Abstract

ABSTRACT The prevalence of chronic kidney failure in the world is increasing every year. Most patients require hemodialysis as a kidney replacement therapy. Complementary and alternative medicine (CAM) researchers revealed that massage therapy is widely used by hemodialysis patients to overcome the side effects caused by hemodialysis. To identify the effectiveness of massage therapy in hemodialysis patients. This scoping review was developed based on the Arskey and O'Malley framework. Study searches were conducted through Scopus and CINAHL. Inclusion criteria in selecting articles included: primary research results discussing the application of massage therapy in patients undergoing hemodialysis in the form of full-text articles, in English, and published in 2019-2023. The 13 articles analyzed in this study revealed that massage therapy has a positive effect on general health, physical symptoms, nausea, fatigue, pain due to muscle cramps, and restless legs syndrome (RLS), pruritus severity, sleep quality, insomnia severity, quality of life, anxiety, and intradialytic hypertension in hemodialysis patients. Foot massage therapy can be used as a complementary therapy to reduce the side effects of hemodialysis. Nurses and health workers need to have certified massage therapy skills in order to provide and teach massage to hemodialysis patients and their families. Keywords: Hemodialysis, Massage Therapy, Nurses.  ABSTRAK Prevalensi penyakit gagal ginjal kronis di dunia meningkat setiap tahun. Abstract

Hemodialysis patients' self-management is critical for maintaining their quality of life and minimizing complications. Patients' health literacy and perception of the disease are essential for their acceptance of the condition and treatment. However, the relationship between these variables and self-management among hemodialysis patients remains unclear. This study aims to identify the relationship between health literacy, perception of disease, and self-management among hemodialysis patients. This correlational study was cross-sectional design, with 129 hemodialysis patients consecutively recruited from a hemodialysis unit of a tertiary hospital in West Java, Indonesia. Data were collected using the Brief-Illness Perception Questionnaire (Brief-IPQ), the European Health Literacy Survey Questionnaire (HLS-EU-Q47), and the End Stage Renal Disease Adherence Questionnaire (ESRD-AQ). The data were analyzed using Pearson correlation analysis. Most of the patients had sufficient or excellent health literacy (74.4%), negative illness perception (50.4%), and low self-management practices (71.3%). Pearson’s correlation analysis revealed a positive correlation was found between health literacy and self-management practices (r = .189; ρ=0.032). Additionally, there was a negative correlation between illness perception and health literacy (r = -.27; ρ= .002) as well as between illness perception and self-management practices (r = -.762; ρ= .000). Positive illness perception and better self-management practices correlate with a higher level of health literacy. These findings highlight the importance for healthcare staff to facilitate positive illness perceptions and self-management practices, and to consider these factors as vital aspects in developing self-management education programs for ESRD patients.

Abstract

Hemodialysis (HD) is part of the therapy for chronic kidney disease (CKD) patients to partially replace kidney function. However, the routine process of hemodialysis, which is performed two to three times a week, has an impact to patient’s quality of life. Spiritual-based interventions, such as listening to Quran recitation, are emerging as a complementary-therapies to address the symtomps experienced by patients undergoing hemodialysis. However, the potential use of listening to the quran in patients undergoing hemodialysis remains limited. Therefore, this study aims to explore and assess the effects of listening to Quran recitation in CKD patients undergoing hemodialysis. The scoping review was used in this study. The eligibility criteria of this study, include all full-text primary studies written in English and published between 2015 and 2024. The literature systematically searched using 3 databases, such as PubMed, Scopus, and EbscoHost with keywords "chronic kidney disease AND listening quran OR listening koran AND hemodialysis.” Additionally, search engine, Google Scholar, also used to identify additional literature. A total of 9 articles were included consisting of 3 RCTs and 6 quasi experimental studies. The effects of listening to Quran recitation were divided into two benefit cluster. 1) Psychological benefits, such as reduced anxiety, lower depression, and improved quality of life. 2) Physical benefits, such as improved physical condition and reduced fatigue, lower blood pressure, and reduced systemic inflammation. The findings suggest that Quran recitation can be a valuable, non-invasive adjunctive therapy to support holistic care models for CKD patients undergoing hemodialysis.

Abstract

Spinal Cord Injury (SCI) level tinggi dapat menyebabkan gangguan pada pernapasan yang dapat mengancam nyawa pasien. One of the interventions that can be given to individuals with SCI in reducing complications of respiratory problems is Respiratory Muscle Training (RMT). Respiratory Muscle Training (RMT) is a technique that aims to improve the function of the respiratory muscles through specific exercises. This study determined the effectiveness of respiratory muscle training in spinal cord injury patients with respiratory problems. This literature review used several databases such as Google Scholar, Pubmed, Ebsco, Sciencedirect, CINAHL, and SAGE with inclusion criteria Articles published less than five years (2018-2022), English, Full text, Systematic review/meta-analysis/RCT. The results indicate that this RMT intervention's effectiveness on respiratory muscles and lung function depends on the characteristics of SCI patients. There were six articles used for the review of this paper. The results show that this respiratory muscle training intervention's effectiveness impacts respiratory muscles and lung function depending on the characteristics of the spinal cord injury patient himself. Furthermore, the results suggest that this RMT intervention's efficacy on respiratory muscle and lung function depends on the features of the SCI patient

Abstract

Chronic renal failure patients undergoing haemodialysis therapy experience various conditions and require long-term care which can cause a burden on the families who care for patients. This family caregiver burden can affect the quality of life of both patients and family caregivers, and also affect the care provided to patients. The purpose of the study was to determine the description of caregiver burden in families with haemodialysis patients. This study is a quantitative descriptive study and the sample was taken using purposive sampling technique (n = 107) with the inclusion criteria of respondents as the main caregiver with family members undergoing haemodialysis therapy at RSUD Sumedang, and physically and mentally healthy. Exclusion criteria in the study were potential respondents who refused to participate and respondents who decided to stop in the middle of the study. The burden of caregivers on the family was measured using the Burden Scale for Family Caregivers Short Scale (BSFC-s) questionnaire with test results, namely r results (0.634) > r table (0.786) as well as Cronbach alpha value of 0.900 which means this questionnaire is valid and reliable. The picture of caregiver burden shows that almost half of family caregivers are in the high category (49.5%) and most experience heavy burden in the physical dimension. The results of this study are expected to be used as a reference for learning and evaluation for nurses, family caregivers, and educational institutions to be able to carry out effective interventions that can be carried out to reduce the burden of care experienced by family caregivers.

Abstract

Families often act as informal caregivers for family members who suffer from chronic diseases. Intensive family involvement improved patients’ adherence in performing post-discharge treatment program, hospital readmission, clinical outcomes, and quality of life. To effectively help the patients managing their diseases, the family caregivers required not only understand but also must be ready for helping patients manage their conditions. This study aims to identify the readiness for hospital discharge among family caregivers of patients with chronic diseases. Data were collected from 121 family caregivers recruited consecutively from adult inpatients wards using the respondents’ characteristics questionnaire and Readiness for Hospital Discharge Scale for Family with Adult Patients (RHDS Fam) questionnaire (inter-item correlation = 0.90, and Cronbach alpha 0.89). Descriptive analysis was applied. The results showed that most of family were female (77.7%%), unemployed (69.4%), graduated from senior high school (57.9%), and had no chronic diseases (86.8%). Generally, family caregivers reported high readiness for hospital discharge (7.75 Out of 10). While the coping abilities identified as the readiness domain with highest average score (9.07±1.15), the personal domain identified as the lowest domain (7.07 ±0.68). Higher readiness was showed in the age group of 20-40 years, female, graduated from senior high school, employed, and are the siblings of the patients. It can be concluded that the family caregivers of chronic disease patients are ready to return home and help patients continue their post-discharge treatment program. However, interventions are needed to improve their readiness related to patient’s personal status, particularly those who are male, lower educational background, unemployed, has a relationship as parents of the patients, and has a chronic illness.

Abstract

Blood circulation disorders in post-stroke patients can cause cognitive disorders. Cognitive disorders in post-stroke patients can include memory impairment, difficulty speaking, difficulty understanding things, difficulty communicating, and mood disorders. The presence of cognitive disorders in stroke patients can worsen the patient's post-stroke condition, requiring longer treatment time. This study aimed to identify the level of cognitive disorders in post-stroke patients. This research is a descriptive study to identify dementia in post-stroke patients. The sampling technique was accidental sampling for two weeks at the Neurology Polytechnic and Stroke Clinic at one of the hospitals in Bandung, totaling 83 people. The questionnaire used in this research was the Mini-Mental State Examination (MMSE). The collected data is analyzed and presented using frequency and percentage distributions. The results of this study showed that the majority of post-stroke patients (72.3%) did not experience dementia or did not experience cognitive impairment, 15.66% experienced mild cognitive impairment, and 12.04% experienced severe cognitive impairment. From the results of this study, it can be concluded that some post-stroke patients do not experience cognitive impairment, but some post-stroke patients experience severe cognitive impairment. Cognitive impairment in stroke patients can worsen the patient's condition and increase the length of treatment time. Based on the results of this research, health workers especially nurses, are expected to be able to carry out early assessments and provide particular interventions for post-stroke patients aimed at preventing the worsening of dementia and improving the patient's cognitive abilities. The intervention can be in the form of education, psychological support, and social support in the form of counseling during rehabilitation.
