Abstract

Background: Several conditions in children can cause nutritional intake via the oral route to be inadequate. Providing enteral nutrition via a nasogastric tube (NGT) is an alternative option. Research on the use of NGTs in Indonesia is still limited, so this research aims to determine the effect of NGT use on the weight of pediatric patients with feeding difficulties at Dr. M. Djamil General Hospital, Padang. Methods: Experimental research with a pre and post-test-only design was conducted on 96 children aged 1 month - 18 years in the children's inpatient room at Dr. M. Djamil General Hospital, Padang. Body weight measurements were carried out before and after using NGT for 4 weeks (January 2022 – January 2023) at the pediatric nutrition and metabolic diseases polyclinic. Data were analyzed using paired t-test and ANOVA. Results: Most subjects (40.9%) were in the 1-12 month age group. The most common type of nutrition was ONS (oral nutritional supplements) (84.4%), the most common diagnosis was pulmonology (33.3%), and the majority were without complications (72.9%). There is an influence on the child's weight before and after using NGT (p-value <0.01). Significant differences were also found in changes in children's weight with diagnosis and complications (p<0.01). Conclusion: The use of NGT can increase the weight of pediatric patients who have feeding difficulties at Dr. M. Djamil General Hospital, Padang. Significant differences were found in changes in children's weight by diagnosis and complications.

Abstract

Background: Hypertrophic pyloric stenosis (HPS) is a common cause of gastric outlet obstruction in infants, but its management can be challenging in the presence of comorbidities like malnutrition and pulmonary tuberculosis (PTB). This case report highlights the complexities and considerations in managing an infant with HPS, PTB, and severe malnutrition. Case presentation: A 6-month-old male infant presented with lethargy, recurrent vomiting, and failure to thrive. He had a history of PTB on anti-tuberculosis treatment and was severely malnourished. Investigations revealed HPS, and he underwent a pyloromyotomy. Postoperatively, he required careful fluid management, nutritional support, and continued anti-tuberculosis therapy. Conclusion: This case highlights the challenges in managing HPS in an infant with PTB and severe malnutrition. It underscores the importance of a multidisciplinary approach involving pediatricians, surgeons, and nutritionists to ensure optimal outcomes in such complex cases.

Abstract

Background: Infective endocarditis (IE) and recurrent acute rheumatic fever (ARF) are two serious cardiovascular conditions frequently associated with rheumatic heart disease (RHD). Their coexistence complicates diagnosis and management due to overlapping clinical features such as fever, migratory arthritis, and valvular dysfunction. This case report aims to elucidate the clinical presentation, diagnostic challenges, and treatment strategies in a pediatric patient with coexisting IE and recurrent ARF.   Methods: A detailed clinical case study was conducted involving a 10-year-old boy with a history of RHD presenting with joint pain and intermittent fever. Diagnostic evaluations included physical examination, laboratory investigations (including blood cultures and antistreptolysin O titers), and serial transthoracic echocardiography. Therapeutic interventions combined targeted intravenous antibiotics, corticosteroids, and secondary prophylaxis with benzathine penicillin G. Multidisciplinary consultations were employed to optimize management. Results: The patient exhibited echocardiographic evidence of mitral valve vegetations along with severe mitral regurgitation. Blood cultures remained negative, likely due to prior antibiotic exposure. Elevated antistreptolysin O titers confirmed recent streptococcal infection supporting recurrent ARF diagnosis. Clinical improvement was observed with symptom resolution and reduction in vegetation size on follow-up echocardiography. Multimodal therapy was well-tolerated, preventing further complications. Conclusion: This case highlights the diagnostic complexity and therapeutic balancing act required in managing coexisting IE and recurrent ARF in children with RHD. Early recognition through comprehensive evaluation and integrated treatment combining antimicrobial and immunomodulatory approaches can improve outcomes. Continued vigilance and multidisciplinary care are essential for preventing morbidity in this high-risk population.

Abstract

Pandemi Covid 19 berdampak pada penurunan penghasilan pelaku usaha mikro sayur organik di kecamatan Gempol, Pasuruan hingga 50%. Penurunan itu terjadi akibat konsumen yang beralih pada sayur konvensional, dan tidak adanya inovasi pemasaran dari produsen. Tujuan dilakukannya pengabdian ini adalah untuk mengenalkan inovasi technopreneurship pada pelaku usaha sayur organik terdampak pandemi Covid 19 berupa integrated marketing communication. Metode yang digunakan adalah partisipasi aktif dari mitra, masyarakat sekitar, dan pemuda desa. Mitra sebagai pelaku usaha bersinergi dengan masyarakat sekitar yang memiliki usaha pembibitan lele dan budidaya ikan cupang untuk membuat rumah produksi bersama, yang di dalamnya terdapat proses packaging dan digital marketing dengan bantuan pemuda desa. Tim pengabdi mengambil peran sebagai inisiator dan konseptor yang merancang bentuk rumah produksi, bentuk pengemasan, dan strategi pemasaran memanfaatkan media online instagram dan youtube serta pemasaran offline melalui wisata eko edukasi. Hasil yang didapat dari pengabdian ini adalah terciptanya rumah produksi, green packaging dan media pemasaran yang saling terintegrasi. Dalam rumah produksi terdapat mini green house yang terhubung dengan akuaponik lele dan pameran ikan cupang sebagai rintisan wisata eko edukasi. Penggabungan usaha warga ini terbukti dapat menarik simpati anak-anak dan pemuda untuk berkunjung ke lokasi mitra untuk membeli sayur organik. Hal ini tentu dapat menjadi konten pemasaran di akun youtube dan instagram mitra, yang selanjutnya dapat dimanfaatkan untuk menjual hasil panen secara pre order.

Abstract

Background: Patent ductus arteriosus (PDA) is a congenital heart defect resulting from the failure of the ductus arteriosus to close after birth. This condition leads to a left-to-right shunt between the aorta and the pulmonary artery, which may cause increased pulmonary blood flow, pulmonary hypertension, and heart failure if left untreated. Physiological closure of the ductus arteriosus after birth depends on postnatal circulatory changes and the regulation of molecular mediators, particularly prostaglandin E2 and nitric oxide, with additional contributions from genetic factors. Objective: To describe the role of molecular mechanisms and genetic factors in the pathophysiology of PDA and to highlight their implications for diagnostic and therapeutic approaches. Methods: This article is based on a narrative review of the scientific literature addressing the pathophysiology of PDA, with particular emphasis on the roles of prostaglandins, nitric oxide, and genetic factors such as the TBX1 gene and the Notch signaling pathway. Results: The literature indicates that persistent prostaglandin E2 levels and ongoing nitric oxide activity contribute to the maintenance of ductal patency after birth, especially in preterm neonates. Moreover, dysregulation of the TBX1 gene and alterations in the Notch signaling pathway are associated with impaired development and reduced contractility of ductus arteriosus smooth muscle, thereby hindering normal physiological closure. These findings provide a mechanistic basis for the use of prostaglandin-inhibiting pharmacological therapy in the management of PDA. Conclusion: Patent ductus arteriosus is a multifactorial condition driven by complex interactions between molecular mediators and genetic determinants. A comprehensive understanding of these mechanisms is essential for optimizing PDA management through more precise, pathophysiology-based therapeutic strategies.
