Abstract

Background: Breast cancer is the cancer with the highest incidence in women in the world. In Indonesia, breast cancer also ranks first in terms of number and cause of death. Bleeding is a common complication of advanced breast cancer. Identification of bleeding factors in breast cancer wounds is very important. Research instruments are needed to analyze various factors related to bleeding and their indicators Purpose: To determine the validity and reliability of the construct of factors related to bleeding in breast cancer wounds and their indicators. Method: A cross-sectional approach, specifically focusing on observational study. The study took place from January to May 2024, in Dr. Soedraso Hospital Pontianak. The independent variable was factors affecting breast cancer wound bleeding the dependent variables is assessment of breast cancer wound bleeding. Using purposive sampling and the Slovin formula, 60 respondents were selected. The validity and reliability tests in this study were conducted using the outer model evaluation in SEM-PLS with SmartPLS 4.0. Validity was assessed through convergent validity using the AVE value and discriminant validity with the Fornell-Larcker criterion. Reliability was evaluated based on outer loading values and composite reliability (Rho-C) to measure internal consistency. Results: the outer loading value, composite reliability (Rho-C), and average variance extracted (AVE) have met the validity and reliability test standards. The outer loading value of the indicators ranged from 0.628 to 1.000, with the highest value in the indicators of cancer stage, leukocytes, nutritional status (MNA), and bleeding at 1.000. Composite reliability for the variables of wound classification, dressing use and wound care, chemotherapy, therapy, and haematology ranged from 0.776 to 0.903, while the variables of cancer stage, infection, nutritional status, and bleeding each amounted to 1.000. AVE values ranged from 0.599 to 0.822, with the highest value of 1.000 for the variables of cancer stage, infection, nutritional status, and bleeding. Conclusion: This research conducted validity and reliability tests using the measurement model (outer model) evaluation in SEM-PLS. The assessment included outer loading, composite reliability (Rho-C), AVE, and the Fornell-Larcker criteria. The results confirmed that the research instrument was valid and reliable, making it suitable for analyzing bleeding factors in breast cancer wounds.

Abstract

Background: Diabetic Foot Ulcer (DFU) is a chronic complication of diabetes mellitus associated with delayed wound healing and an increased risk of lower-limb amputation. The MUNGS (Maceration, Undermining, Necrotic tissue, Granulation tissue, and Signs/Symptoms of infection) instrument is widely used to assess wound conditions; however, evidence regarding the relationship between individual MUNGS components and wound healing outcomes remains limited. Objective: To analyse the relationship between patient characteristics, individual MUNGS component scores, and wound healing outcomes in patients with DFU. Methods: A quantitative observational study with a retrospective cross-sectional design was conducted among 47 patients with DFU treated at Kitamura Pancasila Clinic, West Kalimantan, between January and July 2025 using total sampling. Data were obtained from medical records and wound photographs. Data were analysed using univariate analysis, Spearman Rank and Fisher's Exact tests, and binary logistic regression. Results: Bivariate analysis showed that the necrotic (p=0.006), granulation (p=0.010), and signs/symptoms (p=0.009) components were significantly associated with wound healing outcomes, whereas patient characteristics, maceration, and undermining were not (p>0.05). Multivariate analysis identified no significant independent predictor, although necrotic and granulation showed a tendency to be associated with wound healing. Conclusion: The necrotic, granulation, and signs/symptoms components were associated with wound healing outcomes in patients with DFU. Although no independent predictor was identified, the MUNGS instrument remains clinically valuable for evaluating wound healing progression.

Abstract

Background: Diabetes Mellitus (DM) is a chronic disease with a continuously increasing incidence that requires long-term management. Glycemic control largely depends on patients’ ability to perform self-care. Dorothea Orem’s Self-Care Deficit Nursing Theory emphasizes that individuals require support when they experience limitations in meeting their self-care needs. In primary care settings, family involvement is an important factor in the effective management of DM; however, evidence mapping regarding the family-based implementation of this theory remains limited. Objective: This scoping review aimed to map the scientific evidence regarding the implementation of a family-based Self-Care Deficit Nursing Theory approach for glycemic control among patients with DM in primary care settings. Methods: A scoping review was conducted following the PRISMA-ScR guidelines. Literature searches were performed in Google Scholar, PubMed, and ScienceDirect for studies published between 2015 and 2025. Eligible studies included primary quantitative, quasi-experimental, randomized controlled trial (RCT), and mixed-methods studies addressing self-care, family support, and glycemic control among patients with DM in primary care settings. Results: Most studies demonstrated that family-based interventions contributed to improvements in dietary adherence, physical activity, self-monitoring of blood glucose, and treatment adherence. These approaches also enhanced patients’ motivation and consistency in performing self-care activities, thereby supporting better glycemic stability. Conclusion: Family-based self-care approaches have the potential to improve glycemic control among patients with Diabetes Mellitus in primary care settings. Integrating nursing theories into family-centered interventions may strengthen the delivery of community-based nursing care.
