Abstract

Chemotherapy is a cancer therapy that can prevent the development and spread of malignant cells, but causes various side effects (loss of appetite, nausea, vomiting, mucositis, changes in taste, pain, fatigue, dry mouth, diarrhea, constipation. These side effects result in discomfort for children with cancer, in more severe conditions it can worsen the child's nutritional status due to inadequate nutritional intake. Nursing care using the Kolcaba comfort approach can be applied to overcome children's discomfort cancer who experience nutritional imbalance problems. The method used is a case study. The comfort needs in the five management cases are assessed based on the physical, psychospiritual, environmental and sociocultural contexts. The comfort interventions provided include standards of comfort, coaching, and comfort food for the soul . Nursing evaluation of the problem of nutritional imbalance which is a physical aspect of the patient, namely, the comfort type for the five children is ease comfort and three of the five children also have transcendence type comfort for other aspects. Kolcaba's Comfort Theory can be applied to pediatric cancer patients who experience nutritional disorders and provides opportunities for nurses to develop innovative interventions to improve the quality of nursing care.

Abstract

In addition to the unpleasant symptoms of the disease, invasive procedures, pain, and anxiety resulting from prolonged treatment and hospitalization n pediatic cancer. These can affect a child's physical, emotional and psychosocial functioning. Play is an important part of childhood and a basic need. Play therapy, through the playful nature of games, will create distraction from the pain, help children express emotions and adapt to the hospital environment. Objective: Therefore, it is necessary to identify non-pharmacological interventions to reduce anxiety in children undergoing chemotherapy.. Methodology: the literature Articles were searched using four databases: Pubmed, Scopus, Science Direct, and Sage journal which were published in 2018-2023. From 4 databases, 9317 articles were obtained and totaled final result are found 8 articles. They were reviewed and all were randomized controlled trial (RCT) studies which were assessed for risk of bias by using the Center for Evidence-Based Medicine (CEBM). Result: Distraction play interventions that can reduce anxiety levels in children undergoing chemotherapy include Art Therapy, Music Therapy, Virtual Reality, Muscle Relaxation, and Play Therapy. Conclusion: distraction play intervention can be applied in daily nursing practice to reduce anxiety in children undergoing chemotherapy treatment.

Abstract

Chronic fatigue syndrome is a debilitating illness of unknown cause that affects physical, psychological, emotional, and social aspects of life, manifesting in poor physical stamina, reduced social interaction, and depression. Self-management programs involve the deliberate practice of specific tasks, activities, and behaviors aimed at managing the medical, psychosocial, and daily life impacts of chronic illness. These programs include strategies such as symptom monitoring, adherence to treatment plans, and maintaining healthy practices. Aim to evaluate the effectiveness of self-management programs in reducing fatigue so as to improve quality of life. In a study utilizing the PICO search strategy related to self-management activities for reducing fatigue, 396 articles were found in the Cochrane, PubMed, Medline, Taylor & Francis, ProQuest, ScienceDirect, and Sage databases between 2020-2024; four articles meeting the inclusion criteria were analyzed in depth using quality assessment tools such as PRISMA. The study protocol was registered in the Prospective Register of Systematic Reviews (PROSPERO) with the following registration number: CRD42024622785. Based on the literature search was conducted without restrictions on the start year, including publications up to 2024.", 415 articles were identified, 19 duplicate articles were selected, until 5 eligible articles were obtained based on the selection of design, participant, intervention, and outcome This review concludes that self-management, such as lifestyle advice, web-based programs, and physical activity, is effective in reducing fatigue, and to achieve better quality of life and fatigue reduction outcomes, tailored to patient needs and conditions needed to be strengthened.

Abstract

Chemoport is an implanted device, placed in the central venous system especially for infusion of chemotherapy drugs in oncological diseases that can reduce vascular inflammation during chemotherapy because this device is implanted under the skin, making IV (intravenous) drug administration easier and reducing pain. Nurses as officers must be able to provide more specific care and therapy based on patient needs, and must understand the lives of cancer patients and their families. The purpose of this study was to determine the effectiveness of nursing interventions and nurse competence in the use of chemoport in cancer patients.  The research method used a systematic review with article searches on five trusted journal databases, such as EBSCOhost, ClinicalKey Nursing, Sage Journal and Science direct. Article analysis using the PRISMA method. The results showed that there were ten articles that fit the criteria for articles related to nursing interventions and nurse competence in the use of chemoport in cancer patients. All articles used quantitative research methods with a randomized control trial design. The conclusion is that nursing interventions and nurse competence in the use of chemoport in cancer patients, among others, are divided into nursing interventions to reduce pain, namely virtual reality interventions, giving Valsalva maneuvers, skin stimulation, and giving local anesthesia. While nursing interventions to reduce anxiety include virtual relity interventions, visual blocking information interventions and patient education and nurse competence through increasing nurse knowledge and providing training.

Abstract

Pediatric cancer patients are particularly vulnerable to oral health complications, primarily due to the adverse effects of chemotherapy. This treatment significantly influences both oral health and nutritional status, especially in children diagnosed with acute lymphoblastic leukemia (ALL). Chemotherapy can trigger a range of oral issues, including mucositis, dry mouth (xerostomia), and oral infections, which in turn can hinder proper nutritional intake and compromise overall health. This paper aims to investigate the impact of chemotherapy on oral health and nutrition in pediatric cancer patients, focusing on those with ALL. Through a literature review, the study examines the prevalence and types of oral complications and their effects on well-being. Relevant literature published between 2014 and 2024 was sourced from databases such as EBSCO, ScienceDirect, SpringerLink, ProQuest, and Scopus. Using the PICO (Population, Intervention, Comparison, and Outcomes) framework, articles were selected and critically appraised by two independent reviewers. A descriptive method was employed to analyze the data. Out of 2,465 initially identified articles, eight were selected for in-depth analysis. The findings show that pediatric cancer patients face both immediate and long-term oral health issues, including dental abnormalities and jaw development problems. These complications can cause pain, eating difficulties, and social stigma, negatively affecting the child's quality of life. The study emphasizes the need for early oral health intervention, better training for healthcare providers, and the development of standardized oral care protocols. Future research should aim to assess the effectiveness of targeted oral health interventions in improving nutrition and overall quality of life in pediatric oncology patients.

Abstract

Introduction: Neutropenic fever is commonly found in children with leukemia after chemotherapy. Fever occurs when children's immunity decreases after chemotherapy, and they are prone to infection. This study aims to describe the cultural care behaviors of multidisciplinary health workers in preventing neutropenic fever among hospitalized children with leukemia after chemotherapy. Methods: An ethnonursing study was conducted at a cancer referral hospital in Central Java, Indonesia. Data were collected through observation in two Pediatric ward and chemotherapy unit for three months and FGDs and in-depth interviews with health workers who provided care services to leukemia patients post-chemotherapy. Data were collected from 14 informants: six nurses, four doctors, two clinical pharmacists, and two nutritionists. This study employed four phases of Leininger analysis for qualitative data to develop its thematic structures. Results: The analysis of the entire data has revealed five themes: (1) performing antiseptic and aseptic techniques consistently, (2) nurse and dietician collaboration in handling eating difficulties post-chemotherapy, (3) collaborative interactions interdisciplinary in handling medication, (4) patient placement in an isolation room adjusted to patients' conditions, and (5) nurse's challenge to educate patients and their families. Conclusions: Health workers from multiple disciplines play essential roles in preventing neutropenic fever by involving patients, parents, and other family members. It is necessary for the nurses to collaborate with other healthcare professionals and educate patients, and families to participate in the care interventions for these patients in minimizing the occurrence of infection among leukemia patients.

Abstract

Introduction: Anxious and fear in pediatric patients before surgery is a challenge for health workers in the operating room, especially nurses. Various factors cause and ultimately affect the initial period to post-surgery. Distraction techniques have been known to be effective in reducing anxiety in pediatric patients before surgery. The use of technology can be an alternative distraction technique as part of non-pharmacological atraumatic treatment before surgery. The purpose of this study is to identify several non-pharmacological distraction techniques using technology. Method: Literature study was made by analyzing research articles published from 2013 to 2019 and in English. Data obtained from electronic databases Scopus, PubMed, Proquest, ScienceDirect, and Ebsco by using the keywords: "preoperative anxiety", "children", and "technology". Results: There were 10 quantitative research articles that met the inclusion criteria. The results of the study found that there are several non-pharmacological distraction techniques in reducing the anxiety of pediatric patients before surgery by utilizing technology, including virtual reality, handheld video games, video distraction, and smartphone / tablet-based educational multimedia.

Abstract

Navigasi pasien kanker merupakan pendekatan yang bertujuan untuk membantu individu dalam mengatasi hambatan selama proses diagnosis, pengobatan, hingga pemulihan. Penelitian ini bertujuan untuk mengkaji secara sistematis peran navigasi pasien dalam meningkatkan kualitas hidup penderita kanker melalui telaah literatur ilmiah. Metode yang digunakan adalah studi pustaka dengan menelusuri artikel dari database bereputasi internasional seperti PubMed, Springer, dan ScienceDirect. Hasil tinjauan menunjukkan bahwa navigasi pasien mampu mempercepat diagnosis dan inisiasi terapi, meningkatkan kepatuhan terhadap pengobatan, serta memberikan dukungan emosional dan informatif kepada pasien. Di negara berkembang, peran navigator juga berkontribusi dalam mengurangi ketimpangan akses layanan, khususnya bagi kelompok marjinal. Namun, di Indonesia, implementasi navigasi pasien masih terkendala oleh minimnya regulasi, pelatihan SDM, dan kesadaran masyarakat.. Kajian ini menekankan pentingnya integrasi layanan navigasi dalam perawatan kanker untuk mendukung kesejahteraan pasien secara holistik. Sehingga dapat disimpulkan bahwa navigasi pasien merupakan pendekatan strategis yang terbukti efektif dalam meningkatkan kualitas hidup pasien kanker, dan perlu diintegrasikan dalam sistem layanan onkologi nasional.

Abstract

Thalassemia is a chronic disease that requires lifelong treatment. Children with thalassemia are highly dependent on their parents for daily care and management. This study aims to identify key themes reflecting the challenges, needs, and expectations of parents caring for children with thalassemia through a qualitative meta-synthesis approach based on Roy’s Adaptation Model (RAM). A literature search was conducted in the PubMed, MEDLINE, Scopus, and ScienceDirect databases for English-language articles published between 2014 and 2024. Twelve studies from six countries were analyzed using the Critical Appraisal Skills Programme (CASP). The synthesis identified four meta-themes describing the parents’ adaptation process: (1) physical fatigue and psychosomatic symptoms, (2) psychological stress and parental identity crisis, (3) disruption of family roles and functions, and (4) lack of systemic and social support. These findings emphasize the importance of a multidimensional support approach encompassing emotional, educational, and health policy aspects to improve family quality of life. This study provides a theoretical foundation for family-centered nursing interventions and supports parental resilience.

Abstract

Thalassemia is a hereditary blood disorder that, while incurable, is entirely preventable through early detection. In Indonesia, an estimated 6–10% of the population carries the thalassemia gene but screening is not a nationwide mandatory. Existing initiatives remain fragmented across regions and target only specific groups, such as prospective brides and grooms in Jakarta or pilot programs in Bekasi Regency. Lack of early screening leads to the birth of thousands of children with thalassemia major, requiring lifelong transfusions and iron chelation therapy that costs the government IDR 800 billion per year. To achieve long-term sustainability, Indonesia must shift from curative to preventive strategies by integrating genetic screening into the national health insurance and reproductive health systems. Policy Ask: Develop a joint regulation between the Minister of Health and the Minister of Religious Affairs, or Minister of Health Regulation (Permenkes), mandating premarital and school-based thalassemia screening as part of national health and marriage registration programs.
