Abstract

Background: Jaminan Kesehatan Nasional (JKN) is the health policy of the Indonesian government to achieve Universal Health Coverage (UHC), where each City or District has a varied regional policy in implementing it. Surakarta City is one of the regions that has been able to reach UHC. Policy analysis triangle is used to understand the various factors involved in each policy process. Objective: This study aims to explore deeply the implementation of JKN policy in Surakarta City to be able to achieve UHC based on a policy analysis triangle.Methods: This type of research is a qualitative study with a case study approach. The study was conducted in Surakarta City, Central Java, from September 2018 to March 2019. Data collection was carried out through in-depth interviews and document analysis. Sampling technique with criterion sampling. The research informants were five people who were the leaders of the Surakarta City Government, and 10 people from the Surakarta City community. Data validity was done by triangulation, member check, and peer debriefing. Results: The implementation of JKN policy in the city of Surakarta to reach UHC was influenced by: actors who were highly committed and competent in the success of the JKN program, a dynamic process in adjusting central policy to the regions by implementing various strategies and innovations to increase JKN membership coverage; political factors with policy making that support the achievement of UHC, environmental factors where population density in Surakarta City is the most densely populated in Central Java, there is a high awareness and willingness of the people of Surakarta City to have JKN, and technological support that facilitates JKN coordination and implementation. Conclusion: Achieving UHC in Surakarta City is the result of optimizing the four factors in the triangle analysis policy.

Abstract

Pendahuluan: Keadaan darurat bisa terjadi pada siapa saja, di mana saja, kapan saja. Dalam situasi ini, tenaga medis harus siap untuk bertindak dalam kekuasaan mereka. Perawat adalah profesional medis yang bertanggung jawab untuk perawatan pasien darurat. Sebuah survei tahun 2019 di lapangan menemukan bahwa sebagian besar perawat tidak memahami keadaan darurat kardiovaskular. Masalah terbesar yang dihadapi PPNI setelah dikoordinasikan sebagai ketua tim PPNI RS UNS adalah pengetahuannya tentang pembacaan EKG perawat terbatas, perawat tidak mengetahui kondisi kegawatan jantung dan pelatihan pertolongan pertama kegawatan jantung jarang ditemui, dan tidak ada pedoman khusus kegawatan kardiovaskuler pada perawat. Tujuan pengabdian ini adalah meningkatkan pengetahuan kegawatan kardiovaskuler pada perawat PPNI RS UNSMetode: Kegiatan pengabdian masyarakat yang dilakukan ini meliputi pelatihan secara daring pada hari Kamis 16 April 202 di RS UNS, selain itu juga pembuatan buku Panduan membaca EKG bagi perawat dan leaflet cara membaca EKG bagi perawat.Hasil: Terjadi peningkatan pengetahuan perawat terhadap EKG dengan peningkatan nilai pretes 55,5 menjadi 72,5 dengan skala 100.Kesimpulan: Pelatihan kegawatan kardiovaskuler pada perawat meningkatkan pengetahuan perawat mengenai kegawatan kardiovaskuler

Abstract

Pendahuluan: Prevalensi diabetes melitus (DM) secara global terus meningkat hingga menjadi 3 kali lipat pada tahun 2030. Volume kejadian yang tinggi tentu saja diikuti dengan beban biaya yang tinggi pula. Terapi DM tipe 2 saat ini hanya mencegah progresifitas DM tipe 2 dan mencegah komplikasi yang lebih parah. Salah satu inti tatalaksana DM tipe adalah edukasi ke penderita DM tipe 2 untuk menjaga kadar gula darah, bagaimana diet yang baik, dan agar rutin selalu minum obat DM. Pengabdian yang dilakukan adalah dengan mengadakan penyuluhan dan edukasi Diabetes untuk meningkatkan pengetahuan penderita DM tipe 2, membuat buku Panduan minum obat bagi penderita DM tipe 2, pemberian alat pemeriksaan gula darah, dan pemberian jarum insulin pada penderita DM tipe 2.Metode: Langkah pengabdian yang akan dilakukan meliputi pengumpulan bahan dan materi, perencanaan, dan kegiatan inti.  Penyuluhan dan edukasi sebagai acara inti akan dilakukan pada tanggal 22 April 2022 dengan materi: mengenal DM tipe 2 dan gejalanya, mengenal obat-obatan DM tipe 2, olahraga dan aktifitas fisik pada DM tipe 2, komplikasi pada DM tipe 2.Hasil dan pembahasan: Evaluasi kegiatan menggunakan kuesioner kepuasan kegiatan. Semua responden menyatakan puas terhada isi materi penyuluhan.Kesimpulan: Pengabdian sudah terlaksana dengan baik, masyarakan penderita DM tipe 2 yang menjalani pelatihan mendapatkan penurunan gejala komplikasi DM tipe 2.

Abstract

Since the COVID-19 pandemic occurred at the end of January 2020, the health social security administering body (BPJS Kesehatan) as the payer has an important part in handling COVID-19, namely by carrying out the process of verifying claims for COVID-19 cases while the financing is carried out by Ministry of Health. However, COVID-19 benefits may be transferred to the National Health Insurance benefits during the endemic period. Therefore, this study aims to evaluate services and obtain policy recommendations for the transfer of COVID-19 benefits. Qualitative research through focus group discussions was conducted in September 2021 with policymakers, clinicians, COVID-19 survivors, and BPJS Kesehatan. The results of the qualitative study show that there are still many obstacles in the service process. Therefore, it is necessary to regulate the provisions of service regulations. As for the claim process, health facilities find it difficult to change the dynamic regulations during the pandemic. BPJS Kesehatan requires policy certainty to determine the end of the pandemic period. If the COVID-19 benefits are transferred to JKN after the pandemic, the source of financing may not only be done through the conversion of contributions but there is also an option for assistance funds from the government. In terms of tariffs, it is necessary to calculate costs according to the agreed benefit package through INA-CBG. Gradual and thorough preparations are needed for the implementation of the transfer of COVID-19 benefits in the benefits of JKN.

Abstract

The study aims to assess the difference in knowledge and acceptance levels towards nutritional technology between Generation X and Y nutritionists working in healthcare facilities in East Java Province, Indonesia. This study focused on the quantitative method which a cross-sectional survey applied a purposive sampling method to recruit nutritionists. The online questionnaire was prepared to assess knowledge and acceptance of nutritional technology and circulated to WhatsApp and Telegram groups. The researchers recruited 355 participants based on the inclusion criteria and completed the questionnaire. The data were analyzed using Chi-squared Test and Pareto Principle. This study showed that the knowledge levels among Generations X and Y were 76,8 percent and 83,2 percent, respectively, whereas the acceptance levels were both lower at 72,0 percent and 74,9 percent, respectively. Further analyses showed no different in both the knowledge level (????2 = 2.295, p-value = 0.167) and the acceptance level (????2 = 0.386, p-value = 0.617) of nutritional technology between the two generations. In addition, this study concluded that despite having good knowledge of nutritional technology, Generation Y nutritionists' acceptance of the technology was still less than expected. This information can be used as an evaluation material for Persagi to encourage their members to increase their digital literacy related to nutrition technology by the generation characteristics of the nutritional workforce

Abstract

Obesity is an excess weight that is characterized by the Body Mass Index (BMI) 27kg/m2. Obesity in health professionals can potentially endanger health professionals. Tomatoes are a source of lycopene which has a protective effect against obesity. This study aims to analyze the effect of giving tomato juice on the waist circumference and body fat of health officers with obesity. This experimental study employed the pre-test and post-test control group design. The research sample comprised 66 individuals, divided into 2 groups. The Control group was given education on a low-calorie diet while the treatment group was given education on a low-calorie diet and 500 ml of tomato juice. The data analysis was conducted using paired–t-tests. After 30 days of treatment, there was a decrease in body fat levels in the treatment group by 1.32+1.4 (p=0.000). Meanwhile, in the control group, there was no significant difference (p=0.543). The treatment group experienced a decrease in waist circumference by 5+0.44 (p=0.000) and the control group had a difference of 3.12+2.61 (p=0.000). Tomato juice is effective in reducing waist circumference and body fat in obesity. It is necessary to consider the use of tomato juice in programs aimed at reducing waist circumference and body fat.

Abstract

COVID-19 has been declared a global pandemic affecting many countries, including Indonesia. The worsening of COVID-19 patients may go unnoticed because one of the clinical manifestations of COVID-19 is silent hypoxia, where the patients’ blood oxygen saturation drops with no visible symptoms. Blood gas analysis is needed to detect silent hypoxia. This study aims to confirm whether blood gas analysis results correlate with the mortality of critical COVID-19 patients. This was a retrospective cohort study, the samples were taken from the medical records of critical COVID-19 patients from May 2020-July 2021. The variables observed were gender, age, comorbidities, oxygen saturation (SaO2), partial pressure of oxygen (PaO2), partial pressure of carbon dioxide (PaCO2), bicarbonate (HCO3-), and blood pH. Descriptive statistics, bivariate, and multivariate analyses were done. There were 210 samples, where 137 patients survived (65.24%) and 73 patients died (34.76%). The data analysis showed that comorbidities and blood pH significantly correlate with the mortality of critical COVID-19 patients, with p-values < 0.05; indicating that comorbidities and blood pH can be used as mortality predictors in critical COVID-19 patients. The RR for comorbidities and blood pH were 2.194 and 2>294, respectively, with CI 95% 1.202-4.833 for comorbidities and 1.151-5.295 for blood pH. The cut-off value used for blood pH was 7.310.

Abstract

COVID-19 is still a major problem and characterized by the development and progression of inflammatory responses. Assessment of disease degree includes several tools, such as the pneumonia severity index (PSI). However, because the pandemic occurs in all regions in Indonesia, not all health facilities have a complete diagnostic tool to assess the severity of pneumonia. It is necessary to have an easy, inexpensive, and readily available examination to determine the COVID-19 disease's severity, such as Neutrophil-to-Lymphocyte Ratio (NLR). The purpose of this study was to determine the correlation between NLR and PSI in COVID-19 patients. The research was carried out on the COVID-19 ward of Universitas Sebelas Maret (UNS) Hospital in February 2021. Examination of laboratory parameters and X-rays was carried out when the patients entered the Emergency Room at the hospital. A prospective cohort study, with the population being patients >18 years old with mild to critical COVID-19 and being admitted from February to March 2021. Statistical analysis was conducted using the Pearson correlation test with SPSS 25, in which case p was deemed significant if it was <0.05. Results were obtained from 49 COVID-19 patients treated in the COVID-19 ward, whose mean age was 49.83 + 13.31, NLR and PSI were 8.85 + 13.25, 61.673 + 26.635, respectively. There was a strong positive correlation between NLR and PSI (p = 0.001; r = 0.488). NLR was correlated with PSI in COVID-19 patients.

Abstract

Introduction: Psychological problems can lead to emotional eating and meal skipping, which may impact unhealthy food consumption patterns. This study aimed to assess the interrelationship between emotional eating, meal skipping, and unhealthy food consumption pattern in adolescent girls. Method: This cross-sectional study included 122 samples of adolescent girls aged 13-15 years. The study was carried out in Tangerang, in June 2021. The data of emotional eating were measured by Emotional Eater Questionnaire (EEQ), meal skipping collected by self-reported questionnaire of daily eating frequency, and unhealthy food consumption patterns measured by Food Frequency Questionnaires (FFQ). Result: The results showed majority of emotional eater subjects more often consumed fatty foods (76.6%). It’s also found that adolescents  skipping breakfast were high in consumed sweet  foods (66.0%), while those skipping lunch (63.6%) and dinner (54.5%) were found higher to consume  fatty foods than others. In addition, multivariate regression analyses showed that simultaneously between the variables, only emotional eating variables had a significant  relationship with unhealthy food consumption patterns (p-value = 0.002), while meal skipping variables were not significantly related. Conclusion: It can be concluded that emotional eating can affect unhealthy food consumption patterns among adolescent girls, while meal skipping did not show  any meaningful effect.

Abstract

Pendahuluan: Pasien dengan komorbid diabetes melitus (DM) berisiko tinggi mengalami COVID-19 derajat berat bahkan kematian. Hiperglikemia pada pasien DM menyebabkan perubahan aktivitas dan konsentrasi faktor fibrinolitik dan prokoagulan sehingga terjadi peningkatan risiko trombosis. Pemeriksaan yang dapat digunakan sebagai marker hiperkoagulasi yaitu D-Dimer. Penelitian ini bertujuan untuk mengetahui hubungan glukosa darah admisi dan HbA1c dengan kadar D-Dimer pada pasien COVID-19 dengan DM tipe 2 karena belum banyak penelitian mengenai hal ini. Metode: Desain penelitian yang digunakan yaitu observasional analitik dengan pendekatan cross-sectional. Penelitian menggunakan teknik purposive sampling dari data rekam medis pasien COVID-19 dengan DM tipe 2 di RS UNS pada rentang bulan Agustus 2020 hingga Agustus 2021 yang dirawat inap, dilakukan pemeriksaan kadar D-Dimer admisi, post prandial glucose, dan HbA1c dengan rentang usia ≥ 18 tahun dan < 60 tahun. Data dianalisis dengan Uji Korelasi Sprearman.   Hasil: pasien memenuhi kriteria penelitian. Hasil uji korelasi menunjukkan nilai probabilitas (p) glukosa darah admisi dan HbA1c dengan kadar D-Dimer masing-masing 0,253 dan 0,236 (p > 0,05) dengan koefisien korelasi (r) glukosa darah admisi dan HbA1c secara berurutan 0,098 dan 0,106. Kesimpulan: Penelitian menunjukkan tidak terdapat hubungan antara glukosa darah admisi dan HbA1c dengan kadar D-Dimer pada pasien Covid-19 dengan DM tipe 2.
