 4-8 are about human resource readiness; sub-themes 9-14 are about organizational and governance readiness; and sub-themes 15-18 are about technology readiness. Data analysis involved data reduction, data presentation and conclusion and verification. The research findings show that Human Resources at Ipuh Bangun Jaya Health Center have good efficacy and individual attributes to support the implementation of primary care integration at the Health Center. The organization and governance of Ipuh Bangun Jaya Health Center show the existence of management support as one form of good principal support to move towards the implementation of primary care integration at the Health Center. The information technology available at Ipuh Bangun Jaya Health Center is adequate for the implementation of primary care integration at the Health Center as one of the available principal supports. Although the valence of change is not yet very strong, the efficacy of change and the situation of change have been felt and expressed by employees in their daily activities. Things that need to be improved to encourage change include: widespread socialization to all employees according to clusters, structured socialization of policy products, and adequate internet access. The study concludes that Ipuh Bangun Jaya Health Center has strong human resources, governance, and IT support for primary care integration. However, improvements in socialization and internet access are needed for successful implementation.

Abstract

Hypertension is one of the factors contributing to non-communicable diseases that can have a negative impact on an individual's social, economic, and psychological aspects. Therefore, it has been designated as one of the diseases that requires a referral-back program. However, adherence to the referral-back program still needs to be evaluated. This study aims to evaluate the satisfaction of health insurance patients participating in the referral-back program for hypertension cases at Kumpai Batu Atas Health Center. A qualitative descriptive research was applied in phenomenology approach by interviewing 17 primary informants and 2 triangulation informants from the hospital and health center. Data collection includes in-depth interviews, triangulation, and patient medical records. Interviews are interactive and flexible, using probing techniques to explore participants’ experiences. Triangulation with health center staff, physicians, and specialists ensures validity. Data analysis follows reduction, presentation, and conclusion drawing, refining key themes, structuring findings, and verifying conclusions for accuracy. The thematic analysis yielded 4 themes: 1) the situation of referral-back program participants (diagnosed based on signs, symptoms, and predisposing factors. Some dropped out after 2-6 months due to uncertainty about the availability of medication); 2) satisfaction with the quality of service (satisfactory due to fast and accurate service. Professional attitudes were expressed appropriately and in accordance with needs. Clinical competence was displayed with quality and empathy was expressed with a humble attitude); 3) satisfaction with the value referral-back program facilitates access and fast service. Certainty and innovation are the main values for patients); and 4) perceptions of the best desired service (certainty of medication availability, easy administration, and friendly communication are highly desired).

Abstract

Primary Healthcare Service Integration (PHSI) at Community Health Centers aims to integrate primary healthcare services at Community Health Centers, Sub-Centers, and Integrated service posts for a more comprehensive and integrated approach. Sungai Rangit Community Health Center in West Kotawaringin Regency is planned to implement PHSI in 2024. However, a preliminary study has shown several issues that need to be anticipated, especially in terms of planning and budgeting, organizational governance, and staff assignment. This study aims to explore the operational risks faced by Sungai Rangit Community Health Center so that the implementation of PHSI can run smoothly. This research uses a qualitative descriptive approach. Data was collected through Focus Group Discussions (FGD) involving 59 employees of Sungai Rangit Community Health Center and triangulated interviews with health department employees and qualified risk management experts. The study collected qualitative data through FGDs and document analysis, involving health center employees and policy documents. Triangulation with district officials and risk management experts ensured validity. Data were refined, categorized, and analyzed iteratively, leading to a prioritized risk assessment for healthcare integration while maintaining ethical considerations. The study identified 5 main operational risks that need to be anticipated, namely: 1) risk of unavailable budget; 2) risk of invalid health data; 3) risk of sub-standard work plan preparation; 4) risk of internet connection down; and 5) risk of employee demotivation. Village governments need to allocate budgets for the needs of cadres to conduct household data collection. Cluster Heads need to assist cadres in conducting data collection and filling out the LOCAL AREA MONITORING dashboard. WiFi access in the Community Health Center environment needs to be improved to support internet-based community health center operations.

Abstract

Patient safety is a situation where patients are protected from all risks of injury that should not occur during health services, one effort is by reporting incidents. The aim of this research is to analyze individual and organizational factors regarding nurses' perceptions in reporting patient safety incidents. Quantitative research method survey method with a cross sectional approach. Samples were collected using purposive sampling technique, totaling 243 respondents. The data collection technique uses a questionnaire. Validity is tested using Pearson’s correlation, with items valid if the coefficient exceeds the critical r-value at 5% significance. Reliability is assessed with Cronbach’s Alpha, where a coefficient above 0.600 indicates acceptable consistency. The data analysis technique uses multiple linear regression. The research results show that the factors that have an influence on nurses' perceptions are motivation indicators, reward/punishment and hospital policy, this is because the sig value. < 0.05. Meanwhile, the indicators that have no effect are the knowledge, training and leadership indicators, this is because the sig value. > 0.05. It was concluded that individual and organizational factors, namely indicators of motivation, reward/punishment and hospital policy, had an influence on nurses' perceptions, while the results that had no influence were indicators of knowledge, training and leadership

Abstract

Implementing Electronic Medical Records (EMR) is a strategic step in improving the efficiency and quality of health services in hospitals. So it is necessary to evaluate the implementation of EMR using the HOT Fit method as a complete solution that best suits the difficulties or limitations currently existing at the Aminah Blitar Islamic Hospital. This research aims to analyze the influence of each variable on the implementation of EMR at the Aminah Blitar Islamic Hospital. The design of this research is a quantitative observational analytic type with a cross-sectional approach. The population of this study is 151 profesional pemberi asuhan (PPA) using EMR. This research uses a proportional random sampling technique. The sample size was determined using the Krejcie and Morgan table and a sample of 108 profesional pemberi asuhan (PPA) using EMR was obtained. The research results show that Service Quality has an effect on User Satisfaction with a significance value of <0.05, namely 0.000. Service Quality influences the Environment with a significance value of <0.05, namely 0.000. User satisfaction does not affect net benefit, with a significance value of> 0.05, 0.357. Environment influences Net-Benefit with a significance value of <0.05, namely 0.000. Service Quality mediated by User Satisfaction has no effect on Net-Benefit with a significance value of > 0.05, namely 0.325. Service Quality with environmental mediation has an effect on Net-Benefit with a significance value of > 0.05, namely 0.0000000. So it can be concluded that the presence of professional and reliable IT personnel supported by government and hospital policies influences the optimal implementation of EMR at the Aminah Blitar Islamic Hospital.

Abstract

Stroke is a neurological condition that often leads to brain dysfunction, resulting in paralysis, speech impairments, and a significant decline in patients' quality of life. Speech therapy is one of the critical rehabilitation methods to restore communication abilities in post-stroke patients, which subsequently improves their quality of life. Furthermore, family support plays a vital role in the success of rehabilitation, contributing emotionally, socially, and instrumentally. This study aims to analyze the effect of speech therapy services on the quality of life of post-stroke patients, with family involvement as a mediating variable. A quantitative research method with a cross-sectional design was employed. A total of 100 post-stroke patients undergoing medical rehabilitation were selected using purposive sampling. Data were collected through questionnaires. This study confirms the validity and reliability of the measurement scale, with all variables meeting convergent validity and composite reliability criteria. The data analyzed using path analysis to evaluate direct and indirect relationships between variables. The results revealed that speech therapy services significantly and positively impact patients' quality of life (p < 0.05). Family involvement was also found to have a significant impact, both directly on patients' quality of life and as a mediator that enhances the effectiveness of speech therapy services. The study concludes that optimal speech therapy services, combined with active family involvement, can significantly improve the quality of life of post-stroke patients. This study recommends increasing the number of speech therapists and actively involving families in rehabilitation programs for stroke patients at Ngudi Waluyo General Hospital.

Abstract

Penderita diabetes militus selama ini tidak mudah untuk mencapai sasaran gula darah dan kadar HbA1C yang diinginkan karena salah satu penyebabnya adalah kebanyakan pasien sering hanya mengandalkan obat-obatan saja, padahal banyak faktor lain yang dapat mempengaruhi dan membantu capaian gula darah yang diinginkan. Faktor-faktor tersebut merupakan penatalaksanaan empat pilar diabetes militus. Empat pilar tersebut adalah: pola makan sehat, aktivitaas fisik, obat-obatan, dan edukasi. Penatalaksanaan ini harus diimbangi dengan kepatuhan pelaksanaannya oleh penderita diabetes melitus. Oleh karena itu, penelitian ini bertujuan untuk mengetahui efektivitas kepatuhan pelaksanaan empat pilar Diabetes melitus terhadap kadar glukosa darah dan kadar HbA1c penderita Diabetes Melitus Type 2. Penelitian ini merupakan penelitian kuantitatif dengan desain Quasy-eksperiment with control group design, sampel dalam penelitian ini berjumlah 124 orang yang di ambil menggunakan teknik consecutive sampling. Penelitian ini berlangsung selama bulan Maret – Mei 2023. Hasil yang didapatkan dalam penelitian ini adalah terdapat efektivitas kepatuhan pelaksanaan empat pilar diabetes terhadap peningkatan kadar gula darah dan kadar HbA1C pada penderita diabetes melitus dengan nilai p-value 0,000 (<0,05). Kesimpulan dalam penelitian ini adalah kepatuhan 4 pilar berpengaruh pada kadar gula dalam darah dan kadar HbA1C pada penderita DM di Klinik Azahra Silo Jember. It is not easy for diabetes mellitus sufferers to achieve their desired blood sugar targets and HbA1C levels because most patients rely only on medication alone, even though many other factors can influence and help achieve the desired blood sugar. These factors constitute the management of the four pillars of diabetes mellitus. The four pillars are healthy eating patterns, physical activity, medicine, and education. This management must be balanced with compliance with its implementation by diabetes mellitus sufferers. Therefore, this study aims to determine the effectiveness of compliance with the performance of the four pillars of Diabetes Mellitus on blood glucose levels and HbA1c levels in Type 2 Diabetes Mellitus sufferers. This research is a quantitative study with a quasi-experiment design with a control group design, The sample in this study was 124 people taken using a consecutive sampling technique. This research took place during March - May 2023. The results obtained in this study were that there was effectiveness in complying with the implementation of the four pillars of diabetes in increasing blood sugar levels and HbA1C levels in diabetes mellitus sufferers with a p-value of 0.000 (<0.05). This study concludes that compliance with the 4 pillars affects blood sugar levels and HbA1C levels in DM sufferers at the Azahra Silo Clinic, Jember.
