Abstract

Introduction: Women living with HIV face clinical, psychological, and social challenges that can affect self-care. While demographic and disease-related factors have been widely studied, the roles of adaptation and resilience in self-care remain unclear. Objective: To develop and test a resilience model based on the Roy Adaptation Model to explain self-care among women living with HIV in Surabaya, Indonesia. Methods: A cross-sectional study was conducted among 111 women living with HIV recruited through purposive sampling from primary healthcare centers in Surabaya. Eligible participants were women aged ≥18 years receiving antiretroviral therapy for at least three months, able to complete an online questionnaire, and without diagnosed mental disorders or a history of substance abuse treatment. Data were collected using structured questionnaires measuring focal, contextual, and residual stimuli, coping mechanisms, adaptation modes, resilience, and self-care. SEM-PLS with 5,000 bootstrap resamples was used to evaluate the proposed model. Results: The measurement model showed satisfactory reliability and validity (CR = .564–.953; AVE > .50). Contextual (β = .170; P-value<.05) and residual stimuli (β = .492; P-value<.05) significantly influenced coping mechanisms, whereas focal stimuli did not. Coping mechanisms were associated with adaptation modes (β = .624; P-value<.05), which in turn were associated with resilience (β = .669; P-value<.05) and self-care (β = 0.758; P-value<.05). Self-care showed the highest predictive relevance (Q² = .425). Conclusion: Self-care among women living with HIV is closely associated with coping, adaptation, and resilience rather than clinical or demographic factors. Abstract

This study analyzes pending claims within the INA-CBG–based reimbursement process at the Casemix Unit of RS Islam Masyithoh Bangil in 2025. Under Indonesia’s National Health Insurance (JKN), claim submission functions as a critical interface between clinical services and financing governance; disruptions in this process may affect both operational workflows and hospital cash flow. This research employed a descriptive case study design using semi-structured interviews, non-participant observation, and document review. To strengthen the process-based analysis, the study incorporated descriptive quantitative tabulation of monthly verification documents from January–November 2025 at the claim level (unique SEP), covering claim status, service type, pending categories, and submitted value. The dataset recorded 5,272 claims, of which 2,417 were pending (45.8%), with a cumulative pending submission value of IDR 8,427,504,800 and marked month-to-month fluctuations. The composition of pending categories shifted across months, predominantly involving coding rules and service standards, while administrative completeness remained consistently present at a smaller proportion. These findings indicate that pending claims represent a recurrent process output arising from interdependent documentation practices, coding accuracy, and cross-unit coordination within the INA-CBG framework. The study concludes that strengthening internal verification, standardizing documentation and classification, and establishing routine monitoring indicators are essential to improve claim efficiency and organizational sustainability.

Abstract

Increasingly fierce competition in the healthcare sector requires clinics to continuously improve the quality of their services and organizational competitiveness. Outpatient services are one of the primary services that play a crucial role in enhancing patient satisfaction, the number of visits, and the clinic’s operational sustainability. Therefore, an appropriate development strategy is needed based on an analysis of the organization’s internal and external conditions. This study analyzes the development strategy for outpatient services at Locus Medical Hub Clinic using SWOT analysis and formulates an appropriate development strategy. This research employs a qualitative approach with a case study design. The research informants consist of management, doctors, nurses, administrative staff, and outpatient patients selected using purposive sampling, totaling 30 respondents. Data collection was conducted through in-depth interviews, observation, documentation, and Focus Group Discussions (FGDs). Data analysis was performed using the Internal Factor Analysis Summary (IFAS) Matrix, External Factor Analysis Summary (EFAS) Matrix, and SWOT Matrix. The analysis results show an IFAS score of 3.04 and an EFAS score of 3.15. The clinic’s key strengths include competent healthcare staff, fast and friendly service, a digital registration system, and a strategic location. Key opportunities stem from the growing demand for healthcare services, advancements in healthcare technology, and opportunities for collaboration with various parties. The SWOT analysis results place Locus Medical Hub Clinic in Quadrant I, indicating a strong organizational condition with high development potential. The recommended strategy is an aggressive strategy (SO) that leverages internal strengths to capitalize on external opportunities. This strategy includes the development of technology-based flagship services, improving service quality, optimizing digital systems, enhancing human resource competencies, and expanding strategic partnerships to improve the quality of outpatient care and the competitiveness of Locus Medical Hub Clinic.
