Abstract

Background: Hypertrophic pyloric stenosis (HPS) is a common cause of gastric outlet obstruction in infants, but its management can be challenging in the presence of comorbidities like malnutrition and pulmonary tuberculosis (PTB). This case report highlights the complexities and considerations in managing an infant with HPS, PTB, and severe malnutrition. Case presentation: A 6-month-old male infant presented with lethargy, recurrent vomiting, and failure to thrive. He had a history of PTB on anti-tuberculosis treatment and was severely malnourished. Investigations revealed HPS, and he underwent a pyloromyotomy. Postoperatively, he required careful fluid management, nutritional support, and continued anti-tuberculosis therapy. Conclusion: This case highlights the challenges in managing HPS in an infant with PTB and severe malnutrition. It underscores the importance of a multidisciplinary approach involving pediatricians, surgeons, and nutritionists to ensure optimal outcomes in such complex cases.

Abstract

Background: Infective endocarditis (IE) and recurrent acute rheumatic fever (ARF) are two serious cardiovascular conditions frequently associated with rheumatic heart disease (RHD). Their coexistence complicates diagnosis and management due to overlapping clinical features such as fever, migratory arthritis, and valvular dysfunction. This case report aims to elucidate the clinical presentation, diagnostic challenges, and treatment strategies in a pediatric patient with coexisting IE and recurrent ARF.   Methods: A detailed clinical case study was conducted involving a 10-year-old boy with a history of RHD presenting with joint pain and intermittent fever. Diagnostic evaluations included physical examination, laboratory investigations (including blood cultures and antistreptolysin O titers), and serial transthoracic echocardiography. Therapeutic interventions combined targeted intravenous antibiotics, corticosteroids, and secondary prophylaxis with benzathine penicillin G. Multidisciplinary consultations were employed to optimize management. Results: The patient exhibited echocardiographic evidence of mitral valve vegetations along with severe mitral regurgitation. Blood cultures remained negative, likely due to prior antibiotic exposure. Elevated antistreptolysin O titers confirmed recent streptococcal infection supporting recurrent ARF diagnosis. Clinical improvement was observed with symptom resolution and reduction in vegetation size on follow-up echocardiography. Multimodal therapy was well-tolerated, preventing further complications. Conclusion: This case highlights the diagnostic complexity and therapeutic balancing act required in managing coexisting IE and recurrent ARF in children with RHD. Early recognition through comprehensive evaluation and integrated treatment combining antimicrobial and immunomodulatory approaches can improve outcomes. Continued vigilance and multidisciplinary care are essential for preventing morbidity in this high-risk population.

Abstract

Pandemi Covid 19 berdampak pada penurunan penghasilan pelaku usaha mikro sayur organik di kecamatan Gempol, Pasuruan hingga 50%. Penurunan itu terjadi akibat konsumen yang beralih pada sayur konvensional, dan tidak adanya inovasi pemasaran dari produsen. Tujuan dilakukannya pengabdian ini adalah untuk mengenalkan inovasi technopreneurship pada pelaku usaha sayur organik terdampak pandemi Covid 19 berupa integrated marketing communication. Metode yang digunakan adalah partisipasi aktif dari mitra, masyarakat sekitar, dan pemuda desa. Mitra sebagai pelaku usaha bersinergi dengan masyarakat sekitar yang memiliki usaha pembibitan lele dan budidaya ikan cupang untuk membuat rumah produksi bersama, yang di dalamnya terdapat proses packaging dan digital marketing dengan bantuan pemuda desa. Tim pengabdi mengambil peran sebagai inisiator dan konseptor yang merancang bentuk rumah produksi, bentuk pengemasan, dan strategi pemasaran memanfaatkan media online instagram dan youtube serta pemasaran offline melalui wisata eko edukasi. Hasil yang didapat dari pengabdian ini adalah terciptanya rumah produksi, green packaging dan media pemasaran yang saling terintegrasi. Dalam rumah produksi terdapat mini green house yang terhubung dengan akuaponik lele dan pameran ikan cupang sebagai rintisan wisata eko edukasi. Penggabungan usaha warga ini terbukti dapat menarik simpati anak-anak dan pemuda untuk berkunjung ke lokasi mitra untuk membeli sayur organik. Hal ini tentu dapat menjadi konten pemasaran di akun youtube dan instagram mitra, yang selanjutnya dapat dimanfaatkan untuk menjual hasil panen secara pre order.

Abstract

Background: Patent ductus arteriosus (PDA) is a congenital heart defect resulting from the failure of the ductus arteriosus to close after birth. This condition leads to a left-to-right shunt between the aorta and the pulmonary artery, which may cause increased pulmonary blood flow, pulmonary hypertension, and heart failure if left untreated. Physiological closure of the ductus arteriosus after birth depends on postnatal circulatory changes and the regulation of molecular mediators, particularly prostaglandin E2 and nitric oxide, with additional contributions from genetic factors. Objective: To describe the role of molecular mechanisms and genetic factors in the pathophysiology of PDA and to highlight their implications for diagnostic and therapeutic approaches. Methods: This article is based on a narrative review of the scientific literature addressing the pathophysiology of PDA, with particular emphasis on the roles of prostaglandins, nitric oxide, and genetic factors such as the TBX1 gene and the Notch signaling pathway. Results: The literature indicates that persistent prostaglandin E2 levels and ongoing nitric oxide activity contribute to the maintenance of ductal patency after birth, especially in preterm neonates. Moreover, dysregulation of the TBX1 gene and alterations in the Notch signaling pathway are associated with impaired development and reduced contractility of ductus arteriosus smooth muscle, thereby hindering normal physiological closure. These findings provide a mechanistic basis for the use of prostaglandin-inhibiting pharmacological therapy in the management of PDA. Conclusion: Patent ductus arteriosus is a multifactorial condition driven by complex interactions between molecular mediators and genetic determinants. A comprehensive understanding of these mechanisms is essential for optimizing PDA management through more precise, pathophysiology-based therapeutic strategies.

Abstract

Keselamatan instalasi listrik rumah tangga merupakan aspek penting dalam mencegah terjadinya kecelakaan listrik dan kebakaran, terutama di kawasan permukiman lama. Wilayah Kayutangan Heritage Kota Malang memiliki karakteristik bangunan berusia tua dengan instalasi listrik yang sebagian besar tidak lagi sesuai dengan standar keselamatan. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk meningkatkan kesadaran dan pengetahuan masyarakat terhadap pentingnya keselamatan instalasi listrik rumah tangga. Metode pelaksanaan meliputi sosialisasi, penyuluhan, diskusi interaktif, dan observasi kondisi instalasi listrik warga. Hasil kegiatan menunjukkan adanya peningkatan pemahaman masyarakat mengenai potensi bahaya listrik, standar instalasi listrik rumah tangga, serta perilaku penggunaan listrik yang aman. Kegiatan ini diharapkan dapat menurunkan risiko kecelakaan listrik dan mendukung terciptanya lingkungan permukiman yang aman dan berkelanjutan di kawasan Kayutangan Heritage.

Abstract

Background: Acute Rheumatic Fever (ARF) was an immune-mediated complication of Group A Streptococcus (GAS) infection that could progress to Rheumatic Heart Disease (RHD) through repeated or untreated episodes. While RHD typically developed over years, yet some children in endemic settings can develop severe multivalvular disease rapidly, likely due to unrecognized/subclinical ARF and inadequate secondary prophylaxis. This case adds to the literature by illustrating severe rheumatic mitral stenosis at a young age with clinical features suggestive of recurrent ARF despite no documented prior ARF, emphasizing rapid progression could occur in endemic settings. Case report: An 11-year-old male presented with exertional dyspnea and intermittent joint pain without swelling or redness. There was no previously documented ARF episode. Serology showed positive anti-streptolysin O (ASO) supporting recent streptococcal exposure. Echocardiography demonstrated severe mitral stenosis, moderate mitral regurgitation, moderate aortic regurgitation, moderate aortic stenosis, and severe tricuspid regurgitation with high probability of pulmonary hypertension. Diagnosis of recurrent ARF with severe RHD was established using the modified Jones criteria, supported by echocardiographic evidence of multivalvular involvement. Initial management was adjusted for penicillin allergy and included azithromycin, corticosteroids, beta-blockers, diuretics, and nutritional rehabilitation, followed by erythromycin for secondary prophylaxis. Conclusion: This case highlighted the possibility of rapid progression to severe RHD in children due to subclinical ARF. Early diagnosis, routine echocardiography, strict adherence to secondary prophylaxis, and patient education were vital to prevent long-term complications, including heart failure and surgical interventions.

Abstract

The “Prabowo Gemoy” campaign has become a prominent political communication innovation in Indonesia, particularly in targeting younger voters through humorous, expressive, and highly visual content. This study analyzes how two media outlets, Kumparan (as national media) and BBC News (as international media), frame the campaign using Robert M. Entman’s framing analysis model. A qualitative media text analysis method was employed to examine four key elements of framing: defining problems, diagnosing causes, making moral judgments, and suggesting remedies. The results indicate that Kumparan frames the campaign as a modern and emotionally resonant strategy to connect with Gen Z and millennial voters, depicting it as a breakthrough in digital political marketing. Conversely, BBC News frames the campaign critically, highlighting the risks of image rebranding and the potential whitewashing of Prabowo's controversial human rights record. These divergent frames reflect the cultural and institutional differences between the media outlets. This study concludes that media not only report political events but actively shape public perception and political discourse through strategic framing, which plays a crucial role in constructing the image of political figures.

Abstract

Background: Acyanotic congenital heart disease (CHD) causes increased metabolic demands and nutrient malabsorption, potentially triggering zinc deficiency. Urinary zinc measurement offers a non-invasive method to assess zinc status. This study evaluates the association between urinary zinc levels and nutritional status in children with acyanotic CHD. Methods: An analytical cross-sectional study involving 50 children aged 2–60 months with echocardiography-confirmed acyanotic CHD was conducted at the Pediatric Cardiology Outpatient Clinic, DR. M. Djamil General Hospital, Padang, from October 2024 to August 2025. Nutritional status was assessed by anthropometry using WHO growth standards (weight-for-height z-scores). Spot urinary zinc levels were measured using a colorimetric zinc assay kit (Elabscience, E-BC-K137-M) with absorbance read at 560 nm. One-way ANOVA and post-hoc tests were used for group comparisons. Results: Malnutrition prevalence reached 62% (undernutrition 32%, severe malnutrition 30%). Hypozincuria (urinary zinc <8 µmol/L) was found in 38% of subjects. A highly significant difference in urinary zinc levels was observed across nutritional status groups (p<0.001). Urinary zinc was highest in the well-nourished group (9.6±1.2 µmol/L), followed by undernutrition (8.4±1.8 µmol/L), and lowest in severe malnutrition (6.1±1.5 µmol/L). Post-hoc analysis confirmed significant differences between well-nourished and severely malnourished (p<0.001), and between undernutrition and severe malnutrition (p<0.001). Conclusion: Urinary zinc levels are significantly associated with nutritional status in children with acyanotic CHD, particularly in those with severe malnutrition. Urinary zinc assessment may serve as a practical screening tool to identify zinc deficiency in this population and guide targeted nutritional interventions.

Abstract

Abstrak Artikel ini menganalisis secara mendalam relevansi kejujuran dalam QS. At-Taubah sebagai karakter inti dalam pendidikan di era digital. Dilatarbelakangi oleh tantangan moral seperti disinformasi dan penyalahgunaan teknologi, penelitian kualitatif deskriptif-analitis ini menggunakan kajian pustaka, melibatkan tafsir klasik dan kontemporer, serta hadis. Temuan menunjukkan bahwa QS. At-Taubah: 119 memberikan panduan moral signifikan terkait kejujuran, yang esensial untuk membangun karakter siswa yang bertanggung jawab dan berintegritas di dunia nyata maupun digital. Implementasi praktis kejujuran ini dapat diintegrasikan melalui pendekatan pedagogis inovatif seperti pembelajaran berbasis proyek digital, simulasi studi kasus etika digital, dan integrasi tentang nilai kejujuran dalam kurikulum umum. Pendekatan ini didukung oleh berbagai studi yang menunjukkan bahwa integrasi nilai oleh pendidik efektif dalam mengatasi dekadensi moral, sehingga pelatihan guru secara reguler sangat penting untuk memfasilitasi integrasi nilai ini di seluruh mata pelajaran. Selain itu, pendekatan berbasis narasi dan konsep amanah, seperti kisah Ka’ab bin Malik, menjadi metode efektif dalam penanaman kejujuran. Kesimpulannya, pendidikan berbasis nilai Qur’ani, khususnya kejujuran, sangat krusial untuk membentuk generasi yang etis, bertanggung jawab, dan adaptif terhadap tantangan era digital, memastikan integritas moral di tengah kemajuan teknologi. Abstract This article thoroughly analyzes the relevance of honesty in QS. At-Taubah as a core character trait in digital age education. Driven by contemporary moral challenges such as disinformation and technology misuse, this descriptive-analytical qualitative study employs a comprehensive literature review, drawing upon classical and contemporary tafsir (Qur'anic exegesis), alongside hadith. Findings indicate that QS. At-Taubah: 119 provides significant moral guidance on honesty, essential for cultivating responsible and integrated student character in both physical and digital realms. Practical implementation of this honesty can be achieved through innovative pedagogical approaches, including digital project-based learning, ethical digital case study simulations, and the integration of integrity values into general curricula. This approach is substantiated by various studies demonstrating that educators' integration of values effectively counters moral decadence, underscoring the critical need for regular teacher training to facilitate this across all subjects. Furthermore, narrative-based approaches and the concept of ‘amanah’ (trust/responsibility), exemplified by the story of Ka’ab bin Malik, prove effective methods for instilling honesty. In conclusion, Qur’anic value-based education, particularly emphasizing honesty, is crucial for shaping a generation that is ethical, responsible, and adaptable to the demands of the digital age, thereby ensuring moral integrity amidst technological advancements.

Abstract

The development of digital learning technology requires effective innovation to support more concrete conceptual understanding, particularly in electricity topics at the elementary school level. Science (IPAS) learning on simple electrical circuits still requires tools that can clearly and systematically demonstrate experimental processes. This study aims to develop and determine the feasibility of a live-action instructional video entitled “Assembling an Electric Lamp Powered by a Battery” for fifth-grade elementary school students. The methodology employed was Research and Development (R&D) using the Analysis, Design, Development, Implementation, and Evaluation (ADDIE) model. Data were collected through observation, literature review, and validation questionnaires using a five-point Likert scale. The validation subjects consisted of three subject matter experts and three media experts. The data were analyzed by calculating the feasibility percentage based on the comparison between the obtained scores and the maximum possible scores. The results indicate that validation by subject-matter experts reached a feasibility percentage of 90%, categorized as highly feasible, while validation by media experts reached 95%, also categorized as highly feasible. The aspects of scientific accuracy, language use, and media integration received excellent evaluations, although there were suggestions for improving the clarity of alignment between the content and the learning objectives. Therefore, the developed live-action instructional video is considered feasible for use as a learning medium in elementary science (IPAS) on basic electricity and has the potential to enhance conceptual understanding through authentic and direct demonstrations.
