Abstract

Background: Sleep disturbance accelerates type 2 diabetes mellitus by reducing insulin secretion and by the occurrence of difficulty in controlling sleep behaviour in humans so that it is necessary conduct a study by using experimental animals.Objective: This study aims to determine histophotometric differences of pancreas and differences of glucose tolerance in male albino rats (Rattus norvegicus) after induced by stress paradoxical sleep deprivation (PSD) and total sleep deprivation (TSD). Methods: This study applied a post-test only with control groups consisting of 30 male albino rats (Wistar strain) divided randomly into 3 groups: control group, PSD group and TSD group. The mean number and diameter of islets of Langerhans were calculated per slide and were averaged for each group at a magnification of 400 X. The percentage of beta cells in one islet of Langerhans was (ß-p) that was a number of normal beta cells (Bn) divided for the total beta cells (Bt) (normal and damaged) x 100%. The glucose level based on the sampling time was made a curve shape, and the area under the 0-to-120-minute curves (AUC0-120) was calculated by using a trapezoidal formula.Results: The lowest AUC0-120 level of glucose was the control group (14,082 ± 955mg/dL) and the highest was the TSD group (16,293 ± 2,195 mg/dL); the Post-Hoc LSD test showed a significant difference (p<0.05) between the control-PSD group and the TSD-control group. The mean result of pancreatic ß cells mass with the lowest number was in the TSD group (76.3 ± 4.8), and the highest result was in the control group (94.3 ± 2.7); Post-Hoc LSD showed a significant difference (p <0.05) in the control-PSD group and the TSD-control group. The lowest mean in cell diameter of islets of Langerhans was in the TSD group (0.132 ± 0.031 mm), and the highest mean was in the control group (0.213 ± 0.019 mm). Conclusion: Sleep deprivation could reduce pancreatic ß cell mass and cell diameter of islets of Langerhans and increase glucose tolerance levels.

Abstract

Kawasaki disease (KD) is an acute systemic vasculitis frequently affecting children under five years old. KD can cause severe complications. It can lead to coronary aneurysms in 15-25% of untreated cases. Intravenous immunoglobulin (IVIG) treatment within ten days of initial onset can reduce the rate of coronary artery aneurysms. However, IVIG administration in Indonesia is currently limited due to its high cost. The case reported a 4-year-old girl with a chief complaint of fever. She complained about a high fever seven days before hospitalization. She also complained about vomiting, cough, joint pain, diarrhea, skin peeling, and rash on her body, palms, and soles. On examination, she looked irritable and sluggish. The temperature was 39.9o C. There were red eyes without discharge, strawberry tongue, oral thrush, and red, dry, cracked lips, swollen neck lymph nodes, and skin rash on her body, palms, and soles. Laboratory testing showed microcytic hypochromic anemia with Hb 7.9 g/dL and leukocytosis 24.230/mm3. Chest X-ray showed perihilar and paracardial infiltrates. Electrocardiogram revealed sinus tachycardia. An echocardiogram showed left ventricle dilatation with trivial mitral regurgitation and no coronary abnormalities were found. She was diagnosed with Kawasaki Disease. She was treated by IVIG 30 gr single dose during 12 hours on day 7 of initial onset, methylprednisolone injection 10 mg/8 hours, paracetamol 150 mg/8 hours, and aspirin 400 mg/8 hours orally and discharged from the hospital with improvement. There weren’t any coronary artery abnormalities found. IVIG administration within ten days of initial onset in KD patients can reduce the risk of coronary artery complications. IVIG administration after day 10 of initial onset can achieve resolution of inflammation but can be insufficient for preventing coronary artery lesions (CALs).

Abstract

The association between the risk factors for male infertility including smoking, obesity, male age, and leukocyte count with sperm analysis, still shows mixed results. This study aims to determine the association between smoking, obesity, male age, and leukocytes count with sperm quality (sperm concentration, sperm motility, and sperm morphology) of infertility patients in Purwokerto. This study is an observational study with a cross-sectional design conducted in the medical records section of the Bunda Arif Hospital Purwokerto. The sample was taken by total sampling. The bivariate test of smoking and obesity behaviour variables with the results of sperm quality using the Chi-Square test and Fisher's exact test. Male age variables used the Kruskal-Wallis test and the Spearman correlation test. Variable leukocytes count using the Spearman test. The results showed no association between male age, obesity and smoking behaviour with sperm quality, sperm concentration, sperm motility, and sperm morphology (p>0.05). There is a significant association (p<0,05) between leukocyte semen count and spermatozoa concentration, sperm motility, and morphology of spermatozoa. It is concluded that there was a significant association between leukocyte count and sperm quality in infertility patients in Purwokerto. Research needs to be continued by examining the relationship between leukocytopenia and sperm DNA damage by looking at sperm DNA fragmentation.

Abstract

Background: Exposure to formaldehyde from a preserved cadaver can cause several health problems in students, lecturers, and laboratory technicians working in medical anatomy laboratories. These conditions include respiratory tract irritation and cancer due to acute and chronic exposure, respectively. Lowering the frequency of contact with formaldehyde from preserved organs helps to ensure the safety of students, lecturers, and laboratory technicians. Therefore, this study aims to determine the effectiveness of low concentration formalin as a preservative solution for goat kidneys. The organ was selected due to its anatomical structure similarity to humans.Method: Goat kidneys with 50-60 ml of volume were collected, washed, immersed in preservative solution, and stored for two months. The samples were then divided into four groups, which were preserved with different formalin concentrations, namely 30%, 20%, 10%, and 5%. The preservation parameters, namely organ structure and integrity, color, springiness, odor, and mold growth were qualitatively described, while the kidney volume was quantitatively measured.Results: After the samples were preserved for two months, they were observed, and all groups showed similar characteristics. The kidneys preserved with 5% or 10% formalin showed equal volume before and after two months of preservation.Conclusion: Low concentration formalin has the same effectiveness in preserving goat kidneys as the high concentration. It also has the potential to be applied in human anatomy learning.

Abstract

Paradoxical Sleep Deprivation (PSD) elevates glucocorticoid and Reactive Oxygen Species (ROS) levels that cause oxidative stress, trigger spermatogenic cells damage, and reduce the number of sertoli cells. Vitamin C and E are antioxidants that could prevent spermatogenesis damage by preventing free radical formation. The study aimed to determine the effect of single and combined doses of vitamin C and E in improving spermatogenesis of white male rats (Rattus norvegicus) model of PSD. This is experimental research with post-test only and control group design on 28 white male rats distributed into four groups, i.e. group I (control, PSD), group II (PSD+vitamin C), group III (PSD+vitamin E), and group IV (PSD+a combination of vitamin C and E). Testicular preparations were stained using Hematoxylin-Eosin staining, quantitative scores of spermatogenic cells were measured using the Johnsen method, and the number of sertoli cells was counted in 10 seminiferous tubules in each of the three testicular sections. Group I has the lowest mean of Johnsen score (5.27±0.28), and group IV has the highest mean score (8.95±0.62), while the Mann-Whitney test showed a significant difference (p<0.05) between group II, III and IV compared to group I (control group). Mean sertoli cells number of group I is the lowest (10.66±1.04), and group IV has the highest sertoli cells number. The post-hoc LSD test showed a significant difference (p<0.05) between group IV and the other groups. Thus, the combination of vitamins C and E improves Johnsen score and sertoli cells number of male rats (Rattus norvegicus) experiencing PSD.

Abstract

Pasangan infertil semakin banyak ditemukan di Indonesia, 40% penyebab infertilitas terkait dengan faktor suami. Analisis sperma merupakan metode untuk mengetahui penyebab infertilitas. Metode masturbasi lebih direkomendasikan dibandingkan coitus interruptus. Kekurangan pada metode masturbasi adalah adanya beban psikologis dan latar belakang agama. Tujuan penelitian adalah membandingkan hasil analisis sperma dari hasil coitus interruptus dan masturbasi pada kasus infertilitas. Jenis penelitian adalah observasional analitik. Sampel penelitian berjumlah 97 laki-laki infertil yang melakukan pemeriksaan di RSIA Bunda Arif pada bulan Januari 2017 – Juni 2018. Teknik sampling menggunakan total sampling. Analisis sperma menggunakan klasifikasi dari WHO tahun 2010. Analisis statistik yang digunakan adalah uji t tidak berpasangan. Hasil penelitian menunjukkan perbedaan bermakna konsentrasi sperma (p=0,043) pada kelompok masturbasi 46,5 ± 7,2 dan kelompok coitus interruptus 18,7 ± 5,3. Hasil yang tidak bermakna pada motilitas (p=0,632) dan morfologi (p=0,722). Kesimpulan penelitian adalah analisis sperma dengan proses masturbasi lebih baik konsentrasinya dibandingkan dengan proses coitus interruptus pada kasus infertilitas. Infertile couples are increasingly found in Indonesia and the 40% of many causes are related to husband factors consequently sperm analysis is needed to predict infertility. A method of masturbation is more recommended than coitus interruptus. The deficiency of masturbation method lies on psychological and religious burden. This research aims at comparing the sperm analysis resulted from coitus interuptus and masturbation in infertility cases. It belongs to observational analityc reserach. The subjects of the research were 97 infertile male patients who underwent examination in RSIA Bunda Arif in January 2017-June 2018 and total sampling method was applied. The sperm analysis utilized WHO classification in 2010 and independent t-test was used in statistics analysis. The results of this study showed the sperm comparison and research results (p = 0.043) in masturbation groups is 46.5 ± 7.2 and the coitus interruptus group is 18.7 ± 5.3 and the result is not suitable for motility (p = 0.632) and morphology (p = 0.722) in spite of the fact that it showed better in the masturbation group. The study concludes that the sperm concentration in sperm analysis in the masturbation process is better and more recommended than that of coitus interruptus.

Abstract

Background: One indication of microvascular dysfunction in diabetes mellitus (DM) is decreased nitric oxide (NO) levels. Tithonia diversifolia leaves (TDL) extract has been scientifically demonstrated to decrease glucose levels in diabetic rats.Objective: This study aims to examine the effect of TDL extract on NO levels in diabetic rats.Methods: True experimental with pre-post test control group design used 25 Sprague-Dawley rats. Its were divided into 5 groups: healthy control (K.1); diabetic control (K.2); diabetes rats with ethanol extract of TDL at a dose of 25 (K.3); 50 (K.4); and 100 mg/kg BW (K.5) for 28 days. We use a combination of nicotinamide (230 mg/kg BW) and streptozotocin (65 mg/kg BW) to induce diabetes mellitus. The blood serum was taken before and after extract administration. NO level was assessed using spectrophotometry. Paired T-test, Wilcoxon, one-way ANOVA, and post hoc LSD were performed for statistical analysis.Results: There were significant differences in NO levels before and after treatment in all groups unless K.5 (100 mg/kg BW). Furthermore, there was no significant difference in NO levels in the healthy control and the K.5 group but significant differences in other groups.Conclusion: TDL extract can prevent the decrease in NO level in diabetic rats model with the effective dose of 100 mg/kg BW.

Abstract

Abstract: Sleep deprivation-induced stress decreases the quality of spermatozoa due to the increase of glucocorticoid levels through the hypothalamic-pituitary-adrenal axis. Sleep recovery (SR) restores the effect of stress due to sleep deprivation by decreasing lipid peroxides and other free radicals and increasing glutathione antioxidants. This study aims to determine the spermatozoa quality in male albino rats (Rattus norvegicus) after the stress induction by diverse sleep deprivation protocols. This study is experimental research with posttest-only control group design in 30 male albino rats divided into five groups consisting of group I for negative control (no induction of sleep deprivation), group II for paradoxical sleep deprivation (PSD), group III for total sleep deprivation (TSD), group IV for PSD followed by SR, and group V for TSD followed by SR. PSD and TSD were performed by depriving rats sleep for 20 and 24 hours/day, respectively, for five days. SR was done for the next five days after PSD or TSD. Shapiro-Wilk test was used for normality of data distribution, and Levene's test was used for variance homogeneity. Comparative hypothesis was then analyzed with One Way Analysis of Variance (ANOVA) test, and significance between groups was analyzed with Tukey's Post-Hoc test. Group III (TSD) demonstrated the lowest mean of spermatozoa count (41,53±2,29), spermatozoa morphology (13,00±2,91), spermatozoa viability (31,00±7,39), and percentage of motility rate (37±8,58%) among all groups. In conclusion, total sleep deprivation (TSD) decreases the quality of spermatozoa in male albino rats (Rattus norvegicus).

Abstract

Posyandu merupakan wadah pemberdayaan masyarakat untuk mentransfer informasi dan keterampilan dari petugas kesehatan kepada masyarakat setempat dan antar sesama masyarakat. Kegiatan posyandu digerakkan oleh kader posyandu, warga terpilih yang rela mencurahkan tenaga & waktunya, serta paling memahami kondisi masyarakat setempat. Keberadaan kader sangat strategis sebagai ujung tombak dan garda terdepan pelayanan kesehatan masyarakat. Mengingat perann pentingnya, kader posyandu perlu memiliki pengetahuan dan keterampilan untuk mendukung perannya. Kader posyandu harus berbenah diri dan beradaptasi dengan perubahan agar tetap diminati oleh masyarakat. Kapasitas kader perlu ditingkatkan dan kualitas pelayanan perlu ditingkatkan. Kader perlu berinovasi dan berkreasi dalam penyelenggaraan posyandu agar tidak terjebak pada rutinitas yang menyebabkan masyarakat bosan datang ke posyandu karena kegiatannya hanya “menimbang berat badan dan tinggi badan”. Berbagai permasalahan di Posyandu yang sering muncul antara lain aspek kualitas dan keterampilan kader dan sarana prasarana posyandu. Kegiatan ini dilakukan dengan memberikan transfer pengetahuan dan pelatihan. Materi yang diberikan meliputi “Posyandu, peran, dan tugas kader”; “Buku KMS dan KIA”; “Sukses Menyusui Eksklusif”; dan “Masalah Diare pada Anak”. Pelatihan dilakukan dengan memberikan skenario kasus kemudian kader melakukan role play memberikan edukasi tentang kasus terkait. Kegiatan ini diharapkan dapat menjadi langkah untuk meningkatkan pengetahuan dan keterampilan kader yang mendukung perannya di Posyandu. Posyandu is a forum for community empowerment to transfer information and skills from health workers to the local community and between fellow communities. Posyandu activities are driven by posyandu cadres, selected citizens who are willing to put in their energy & time, and best understand the conditions of the local community. The existence of cadres is very strategic as the spearhead and the frontline of public health services. Given their important role, posyandu cadres need to have knowledge and skills to support their role. Posyandu cadres must improve themselves and adapt to changes to remain in demand by the community. Cadre capacity needs to be upgraded and service quality needs to be improved. Cadres need to innovate and be creative in organizing posyandu, so they don't get stuck in routines that cause people to get bored of coming to the Posyandu because their activities are only "weighing weight and height". Various problems in Posyandu that often arise include aspects of the quality and skills of cadres and posyandu infrastructure. This activity is carried out by providing knowledge transfer and training. The material provided includes “Posyandu, roles, and duties of cadres”; “KMS and KIA Books”; “Success for Exclusive Breastfeeding”; and “Diarrhea Problems in Children”. The training was carried out by providing case scenarios and then cadres did role-play providing education about related cases. This activity is expected to be a step to improve the knowledge and skills of cadres who support their roles in Posyandu.

Abstract

The results of Riskesdas (Baseline Health Research) by the Indonesian Ministry of Health in 2018 revealed that the caries rate in rural areas was higher than in urban areas. However, the number of villagers receiving dental and oral health care was lower than in urban communities. Karangtengah Village was located in the Baturraden District, Banyumas Regency, which is socio-demographically rural. This study aims to determine the correlation between knowledge and affective of oral health on the caries rate in Karangtengah Village. The study was observational analytical research. Subjects were selected using a purposive sampling technique. The total subjects were 388 residents. The knowledge and affective data were collected using a questionnaire, and the caries rate was scored using DMF-T. Data were analyzed using the Pearson correlation test (SPSS 22). The results revealed that the oral health knowledge and affective of the majority of Karangtengah Village residents were in the moderate category (48.7% and 66.8%, respectively), while the average DMF-T score was 13.12 (very high). The statistical analysis demonstrated no significant correlation between knowledge and DMF-T score and between the affective aspect of oral health and DMF-T score (p0.5).  The moderate category of oral health knowledge and affective aspect in rural areas may not have been applied as oral health behaviors contribute to high dental caries rates.
