Abstract

Aims: This study aims to qualitatively explore the positive experiences of caregivers for people with dementia (PwD. Methods: A descriptive phenomenological design was employed with purposive sampling. This study was conducted in Indonesia by the Alzheimer’s Indonesia organization. Participants were selected using purposive sampling methods. The study involved 11 caregivers, including paid caregivers and family members, including children, grandchildren, spouses, siblings, and other close relatives, who had provided care for at least one year. Data were collected through in-depth interviews conducted face-to-face or via Zoom in 2022. Colaizzi’s method guided the data analysis to extract significant statements, formulate their meanings, and identify emerging themes. Rigor was ensured through data triangulation, peer checking, and participant validation. Results: The study identified four main themes reflecting the positive experiences of caregivers of persons with dementia: (1) Personal Growth and Development, including increased patience, empathy, and caregiving competence; (2) improved relationships and connections, characterized by stronger emotional bonds and supportive family dynamics; (3) Search for Meaning and Role Satisfaction, where caregiving was seen as a purposeful, spiritually meaningful role; and (4) Positive Psychological Outcomes, such as reduced stress, greater emotional resilience, and enhanced well-being. Conclusion: Despite the challenges, caregiving for individuals with dementia can lead to personal growth, stronger relationships, a sense of meaning, and emotional resilience. These positive experiences highlight the importance of supporting caregivers and recognizing their strengths.

Abstract

Background: Insomnia is one of the most common sleep problems in older adults (elderly). Insomnia in the elderly is caused by several factors, namely health status factors, drug use, environmental conditions, psychological stress, diet/nutrition. Purpose: overcoming insomnia in the elderly by combining two theories, namely Comfort and Environmental. Methods: The type of research used in this study is qualitative with a phenomenological approach. the number of participants who participated in this study were fifteen people. Collecting data using Focus Group Discuss (FGD), Observation, and in-depth interviews, the results of data collection in this study using analysis techniques Triangulation method (Focus Group Discuss, in-depth interviews, and observation). Results: This study produced five main themes, namely: 1) physical, 2) psychology, 3) religion, 4) social, 5) environment. Conclusion: In achieving the quality and quantity of sleep for the elderly who experience insomnia, it is expected that the elderly can fulfill their comfort and cleanliness in their surrounding environment.

Abstract

Background: Hypertension management in the elderly is crucial to prevent complications. The role of the family in healthcare is crucial for the successful management of hypertension in the elderly. Purpose: To develop a family role model based on family-centered nursing for the management of hypertension in the elderly. Method: This exploratory study used a cross-sectional approach. This phase identified cultural and social structural factors, family factors, individual factors, and the family's role in the management of hypertension in the elderly. The study was conducted in Kudus Regency, Central Java Province, from December 2023 to March 2024. The study population was primary caregivers of elderly people who were active smokers and diagnosed with hypertension. The sampling technique used was multistage cluster sampling, with a sample size of 125 respondents. Data were collected using a structured questionnaire that had undergone validity and reliability testing. The data were then analyzed descriptively and using SEM-PLS. Results: The role of the family significantly influenced the management of hypertension in the elderly (T-statistic = 8.786). Conclusion: The more optimal the family's role as educator, motivator, and facilitator, the better the management of hypertension in the elderly.   Keywords: Elderly; Family-Centered Nursing; Family Role; Hypertension Management; Model Development.   Pendahuluan: Manajemen hipertensi pada lansia merupakan hal yang penting dalam pencegahan komplikasi hipertensi. Salah satu penentu keberhasilan manajemen hipertensi pada lansia yang ditentukan oleh peran keluarga dalam perawatan kesehatan. Tujuan: Untuk mengembangkan model peran keluarga berbasis keperawatan family centered terhadap manajemen hipertensi pada lansia. Metode: Penelitian eksplanatori dengan pendekatan cross-sectional. Pada tahap ini dilakukan identifikasi terhadap faktor cultural and social structure, faktor keluarga, faktor individu, dan peran keluarga terhadap manajemen hipertensi pada lansia. Penelitian dilaksanakan di Kabupaten Kudus, Provinsi Jawa Tengah, pada bulan Desember 2023 hingga Maret 2024. Populasi penelitian adalah caregiver primer dari lansia yang merupakan perokok aktif dan terdiagnosis hipertensi. Teknik pengambilan sampel yang digunakan adalah multistage cluster sampling dengan jumlah sampel sebanyak 125 responden. Data dikumpulkan menggunakan kuesioner terstruktur yang telah melalui uji validitas dan reliabilitas. Data selanjutnya dianalisis secara deskriptif dan menggunakan SEM-PLS. Hasil: Peran keluarga berpengaruh signifikan terhadap manajemen hipertensi pada lansia (nilai T-statistik = 8.786). Simpulan: Semakin optimalnya peran keluarga sebagai edukator, motivator, dan fasilitator, maka semakin baik pengelolaan hipertensi pada lansia.   Kata Kunci: Family Centered Nursing; Lansia; Manajemen Hipertensi; Pengembangan Model; Peran Keluarga.

Abstract

Background: Type 2 Diabetes Mellitus (T2DM) presents a chronic health challenge that extends beyond the patients, significantly affecting family members who assume caregiving responsibilities. Family caregivers often experience emotional, physical, and psychological burdens, particularly in low-resource settings. However, the interplay between family communication, decision-making, caregiver burden, and self-efficacy, and how these factors jointly influence caregiver quality of life, remains insufficiently explored, particularly in culturally specific and resource-limited settings.Purpose: This study aimed to examine the structural relationships among family communication, shared decision-making, caregiver burden, self-efficacy, and their influence on the quality of life of family caregivers of individuals with T2DM.Methods: A cross-sectional study was conducted involving 327 family caregivers recruited from 16 primary health centers using cluster random sampling. Data were collected using validated instruments to measure family communication, decision-making, caregiver burden, self-efficacy, and quality of life. Structural Equation Modeling (SEM) was used to analyze both direct and indirect relationships among the variables.Results: The findings revealed that family communication (β = 0.35) and decision-making (β = 0.42) had significant positive effects on self-efficacy, whereas caregiver burden (β = -0.33) had a significant negative effect. Self-efficacy, in turn, significantly improved caregivers’ quality of life (β = 0.51). Furthermore, self-efficacy mediated the indirect effects of family communication, decision-making, and caregiver burden on quality of life. The model explained 49% of the variance in self-efficacy and 64% of the variance in caregiver quality of life.Conclusion: Self-efficacy plays a central role in enhancing caregiver well-being and quality of life. Interventions that promote open family communication, shared decision-making, and burden reduction are essential. Nursing implications include the need for culturally sensitive, family-centered nursing interventions to enhance caregiver self-efficacy through communication training, psychosocial support, and empowerment in decision-making.
