Abstract

Cancer is one of the catastrophic diseases that requires high treatment costs, long-term care, and poses a significant burden on the health system. In Indonesia, there are 514 hospitals stratified according to their expected capacity to provide cancer services; however, assessments in Central Sulawesi Province revealed that network hospitals remain at the basic level. This phenomenon highlights a gap between the growing need for cancer services and the actual capacity of hospitals. This study aims to evaluate the readiness of available resources and to analyze the strategic planning of hospitals in supporting the implementation of priority cancer service programs. A qualitative method with a case study design was applied, using interviews, observations, and document reviews. Data were analyzed through the stages of data reduction, data display, and conclusion drawing. The findings indicate that Anutapura Palu Hospital has reached an intermediate level with better readiness in terms of input, process, and product, while Torabelo Sigi Hospital remains at the basic level with significant limitations in human resources, facilities, operational documents, and funding. Strategic planning at Anutapura Palu Hospital has been more adaptive, enabling faster operationalization of cancer services, while Torabelo Sigi Hospital requires acceleration in governance documents, infrastructure development, and technical assistance. This study emphasizes that the success of cancer service implementation is highly determined by input readiness, process effectiveness, product outcomes, and adaptive strategic planning.

Abstract

Waktu tunggu pelayanan obat di Unit Farmasi Rawat Jalan RSU BaliMed Karangasem secara konsisten tidak memenuhi Standar Pelayanan Minimal (SPM), dengan rata-rata 45-60 menit untuk obat non-racikan (standar: 30 menit) dan 75-90 menit untuk obat racikan (standar: 60 menit), yang menjadi keluhan utama pasien. Penelitian tindakan (action research) kualitatif ini bertujuan untuk menganalisis akar masalah dan merancang strategi perbaikan dengan menerapkan konsep Lean Hospital. Pengumpulan data melalui observasi partisipatif, wawancara mendalam, dan studi dokumentasi selama 4 bulan mengungkap tiga pemborosan (waste) dominan: waiting (33%) akibat bottleneck verifikasi resep BPJS manual, motion (26%) karena tata letak tidak ergonomis, dan inventory (22% obat kadaluarsa). Analisis Value Stream Mapping (VSM) menunjukkan Process Cycle Efficiency yang sangat rendah, yaitu 22%. Berdasarkan temuan tersebut, dirancang Future State VSM yang mengintegrasikan tiga strategi utama: (1) Penerapan 5S dan analisis ABC untuk optimalisasi tata letak gudang, (2) Implementasi Sistem Kanban untuk mengelola persediaan obat, dan (3) Integrasi teknologi barcode scanner untuk mempercepat verifikasi resep. Simulasi strategi ini memproyeksikan penurunan waktu tunggu hingga 42-50% dan peningkatan efisiensi proses menjadi 38%. Analisis SWOT digunakan untuk merumuskan roadmap implementasi yang realistis, dengan mempertimbangkan kekuatan komitmen manajemen dan peluang integrasi teknologi, serta mengantisipasi ancaman seperti keterbatasan anggaran dan resistensi perubahan. Disimpulkan bahwa pendekatan Lean Hospital efektif untuk mengidentifikasi dan merancang solusi sistematis dalam menangani inefisiensi waktu tunggu. Penelitian ini merekomendasikan implementasi bertahap, dimulai dari quick wins seperti 5S, diiringi penguatan kapasitas SDM dan integrasi sistem informasi untuk keberlanjutan perbaikan.

Abstract

Medical tourism is the activity of patients traveling across countries to obtain better or more affordable medical services. Since 2012, Bali International Medical Center (BIMC) Siloam Nusa Dua Hospital has focused on cosmetic surgery and dental tourism services through the CosMedic Center. After the Covid-19 pandemic, patient visits decreased by 70%, while other medical tourism centers in Bali have grown rapidly, thus requiring BIMC to increase its competitiveness. This study aims to analyze the implementation of medical tourism services in improving hospital competitiveness. This study uses a qualitative descriptive approach with an in-depth interview method with key informants. The results show that at the pre-hospital stage, 24-hour service is not optimal and patient interest in choosing a complete package (transportation, accommodation, tour) is still low. At the hospital service stage, there is no special inpatient room for medical tourists, while the post-hospital stage runs according to the SOP. Collaboration with the Travel Agency (BPW) revealed the limitations in the quality and quantity of certified medical tourism guides. In conclusion, the implementation of the SOP for medical tourism at BIMC Siloam Nusa Dua has been running according to Permenkes No. 76 of 2015, but to strengthen competitiveness, it is necessary to improve 24-hour services, strengthen promotion and branding, develop special inpatient room facilities, and improve the quality and number of medical tourism guides. These findings are expected to be the strategic basis for developing sustainable medical tourism in Bali.

Abstract

ABSTRAK Rendahnya tingkat partisipasi peserta Program Pengelolaan Penyakit Kronis (Prolanis) menjadi permasalahan yang dihadapi oleh berbagai fasilitas kesehatan tingkat pertama, termasuk Klinik SMC Banyusari di Kota Denpasar. Data klinik menunjukkan bahwa rata-rata kehadiran peserta Prolanis hanya sekitar 21,2% per bulan, masih berada di bawah target minimal BPJS Kesehatan sebesar 50%. Penelitian ini bertujuan untuk menganalisis faktor internal dan eksternal melalui analisis SWOT guna menentukan strategi manajerial yang tepat dalam meningkatkan pelaksanaan Program Prolanis di Klinik SMC Banyusari. Penelitian ini menggunakan pendekatan kualitatif dengan desain studi kasus. Pengumpulan data dilakukan melalui wawancara mendalam, observasi, dan telaah dokumen terhadap enam informan yang terdiri dari kepala klinik, petugas Prolanis, dan peserta Prolanis. Data dianalisis menggunakan matriks Internal Factors Analysis Summary (IFAS) dan External Factors Analysis Summary (EFAS). Hasil penelitian menunjukkan bahwa Klinik SMC Banyusari memiliki kekuatan internal yang lebih dominan dibandingkan kelemahannya, dengan skor IFAS sebesar 1,31, serta peluang eksternal yang lebih besar dibandingkan ancaman, dengan skor EFAS sebesar 1,12. Klinik berada pada posisi strategi pertumbuhan stabil yang memungkinkan penguatan program melalui optimalisasi sumber daya internal dan pemanfaatan peluang eksternal. Strategi manajerial yang direkomendasikan meliputi penguatan edukasi berbasis peserta, optimalisasi peran petugas Prolanis, pemanfaatan teknologi informasi, serta pengembangan jejaring dengan masyarakat dan pemerintah desa. Dengan penerapan strategi tersebut, diharapkan partisipasi peserta Prolanis di Klinik SMC Banyusari dapat meningkat secara berkelanjutan Kata Kunci: prolanis, analisis SWOT, Strategi Manajerial     ABSTRACT Low participation in the Chronic Disease Management Program (Prolanis) remains a major challenge for primary healthcare facilities, including SMC Banyusari Clinic in Denpasar. Clinic records show that the average monthly attendance rate of Prolanis participants is approximately 21.2%, which is below the minimum target of 50% set by the National Health Insurance Agency (BPJS Kesehatan). This study aims to analyze internal and external factors using a SWOT analysis to determine appropriate managerial strategies for improving the implementation of the Prolanis program at SMC Banyusari Clinic. This study employed a qualitative approach with a case study design. Data were collected through in-depth interviews, observations, and document reviews involving six informants, consisting of the clinic head, Prolanis officer, and Prolanis participants. The data were analyzed using the Internal Factors Analysis Summary (IFAS) and External Factors Analysis Summary (EFAS) matrices. The findings indicate that the clinic’s internal strengths outweigh its weaknesses, reflected by an IFAS score of 1.31, while external opportunities exceed existing threats, with an EFAS score of 1.12. These results place the clinic in a stable growth strategic position, enabling program improvement through the optimization of internal resources and the utilization of external opportunities. Recommended managerial strategies include strengthening participant-centered health education, enhancing the role of Prolanis officers, utilizing information technology, and developing collaboration with local communities and village authorities. Implementing these strategies is expected to sustainably increase Prolanis participant attendance at SMC Banyusari Clinic. Keywords: Prolanis, SWOT Analysis, Managerial Strategy

Abstract

Latar Belakang: Penelitian ini dilatarbelakangi oleh tingginya kontribusi biaya farmasi terhadap total pengeluaran rumah sakit, serta munculnya permasalahan seperti keterlambatan distribusi, risiko kedaluwarsa obat, dan ketidaksesuaian stok di Instalasi Farmasi RS Samarinda Medika Citra. Tujuan: Penelitian ini bertujuan untuk menganalisis fungsi manajemen logistik pada instalasi farmasi serta merumuskan strategi perbaikan guna meningkatkan efisiensi operasional rumah sakit. Metode: Penelitian menggunakan pendekatan kualitatif dengan teknik wawancara mendalam terhadap tujuh informan kunci, meliputi pimpinan manajemen, kepala unit dan staf farmasi, serta didukung oleh hasil pengamatan dan telaah dokumen terkait. Hasil: Hasil penelitian menunjukkan bahwa fungsi logistik meliputi perencanaan, penganggaran, pengadaan, penerimaan, penyimpanan, distribusi, pemeliharaan, penghapusan, dan pengendalian telah berjalan, namun masih menghadapi beberapa kendala, seperti keterbatasan sistem informasi, koordinasi antarunit yang belum optimal, dan pengendalian stok yang lemah. Strategi: Strategi perbaikan yang dirumuskan melalui pendekatan SWOT dan Business Model Canvas mencakup peningkatan kompetensi sumber daya manusia, penguatan SOP, digitalisasi sistem logistik, serta optimalisasi koordinasi lintas unit. Kesimpulan: Penerapan strategi berbasis SWOT dan Business Model Canvas dapat menjadi instrumen efektif untuk meningkatkan efisiensi, mengurangi pemborosan, serta memperkuat profitabilitas rumah sakit.

Abstract

Human resources play a vital role in determining the existence and sustainability of an institution, including hospitals as complete health care facilities. The existence of doctors as the main asset demands a continuous transformation of work culture to maintain competitive service quality. This study discusses the transformation of doctors' work culture in supporting the improvement of service quality at RSPAU dr. S. Hardjolukito. The method used is descriptive qualitative with secondary data collection techniques, observation, and interviews. The results of the study show that doctors' work culture is influenced by regulations, SOPs, and internal and external factors, with quite good results (score 4). However, there are challenges in the form of information technology disruption, high workload, and competition between health facilities (score 3). Hospital management responded by clarifying procedures and providing appreciation to improve performance, career, and service quality (score 4).

Abstract

The long waiting time for medication services at the Outpatient Pharmacy Depot of Mokopido Tolitoli Regional Hospital has become a major issue in improving the quality of hospital services, as it indicates inefficiencies in the human resources (HR) management of pharmacists. This study aims to evaluate HR governance at the pharmacy depot to improve the efficiency of drug services. Identified problems include workload imbalance, patient waiting times that far exceed standards, and increasing public complaints about the lengthy service process. This study used a qualitative approach with in-depth interviews, observation, and document analysis as data collection techniques. Research participants included hospital leaders, medical support officers, pharmacy installation managers, as well as pharmacists, pharmaceutical technicians, and outpatients. The results of the study indicate that HR governance is not yet optimal. Staff needs planning is not based on workload analysis, task division is still rigid, rotation mechanisms are unstructured, competency training is limited, and the performance evaluation system is not integrated with quantitative indicators such as prescription service waiting times. These conditions indicate a gap between managerial policies and operational needs in the field. Based on these findings, improving HR governance is a strategic step to increase service efficiency. The practical implications of this research emphasize the importance of workload-based HR planning, task flexibility and rotation, and the development of measurable training and performance evaluation to support continuous improvement.

Abstract

Emergency Department (ED) services are crucial as they directly impact patient survival. A decline in ED visits at Permata Depok Hospital in 2024 and early 2025 has raised concerns, particularly related to patient complaints shared via social media. This study aims to analyse the influence of healthcare workers’ competence, doctor consultation time, and facility availability on patient complaints in the hospital’s ED. This is a quantitative study with a cross-sectional design, involving 93 respondents selected using the Slovin formula from a population of 1,180 patients. Data were collected through a Likert-scale questionnaire and analysed using chi-square tests and logistic regression. The results show that all three variables significantly affect patient complaints (p-value < 0.05), with facility availability especially basic sanitation like soap, tissue, and clean water emerging as the most dominant factor. Managerial implications include the need to prioritize supporting facilities, implement SOPs and regular sanitation audits, adopt rapid reporting systems using QR codes, and make data-driven decisions through satisfaction surveys and WASH-FIT audits. Future research is recommended to expand variables, apply mixed-methods approaches, conduct inter-hospital comparisons, and monitor intervention outcomes.

Abstract

This study aims to analyze the implementation of training programs in improving hospitality services at RSUD dr. R. Goeteng Taroenadibrata Purbalingga. The study is motivated by the need to enhance hospital service quality, which should not only focus on medical aspects but also on non-medical aspects such as friendliness, empathy, and effective communication with patients and their families. A qualitative descriptive approach was employed involving 13 informants consisting of hospital employees, health workers, and non-health staff. Data were collected through in-depth interviews, observation, and documentation, and were analyzed using the interactive model of Miles and Huberman, which includes data reduction, data display, and conclusion drawing. The findings indicate that hospitality training has been conducted regularly, covering interpersonal communication, service excellence, and the development of patient-friendly attitudes. The implementation of the training has increased employees’ awareness of patient-oriented services. However, several challenges remain, including limited training duration, lecture-dominated learning methods, unequal training opportunities for all staff including outsourced workers, and limited post-training evaluation to measure behavioral changes. Therefore, this study recommends improving interactive training design, expanding participant coverage, and strengthening continuous evaluation to ensure sustainable improvements in hospital hospitality services.
