Abstract

Introduction: Universal Health Coverage (UHC) is a significant agenda outlined in the SDG 2030. In order to achieve UHC, Indonesia established the Social Security Management Agency (BPJS) in 2014. Nearly a decade after its establishment, BPJS experienced a financial deficit in its first six years, followed by a surplus during the COVID-19 pandemic. This has impacted the sustainability of BPJS Health. One of the factors contributing to the financial deficit is the non-compliance of participants in paying BPJS contributions, particularly in the Non-Wage Earner/Self-Employed/Independent (PBPU) and Non-Worker segments who are required to pay contributions individually. When participants fail to pay contributions for six consecutive months, their membership status is converted to inactive. From the 2022 BPJS Sample Data, it was found that 799,384 out of 2,305,435 (34.67%) participants had inactive membership status. Objective: This study aims to analyze the factors influencing BPJS membership status in the Independent and Non-Worker segments. Method: The method that used in this study is quantitative analysis of secondary data from the 2022 BPJS Sampel Data with the Linear Probability Model (LPM), Logit and Probit Models. The analysis is conducted using STATA to obtain univariate, bivariate and multivariate analyses. Result: From a sample of 545,507 participants, if was found that 54.20% had inactive membership status. Factors such as age, gender, marital status, treatment class, healthcare facility type, and participant segmentation affect membership status. The LPM regression results indicate that the factors leading to inactive membership status are being of productive age, male, belonging to class 1 and 2, utilizing primary healthcare facilities, and being in the Independent segment. Conclusion: Therefore, stateholders such as the central government and BPJS need to collaborate in efforts to reduce the number of inactive membership statuses.

Abstract

Background: Indonesia is experiencing rapid population aging, raising concern about muscle weakness in later life; possible sarcopenia offers early identification through low handgrip strength. Objective: To estimate the prevalence of possible sarcopenia and its determinants among older adults in Indonesia. Methods: This secondary analysis used baseline data from the Indonesian Longitudinal Aging Survey 2023 and included adults aged sixty years or older with valid handgrip measurements. Possible sarcopenia followed the Asian Working Group for Sarcopenia 2019 thresholds using the maximum of two trials per hand. Covariates included sociodemographic, behavioral, functional, and clinical factors. We applied descriptive statistics, bivariate tests, and multivariable logistic regression, with probit average marginal effects. Sampling weights were unavailable in the public-use microdata; we treated enumeration areas as the primary sampling units, with available stratification and no weights; estimates were unweighted, and standard errors were design-based (survey-corrected). Results: Among 1,598 participants, the prevalence of possible sarcopenia was 51.1%. Older age, low physical performance, and urban residence were associated with higher odds. Higher body mass index and better cognition were associated with lower odds. A prespecified sex-by-body mass index interaction suggested attenuation of the protective association of body mass index among women. Conclusion: Possible sarcopenia is common among older Indonesians and is patterned by age, body composition, cognition, functional status, and residential context. Community health services, including community nursing services, can integrate routine handgrip assessment with brief interventions on strength activities, nutrition, and cognitive engagement to identify risk early and inform preventive care.

Abstract

Developing Asian countries continue to face financial challenges in ensuring equitable access to HIV services. Despite numerous initiatives to expand coverage and improve treatment outcomes, affordability remains a major barrier. This systematic review, conducted using PRISMA methodology, analyzes willingness to pay (WTP) and readiness to pay (RTP) for HIV-related services, based on 14 studies selected from an initial pool of 6,773 articles identified through three major databases (PubMed, Scopus, and ProQuest) covering peer reviewed publications between 2015 and 2025. Focusing on at-risk populations in India, China, Vietnam, Iran, and the Philippines, the review examines services such as HIV self-testing kits, antiretroviral therapy (ART), pre-exposure prophylaxis (PrEP), and viral load monitoring. Findings reveal significant variation in WTP/RTP influenced by education, income, HIV awareness, stigma, and privacy concerns. For instance, 69% of respondents in India were willing to pay for self-testing kits, compared to only 6.8% of MSM in China willing to pay market rates for PrEP. RTP extends beyond financial capacity to include behavioral, social, and cultural readiness, which plays a critical role in service uptake. The review concludes that WTP/RTP alone is insufficient for achieving universal HIV care. It recommends adaptive policies such as hybrid financing models, targeted health education, and government subsidies to ensure sustainable and inclusive HIV service delivery.

Abstract

Pre-hospital care is the initial phase of emergency management for patients with acute conditions. Emergency patients' mortality rates can be lowered with appropriate pre-hospital care. The utilization of telemedicine has been increasing rapidly, and one of its applications is the provision of pre-hospital care. This study aimed to identify and evaluate published research on the use of telemedicine to facilitate emergency patient management in pre-hospital care. ProQuest, Scopus, EMBASE, and EBSCOhost databases were used for article search and PRISMA flow diagram was used to perform the articles selection. The search results obtained 443 articles. Title and abstract screening removed 347 articles, leaving 41 to be assessed for eligibility. Articles relevant to the study question and meet the criteria were 14 articles. Most of the studies were conducted in developed countries. The use of telemedicine has a significant potential to improve the quality of pre-hospital care provided to emergency patients, by aiding in diagnosis and treatment selection for patient’s better outcomes. However, to establish this system, a well-organized EMS system is required. Nevertheless, the use of this technology requires more development, and additional study is required in the future.
