Abstract

Chronic renal failure patients undergoing haemodialysis therapy experience various conditions and require long-term care which can cause a burden on the families who care for patients. This family caregiver burden can affect the quality of life of both patients and family caregivers, and also affect the care provided to patients. The purpose of the study was to determine the description of caregiver burden in families with haemodialysis patients. This study is a quantitative descriptive study and the sample was taken using purposive sampling technique (n = 107) with the inclusion criteria of respondents as the main caregiver with family members undergoing haemodialysis therapy at RSUD Sumedang, and physically and mentally healthy. Exclusion criteria in the study were potential respondents who refused to participate and respondents who decided to stop in the middle of the study. The burden of caregivers on the family was measured using the Burden Scale for Family Caregivers Short Scale (BSFC-s) questionnaire with test results, namely r results (0.634) > r table (0.786) as well as Cronbach alpha value of 0.900 which means this questionnaire is valid and reliable. The picture of caregiver burden shows that almost half of family caregivers are in the high category (49.5%) and most experience heavy burden in the physical dimension. The results of this study are expected to be used as a reference for learning and evaluation for nurses, family caregivers, and educational institutions to be able to carry out effective interventions that can be carried out to reduce the burden of care experienced by family caregivers.

Abstract

Families often act as informal caregivers for family members who suffer from chronic diseases. Intensive family involvement improved patients’ adherence in performing post-discharge treatment program, hospital readmission, clinical outcomes, and quality of life. To effectively help the patients managing their diseases, the family caregivers required not only understand but also must be ready for helping patients manage their conditions. This study aims to identify the readiness for hospital discharge among family caregivers of patients with chronic diseases. Data were collected from 121 family caregivers recruited consecutively from adult inpatients wards using the respondents’ characteristics questionnaire and Readiness for Hospital Discharge Scale for Family with Adult Patients (RHDS Fam) questionnaire (inter-item correlation = 0.90, and Cronbach alpha 0.89). Descriptive analysis was applied. The results showed that most of family were female (77.7%%), unemployed (69.4%), graduated from senior high school (57.9%), and had no chronic diseases (86.8%). Generally, family caregivers reported high readiness for hospital discharge (7.75 Out of 10). While the coping abilities identified as the readiness domain with highest average score (9.07±1.15), the personal domain identified as the lowest domain (7.07 ±0.68). Higher readiness was showed in the age group of 20-40 years, female, graduated from senior high school, employed, and are the siblings of the patients. It can be concluded that the family caregivers of chronic disease patients are ready to return home and help patients continue their post-discharge treatment program. However, interventions are needed to improve their readiness related to patient’s personal status, particularly those who are male, lower educational background, unemployed, has a relationship as parents of the patients, and has a chronic illness.

Abstract

Nursing profession’s education is responsible for preparing competent graduates ready to face healthcare challenges. A key indicator of this readiness is the self-efficacy of nursing students, which reflects their confidence in applying clinical skills. It is crucial for these students to be prepared to provide safe, high-quality care as they enter the workforce. This study aims to evaluate the self-efficacy levels in clinical skills among students enrolled in the Nursing Profession Study Program at the Faculty of Nursing, Universitas Padjadjaran. This study employed a quantitative descriptive method using a cross-sectional approach. The population in this study amounted to 150 active students of the Nursing Profession Study Program at the Universitas Padjadjaran and the sample was taken using total sampling type. Data was collected using the Clinical Skills Self-Efficacy Scale (CSES) to measure the level of self-efficacy in performing clinical skills with validity test results after translated to Bahasa ranging from 0.415-0.727, and the reliability test results 0.825. Univariate analysis was carried out using descriptive statistics. The results of the study show that students of the Nursing Professional Education Program at the Faculty of Nursing, Universitas Padjadjaran, have a high level of self-efficacy in performing basic clinical skills, with 80 students (53.3%) demonstrating high self-efficacy. Where the percentage of class 46 students who have a high level of self-efficacy is greater than class 47. This is largely due to the experience they have in carrying out clinical procedures. That’s related to the statement one of the main factors that can influence self-efficacy according to Albert Bandura is experience, the experience of someone who is successful in carrying out a task or achieving a goal can increase self-efficacy.

Abstract

Renal failure patients will experience a variety of metabolic disorders included in the production process of various vitamins and minerals. Lack of vitamin and mineral intake will aggravate vitamin and mineral deficiencies so that patients can experience various musculoskeletal disorders. This condition can cause deterioration in the patient's condition so that the quality of life of the patient can decrease. This study aims to identify the fulfillment of vitamin and mineral needs in patients with chronic kidney diseases.This research used descriptive quantitative method with purposive sampling technique. The Sample in this study was 80 patients with criteria, experience full awareness (composmentis) and literacy. Data was taken using a food record, then analyzed with Nutrisurvey software. Data were analyzed using frequency distribution. The results showed that sodium intake (55%), potassium (87.5%), calcium (53.75%), and phosphorus (46.25%) in most respondents in the normal category. Vitamin D intake in most respondents (70%) is in the less category. Most respondents get enough vitamin and mineral intake except vitamin D.Health workers are expected to provide ongoing education on the importance of maintaining intake of vitamins and minerals especially vitamin D in patients with chronic kidney diseases. In addition, health workers are expected to collaborate well in meeting the vitamin and mineral needs of patients with kidney diseases.

Abstract

Coronary Heart Disease (CHD) was the number two cause of death; one of the treatments for CHD was Coronary Artery Bypass Graft (CABG). Patients after undergoing CABG will undergo treatment in the Intensive Care Unit (ICU). In patients who experience treatment in the ICU, many patients experience sleep disturbances related to therapy regime rhythm in the ICU. This literature review aims to identify the intervention for managing sleep quality in post-CABG patients undergoing treatment in the ICU. The method use was a narrative review study, The review was conducted two databases, PubMed and ScienceDirect, to search data from 2012 to 2022. The keywords used were Coronary Artery Bypass Graft, Intensive care unit, and Sleep Quality. The results of this study included nine articles reviewed, which were entirely randomized control trials and randomized clinical trials. The review found that the interventions to manage sleep quality are deep breathing therapy, aromatherapy with roses, acupressure, self-care, and drug administration. From the results of the review, it was found that all articles reported using several specific interventions in the intervention group that were proven to improve sleep quality in post-CABG patients. The study results provide information for nurses regarding sleep quality in post-CABG patients.

Abstract

Gagal ginjal terminal yang dialami pasien hemodialisis dapat menimbulkan berbagai perubahan dalam kehidupannya. Persepsi yang positif terhadap penyakit dapat membantu pasien hemodialisis dalam menerima keadaannya dan meningkatkan motivasi untuk menjalankan berbagai tindakan pengobatan. Apabila pasien hemodialisis memiliki persepsi yang negatif terhadap penyakit, maka akan cenderung mudah mengalami berbagai masalah psikologis. Penelitian ini dilakukan untuk mengetahui persepsi terhadap penyakit pada pasien hemodialisis di Bandung. Penelitian ini merupakan penelitian deskriptif kuantitatif yang dilakukan kepada pasien hemodialisis di salah satu Rumah Sakit di Bandung. Teknik sample yang digunakan adalah consecutive sampling sebanyak 126 orang. Kriteria inklusi dalam penelitian ini adalah pasien gagal ginjal yang menjalani hemodialisis dan memiliki tanda-tanda vital yang stabil. Instrumen yang digunakan untuk mengukur persepsi terhadap penyakit adalah kuesioner persepsi penyakit singkat (Brief-IPQ) yang dikembangkan oleh Broadbant, et.al. tahun 2005, dan sudah dilakukan back translate ke dalam bahasa Indonesia. Hasil penelitian menunjukkan bahwa sebagian besar pasien hemodialisis memiliki persepsi terhadap penyakit yang negatif (50,4%). Sebagian besar pasien merasakan berbagai dampak penyakit terhadap kehidupannya dan mengalami perubahan secara emosional semenjak mengalami gagal ginjal terminal. Persepsi terhadap penyakit yang negatif pada pasien hemodialisis dapat mempengaruhi kualitas hidup, angka kesakitan dan capaian pengobatan yang dijalaninya. Oleh karena itu, dibutuhkan dukungan keluarga dan sosial. Petugas kesehatan khususnya perawat diharapkan dapat memberikan edukasi dan konseling pada pasien hemodialisis untuk meningkatkan persepsi pasien terhadap penyakit.

Abstract

ABSTRAKIndonesia sudah menetapkan program skrining kanker serviks. Namun, program skrining kanker serviks di Indonesia belum mencapai target. Banyak faktor yang dapat memengaruhi sehingga perlu adanya identifikasi lebih lanjut untuk mengetahui hal tersebut. Tujuan penelitian: untuk memetakan faktor-faktor yang memengaruhi penerapan skrining kanker serviks pada wanita. Metode: scoping review dengan mencari melalui Google Scholar, EBSCO, PubMed, dan ProQuest. Kata kunci yang digunakan antara lain “woman” OR “women” OR “female” OR “females” AND “cervical cancer screening”,“motivation” OR “participation”,“barrier” OR “challenges”,“influence factors”, terbit tahun 2017 hingga 2020, bahasa Indonesia dan bahasa Inggris, teks lengkap, penelitian kuantitatif, kualitatif, dan tinjauan sistematis. Critical appraisal yang digunakan dalam penelitian ini ialah JBI Form Checklist untuk penelitian cross sectional, kualitatif, dan systematic review.  Hasil: delapan belas artikel ditemukan dan digunakan dalam studi literatur, dengan mayoritas berasal dari Asia (77,7%).  Terdapat 23 faktor, kemudian dikelompokkan ke dalam 4 kelompok faktor antara lain faktor demografi dan biologi yang meliputi tempat tinggal, usia, pekerjaan, pendapatan, usia menikah, tingkat pendidikan, asuransi kesehatan, dan riwayat kesehatan; faktor kognitif antara lain rendahnya pengetahuan tentang kanker serviks, skrining, dan faktor risikonya; faktor emosional termasuk rasa malu; faktor sosial budaya yang meliputi dukungan dari pemberi pelayanan kesehatan, keluarga, relawan kesehatan masyarakat, otoritas dalam keluarga; dan faktor infrastruktur seperti akses informasi. Diskusi: skrining kanker serviks pada wanita dibangun oleh berbagai faktor.  Puskesmas sebagai fasilitas pelayanan kesehatan primer diharapkan mampu mengoptimalkan pelayanan dan penyebarluasan informasi deteksi dini kanker serviks.  Kesimpulan: penelitian selanjutnya agar lebih fokus mengidentifikasi salah satu faktor pengaruh serta penanggulangan faktor tersebut. Kata Kunci: kanker serviks, scoping review, skrining Factors Influencing Cervical Cancer Screening in Indonesia: A Scoping Review ABSTRACTCervical cancer screening programs have been established in Indonesia; however, these programs have not yet reached their intended targets. Multiple factors can impact the successful implementation of cervical cancer screening, highlighting the need for further identification and understanding of these factors. Objective: to map the factors that influence the implementation of cervical cancer screening among women. Methods: A scoping review was conducted using the databases Google Scholar, EBSCO, PubMed, and ProQuest. The search employed keywords such as "woman" OR "women" OR "female" OR "females" AND "cervical cancer screening", "motivation" OR "participation", "barrier" OR "challenges", "influence factors", with the inclusion criteria of studies published between 2017 and 2020, in either Indonesian or English languages, with full-text availability. The review encompassed quantitative, qualitative, and systematic review studies. The critical appraisal used in this study is the JBI form checklist for cross-sectional, qualitative, and systematic studies.  Results: Eighteen articles were included in the literature analysis, with the majority originating from Asia (77.7%).  Through thematic analysis, twenty-three factors were identified and categorized into four groups: demographic and biological factors (e.g., place of residence, age, occupation, income, age at marriage, education level, health insurance, and health history); cognitive factors (e.g., insufficient knowledge about cervical cancer, screening, and risk factors); emotional factors (e.g., feelings of shame); sociocultural factors (e.g., support from healthcare providers, family, community health volunteers, and authority within the family); and infrastructure factors (e.g., access to information). Discussion: Cervical cancer screening among women is influenced by various factors. Primary healthcare facilities, such as community health centers (puskesmas), are expected to optimize services and disseminate information on early detection of cervical cancer. Conclusion: further research to focus more on identifying one of the influencing factors and overcoming these factors.Keywords: Cervical cancer, scoping review, screening

Abstract

Diabetic ketoacidosis (DKA) is a life-threatening form of diabetes mellitus emergency. Rapid and appropriate management is needed to prevent worsening condition. Fluid management in DKA is important to improve tissue perfusion, correct electrolyte imbalance, reduce blood glucose concentration and counterregulatory hormones. This case report aims to describe the fluid management in a client with instability of blood glucose levels in DKA. A 42-year-old woman was rushed to the emergency room with decreased consciousness for 3 hours. Laboratory examination results: blood glucose >600mg/dL, Base Excess <-30mmol, urine ketones +3, urine protein +2, urine glucose +4, pH 6.8, HCO3 1.9mmol/L. The client had unstable blood glucose levels while in the treatment room. Fluid management was performed by combining 0.9% NaCl, WIDA 2A, 40% dextrose bolus, and continuous insulin drip, that was adjusted according to the client's blood glucose level. After receiving fluid management, the client's blood glucose levels are stable in the range of 100 - 200mg/dL on the fifth day until the client recovered. Providing fluid management tailored to the management of DKA and client needs can improve the stability of blood glucose levels so that this fluid management intervention can be utilized for DKA clients with similar complaints.

Abstract

Kanker paru primer adalah tumor ganas yang berasal dari epitel bronkus. Kanker paru merupakan ancaman bagi penduduk di dunia. Kanker paru seringkali terlambat ditangani karena gejala dirasakan muncul setelah stadium lanjut.hingga muncul gejala yang menyerupai penyakit lain. Gejala yang dapat muncul dapat berupa batuk, nyeri, sesak, sesak semakin berat karena dapat muncul komplikasi efusi pleura. Kasus kanker paru ini menjadikannya penyakit yang menarik untuk disajikan sebagai kasus klinis dengan rencana model asuhan keperawatan. Tujuan studi kasus ini adalah untuk mendeskripsikan dan intervensi berdasarkan model adaptasi Roy. Kasus: pasien laki-laki berusia 59 tahun dengan kanker paru dextra disertai komplikasi dan mengalami batuk berdarah, sesak dan nyeri. Rencana perawatan menerapkan model adaptasi Roy. Kebutuhan utama berdasarkan pendekatan adaptasi Roy yang ditemukan adalah oksigenasi (pola napas tidak efektif), sensasi (nyeri), kebutuhan akitivitas dan istirahat (intoleransi aktivitas) perlindungan (resiko infeksi), dan konsep diri (kecemasan). Metode penelitian yaitu studi kasus dengan asuhan keperawatan secara komprehensif melalui pendekatan adaptasi teori model Callista Roy. Pendekatan model adaptasi Roy berguna dalam kasus ini karena berfokus pada kemampuan adaptasi pasien yang sesuai untuk digunakan dalam manajemen pasien kanker paru disertai perawatan kolaboratif. Abstract

Fatigue merupakan salah satu gejala yang paling banyak dirasakan pasien hemodialisis. Fatigue dapat memperberat kondisi pasien, dan menurunkan kualitas hidup. Fatigue pada pasien hemodialisis harus dikurangi melalui berbagai intervensi. Tujuan studi literature  ini adalah untuk mengidentifikasi intervensi komplementer yang terbukti dapat menurunkan fatigue pada pasien hemodialisis. Literatur review ini menggunakan metoda scoping review dengan mencari literatur dari beberapa databased seperti Ebsco Host, Scopus, Science Direct, Sage Journals, Springer, dan Pubmed. Selain itu, pencarian literatur juga dilakukan dalam mesin pencari yaitu Google Scholar dengan menggunakan beberapa kata kunci pencarian yaitu “hemodialysis” OR “haemodialysis” OR “dialysis” AND “Complementary Intervention” AND “fatigue management” OR “energy concervation” OR “energy management” OR “managing fatigue”. Kriteria inklusi yang digunakan dalam literature review ini yaitu artikel terbit antara 2014-2024 (10 tahun terakhir), menggunakan metoda Randomized Controlled Trials (RCT), berbahasa Inggris. Untuk artikel dengan hasil tidak terbukti dapat menurunkan fatigue dieksklusi dari  review ini. Berdasarkan hasil pencarian  didapatkan 13 artikel yang membuktikan bahwa terapi akupresure, terapi pijat dengan menggunakan minyak zaitun, terapi recreasional, dan beberapa latihan fisik dapat menurunkan fatigue.  Kata Kunci: fatigue, hemodialisis, penyakit  ginjal  kronik, terapi komplementer
